2.Bougie Dilatation of a Patient with Esophageal Lye Stricture Fed with Gastrostomy Tube for 21 Years: A case report.

The patient was a 51 year-old woman suffering fraen dysphagia due to upper esoyhageal lye stricutue whieh had developed as a result of a suicide attempt 21 years ago. Shortly after that, she underwent feeding gastrostomy and has lived in the gastrostomy state for 2l years. After admission, she underwent a barium esophagoram which revealed a near total obstruction at the cricoid cartilage level. Bougie dilatation with American Dilation System was tried on day 3. But the spring tip marked guide wire which was to be used with the American Dilation System could not be passed through the stricuture. Therefore, we performed a bougie dilatation using angiographic guide wire M(H-AG-35in-150 cm) with success. On day 14, she underwent a barium esophagogram which revealed her improved condition, and she was discharged on day 16.

3.Virtual Plastic Surgery using Adobe Photoshop(R).

Digital imaging is emerging as a standard method for patient documentation in clinical setting. Managing patient expectations before aesthetic surgery can greatly improve patient satisfaction after surgery. The patient who visited for plastic surgery wants a predicted figure after the operation. A virtual plastic surgery software is necessary in order to satisfy the desire of patients. Adobe Photoshop(R) is the professional standard in desktop digital imaging, offers indispensable new features for graphic and web design, photography, and video. Using imaging technology, it takes just minutes to realistically simulate the results of double eyelid operation, liposuction, rhinoplasty or any other aesthetic procedure. The aim of this study is to analyze the significance of the digital image processing and to introduce the virtual plastic surgery using Adobe Photoshop(R).

4.Treatment of Facial Scars with CO3 Laser.

Numerous modalities have been used to treat facial scars. Although excellent results can be obtained by high-energy, pulsed carbon dioxide(CO2) laser, significant morbidity exists due to the considerable thermal damage to the skin. CO3 Laser combines the performance of a CO2 system with the safety and convenience of an Er:YAG laser. The CO3 effect is due to a wide range of pulse durations, up to 20 times longer than those delivered by conventional Er:YAG lasers. The result is a thermally tunable laser, with a zone of thermal effect up to 100micrometer in depth. With the CO3 laser, the user chooses the correct depth of thermal effect for maximum safety and efficacy. This study was performed to evaluate the effect of the long-pulsed Er:YAG CO3 laserbrasion in treatment of scars and propose that it can be a effective method to improve cosmetic results. The authors treated 47 cases (8 males and 39 females) including acne scar(n=8), annular atrophic scar(n=9), posttraumatic scar(n=19), postsurgical scar (n=11) with CO3 laser. Questionaire were completed for clinical assessment by the patients and clinicians. Excellent and good results were achieved in 87.3% of scars in clinician's assessments and in 78.8% of scars in patient's assessments. In conclusion, resurfacing with a long-pulsed Er:YAG laser is a safe and very effective treatment modality for facial scars.

Facial rhytides are a common problem that often presents for treatment. Laser facial skin resurfacing has become a method of treatment for photoaging and pigmentary conditions. Although excellent results can be obtained by high-energy, pulsed carbon dioxide(CO2) laser, significantly morbidity exists due to the considerable thermal damage to the skin. Long-pulsed Er:YAG laser (CO3 laser, Cynosure) combines the performance of a CO2 system with the safety and convenience of an Er:YAG. Yet the CO3 laser is more than the combination of two complementary resurfacing lasers. It brings thermal management to skin resurfacing. The CO3 effect is due to a wide range of pulse durations, up to 20 times longer than those delivered by conventional Er:YAG lasers. With the CO3 laser, the user chooses the correct depth of thermal effect for maximum safety and efficacy. The purpose of this study was to evaluate the results of the CO3 laser in the treatment of facial wrinkles. Forty-three patients, aged between 35 and 70 years, with facial wrinkles, were evaluated. Wrinkle improvement was evaluated by the patient's satisfaction, clinical improvement, and photographic analysis before and after the resurfacing procedure. The long-pulsed Er: YAG laser for facial wrinkles is a safe and effective mode for laser skin resurfacing.

7.A Case of Pancreatic Serous Cystadenoma Associated with Papillary Thyroid Cancer.

The most common pancreatic cystic lesion is pancreatic pseudocyst which represents about 85%. Primary cystic neoplasms represent about 10 to 15% of the lesion. Pathologically cystic neoplasms can be classified into serous cystadenoma, mucinous cystadenoma and papillary cystic neoplasm by epithelial lining-cell, whereas pseudocyst is characterized by fibrotic capsules. Mucinous form is known to be premalignant or malignant and serous cystadenoma was known to be benign in the past, but recently 4 cases of malignant transformation have been reported. Serous cystadenoma is described under a variety of names, including microcystic adenoma and glycogen-rich cystadenoma but recently macroqystic variants have been reported. Serous cystadenoma is most commonly seen in middle aged women with symptoms of vague upper abdominal pain or palpable mass. It is sometimes associated with extra- pancreatic diseases such as gallstones, diabetes mellitus, hypertension, duodenal ulcers, sterility, obesity and thymic dysfunction, but coexisting papillary thyroid cancer have been reported in only 2 cases to our knowledge. The pathogenesis of associated diseases is unknown and appears to be due to function of age of the patients or incidental occurrence. Herein, we report a patient who had a pancreatic serous cystadenoma coexisting with papillary thyroid cancer. Since pancreatic serous cystadenoma can occur in association with papaillary thyroid cancer, examination of thyroid seems to be advisable when pancreatic serous cystadenoma is found.

10.A Clinical Study of Pregnancy and Delivery in Women Aged 40 Years and Older.

OBJECTIVE: To access the pregnancy tendency and delivery in women aged 40 years and older. METHODS: From January 1999 to June 2001, 7882 deliveries were in Gacheon Medical School. Among them we compared 95 cases of pregnant women aged 40 years and older with 94 cases of those aged under 40 years as a control group. RESULTS: The parity was higher in women aged older than 40 years compared to control group. Number of prenatal special studies was higher in women aged older than 40 years (n=82) compared to control group (n=14). In modes of delivery and indications for c/sec, there were no difference in two groups. There were no difference in prenatal complications between two groups. But, number of cases related to placenta previa, myoma and anemia were increased in women aged older than 40 years. In postpartum complications, uterine atony and hysterectomy, postpartum bleeding and placenta accreta were higher in women aged older than 40 years compared to control group. CONCLUSION: For pregnant women aged 40 years and older, a careful approach and appropriate management will be necessary for good pregnant outcome.
