4.A Case of Pigmented Bowen's Disease.

A 38-year-old woman was presented with a dark brown plaque on the abdomen. Clinically, the tumor was simulating the appearance of dysplastic nevus. Microscopically, the cells of the stratum malphighii lay in a disordered pattern. Many cells in the epidermis were atypical and melanin pigment was mainly in the basal layer of the epidermis and the upper dermis. Diagnosis of pigmented Bowen's disease was made. Pigmented Bowen's disease is rarely found at body sites other than the anogenital area.

6.Clock drawing test to screen for dementia in parkinsonian patients with low educational backgrounds

Objectives: This study was aimed to assess the usefulness of the quantitative assessment of clock
drawing test (CDT) combined with the Mini-Mental State Examination (MMSE) compared to that
of the Montreal Cognitive Assessment (MoCA) or the MMSE alone for screening of dementia in
Parkinson disease (PD) in patients with a low educational level. Methods: A representative sample of
91 PD patients was administered MMSE, MoCA and CDT. The discriminative validity of the MMSE,
MoCA, and a MMSE+CDT combination for dementia screening was determined by estimating the
sensitivity and specificity of each test and by testing integrated discrimination improvement (IDI).
Results: The mean age and educational years were 69.0 (years) and 7.3 in the study population. The
best screening cut-off points for the MMSE, MoCA, and MMSE+CDT were 25/26, 21/22 and 41/42.

7.The prognostic factor for predicting neurologic sequalae in glufosinate ammonium intoxication

Objective:
This study aimed to analyze the clinical characteristics of patients with acute glufosinate ammonium (GLA) poisoning and investigate the indicators associated with the severity of neurotoxicity in GLA-poisoned patients.
Methods:
We retrospectively collected the data of patients who were admitted due to GLA poisoning from 2018 to 2022, to gather the factors that could influence neurologic outcomes. These outcomes were estimated based on the Glasgow Coma Scale (GCS) scores at admission, the worst GCS score during impaired consciousness, the GCS score at discharge, and the presence and duration of seizures.
Results:
Among the 67 GLA-poisoned patients, the average GCS score at admission was 13.8±2.6 points, the worst GCS score recorded was 11.2±3.5 points, and the GCS score at discharge was 13.7±2.9 points. The factors significantly influencing the GCS score at the initial admission included respiration rate, saturation, white blood cell count, and pH (P=0.037, P=0.005, P=0.021, and P=0.001, respectively). Factors affecting the worst GCS score included age, diastolic blood pressure, platelet count, blood urea nitrogen (BUN), and pH (P=0.001, P=0.016, P=0.015, P=0.002, and P<0.001, respectively). The GCS score at discharge exhibited significant correlations with age, BUN, and pH (P<0.001, P<0.001, and P=0.011, respectively). The average age of the patients who experienced seizures after GLA poisoning was significantly higher at 75.1±12.9 years compared to that of patients without seizures (mean age, 65.0±15.2 years; P=0.006). Additionally, the average bicarbonate level was lower in patients with seizures, measuring 19.0±4.7 mmol/L, when compared with that of patients without seizures (average, 21.4±4.3 mmol/L; P=0.045).
Conclusion
The elderly, metabolic acidosis, and elevated BUN could serve as good indicators for adverse neurological outcomes in GLA-poisoned patients.
8.A Case of Actinic Keratosis Treated with Topical Imiquimod.

Actinic keratoses (AK) are premalignant lesions, which can develop into invasive squamous cell carcinoma(SSC). Therapies for AK include cryotherapy, curettage, topical 5-fluorouracil, laser therapy and so forth. All therapies currently prescribed can be painful and may result in scarring and changes in skin pigmentation. A potential new treatment modality for AK is the imiquimod (Aldara(R)), which can be applied by patients themselves. We reported a case of AK which was very extensive on the scalp of the old man, who was treated with topical imiquimod.

9.Angiotensin Converting Enzyme Gene Insertion/Deletion Polymorphism and E-selectin S128R Polymorphism in Peripheral Atherosclerotic Occlusive Disease.

PURPOSE: Studies concerning the relationship between gene polymorphisms and potentially implicated cardiovascular disease have produced conflicting findings, in part due to differences in ethnic background between populations. These led us to evaluate the impact of polymorphisms in the ACE and E-selectin genes on peripheral artery atherosclerosis in a Korean population. METHODS: We studied 92 male patients (median age: 65.9, range: 48~82) with severe peripheral atherosclerosis documented by angiography and ABI (ankle brachial index). The control group comprised 290 healthy persons (male 216, female 64, median age 61.3, range 20~90) without symptoms for peripheral vascular disease. The blood samples were stored at -20oC until DNA was ready to be extracted. The inorganic procedure for DNA extraction was based on the method described by Miller et al. The ACE and E-selectin polymorphisms were detected by polymerase chain reaction (PCR) amplification. RESULTS: The distribution of ACE genotypes of the patient group was as follows: II, 34 (37.0%); ID, 46 (50.0%); and DD, 12 (13.0%). It was not significantly different from that of the control subjects: II, 104 (37.1%); ID, 133 (47.6%); and DD, 43 (15.3%) (P=0.80). The allele frequencies of the patient group were as follows: I, 114 (62.0%); and D, 70 (38.0%). It was not significantly different from that of the control subjects: I, 341 (60.9%); and D, 219 (39.1%) (P= 0.80). The frequencies of E-selectin genotypes in the patient group were as follows: Ser/Ser 85 (93.4%); Ser/Arg, 6 (6.6); and Arg/Arg, 0 (0%). It was not significantly different from that of the control subjects: Ser/Ser, 262 (93.6%); Ser/Arg, 18 (6.4%); and Arg/Arg, 0 (0%) (P=0.95). In addition, the allele frequencies of the patient group were as follows: Ser, 176 (96.7%); and Arg, 6 (3.3%). It was not significantly different from that of the control subjects: Ser, 542 (96.8%); and Arg 18 (3.2%) (P=0.95). CONCLUSION: The I/D polymorphism of the ACE gene and E-selectin S128R polymorphism were not significantly different between the atherosclerotic patient group and the normal control group in Koreans.

10.En Bloc Orbitectomy for Recurred Adenoid Cystic Carcinoma of the Lacrimal Gland.

A 54-year-old woman presented with an exophthalmos and a mass on her right eye, which proved to be recurred adenoid cystic carcinoma of the lacrimal gland. Orbital CT and MRI showed that tumor was located in the right orbit, but skin over right eye brow was also involved. There was no visualization of tumor extension into the intracranial compartment. The authors have performed an en bloc orbitectomy. Although en bloc orbitectomy is known to be useful in reducing recurrence, this case showed the tumor recurrence and even distant metastasis that occurred after total tumor removal via en bloc orbitectomy. As adenoid cystic carcinoma of lacrimal gland is known to have high rate of recurrence and metastasis, en bloc orbitectomy should be performed to reduce local advancement and distant metastasis. In case of distant metastasis, radiotherapy could be useful strategy in selected case.
