1.An analysis of a telephone survey on knowledge of iodine deficiency disorders in Shanghai residents

Objective To find out whether Shanghai residents have mastered the knowledge related to iodine deficiency disorders (IDD),and how they choose different kind of salt.Methods Residents were selected by stratified random sampling from all 18 districts(counties) of Shanghai in 2010.Simple random sampling was used at the first level; random function was used at the second level to produce the last 4 numbers of a phone number.People who own the number were selected to be called.Results Totally 219 people completed the investigation.49.3% (108/219) of the residents only selected iodized salt,and 25.6%(56/219) choose non-iodized salt; 6.8% (15/219) selected both,and 18.3% (40/219) don't care.About the reason of choosing iodized salt,25.9% (28/108) thought it can prevent IDD,6.5% (7/108) thought it's good to children's intelligence.About the reason of choosing non-iodized salt,35.7% (20/56) thought they were not iodine deficiency,17.9% (10/56) thought Shanghai was not an IDD epidemic region.Among the 126 people who had heard of iodine deficiency disorders,7.1% (9/126) believed that iodine deficiency disorders can lead to varying degrees of mental impairment,65.1% (82/126) thought it can lead to endemic goiter; 45.2%(57/126) thought eating iodized salt and 33.3% (42/126) thought eating kelp and laver can prevent IDD.58%(127/219) had no idea of IDD and/or its hazards.Conclusions The resident's knowledge on iodine deficiency disorders is not satisfactory.We should make more effort in health education and help people to choose salt reasonably.
2.The Analysis of Risk Factor and Infection Control of Carbapenem-Resistant Acinetobacter baumannii in a Medical Intensive Care Unit.

BACKGROUND: An epidemiologic study was performed after the outbreak of carbapenem-resistant Acinetobacter baumannii (CRAB) in the medical intensive care unit (MICU) from December 2006 to May 2007. METHODS: A retrospective case-control study was performed using the medical records of the patients. The case and control patients were compared for age, gender, total length of stay in MICU, prior carbapenem use, Acute Physiology and Chronic Health Evaluation II (APACH II) score, presence of central line, effect of mechanical ventilation, and sputum suction. Environmental and hand-washing studies were performed during the outbreak. RESULTS: Ten CRAB-affected patients and 29 controls were enrolled in this study. Univariate analysis showed that the age, total length of stay in MICU, presence of central line, and prior carbapenem use were associated with the CRAB outbreak. However, multivariate analysis showed that only prior carbapenem use was associated with the CRAB outbreak (odd ratio: 8.67, P=0.01). The outbreak disappeared after implementing a combined infection control strategy, including the sequential disinfection of MICU and strict compliance with cross-transmission prevention protocols. CONCLUSION: The use of carbapenem was associated with an increased risk of CRAB infection. This study suggests that the MICU contamination and infection transmission by health-care workers played a major role in the CRAB outbreak. Novel strategies such as restricted use of broad-spectrum antibiotics, strict hand hygiene, strict isolation of the patients, and MICU disinfection may be required to prevent the CRAB outbreak.

3.A hospital based case control study on the risk factors of stomach cancer.

BACKGROUND: For gastric cancer, primary prevention by risk factor modification might have some important roles. However, previous studies having investigated the factors associated with stomach cancer reported various results. In addition, there were only a few studies based on Korean population. METHODS: A case control study was carried out on 106 cases matched for age and sex with 106 controls in a tertiary care hospital. In patients who were aged 75 years or less and had been newly diagnosed as adenocarcinoma of stomach between July 1996 and January 1997 were included into the case group. Information on baseline characteristics, health habits, dietary habits of study subjects was obtained through an interview using structured questionnaire. Conditional logistic regression analysis was used to evaluate the factors associated with stomach cancer. RESULTS: Salt preference was associated with significantly increased risk of stomach cancer (odds ratio[OR]=9.81, 95% confidence interval[CI]=2.28 42.2). Eating broiled food more than three times a week significantly increased the risk of stomach cancer (OR=3.33, 95% CI=1.16 9.55) compared to eating it less than once a week. Blood type, family history of stomach cancer, smoking, and alcohol consumption were not associated with stomach cancer. CONCLUSION: Salt preference and frequent eating of broiled food are the risk factors significantly associated with stomach cancer in this study subjects.

4.Plasmacytoma on the upper cyclid.

Plasmacytoma is one of the plasma cell dyscrasia. It occurs commonly on nasopharynx and paranasal sinus, but rare on the eyelid. If it involves the orbit, variable symptoms are presented, such as painless growing mass, proptosis, diplopia, decrease of vision, eyeball deviation etc. Irradiation is generally accepted as the treatment of choice for extramedullary plasmacytoma. Although the patient treated with irradiation safisfactory, wer must carefully follow up the patient about progression of multiple myeloma. We experienced a 15-years-old female who had a progressive growing palpable and non-tender mass on the right upper eyelid. The mass was confirmed as plasmacytoma by pathologic examination. We treated the mass with radiation. So, We present a case of plasmacytoma on the upper eyelid.

5.A comparison between attitude to the guideline and reported practice pattern of hypertension management in family physicians working in private clinic.

BACKGROUND: Importance of the participation of clinical practitioners in the development of guideline is increasingly emphasized. We studied the attitude of family physicians to the guideline for hypertension management and compared it with their reported practice patterns. METHODS:We developed a guideline for hypertension management with reference to other guidelines previously published and used by WHO and health organizations in many countries. A questionnaires asking attitude to the contents of the guideline and real practice pattern was sent to 200 Korean family physicians. Response rate to the questionnaire was 27.5%. RESULTS: The agreement rate to annual blood screening in persons aged 15 and older was 87%, while the reported practice rate in most of patients was only 42%. The agreement rates to medical history taking in hypertensives were high in all items, but reported practice rate was very low. The agreement rates to physical examination in hypertensives were relatively high in all items, while reported practice rate was high only in lung and heart examination. The reported practice rate of laboratory tests were less than 60% and that of electrolyte check was only 22%. Most of the study subjects agreed with all behavior modification methods. Although 80% of the subjects agreed that diuretics or beta blocker should be used as an initial regimen, the reported practice rate was only 36 %. CONCLUSION: There observed a big difference between attitude to the guideline and reported practice pattern in this study. It showed the need to narrow the gap between them.

6.Wandering Behavior in Korean Elders with Dementia Residing in Nursing Homes.

PURPOSE: This study investigated various factors relatied to wandering behavior of Korean elders with dementia (KED). METHODS: A sample of 160 ambulatory residents with dementia from 14 long term care facilities was used to examine demographic, individual, cognitive, physical health, and environmental characteristics by comparing wanderers (N=108) to nonwanders (N=52). Subjects were evaluated by Korean versions of the Mini-mental State Exam (K-MMSE), the Physical and Instrumental Activities of Daily Living (K-PIADL), and the Revised Algase Wandering Scale Nursing Home version (KRAWS-NH) along its six dimensions. Demographic and environmental data were also obtained. Independent sample ttests, Chi-square test, Fisher's Exact tests, and ANCOVAs were used to examine differences between wanderers and nonwanders. RESULTS: Wanderers were significantly (p<.05) older and had more limitations in K-PADL and K-IADL. The degree of overall wandering and certain features of wandering were significantly different (p<.05) by total number of residents in the facility, type of bedroom (i.e., "Ondol"), and color of bedroom and living-room walls (i.e., sky blue). CONCLUSIONS: Findings of this study may be useful in understanding wandering behavior of KEDs and thus developing more culturally specific management strategies.

7.An Analysis on the Job Satisfaction and Job Characteristic for the dietitians who perform Nutrition Service in the field of Industry Foodservice.

This study has been focused on understandings for the problems of dietitian who perform nutrition service in the field of industry foodservice and then focused on using of its findings as basic material for smooth nutrition service performance through analyzing job satisfaction, job characteristic and its importance of dietitians' task in industry foodservice. A questionnaire survey of 120 nutritionists who have engaged themselves in industry foodservice-60 are under direct management and 60, held in trust-has been performed, and 95 responses (79%) have been collected and categorized, except some unfinished responses. The examined data have been classified statistically by using of SPSS, and then analyzed into frequency, percentage, mean value, standard deviation, and correlation among factors, according to questionnaires. The findings of the research can be summarized as following: The details of the surveyed dietitians were: 20~25 years old on an average; working less than two years; college graduates; mere employees; receiving monthly pay of 70~100 won on an average; working more than 52 hours weekly; and providing with four meals a day in a single menu. For job satisfaction and job characteristic, the service itself and the understanding of the service appear as main features. For the relative importance of the service, the findings show that the menu making, sanitation and cost control occupy an important position, while nutrition counseling, nutrition education and dietary control by ailments make up very low portion. For the cause of not enacting the nutrition service, the lack of counseling ability and the overburden of food service are at the top. The findings of this research, therefore, present the needs of the service capacity education and the reduction of excessive foodservice hours of dietitians in order to secure the efficient nutrition service in industry foodservice. To achieve this goal, first of all, there should be an intensive education course in school by using of practice hours. for enhancing practical service adaptability, and then the computerization of foodservice should be executed perfectly to reduce the excessive foodservice hours.

9.Effects of Video Assisted Education Using Smartphone on Bowel Preparation for Colonoscopy.

PURPOSE: The purpose of this study was to develop video assisted education on bowel preparation for colonoscopy (VEBPC) and use a snartphone to evaluate effects of the VEBPC. METHODS: Adult patients who were scheduled for colonoscopy were recruited from a university general hospital and randomly assigned to three groups. Group 1 (n=30) watched the video using a computer set in the endoscope consulting room. Group 2 (n=29) watched it using a smartphone, and group 3, the control group (n=29) received education with existing instructions at the reservation- reception desk. Participants were evaluated on knowledge on taking bowel preparation agents and diet, compliance on taking bowel preparation agents and diet, satisfaction with education, and actual level of bowel preparation. RESULTS: Group 1 and 2 showed significantly (p<.001) higher scores for knowledge, compliance, and satisfaction compared to the control group. However, in post-hoc test analyses there were no significant differences in these variables between group 1 and 2. No significant difference was found in the actual level of bowel preparation among the three groups. CONCLUSION: Findings from this study show that VEBPC using smartphone is a better option than existing educational methods. However, replication studies are necessary to confirm these findings.

10.Association between Endometriosis and Polymorphisms of N-acetyl Transferase 2 (NAT2), Glutathione S-transferase M1 (GSTM1) and Cytochrome P450 (CYP) 1A1 Genes in Korean Infertile Patients.

OBJECTIVE: To investigate the association between endometriosis and polymorphisms of N-acetyl transferase 2 (NAT2), glutathione S-transferase M1 (GSTM1), and cytochrome P450 (CYP) 1A1 genes in Korean infertile patients. MATERIALS AND METHODS: A total of 303 infertile patients who had undertaken diagnostic laparoscopy during January, 2001 through December, 2003 at Samsung Cheil Hospital enrolled in this study. The patients were grouped according to laparoscopic findings: minimal to mild endometriosis (group I: n=147), moderate to severe endometriosis (group II: n=57), normal pelvic cavity (n=99). Peripheral blood was obtained and genomic DNA was extracted. The genotypes of each genes were analyzed using polymerase chain reaction (PCR) and restriction fragment length polymorphism (RFLP). For NAT2, RFLP was used to detect the wild type (wt) and mutant (mt) alleles, enabling classification into slow (mt/mt) or fast (wt/wt or wt/mt) acetylation genotypes. For GSTM1, PCR was used to distinguish active (+/- or +/+) from null (-/-) genotypes. For CYP1A1, MspI digestion was used to detect the wild type (A1A1), heterozygote (A1A2) or mutant (A2A2) genotypes. RESULTS: The genotype frequencies of NAT2 slow acetylator was 12.8%, 10.9%, 12.8% in group I, group II and control, respectively. The genotype frequencies of GSTM1 null mutation was 55.3%, 41.8%, 53.2% in group I, group II and control, respectively. The genotype frequencies of CYP1A1 MspI polymorphism was 16.3%, 9.1%, 18.1% in group I, group II and control, respectively. No significant difference was observed between endometriosis and normal controls in the genotype frequencies of the NAT2, GSTM1, CYP1A1 MspI polymorphism. CONCLUSION: The NAT2, GSTM1, CYP1A1 gene polymorphism may not be associated with the susceptibility of endometriosis in Korean women.
