Neovascular age-related macular degeneration (AMD) is the main cause of vision loss in AMD patient. Now,the application of anti-vascular endothelial growth factor(VEGF) has become the preferred therapy for the treatment of neovascular AMD,and it has been proved to improve the visual acuity and reduce the blindness rate. But the formation of choroidal neovascularization (CNV) in AMD is a comprehensive and integrated pathological process. It is difficult to cure the CNV with only anti-VEGF drug for all the neovascular AMD patients. Therefore, the targeting drugs toward other signal pathway associated with CNV are being in different phases of clinical trials.Resent years, with the development of science and technology, small interference RNA(siRNA) and genome technology has been applied to treat neovascular AMD and show a potential prospect, and gene therapy and stem cell therapy are the most remarkable and predominant ways for AMD.Doubtlessly, these therapies offer novel choice for the management of neovascular AMD.

Journal of Acupuncture and Tuina Science 2020;18(3):203-206
Objective: To observe the clinical effect of auricular point sticking therapy for cardiac syndrome X (CSX) in women. Methods: A total of 64 patients were randomized into two groups by the random number table, with 32 cases in each group. Patients in the control group received conventional treatment for angina, while patients in the treatment group received auricular point sticking therapy on the basis of conventional treatment. After 8 weeks of treatment, the levels of serum estradiol (E2), nitric oxide (NO) and endothelium-1 (ET-1) were compared to evaluate the therapeutic effect. Results: After 8 weeks of treatment, the total effective rate and markedly effective rate in the treatment group were significantly higher than those in the control group, and the between-group comparisons showed statistical significance (both P<0.05). After treatment, the scores of chest pain and tightness in the control group dropped significantly, and the intra-group comparisons showed statistical significance (both P<0.05); the scores of chest pain and tightness, palpitations, weakness and shortness of breath in the treatment group dropped significantly, and the intra-group comparisons showed statistical significance (all P<0.05); there were significant inter-group differences in the scores of chest pain and tightness. After treatment, the intra-group comparisons of E2, NO and ET-1 contents in the control group showed no statistical significance (all P>0.05), while the E2, NO and ET-1 contents in the treatment group changed significantly after treatment and were significantly different from those in the control group (all P<0.05). Conclusion: Auricular point sticking on the basis of conventional treatment was effective for CSX in women, and is worth clinical application.

Objective To investigate the clinical efficacy, operative essentials and indications of hybrid spinal fusion surgery for cervical spondylotic myelopathy. Methods From August 2008 to December 2011,thirty-eight patients with cervical spondylotic myelopathy underwent hybrid spinal fusion surgery in our hospital. There were 27 males and 11 females, aged from 33 to 70 years (average,51 years).A total of 86 segments were treated (fusion 48 vs.non-fusion 38).Twenty-eight patients underwent a two-level surgery, and ten patients received a three-level surgery. The Japanese Orthopaedic Association (JOA) score and Visual analogue scale (VAS) were used to evaluate pre-and post-operative neurological function and pain, respectively. The pre-and post-operative range of motion of the cervical spine was measured according to Xrays. Moreover, the surgical complications were recorded and analyzed. Results Thirty-seven patients were followed up for 15 to 55 months (average,29.1 months). The improvement of neurological function was obtained in 36 patients. The JOA score was improved from preoperative 10.5±1.57 to 14.3±1.97 at final follow-up,with an improvement rate of 58.46%,and the results were excellent in 16 cases, fair in 20 cases and poor in 1 case. The VAS was improved from preoperative 7.3±1.04 to 3.2±1.41 at final follow-up.The Cobb angle changed from preoperative 25°±3.21°to 20°±2.56°at final follow-up.After operation, the neurological function was not restored in 1 case; hoarseness and bucking occurred in 2 cases; sore throat occurred in 22 cases; anterior displacement of prosthesis (PCM) occurred in 3 cases.Other patients had no complications, such as displacement, loosening and heterotopic ossification. Conclusion In hybrid spinal fusion surgery, the lesions segments are decompressed fully, the severely degenerative segments are fused, and the motion of the non-fusion segments is reserved. As a result, not only the stability of the cervical spine is achieved, but also an obvious improvement of symptoms and a satisfactory short-term efficacy can be obtained. Therefore, this method is an alternative procedure for cervical spondylosis myelopathy.

Objective CXCR4-overexpressing bone marrow-derived mesenchymal stem cells (CXCR4-BMSC) were constructed and co-cultured with hypoxia/re-oxygenation pretreated renal tubular epithelial cells (HR-RTEC).Repair of HR-RTEC was detected and the possible mechanism was also discussed. Methods CXCR4-BMSC (CXCR4-BMSC/eGFP,eGFP as the tracer gene) and null-BMSC (BMSC/eGFP) were obtained by gene transfection technique, and the level of CXCR4 in the transfected cells was detected.RTEC was cultured under hypoxia/re-oxygenation condition for 12 h,respectively,to obtain HR-RTEC,which was used to simulate AKI in vitro.BMSC and HR-RTEC were co-cultured for 12 h,and the proportion of apoptotic cells among the HR-RTEC was assayed by immunofluorescence technique.Western blot was used to test the protein levels of cleaved Caspase-3 and Bcl-2.The number of migrating BMSC was also assayed. After culturing with the HR-RTEC culture supernatant, the expression of cytokeratin 18 (CK18) in BMSC was tested by immunofluorescence staining.Cytokines including bone morphogenetic protein-7 (BMP-7),hepatic growth factor (HGF) and interleukin-10 (IL-10) in the BMSC culture supernatant were detected by ELISA method. Results Expression of CXCR4 was enhanced in CXCR4-BMSC.Proportions of the apoptotic cells among HR-RTEC after being co-cultured with BMSC,CXCR4-BMSC and null-BMSC were all decreased, especially in the C/H group. The decreased cleaved Caspase-3 and enhanced Bcl-2 were also observed in HR-RTEC.The number of migrating CXCR4-BMSC was the highest.Proportions of CK18+ cells in BMSC,CXCR4-BMSC and null-BMSC were all low and showed no difference.However,CXCR4 overexpression in BMSC stimulated secretions of BMP-7,HGF and IL-10.Conclusions CXCR4-overexpressing BMSC has more repair effect on the co-cultured HR-RTEC,the enhanced migration ability and secretion ability of CXCR4-BMSC are the possible mechanisms.
