Objective To evaluate the safety and efficacy of epirubicin (EPI) and gemcitabine (GEM) alternating sequential intravesical chemotherapy after transurethral resection in the treatment of non-muscle invasive bladder cancer. Methods 240 patients with primary non-muscle invasive bladder urothelial carcinoma were randomly divided into 2 groups. There were 120 cases for each group,EPI group was given EPI 50 mg (once a week),bladder perfusion, while the EPI+GEM group was given EPI 50 mg (once every other week),GEM 1000 mg (once every other week),alternating sequential perfusion. The follow-up time ranged from 6 to 24 months when the time of tumor recurrence and adverse reactions of chemotherapy were observed and recorded. Results The 2-year tumor free survival rate for EPI group recurrence was 60.0 % (72/120),and 75.0 % (90/120) for EPI+GEM group. There were statistical significance between the differences of the 2 groups (x2 =5.489,P < 0.05).3 cases in EPI group and 2 cases in EPI+GEM group progressed to muscle invasive bladder cancer, and there was no statistical significance between the differences of bladder cancer progression rates for the 2 groups (2.5 % and 1.7 %) (x2 =0.000,P < 0.05).The main adverse reaction during the treatment was gastrointestinal discomfort (15 cases and 6 cases respectively),no serious haematological toxicity and other adverse reactions were frequent urination, urgency, dysuria and hematuria. The differences between the occurrence rates of adverse reactions for the 2 groups were 25.0 % and 11.7 %,which were statistically significant (x2 =6.252,P < 0.05).Conclusion For non-muscle invasive urothelial bladder carcinoma, the curative effect is better to use EPI and GEM sequential intravesical chemotherapy than to use EPI alone, which can not only reduce the 2 years recurrence rate after the operation, but also reduce the incidence rate of adverse reactions.Yet,this method cannot change the progress of bladder cancer.

Objective To investigate the effect of photodynamic therapy with chlorin e6 (Ce6) and 5-aminolevulinic acid (5-ALA) on mouse model of malignant melanoma. Methods The mouse model of malignant melanoma was established by injecting 0.1 ml A-375 cells (about 2?10 6 cells) under the right hind leg of BALB/c-6 mice, and that the yellowish white node appears at injection site proves the successful model. Twenty-four of 27 successful mouse models were irradiated at the tumor site with semiconductor laser (wavelength 652 nm) with a total dose of 100 J/cm 2 . Before laser exposure, the mice were treated with 10% 5-ALA by topical compress for 2 h or 7.5 mg/kg Ce6 by intraperitoneal injection for 1 hour or 5-ALA topical application combined with intraperitoneal injection of Ce6 (n=6 in each group). Another six mice as control only underwent PDT. One week after PDT,the mice were killed, the tumor mass was peeled off and weighed, whether the metastasis occurred or not was detected, and the tumor, liver, spleen, lung, kidney were sent to histopathological examination. Results The tumor weight in 5-ALA group,Ce6 group, and the combined group had significant difference as compared with control group (P0.05). The dehydration and scab formation and necrosis could be seen in tumor sites at 1 week after PDT. The cell collapse and necrosis, subdermal thrombosis and cell outline clouding could be observed by histopathological examination. Metastasis of melanoma were found in 5-ALA group,Ce6 group, and the combined group. Conclusion PDT with Ce6 and 5-ALA could kill the malignant melanoma effectively in animal experiment but could not affect the metastasis of melanoma.

[Objective]To investigate the technical advantages of micro-CT in the research of three dimensional structure of bone tissues over histological section technology. [Method]Different kinds of bone tissue related specimen were observed by micro-CT and histological section, respectively. The diversity of the sample treatments, result expressions and test data were compared.[Result]Micro-CT showed great advantages in displaying three dimensional structure characteristics of bone tissues. Both quantity and quality changes of bone tissues could be precisely determined by micro-CT which was much better than histological section technology. However, histology showed great advantages in displaying the cell figures, growth and differentiation.[Conclusion]Micro-CT provides a novel test method for both sample treatments and result expressions.Compound with histological technology,micro-CT can display the appearance and structure of bone tissues more sufficiently.
8.Combined fixation for comminuted intra-articular fracture in distal radius
Orthopedic Journal of China 2006;0(08):-
[Objective]To explore the effective treatment method of the articular fracture in distal radius.[Method]Severely comminuted AO type-C3 intra-articular fractures of the distal end of the radius were treated with combined internal and external fixation. The Gartland and Werley system and the Green and O'Brien system were used for comparison of the pre-operative and after operative evaluation.Total articular congruity had been assessed with both clinical rating systems.[Result]At a mean of ninteen months following-up,the mean arc of flexion-extension was 78% of that on the uninjured side and the mean grip strength was 82% of that on the uninjured side. The mean total articular incongruity(the gap plus the step-off)averaged 2 mm,and the radial length was restored to a mean of 11 mm.According to the Gartland and Werley demerit-point system, seventeen of the patients had a good or excellent result.According to the modified Green and O'Brien clinical rating system, eleven had a good or excellent result.Postoperative total articular incongruity had a moderately strong correlation with the outcome as assessed with both clinical rating systems.[Conclusion]Open reduction and combined internal and external fixation of AO type-C3 fractures can restore radiographic parameters to nearly normal values, maintain reduction throughout the period of fracture-healing,and provide satisfactory functional results.
9.Retrospective analysis on 1 124 case-times of renal transplantation
Chinese Journal of Organ Transplantation 1996;0(02):-
Objective To sum up the experience of renal transplantation in order to improve transplant effect and enhance the long-term survival and recovery rate. Methods The data of renal transplantation of 1 124 cases who received renal transplantation from Jul. 1978 to Oct. 2001 were summarized. All the possible factors that could influence the transplant effect such as pre-operative preparation, HLA match, donors' quality, surgical procedure, immunosuppressive therapy and so on were analyzed. Results The 1-, 3-, 5-year survival rate (%) of patient/kidney was 83.1/ 70.3, 74.6/ 68.6 and 62.6/ 56.1 respectively. The rate of AR was decreased significantly from 3.09?% only with LXM to 0.89?% with LXM test in combination with HLA serological test. The rate of HAR and AR in 130 cases with additional PRA test were lower than those only with LXM test. Conclusions The well pre-operative preparation, the ideal HLA match, the high quality of donor's kidney and transplant operation were keys for successful renal transplantation. Reduction of the complications, individual and combined medication in reason and long-term follow-up were very important in the long-term survival time.
