Objective To investigate the VEGF-C on lymph node metastasis of b-reast cancer.Methods A group of immune PV9000 law on human breast infiltrating ductal breast cancer and fibroids,breast disease organizations paraffin sections were detected on the expression of VEGF-C status.Results 20 patients with breast fibroids and breast disease were negative VEGF-C,80 cases of breast cancer VEGF-C positive rate was 72.5%(58/80);VEGF-C exists primarily on the cancer cells and normal breast fibroids and in the cytoplasm of breast disease is not the expression of VEGF-C.Conclusion Breast cancer cells in the VEGF-C,the expression of close ties with lymph node metastasis.Detection of VEGF-C expression in human breast cancer may predict lymph node metastasis,and therapeutic value.

5.Current diagnosis and treatment of male genital lichen sclerosus.

Male genital lichen sclerosus (MGLSc) is a chronically relapsing disease characterized by a long course, gradual aggravation, and a tendency towards malignancy. Once called balanitis xerotica obliterans, MGLSc has a distinct predilection for the prepuce and glans, involving the urethra when aggravating, forming scarring tissues, and causing urethral stricture, which may seriously affect the patients'quality of life with such symptoms as urinary stream narrowing, dysuria, and painful penile erection. The etiology and pathogenesis of MGLSc have not yet been adequately explained though it is generally thought to be associated with autoimmune mechanism, genetic factors, infections, local trauma, and chronic urinary irritation. MGLSc can be fairly easily diagnosed according to its clinical manifestations and histopathological results, but can be hardly cured. Early diagnosis and prompt treatment are the most important approaches, which may relieve its symptoms, check its progression, and prevent its long-term sequelae. Ultrapotent topical corticosteroids are the choice for the treatment of MGLSc. For those who fail to respond to expectant medication or have dysuria due to urethral stricture and painful erection, rational surgery may be resorted to, with importance attached to long-term follow-up. This article presents an update of the diagnosis and treatment of MGLSc and MGLSc-induced urethral stricture.

6.Therapeutic Observation of Fire-needle plus Round-sharp-needle Acupuncture for Primary Trigeminal Neuralgia

Objective To observe the clinical efficacy of fire-needle plus round-sharp-needle acupuncture in treating primary trigeminal neuralgia.Method Sixty-two patients with primary trigeminal neuralgia were randomized into a treatment group and a control group, 31 cases in each group. The treatment group was intervened by fire-needle therapy at the trigger points followed by deep needling with round-sharp needle at Xiaguan (ST7), and the distal points Hegu (LI4) and Neiting (ST44) were also treated. The same acupoints were selected in the control group but treated with ordinary needling method. The clinical efficacies were evaluated after 2 treatment courses.Result The total effective rate was 93.5%(29/31) in the treatment group versus 77.4% (24/31) in the control group, and the difference was statistically significant (P<0.05).Conclusion Fire-needle plus round-sharp-needle acupuncture is an effective approach in treating primary trigeminal neuralgia.
7.Sonablate-500 transrectal high-intensity focused ultrasound (HIFU) for benign prostatic hyperplasia patients.

To evaluate the safety and efficacy of transrectal high-intensity focused ultrasound (HIFU) in the treatment of benign prostatic hyperplasia (BPH), serial studies were conducted in 150 BPH patients before and 30 min, 1, 2, 6 and 12 month(s) after Sonablate-500 HIFU treatment. A silicon-coated indwelling 16F latex catheter was placed during the determination of the therapy zone. Preoperative and postoperative evaluations were made by using the international prostate symptom score (IPSS), quality of life (QOL), uroflowmetric findings and transrectal ultrasound, and incidence of complications. The cystourethrography was done in 23 patients within 1 year postoperatively. The results showed that after HIFU treatment, IPSS and QOL scores were significantly decreased at 1, 2, 6 and 12 month(s) (P<0.01). Maximum urine flow rate (6.0 to 17.2 mL/s, P<0.01), PVR (75.0 to 30.3, P<0.01) and prostatic volume (65.0 to 38.1 mL, P<0.05) were significantly improved 12 months after the operation. Recurrent urinary retention (n=2) and urethrorectal fistula (n=1) occurred at the 15(th) postoperative day. The duration of the HIFU prostate ablation was 25-90 min. The mean time for an indwelling catheter was 3-19 days. These data demonstrate that treatment of BPH with Sonablate-500 HIFU is safe and effective.

8.The establishment and implementation of specialist nursing outpatient clinic for chronic obstructive pulmonary disease

For the purposes of meeting the needs of COPD patients for health education, decreasing times and costs of re-hospitalization, and increasing patient satisfaction and nurses' working enthusiasm. In 2011, our hospital established a specialist nursing clinic for chronic obstructive pulmonary disease (COPD), and two nursesentitled-deputy directorswith rich clinical experiences for more than fifteen years and strong communication skills were se-lected to take charge of the clinic, and they were responsible for nursing assessment, making individualized pul-monary rehabilitation plan, health guidance, long-term follow up, etc., for discharged stable COPD patients and COPD patients with no need for hospitalization. Patient satisfaction was improved from 83% to 96% after implemen-tation.

Objective To review and analyse the CT distinguishing features of pulmonary cryptococcosis (PC)for improving the level of image diagnosis of the disease.Methods CT images and clinical details of 19 patients with biopsy or surgery pathology con-firmed PC were reviewed and analysed.Results Nodules or masses were presented in 10 cases,in which most casescases were nod-ules,lesions ranged in diameter from 1 cm to 2.5 cm.Multiple nodules were easy to fuse.Consolidation were presented in 6 cases,with high attenuation and clear border.3 cases were mixed type,with nodule,mass consolidation and ground glass attenuation.The lesions were mostly located in the inferior lobes(47%)and peripheral lung(68%).The features of pleural thickening,air bronchus sign,halo sign and cavitations were observed in 42%,36%,26%,21% of patients respectively.Conclusion CT findings of PC are diversity.The lesions are mostly located in the inferior and peripheral lung.Lesions are mild to moderate enhanced.Pleural thickening,air bronchus sign,halo sign,cavitations will be useful for the diagnosis.

OBJECTIVEThis study aims to investigate the regulatory effects of calcitonin gene-related peptide (CGRP) on Nod-like receptor protein 3 (NLRP3) and interleukin-1β (IL-1β) to promote osteoblast differentiation.
METHODSDifferent concentrations of CGRP (0, 10, 30, 100 ng · mL⁻¹) were added to mouse osteoblasts in vitro. The mRNA and protein expression levels of both NLRP3 and IL-1β were examined using Real-time polymerase chain reaction and Western blot, respectively. Moreover, the concentrations of IL-1β protein and intracellular reactive oxygen species (ROS) were detected using enzyme-linked immunosorbent assay and flow cytometry, respectively. The osteogenic differentiation of mouse osteoblasts was identified through alizarin red staining.
RESULTSThe protein and mRNA expression levels of both NLRP3 and IL-1β significantly decreased (P < 0.05) with increasing CGRP concentration. Moreover, the contents of intracellular ROS gradually decreased (P<0.05). The osteogenic differentiation of the osteoblasts was more enhanced in the group treated with 100 ng · mL⁻¹ CGRP than in the empty group (0 ng · mL⁻¹ CGRP).
CONCLUSIONCGRP promotes osteoblast differentiation by inhibiting the expression of inflammatory factors.
