3.Usefulness of Endoscopic Ultrasound in Esophagogastric Varices.

Endoscopic ultrasound (EUS) is a useful diagnostic tool for evaluation of esophagogastric varices and guidance of endoscopic therapy. EUS can visualize not only collateral veins around the esophagus but also perforating veins that connect esophageal varices with collateral veins. They are associated with high risk of bleeding and early recurrence after initial variceal eradication. Isolated gastric varices can be easily diagnosed using EUS that mimic thickened gastric folds or subepithelial tumors. EUS-guided endoscopic therapy is a challenging field of variceal bleeding. It has a potential role for assistance of interventions and evaluation of treatment outcome.

4.Current Status of Research on Liver Cancer in Korea.

Liver cancer is one of the leading causes of cancer-related death in Korea. Liver cancer imposes a considerable societal burden due to its high incidence and high mortality rate in younger patients, as compared to other cancers. However, interest in liver cancer among researchers and health-policy makers is low. In this review, recent trends in the number of published articles on liver cancer in Korea and internationally were analyzed. The key finding is that the rate of growth in the number of published articles on liver cancer is slowly decreasing and financial investment for research into liver cancer is very limited, despite the increasing research and development investment budget in Korea. Meanwhile, the rate of growth of research into liver cancer in China has recently increased markedly. Therefore, the scale and rate of growth of research into liver cancer in Korea should be enhanced.

6.A Case of Digital Mucous Cyst Treated by Minocycline Sclerotherapy.

Digital mucous cysts are observed frequently on the lateral or dorsal aspect of the digital fingers. Digital mucous cysts are prone to recur after simple excision, electrocoagulation, intralesional injection of corticosteroid and a variety of other therapeutic maneuvers. In this report, we describe one patient with digital mucous cyst who was successfully treated with intralesional injection of 1 mg/ml of minocycline hydrochloride without any sequelae.

7.Clinical observation of acute drug intoxications.

Clinical observations were made on 349 cases of acute drug intoxication who were visited to emergency room of Yeungnam University Hospital during recent 7 years from January 1984 to December 1990. The following results were obtained. 1) Total number of cases of acute drug intoxication was 349 which was 0.39% of the total patients of the emergency room during the same period. 2) The ratio of male to female was 1.1:1. The age incidence was highest in the third decade (26.7%). The monthly incidence was highest in May. Higher frequency was observed in summer season. 3) The most common drug of the intoxication was pesticides and herbicides (71.9%), the remainders were miscellaneous drugs (11.2%), sedatives (7.7%), rodenticides (6.3%) and unknown drug (3.2%) in orders. 4) The most common cause of drug intoxication was suicide (69.1%) and the others were accident, unknown cause, intention in orders. 5) Main clinical manifestations were the impairment of consciousness, nausea, vomiting and convulsion. Physical examination revealed increased pulses, increased blood pressure, miosis of the pupil and sweating. Above symptoms and signs were more prominent in pesticide intoxication. Leukocytosis, glycosuria and abnormal LFT were common findings in acute intoxications. 6) The complications were developed in 18.3% among 349 cases and the most common complication was respiratory failure, pneumonia, cardiovascular collapse and pulmonary edema in orders. 7) Overall mortality rate was 8.3% of total cases and mortality rate was highest in herbicide intoxication (22.2%).

8.Novel Compound Heterozygous Mutations in the Vitamin D Receptor Gene in a Korean Girl with Hereditary Vitamin D Resistant Rickets.

Hereditary vitamin D resistant rickets (HVDRR) is a rare genetic disorder caused by a mutation of vitamin D receptor (VDR) gene. A number of cases had been reported in many countries but not in Korea. We examined a three-year old Korean girl who had the typical clinical features of HVDRR including rickets, hypocalcemia, hypophosphatemia, elevated serum calcitriol level and secondary hyperparathyroidism. The girl and her father were both heterozygous for the 719 C-to-T (I146T) mutation in exon 4, whereas she and her mother were both heterozygous for 754 C-to-T (R154C) mutation in exon 5 of the VDR gene. In this familial study, we concluded that the girl had compound heterozygous mutations in her VDR gene which caused HVDRR. This is the first report of a unique mutation in the VDR gene in Korea.

9.A Clinical Study of Ifosfamide/Mesna and Cisplatin in Recurrent Epithelial Ovarian Cancer.

This study was planned to evaluate the efficacy and toxicity of fosfamide/mesna plus cisplatin combined regimen in patients with recurrent and initial therapy-resistant epithelial ovarian cancer. Controversy still remains over the efficacy of cisplatin containing combined chemotherapy as the treatment of recurrent ovarian cancer and refractory to first line chemotherapy. Ifosfamide(isophosphamide) is an analog of cyclophosphamide. It has shown evidences of activity in ovarian cancer and lack of cross-resistance with cyclophosphamide. This study was undertaken for the clinical analysis and evaluation on 16 patients, who were admitted and treated with advanced or recurrent ovarian cancer at the Sooncheunhyang University Chunan Hospital from January 1994 to July 1996. The results were as follows: 1. The mean age of patients was 62 year old with range from 44 to 72. 2. The most frequent side effect was nausea and vomiting(75%), followed by leukocytopenia(68%), alopecia(44%), nephrotoxicity(37%), neurotoxicity(25%), and hepatotoxicity(12%). 3. The mean survival period was 24 months in clinical response cases ranged from 11 months to 36 months, 5 months in progressive cases ranged from 1 month to 14 months. 4. In clinical stages, stage II was 6 cases(37.5%), III was 6 cases(37.5%) and IV was 4 cases(25%). 5. Previously all patients were received debulking surgery and PAC hemotherapy. 6. Overall respons rate was 44%(7/16).

10.A Case of Candida Glabrata Infection after Total Knee Arthroplasty.

Candida infection after total knee arthroplasty is very rare. The diagnosis is difficult due to lack of obvious clinical symptoms. Candida albicans is the most common strain in Candida infections after total knee arthroplasties and Candida glabrata has been reported in only 5 cases in the literature. We report here on a case of Candida glabrata infection after total knee arthroplasty, and this was treated by two-stage revision and using an antibiotics-loaded cement spacer.
