1.Expression of type IV collagen and laminin genes by skin fibroblasts in culture.

Basement membrane zone gene expression by fibroblast cultures was examined by molecular hybridizations with human sequence specific cDNAs corresponding to type 1V procollagen and laminin subunit polypeptides. Northern transfer analysis with total RNA revealed the presence of specific mRNA transcripts for al (IV) and a2 (1V) chains of type 1V procollagen as well as Bl and B2 chains of laminin. Laminin A chain mRNAs were not detected using the same RNA preparations. The molecular size of al (1V) and a2 (1V) procollagen mRNA revealed 6.8kb and 6.7kb, respectively. The molecular size of Bl chain of laminin revealed 5.6kb, and B2 chain revealed 8.2 and 5.5kb polymorphic transcripts. In slotblot analysis using densitometer, steady-state levels of type IV procollagen and laminin mRNAs indicated that they were in relatively low abunclance, as compaired with type I procollagen mRNA. Quantitative levels of al (IV) and laminin Bl chaii mRNAs were more abundant than those of a2 (IV) and laminin B2 mRNAs. The mRNA ratio of al (IV)/a2 (lV) and laminin Bl/B2 were 1.9 and 1.5, respectively. These results demonstrate evidence for differential regulation of the expression of different basement membrane zone molecules during the formation of basement membrane.

4.Increased expression of the epidermal growth factor receptor gene in psoriasis.

Psoriasis is characterized by disregulation of keratinocyte growth with profound epidermal hyperplasia. Keratinocyte hyperplasia in psoriasis may be expained in part by overproduction of growth factor, and by altered metabolism of the epidemal growth factor receptors (EGFR) in affected skin. The expression of epidermal growth f ictor receptor was investigated by Northern blot and slot-blot analysis of total RNA extrated from biopsies of normal skin and psoriatic lesions. In Northern blot analysis, EGFR-specific mRNA transcripts from psoriatic tissues demonstrated the specificity of hybridizarion with a EGFR mDNA probe. The size of EGFR mRNA transcript was 6.7kb in psoriasis lesions which showed no change of quality. In slot-blot analysis, the levels of EGFR mRNA in poriasis revealed a 1.2 fold to 4.1 fold elevation when compared to normal skin. EGFR were present in all epidermal layers by immunoperoxidase staining, whereas in normal skin they were primarily present in the stratum basalis. These results indicate that the increased expresion of the EGFR gene may be, in part, responsible for the hyperproliferation of the epider nis and that retained EGFR may reflect incomplet; abnormal differentiation in active porasis. This altered process of EGFR metabolism may be involved in the pathogenesis of psoriasis.

5.Delayed Radionecrosis of the Brain: A case simulating recurrent glioma.

An appreciation of the radiation changes in the brain adjacent to radiated glioma is of considerable importance from both clinical and pathological standpoint because the alteration in the brain parenchyma may result in a picture that mimics recurrence of the neoplasm. A 35 years old woman was admitted because of deterioration of consciousness which has started about 20 days ago. Past history revealed removal of left frontal brain tumor 4 years earlier and the diagnosis was grade II astroma. Postoperative radiation was done at that time. Sine then, she has been doing well. Computerized tomogram showed an ill defined huge low density mass at right frontal and left posterior parietal areas, which was irregularly enhanced in postcontrast study. Unlikely to the usual brain tumors, there was no mass effect on adjacent structures. Histologically acellular necrotic zone was alternating with hypercellular zone and most striking changes were vascular and glial reaction. Most of the vessels showed acellular hyliane thickening of their wall and some vessels were almost completely occluded. There was marked glial proliferation with considerable cellular and nuclear irregularities. Gemistocytic astrocytes were frequently seen and some were multinucleated.

10.Clinical Evaluation of Spinal Tuberculosis Treated with Hemilaminectomy.

OBJECTIVE: The variable operative methods are underwent as a treatment of tuberculous spondylitis. We propose hemilaminectomy and debridement as a one of operative method at a certain circumstance. METHODS: From July 1998 to June 2002, 13 consecutive patients with spinal tuberculosis were treated surgically in our institution. Among them, the authors analysed 7 patients in whom posterior approach were performed. The lumbar spine was involved in 6 patients, the thoracic in 1. The hemilaminectomy with debridement was done in all cases. The changes in the Kyphotic angle and the height of involved vertebras retrospectively measured from lateral spinal radiographs obtained preoperatively and postoperatively. Mean follow up periods were 16 months. RESULTS: Clinical symptoms and signs were improved in all cases. One patient (14%) was needed anterior fusion because of aggravation of lesion. Another one needed reoperation because of relapse of epidural abscess. The arithmethical average of kyphotic angle was worsened about 2 degrees and that of height loss was lesser than 5% postoperatively. CONCLUSION: The hemilaminectomy with debridement for spinal spondylitis can be a first therapeutic modality in a mild neurologic deficit and minor lesions with extended epidural abscess and granulation tissue to the adjacent vertebras in radiologic finding. Especially if epidural abscess and granulation tissue involve the multiple vertebras, we recommend this operative method.
