2.A Case of Iris Metastasis from Non-small Cell Lung Cancer.

PURPOSE: Metastatic carcinoma of the eye is a common sequela of widespread dissemination of malignancy. Metastasis to the iris is less frequent than that of the cilliary body and choroid. We report a case of iris metastasis in a patient with non-small cell lung cancer. METHODS: 57-Year-Old man was seen in the department of ophthalmology because of blurred vision in his right eye for seven days. The patient had a metastasis of non-small cell lung cancer in the brain. Trabeculectomy and iridectomy were performed due to secondary glaucoma. RESULTS: The case was diagnosed as iris metastasis of non-small cell lung cancer by the clinical and histopathologic findings.

3.Correlation between Foveal Thickness and Visual Acuity in Unilateral Resolved Central Serous Chorioretinopathy.

PURPOSE: To evaluate the correlation between foveal thickness, as measured with optical coherence tomographic evaluation, and the visual acuity in patients who suffered from unilateral resolved central serous chorioretinopathy. METHODS: Twenty-eight consecutive patients who were diagnosed with unilateral resolved central serous chorioretinopathy were studied. High-resolution optical coherence tomography was performed on all of the patients. The foveal thickness of the involved eye was normalized by dividing its thickness with that of the uninvolved eye. The best-corrected visual acuity of the involved eye was normalized as well. The subjects were divided into two groups according to the minimum value of the normalized foveal thickness in the 22 healthy subjects. RESULTS: The mean foveal thickness was 128.68+/-22.06 micrometer in the involved eyes and 148.18+/-13.88 micrometer in the uninvolved eyes. There was a statistically significant correlation between the normalized foveal thickness and the normalized visual acuity. There were statistically significant differences in the normalized visual acuity and the duration of the symptoms between the group with foveal thickness above the minimum value and the group with foveal thickness below the minimum value. CONCLUSIONS: There was a statistically significant correlation between foveal thickness and visual acuity. The patient group with a foveal thickness value above the minimum value of the normalized foveal thickness showed a statistically significant improvement in visual acuity and a shorter duration of symptoms. It seems that the foveal thickness is associated with visual acuity and the duration of symptoms.

4.2-Year Results of MemoryLens Hydrophilic Acrylic Intraocular Lenses after Cataract Surgery.

PURPOSE: To evaluate the clinical results of thermoplastic, hydrophilic acrylic IOL CV232 (MemoryLens(R), CIVA Vision, USA) to those of hydrophobic acrylic IOL AR40e (Sensar(R), AMO, USA). METHODS: Twenty patients had in-the-bag implantation of a MemoryLens(R) IOL in 1 eye and a Sensar(R) in the opposite eye in a randomized fashion after uneventful phacoemulsification. We evaluated the best corrected visual acuity (BCVA) and posterior capsule opacification (PCO) at a mean 30.05 months after surgery. RESULTS: Two years postoperatively, a BCVA of 0.8 or better was found in 65% of the MemoryLens(R) group and in 60% of the Sensar(R) group. There was no statistically significant differences between the two groups (p>0.50). PCO developed in 55% in the MemoryLens(R) group and in 65% of the Sensar(R) group. Again, no statistically significant differences were found between the two groups (p>0.50). CONCLUSIONS: There were no significant differences between the two different intraocular lenses in BCVA or PCO postoperatively.

5.Amplification of the UQCRFS1 Gene in Gastric Cancers.

PURPOSE: The specific aim of this study is to unravel a DNA copy number alterations, and to search for novel genes that are associated with the development of Korean gastric cancer. MATERIALS AND METHODS: We investigated a DNA copy number changes in 23 gastric adenocarcinomas by array-comparative genomic hybridization and quantitative real-time polymerase chain reaction analyses. Besides, the expression of UQCRFS1, which shows amplification in array-CGH, was examined in 186 gastric cancer tissues by an immunohistochemistry, and in 9 gastric cancer cell lines, as well as 24 gastric cancer tissues by immunoblotting. RESULTS: We found common gains at 48 different loci, and a common loss at 19 different loci. Amplification of UQCRFS1 gene at 19q12 was found in 5 (21.7%) of the 23 gastric cancers in an array-comparative genomic hybridization and DNA copy number were increased in 5 (20.0%) out of the 25 gastric cancer in quantitative real-time polymerase chain reaction. In immunohistochemistry, the overexpression of the protein was detected in 105 (56.5%) out of the 186 gastric cancer tissues. Statistically, there was no significant relationship between the overexpression of UQCRFS1 and clinicopathologic parameters (P>0.05). In parallel, the overexpression of UQCRFS1 protein was confirmed in 6 (66.7%) of the 9 gastric cancer cell lines, and 12 (50.0%) of the 24 gastric cancer tissues by immunoblotting. CONCLUSIONS: These results suggest that the overexpression of UQCRFS1 gene may contribute to the development and/or progression of gastric cancer, and further supported that mitochondrial change may serve as a potential cancer biomarker.

6.Early Onset Renal Failure in Congenital Nephrotic Syndrome associated with Congenital Diaphragmatic Hernia by WT1 Gene Mutation.

We experienced a female neonate with congenital nephrotic syndrome (CNS) associated with congenital diaphragmatic hernia (CDH). Because of the rare combination of two conditions, we report this case with literature review. CDH was found immediately after birth and emergency operation was done for hernia repair. But on the next day, generalized edema and oliguria(0.59 mL/kg/hour) was found and her blood chemistry showed hypoalbuminemia (1.6 g/dL), increased BUN (27.7 mg/dL) and serum creatinine( 1.8 mg/dL) along with heavy proteinuria (4+). We started albumin infusion with a bolus of intravenous furosemide. We suspected the neonate had congenital nephrotic syndrome and her 24hr urine protein was 1,816 mg/day. In spite of immunosuppressive therapy, the nephrotic syndrome and renal failure progressed. We started peritoneal dialysis on the day of life 22 but it was not satisfactory. She was complicated by intracranial hemorrhage and multi-organ failure and expired at 34 days of age. Kidney necropsy was performed which showed diffuse mesangial sclerosis (DMS). Her chromosome study revealed 46, XX and her gene study revealed a heterozygous missense mutation, Arg366His, in Wilms tumor suppressor gene (WT1). This case deserves attention on account of the 4th case of CNS with CDH revealing the Arg366His mutation in the WT1 gene andG the 1st case of early onset renal failure without male pseudohermaphroditism and Wilms tumor with CNS, CDH and the Arg366His mutation in the WT1 gene. So, this report gives support to the hypothesis that Arg366His mutation in the WT1 gene can result in CNS and CDH.

The gastric polyps may be premalignant lesions and they should be removed as possible. The gastric polyps can be remoued by various endoscopic methods. The safety and therapeutic effectiveness of snare polypectomy, neodymium YAG laser therapy and electrocutery were compared in a total number of 106 patients who were diagnosed as benign polyps by means of endoscopic biopsy.Safety was evaluated by the rate of complications such as perforation and bleeding, and therapeutic effectiveness was evaluated by the average numbers of treatment session of each modality. Average numbers of treatment session of snare polypectomy, laser therapy and electrocautery were 1.04, 1.38 and l.44, respectively. Average numbers of treatment session of snare polypectomy were significantly (p<0.05) lower than those of other two methods. Perforation was not occurred in any group. Bleeding rates after snare polypectomy, laser therapy and electrocautery were 17.4%, 1.0% and 3.8%, respectively. Bleeding was significantly (p<0,05) more frequent after snare polypectomy than after other two methods. In conclusion therapeutic effectiveness was best in snare polypectomy but complication was most frequent after snare polypectomy. There was no difference between laser therapy and electrocautery for therapeutic effectiveness and safety. Therfore, laser therapy and electrocautery caa be used for the removal of flat adenoma, alternatively.

10.Balloon Dilation Angioplasty of Aortic Coarctation in Adult.

A 21-year-old woman found to be hypertensive was referred for hypertension. On examination, blood pressure was 170/110mmHg in the right arm, 160/100mmHg in left arm,and 120/70mmHg in legs. A grade 2/6 systolic ejection murmur was present at the left upper sternal border, and a chest x-ray revealed a rib notching on the inferior margin of 4th rib. Two-dimensional echocardiogram showed the coarctation of aorta beyound the origin of the left subclavian artery. Biplane TEE demonstrated a discrete narrowing of the descending aorta at the site of coarctation. The blood pressure was 169/86mmHg in ascending aorta and 118/84mmHg in descending aorta. Aortogram showed a localized coarcted aortic segment of 7mm in diameter and 5mm long just distal to the left subclavian artery. Balloon coarctation angioplasty was performed with 7F 30x15mm pediatric balloon dilatation catheter. Balloon position was confirmed on fluoroscopy by the hourglass appearance of the balloon inflation and thereafter, the balloon was inflated until the waist of the balloon disappeared. After procedure, a pull back pressure tracing across the coarctation of aorta revealed no pressure gradient between ascending and descending aorta with 141/90mmHg. Aortogram showed an increase in diameter of the coarctation of aorta to 18mm with aneurysmal formation. 3 months later, follow up aortogram showed no significant change in diameter of coarctation of aorts or aneurysmal formation. Nonsurgical balloon coarctation angioplasty appears to be an alternative therapy for the coarctation of aorta in adults.
