2.Rabbit Corneal Endothelial Cell Damage by Q-switched Nd:YAG laser.

Corneal endothelial damages following Nd: YAG laser application were evaluated rabbits. The first group underwent anterior capsulotomy, while the second group received laser applications at the corneal center and anterior capsule. The rabbits were evaluated with the intraocular pressure, the corneal endothelial cell number, the thickness of the corneal center, and the morphologic change of the cornea, before laser application and at postaplication 1 day, 7 day, and 14 day. But the endothelium was damaged when laser shots were applied at the cornea center and anterior caps ulotomy. In ophthalmologic procedure with Q-switched Nd:YAG laser, the minimal effective poser should be selected, the laser beam should be applicated on extremely fine focus and contact lens should be used to minimize the corneal endothelial injury.

3.The Effect of Learning Organization Construction and Learning Orientation on Organizational Effectiveness among Hospital Nurses.

PURPOSE: This study conducted to identify the effect of learning organization construction and learning orientation on organizational effectiveness among hospital nurses. Method: Data was collected from convenient sample of 296 nurses who worked for the major hospitals in Seoul, Gyeonggi-do and Gangwoen-do. The self-reported questionnaire was used to assess the general characteristics, the level of the learning organization construction, learning orientation and organizational effectiveness. The data were analyzed using descriptive statistics, pearson's correlation coefficient and multiple regression. Result: The mean score of learning organization construction was 3.61(+/-.32), learning orientation got 3.26(+/-.39), and organizational effectiveness obtained 3.38(+/-.42). The learning organization construction affects of organizational effectiveness by 44.18% and learning orientation by 37.43%. CONCLUSION: This finding indicates that learning organization construction and learning orientation affects the nurses' organizational effectiveness in hospital.

4.Nasolacrimal Duct Reconstruction with Pyrex Glass Tube.

From March, 1988 to August, 1989, the authors performed nasolacrimal duct reconstruction with Pyrex glass tube without bony perforation in 26 eyes of 25 patients who has chronic dacryocystitis with nasolacrinal duct obstruction. This method is implantation of permanent indwelling pyrex glasstube from base of the sac through the interosseous canal into the vault of inferior nasal meatus and there are several advantages in this method; The technique is more simple and less traumatic than other dacryocystorhinostomy, tear drainage inferioly prevents pooling of tear in inferior portion of lacrimal sac after surgery, normal anatomic relationships are retained and less postoperative care is required. The results were as follows: 1. Success was achieved in 24 of 26 cases including partial success. 2. Postoperative complications were developed that hypertrophic scar 2 cases, superficial punctate keratitis 1 case, wound infection 1 case, nasal bleeding 1 case.

5.Secondary Intraocular Lens Implantation in Aphakia.

The secondary implantation has gradually increased year by year. The authors conducted a study on eighty-two patients(92 eyes) recieving secondary intraocular lens implantation from Jan 1989 to Jan 1993. Sulcus fixation of posterior chamber IOL implantation was done in cases of intact or small posterior capsule rupture(56 cases). Anterior chamber intraocular lens(8 cases) and posterior chamber intraocular lens implantation by scleral fixation(28 cases) was done in eyes with large posterior capsule rupture or no zonular support. Final postoperative visual acuity of 0.5 or better was acquired in 77 eyes(83.7%) and didn't show statistically significant difference in three groups. The postoperative complications induced IOL decent ration(9.8%), uveitis(6.7%), cystoid macular edema(3.3%), vitreous hemorrhage(3.3%), and retinal detachment(3.3%). The causes of postoperative decreased visual acuity were cystoid macular edema(2.2%), fibrous membrane on IOL(2.2%), IOL decentration(1.1%) and IOL dislocation(1.1%), No statistically significant difference was noted in postoperative central corneal endothelial cell loss in three groups.

6.Expression of Dopamine D1 and D2 Receptor mRNAs in the Fetal Rat Retina.

In this study, the ontogeny of the dopamine D1 and D2 receptors on fetal rat retina was investigated by using in situ hybridization technique. The expression of the D1 and D2 receptor mRNAs showed different pattern of appearance, distribution and density. At gestational day(GD) 13.5, D1 receptor was first detected in neuroepithelium of the retina. Both D1 and D2 receptors were detected at GD 15.5 in the ventricular layer and ganglion cell layer of developing retina of the rat, and D1 receptor showed more strong density than that of D2 receptor. At GD 17.5, Both D1 and D2 receptors were detected in the ganglion cell layer, ventricular layer and pigment epithelium. The labelling density of D1 receptor was higher than that of D2 receptor. At GD 19.5, both D1 and D2 receptors were additionally detected in the optic nuclear layer and plexiform layer. These results imply that D1 and D2 receptor may mediate different important function of the retina.

With the increasing number of air bag-equipped vehicles, there has been a corresponding increase in the incidence of air bag-associated ocular trauma. We encountered a 38 years old man who sustained corneal damage, hyphema, angle recession, medial wall fracture, and visual damage by choroidal rupture of macula due to an inflated air bag in motor vehicle accident and brief review of related literature is presented.

Malignant mixed mullerian tumor (MMMT) of the uterus are uncommon tumor and highly malignant containing epithelial and stromal components which is classified as homologous or heterologous based on the nature of sarcomatous component. It is highly malignant and the prognosis is poor due to frequent metastasis and recurrence, which most patients were associated with postmenopausal status. We experienced one case of malignant mixed mullerian tumor of uterus with neuroendocrine differenciation. So we report this case with a brief review of literature.

9.Evaluation of Anxiety and Depressive Levels in Tinnitus Patients.

BACKGROUND AND OBJECTIVES: The aims of this study were to evaluate the relationship between tinnitus and the level of anxiety and depression experienced by subjective tinnitus patients, and to determine the effect of the level of anxiety and depression to the results of tinnitus treatment. SUBJECTS AND METHODS: A total of 104 patients were included in this study. All the patients conducted Tinnitus Handicap Inventory (THI), Beck Depression Inventory (BDI), State Trait Anxiety Inventory 1, 2 (STAI 1, 2) to evaluate their psychometric properties. The subjective patterns of tinnitus before and after treatment were assessed through questionnaires. RESULTS: Significant correlation between THI scores and BDI, STAI 1, 2 scores were observed in the moderate and severe tinnitus group. The patients were classified into two groups depending on their THI scores, and also classified into two groups according to BDI, STAI 1, 2 results. Analysis of differences after treatment indicated significantly improved VAS scores after treatment in all groups. CONCLUSIONS: The subjective level of tinnitus is significantly related with depression and anxiety symptoms when inconvenience caused by tinnitus was moderate or over. Assessments on tinnitus after treatment appeared to have improved significantly regardless of the severity of tinnitus, level of depression or anxiety disorder.

10.Chemomyectomy Effect of Doxorubicin and Verapamil on Orbicularis Oculi in Rabbit.

Doxorubicin has characteristic chemomyectomy effect of the eyelid without disturbing other eyelid structures, but the major side effect of doxorubicin is the potential for eyelid skin injury as a result of the drug's toxicity in both animal and clinical studies. Verapamil may be used to reduce the dose of doxorubicin and the number of injections that would amplify the toxic effects of doxorubicin. This study was performed to determine whether there is an increase in the toxic effect of the doxorubicin as a result of verapamil pretreatment of the muscle. After 0.5mg, 1.0mg, and 2.0mg doxorubicin was injected in lower eyelids of each group, and equal dose of doxorubicin was injected fo11owing 1.0mg of verapamil injection in lower eyelid of each group, muscle cell loss were measured by light microscopy and side effect was observed. In verapamil and doxorubicin injection group, there was significant differences in the amount of preseptal muscle and even in the pretarsal muscle than the doxorubicin injection group in all doxorubicin doses. Verapamil, injected with a range of doses of doxorubicin, caused suhstantia11y increased muscle loss in the eyelid, compared with injection of doxorubicin alone. Skin ulceration, entropion or ectropion were not visible. Clinically, verapamil cotreatment might be useful to decrease the dose of doxorubicin injected and/or the total number of injections.
