1.Establishment and application evaluation of rapid detection method of Mycoplasma pneuomoniae

Objective To prepare the specific monoclonal antibodies(mAb) of Mycoplasma pneumonia(MP) and to establish a colloidal gold rapid detection method of MP by using mAb .Methods The nasopharyngeal swab samples were collected from chil‐dren patients with acute respiratory tract infection ,separated and cultured and performed the MP identification ,MP antigen was pu‐rified ,mAb of MP was prepared ,then the MP colloidal gold test strip was established by using MP mAb .The throat swab sample was collected from the patients with suspected MP infection and detected by using the fluorescent quantitative PCR and colloidal gold test strip .The detection results were observed and statistically analyzed .Results MP strain was successfully isolated and inac‐tivated ,the MP antigen was purified .The mouse was immunized by using this antigen ,19 strians of mAb against MP were prepared by using the hybridoma technique .Among them ,2 strains of mAb with high titer and good specificity(MP‐5 and MP‐19) were se‐lected as the raw materials for preparing the MP colloidal gold rapid test strip .The lowest detection limit was 20 ng .The clinical samples were detected by using the MP colloidal gold rapid test strip and fluorescent quantitative PCR .The results showed that the sensitivity of MP colloidal gold rapid test strip by using this established method was 88 .2% and its specificity was 82 .6% .Conclu‐sion specific mAbs against MP is prepared and the colloidal gold rapid detection method is preliminarily established which provides the help for rapid diagnosis in the patients with MP infection .

Neuronal nicotinic acetylcholine receptors(nAChRs) are widely distributed in the central nervous system,and they participate in complex neuronal signal transduction and regulation in the brain.The high Ca~(2+) permeability of nAChRs,plays a pivotal role in shaping nAChRs-induced neuromodulatory effects.The nAChRs-mediated Ca~(2+) signals may regulate diverse neuronal activities such as the improvement of learning and memory,and are closely related to drug addiction or neuroprotection mechanism.
3.Effect of IMA,Organic Tin Heat Stabilizer,on the Level of Sex Hormones in Rats

Objective To understand whether Imarcaptoacetate dioctyltin(IMA) is a environmental endocrine disruptor or not. Methods The new born Wistar rats were randomly divided into six groups including positive control group, negative control group and four experimental groups. The rats in the experimental groups were given IMA at different concentrations (0.100 0, 0.010 0, 0.001 0, 0.000 1 ?g/10 g) 1/1 000 000 by hypodermic injection, the rats in the negative control group were treated with corn oil and those in the positive control group were treated with dibutyltin dilaurate(DBTD) at concentration of 0.001 ?g/10 g bw. The rats were injected one time each day for five consecutive days. After one and half months, the levels of T, LH and FSH in the blood, the weight of the testicles, ovaries and the sexual organ exponent were determined. The pathological examination on some samples was conducted. Results In both of female and male, the levels of T in IMA treated rats were significantly increased and showed an doge-effect relationship (P0.05). Objective To identify the isolated rat bone marrow derived mesenchymal stem cells(MSCs),and evaluate the efficiency of adenoviral vector expressing human urokinase type plasminogen activator(uPA) in transfection of rat MSCs and its effect on proliferation of MSCs. Methods MSCs were isolated and purified by pasted wall purification,and were identified by immunicytochemistry.The transfection efficiency of uPA was detected by fluorescent microscopy,the expression of uPA in MSCs was detected by Western blotting,and the proliferation of MSCs was evaluated by MTT. Results The harvested MSCs exhibited the typical appearance of MSCs,and it was revealed by immunohistochemistry that the expression of MSCs markers CD29 and CD90 was positive,while that of CD34 and CD45 was negative.A tendency of increase in expression of green fluorescent protein(GFP) was observed with increase of multiplicity of infection(MOI).After transfection with AduPA for 72 h,the transfection efficiency reached(94.0?1.5)% at MOI of 80,and positive GFP cells could still be observed even after 7 d.The transfected uPA had no effect on the proliferation of MSCs. Conclusion MSCs are favourable genetic vectors to express uPA,and can be used for treatment of liver fibrosis.
5.Expressions of C-Met and HGF in tongue squamous cell carcinoma and their clinical significance

ABSTRACT:Objective To study the expressions of C-Met and hepatocyte growth factor (HGF)in tongue squamous cell carcinoma and their clinical significance.Methods We analyzed 46 patients (the treatment group) with tongue squamous carcinoma and 27 patients (the control group)with benign tumor treated in our hospital between June 201 1 and May 2013.The expressions of C-Met and HGF were detected by immunohistochemistry. Results In the experimental group,C-Met negative expression rate was 21.7% (10/46),weak positive expression rate was 26.1% (12/46),and strong positive expression rate was 52.2% (24/46).The above expression rates were 77.8% (21/27),1 5.8% (5/27),and 3.7% (1/27)in the control group.The two groups differed significantly (P <0.001).In the experiment group,the negative expression of HGF protein was 1 7.4% (8/46 ),weak positive expression rate was 58.7% (27/46),and strong positive expression rate was 23.9% (1 1/46).The above expression rates in the control group was 63.0% (1 7/27 ),37.0% (10/27 ),and 0.0% (0/27 ).The two groups differed significantly (P <0.001).The expression of C-Met protein was significantly related to lymph node metastasis (P <0.05).The expression of HGF protein was significantly related to pathological grading and lymph node metastasis (P <0.05).Conclusion The expressions of C-Met and HGF in tongue squamous cell carcinoma are significantly higher.And the occurrence and development of tongue squamous cell carcinoma are closely related to the expressions of C-Met and HGF.
6.Effects of recombinant human bone morphogenetic protein-2 combined with pedicle screw fixation via dorsal-approach and vertebral body bone graft in treating type A thoracolumbar vertebral fractures

Objective To observe the effects of recombinant human bone morphogenetic protein-2 (rhBMP-2) on patients with type A thoracolumbar fractures treated by pedicle screw fixation via dorsal-approach and vertebral body bone graft.Methods A retrospective case-control study was conducted to analyze the clinical data of 64 patients with type A thoracolumbar fractures treated from June 2012 to July 2015.The patients were divided into control group (32 cases) and research group (32 cases) according to the random number table.There were 22 males and 10 females aged (48.2 ± 11.2)years old in control group.The injury was located at T11 in 2 cases,T12 in 4,L1 in 9,L2 in 7,L3 in 7 and L4 in 3 in control group.There were 24 males and 8 females aged (50.7 ± 11.4) years old in research group.The injury was located at T11in 2 cases,T12in 4,L1in 8,L2 in 8,L3 in 7 and L4 in 3.The control group was treated with posterior pedicle screw reduction,internal fixation and allograft cancellous bone grafting through pedicle of vertebral arch.The research group was treated with rhBMP-2 (mixed with cancellous bone implants) on the basis of the control group.The time of operation,the amount of bleeding during operation,the volume of postoperative bleeding,the amount of postoperative drainage,the recovery of the injured vertebra,the Lane-Sandhu score,the time of fracture healing,and the postoperative complications were compared.Results There was no difference between two groups on operation time,amount of bleeding during operation or amount of postoperative drainage (P > 0.05).No difference existed in two groups about Cobb angle and injured vertebral leading edge height at one week after operation (P > 0.05).At the time of last follow-up (12 months),in research group,Cobb angle was lower while injured vertebral leading edge height was much higher than that of control group (P < 0.05),both of which were better than before (P < 0.05).At 12 months after surgery,the lost Cobb angle of [(2.0 ± 0.7) °] and lost height of injured vertebral leading edge [(3.2 ± 1.0) %] were smaller than those in control group [(5.6 ± 1.7) ° and (6.8 ± 2.4) % respectively] (P < 0.05).The Lane-Sandhu score and healing time of fracture in research group were (8.9 ± 0.8) points and (6.9 ± 0.9) months,which were better than that (6.8 ±0.8)points and (8.4 ± 1.6)months in control group respectively (P<0.05).The complication rate of research group was lower than that of control group,with no significant difference (P > 0.05).Conclusion Compared with simple vertebral bone graft,posterior thoracolumbar vertebrae pedicle screw fixation with vertebral bone graft and hBMP-2 treatment for type A thoracolumbar fractures can restore vertebral stability,shorten the time of fracture healing and reduce the incidence of complications.

Objective To investigate the life quality and influencing factorin stroke patient.MethodTotally 205 hospi-talized stroke patientin the neurology departmenwere investigated with SF-36 scale .The datwere performed the descriptive sta-tistic,t-tesand multiple linearegression analysi.ResultThe scoreof general healthy statu,physiological function ,physiolog-ical role ,somatipain and social function in the patientwith stroke were significantly lowethan thain the national norm (P<0 . 01) .The recurrence frequency ,age ,comorbidity ,education level and income were significanfactoraffecting the life quality in the stroke patien.Conclusion The level of general health statu,physiological function ,physiological role ,somatipain and social function are decreased significantly .Therefore iirecommended to perform the pertinenintervention in the clinical work .
8.Prospective randomized controlled trial of bilingual education in surgery

Objective To explore the feasibility of "immersion program" in French-taught surgical lessons,as to provide multiple educational methods and practical experiences for the application of bilingual education in clinical medicine.Methods Twenty-nine senior students of French-taught class were randomly divided into group A(n=15) and group B(n=14)."Immersion program" and "transitional bilingual education" were employed for group A and group B,respectively for the first half of teaching session,and "transitional bilingual education" and "immersion program" for the second half,respectively.The differences between the two bilingual education models were compared through quiz.Results In the prior 2 of the 4 quiz,the scores of French quiz and the total scores were much higher in "immersion program" group,and there were significant differences between the two groups(P0.05). Conclusion "Immersion program" helps to improve the ability of presentation,comprehension and application of French in the precondition of equal educational content,and it will be more beneficial when accessing the "immersion program" on the basis of "transitional bilingual education".
9.Transvesical approach laparoendoscopic single-site radical prostatectomy: for organ-confined prostate cancer: report of 8 cases

Objective To investigate the feasibility of applying transvescal approach laparoendoscopic single-site radical prostatectomy (TVSSLRP) and assess the oncological and functional outcomes.Methods Eight patients with clinically localized prostate cancer (PCa) of low risk underwent TVSSLRP.Demographic data were accrued including patient age,body mass index (BMI),preoperative PSA level,the International Index of Erectile Function 5,biopsy Gleason score,clinical TNM stage and D'Amico risk classification.One surgeon performed all TVSSLRP procedures.A homemade triple-port was introduced percutaneouly into the bladder to establish pneumovesicum through a 4 cm incision.The major steps of the surgery were described as follows:initial incision was made along posterior margin of the bladder neck to expose bilateral vas deference and spermatic vesicle.After opening Denonvilliers' fascia and extending the space to lateral prostatic pedicles,an intra-fascial nerve sparing procedure was performed.The puboprastatic ligaments were then separated close to the prostate surface and the dorsal vein complex was cautiously swept off.Subsequently,careful apical dissection and urethral transection was sequentially conducted. To reduce the tension of vesico-urethral anastomosis,3 additional incisions parallel to vesio-urethral margin were created and a novel tension - reduced V-LocTM barbed polydioxanone sutures was used. Results All the operations were successfully performed and there was no conversion to standard laparoscopic approach or open surgery.The total operative time range was 75 - 180 min with mean time of 125 min.The blood loss was 85 -450 ml with mean 140 ml and no blood transfusion was required.The catheter was removed after a mean (range) of 14 (9 -16) days.No intra-operative complications occurred. No patient had positive surgical margins.The mean (range) hospital stay was 17 (13 -25) days after surgery. All the cases were continent after removal of the catheter.No cases demonstrated vesico-urethral stricture and biochemical recurrence on 12 - 18 months follow up postoperatively. Conclusions TVSSLRP is technically feasible for cases with organ-confined prostate cancer with good oncological and functional results.
10.Pyrosequencing for rapid detection of resistance to rifampin, isoniazid, ofloxacin and amikacin in Mycobacterium tuberculosis

Objective To develop an assay to determine Mycobacterium tuberculosis resistance to rifampin, isoniazid, ofloxacin and amikacin by pyrosequencing and evaluate the value of this method in clinical application. Methods Eighty-nine clinical isolates of Mycobacterium tuberculosis from tuberculosis patients were collected from Shanghai Pulmonary Hospital during 2008 to 2009. All strains were isolated from decontaminated sputum, cultured on Lowenstein-Jensen media and identified by traditional biochemical tests with standard methods. Ten Mycobacterium tuberculosis were selected from the strain bank of Shanghai Pulmonary Hospital. The optimal conditions of detection of rpoB, katG, gyrA and rrs gene by pyroseuencing were determined, using the 10 Mycobacterium tuberculosis strains whose drug susceptibility of Bactec 960 and sequence of rpoB, katG, gyrA, rrs gene were known. Then the drug susceptibility of 89 Mycobacterium tuberculosis clinical isolate strains were detected by pyrosequencing using this conditions and the results were compared with that of the Bactec 960 methods. Results The pyrosequencing program of sequence analysis was suitable for the detection of the mutations of rpoB and gyrA genes, and the program of single nucleotide polymorphism was suitable for katG and rrs genes. Among the 89 Mycobacterium tuberculosis clinical isolates,when using the drug susceptibility of Bactec 960 as the standard, the sensitivity of rifampin, isoniazid,ofloxacin and amikacin is 98.0%, 64. 1%, 79.5%, 78. 3% respectively, the specificity is 97.5%,100. 0%, 90. 0%, 100. 0% respectively, the accuracy is 97.8%, 77. 5%, 85.4%, 94. 4% respectively,tested by pyrosequencing. The results of the detection of resistance to rifampin, isoniazid, ofloxacin and amikacin in Mycobacterium tuberculosis using pyrosequencing technique were almost the same with that of Bactec 960, and Kappa ≥0. 7 in each detection. Conclusion Pyrosequencing is thus a rapid, accurate and high throughput method for detecting Mycobacterium tuberculosis resistance to these four drugs.
