1.A Study on UVI - induced DNA Synthesis in Mouse Skin in Vivo Studied by Autoradiography.

The status of DNA synthesis and the effect of UV radiation on the DNA synthesis were studied in mouse skin by microautoradiography. Mice exposed to 100mJ/cm of UVB were injected intradermally with tritiated thymidine, 5 minutes, 2, 6 and 24 hours after irradiation and biopsies were processed for light microscopic autoradiography, A total of 25 ICR female albino haired mice were used as subjects. We compared heavily labeled cells(>10 grains/nucleus) and sparsely labeled cells (3-10 grains/nucleus) in UVL irradiated skin with nonirradiated control skin. 1. Within 5 minutes after UVL exposure an apparent depression in the number of heavily labeled cells occurred. This reduction was statistically significant and remained so for at least six hours post,irradiation. By 24 hours after UVL exposure, the reduction was recovered to nearly control level. 2. By five minutes after UVL exposure, sparsely labeled cells were observed in basal cell layer and differentiated cell layer. The nurriber of sparsely labeled cells appeared highest at five minutes after UVL exposure and then tended to fall to nearly control level at 24 hours. This aberrant type of TdR H incorporation is thought to represent unscheduled DNA synthesis to repair pyrimidine dimers formed in UVL injured DNA molecules,

5.A clinical study on patients with porencephaly.

Porencephaly is relatively rare condition defined by an defect or a defect or cavity in the cerebrum owing to a developmental malformation or to a destructive lesion. Fory-five porencephaly patients diagnosed by Brain CT were clinically analyzed and the following results were obtained. 1) By the age group presenting initial symptoms, the peak incidence was from 1 month to below 3 years old. 2) In initial symptoms, seizure, spastic weakness, headache were showed in order of frequency. But 7 cases (15.5%) were asymptomatic. 3) The latency of diagnosis after presenting initial symptoms from the symptom onset time to 10 years. 4) The subsequent symptoms were as follows: spastic weakness, speech disturbance, gait disturbance, mental retardation, sensory loss and seizure showed independently or combined. 5) As etiologic factor, 21 cases (46.7%) were congenital, 16 cases (35.5%) were post-traumatic or post-operative and 8 cases (17.8%) were perinatal. 6) The prognosis was seen various from mild to severe. Out of 45 cases, 29 cases (64.4%) were no complications. But the prognosis in patients with post-traumatic or postoperative etiological factors was poor. With the advent of brain CT and the resultant capability of detecting structural defect and cerebral lesions responsible for epilepsy or focal neurologic signs, porencephaly was seen to be readily recongizable by CT examination. Since porencephaly is a significant contributor to the spectrum of CNS lesion and benign condition, ist recognition is important in determining prognosis and therapy.

6.A Clinical Experience on Pneumomediastinum: Report of 2 cases.

Pneumomediastinum is a relatively uncommon, infrequently reported entity, In the evaluation of these entity, it is important to exclude pathological causes, including Boerhaave's syndrome which carries a high mortality. Spontaneous pneumomediastinum is related to excessive intraalveolar pressure leading to rupture of perivascular alveoli in the setting of a Valsalva maneuver without communication to gut material. So, it has a benign self-limited course and rarely requires medical intervention. On the contrary, secondary pneumomediastinum caused by instrumental, traumatic, and spontaneous perforation of esophagus. Although the prognosis have been improved since the advent of broad-spectrum antibiotics and nutritional support, pneumomediastinum due to esophageal perforation still has a high morbidity and mortality. The most important prognostic factor is the time interval between perforation and initiation of therapy, and an awareness and a high clinical suspicion is critical in the early diagnosis and treatment. Recently, we have experienced 2 cases of pneumomediastinum, one case was spontaneous pneumomediastinum and the other may be caused by instrumental esophageal perforation. We report the clinical course of the patients with a current literature review.

7.A Case of Recurrent Transverse Myelitis in an Eighty Year Old Woman.

An eighty year old woman was admitted at our hospital diagnosed as a cervical transverse myelitis at a previous hospital. She had a sudden weakness of right upper extremity and left lower extremity 2 months after admission. She received magnetic resonance imaging of brain and spinal cord and cerebrospinal fluid examination and was diagnosed as a recurrent transverse myelitis and midbrain infarction. After a steroid treatment, the weakness of extremities was improved.

9.Comparison of the Hyoid Movement during the Pharyngolaryngeal Swallowing at the Healthy Young and Old Person.

OBJECTIVE: To compare the movement pattern of the hyoid during the pharyngeal swallowing between healthy young and old person through the motion analysis using the videofluoroscopic study. METHOD: Eight young and eight old volunteers without dysphagia were included in this study. Movement patterns of the hyoid (degree of anterior and superior displacement, the speed, trajectory and temporal speed curve of the hyoid) were analyzed through the videofluoroscopy and the motion analysis system. RESULTS: The maximal anterior displacement and the maximal speed of the hyoid were decreased in the old group compared with the young. The trajectories of the young and old group showed asymmetrical pathway (returning pathway was different from the departing one). All groups showed two speed peaks at the temporal curve but the peaks at the old group were blunted. CONCLUSION: Decrease of the maximal anterior movement and the peak speed of the hyoid during the pharyngeal swallowing was found in the old group. It may be due to decrease in the contraction speed of the muscles related to the hyoid movement and elasticity of the laryngeal wall according to the aging.
