2.A Clinical Study of Proximal Humeral Fractures

Fractures of the proximal humerus occur more frequently in older patients but are seen in all ages and emerge epiphyseal separations. The objectives of treatment are restoration of all involved structures to normal anatomical state and a completely functional limb in the shortest time possible. One part and two part fractures were indicated in conservative treatment and surgical treatment in most of three, four part fractures. The results of treatment of forty four cases according to Neer's classification were analysed at the Department of Orthopedic Surgery, Chonbuk National University Hospital from Mar. 1978 to Dec.1985. The results were as follows. 1. The prevalent age distribution was third and fifth decade(48%) . 2. The most common cause of injury was slip down(14 cases, 32%). 3. The most frequent cases of the fractures in Neer's classification were two part fractures(22 cases, 50%). 4. Thirty one cases(71%) were treated by conservative treatment, twelve cases(27%) by open reduction and 1 case(2%) by prosthesis. 5. Good results were in 23 cases(52%) of one and two part fractures, fair in 17 cases(39%) and poor in 4 cases(9%) of two, three and four part fractures.

3.A Study on the Serologic Parameters in Petients with Anemia of Chronic Renal Failure-According to Erythropoietin Treatment.

Clinical study was carried out on the 64 hemodialysis patients(HD) with chronic renal failure who had been treated from December 1992 to July 1993 in Yeungnam University Hospital. The following results were obitained. In hematologic parameters, MCH was 28.8±2.0pg, and MCV was 92.4±4.7fl. Result revealed normochromic and normocytic anemia. Mean values of serum ferritin were 657.4±292.0ng/ml in men and 511.5±370g in women. Mean values of serum iron were 145.5±63.7µg/dl. Mean values of transferrin saturation was 61.6±28.4%. Serum frerritin, serum iron and transferrin saturation were higher in HD group than normal reference. In erythropoeitin treatment group, Hb and Hct were significantly higher than non-erythropoietin treatment group. Amount of transfusion was significantly higher in non-erythropoietin treatment group than erythropoeitin treatment group(p<0.05). Values of iron, transferrin saturation were significantly higher in abnormal liver function test(LFT0 hemodialysis group than normal LFT group(p<0.05). Transfusion amounts revealed positive correlation with ferritin(r=0.4675), transferrin satruation (r=0.3823) and iron(r=0.3386) (p<0.05).

Neurofibroma, one of the peripheral nerve tumor, is a complex benign tumor arising from skin, deep soft tissue, nerve tissue and bone. The incidence of peripheral nerve tumor is relatively low and it may occur either solitarily or diffusely. Plexiform neurofibroma, a type of neurofibroma, develops characteristically in the patients with von Recklinghausen's disease and its exact incidence is unknown. The case reports of solitary plexiform neurofibroma not associated with von Recklinghausen's disease were extremely rare. We reported a case of solitary plexiform neurofibroma on the median nerve.

10.The Motor Cortex Mapping Using Transcranial Magnetic Stimulation in Stroke Patients.

OBJECTIVE: To investigate the clinical usefulness of the motor cortex mapping using transcranial magnetic stimulation (TMS) in stroke patients. METHOD: Five stroke patients were studied. A piece of cloth which marked at 1 cm interval was fixed on the patient's head. Motor cortex mapping for abductor pollicis brevis muscles (APB) was performed with a butterfly coil or with a round coil if motor cortex mapping was impossible. RESULTS: Ipsilateral motor pathways were discovered from the unaffected motor cortex to the affected APB in patient 1. This patient showed delayed latency and low amplitude of ipsilateral motor evoked potentials (MEP) that seems to be evoked from the descending motor pathway rather than the corticospinal tract. In patient 2 and 3, contralateral motor pathways traveled from the affected hemisphere to the affected APB. The short latency and high amplitude of MEPs seems to be attributed to the corticospinal tract. In patient 4, no MEP was evoked by any hemisphere or magnetic stimulator. We believe that the affected APB had no motor pathway, and it correlated well with the poor motor function of her hand. In patient 5, contralateral pathways from the affected hemisphere to the affected APB were present. In this patient, the parameters of the motor cortex map such as the amplitude of MEP, the number of MEP evoked site, and the excitatory threshold were improved after 2 months, which correlated well with clinical improvement. CONCLUSION: Motor cortex mapping using TMS is clinically useful for the evaluation of the characteristics of motor pathways and the change of motor cortex excitability in stroke patients.
