1.Two Cases of cutis Marmorata Telangiectatica Congenita.

Cutis naarmorata telangiectatica congenita(CMTC) was first lesribed by Van Lohuizen in 1992. Cases of CMTC were reported later under various names, includir gan Lohuizens syndrome, congenital generalized phlebectasia. congential phlebectasia and reticulatever scular nevus. It is charaterized by persistent livedo reticularis, telangiectasia and phlectnsia from birch hand atrophy and superficial ulceration of the overlying skin are uncornmonly encountered. They have a steady improvement with time, however various congenital abnormalties, such as varicosities, hen ariomas, hypoplasia or hypertrophy of soft tissue and bones, rarely generalized congenital fibrornatos ectal and genital anomaliies may be associated in approxirnately 50 percent of patients. In case 1, the patient was 45-day-old female infant with reticulated mottling and atrophy of the skin on the right leg. In case 2, the patient was 30-day-old male infant who depressed, bluish, and reticulated patches on the both upprarms, right side trunk, buttock and loen leg. Decreased circumferences of the affected extremities vas observed in case 2. No other assotated anomalies were found in physical and X-ray examinatiorwlin both cases. The skin biiopsy of bothed similar findings, which revealed mild dilatation and proliferation of capillaries, mild inflammatoryel infiltration and extravasated red blood cells in the dermis. The clinical follow-up, alneast all of th lisions disappeared spont.anously in 1 year at case 1, and some of lesions also disappeared in 3 month, 11 case 2. In addition, we reviewed and summerized the reported cases in Korea.

2.A Case of Eccrine Carcinoma with Lung Metastasis.

Eccrine carcinoma does not have a characteristic clinical appreranc, but does have a high incidence of metastatic spread. It may arise anywhere in the skin. The microscopic patterns that of an adenocarcinoma, which may be confused with metastatic adenocarcinoma. In the classical type the histologic features varies from fairy well-differentiated tubular structures in some areaslo anaplastic carcinoma in other areas. The patient was a 51-year-old man who showed a painful, tender, right red-calored, 4 x 5crn-sized tumor mass on the right angle of the lower lip for ten years. In the diological examinations multiple metastatic foci were seen on both lung fields, Histopathologic findings of the skin and lung biopsy specimens revealed that of the classic type of eccrine carcinoma like the bove descriptions. Immunocytochemical studies showed a positive reactivity to anti-cytokeratin anti-ibody, anti-CEA antibody, and anti-S-100 protein on the skin and lung biopsy specimens and no he reactivity to anti-S-100 protein on the lung mass. Electronrnicroscopic findings revealed eccrine glaidlar structures such as nunierous mitochondria and dense core granules in the cytoplasm of neoplastic cells. We report a case of eccririe carcinoma with lung metastases baed on the clinical, radiological and histologic findings.

3.Staining Resistance of The Soft Denture Liners.

This study was investigated to compare the staining resistance of soft denture liners. Specimens wee made of Coe-soft. Coe-Comfort, Soft-liner, Visco-gel, and were stored in 1% methyleneblue solution for 24 hours. The amounts of color change before and after treatment with mono-poly and thermocycling were measured by colorimeter(TC-6FX, Tokyo Denshoku Co. Ltd, Japan) for evaluation of staining resistance. The following conclusions were drawn from this study. 1. The staining resistance of Visco-gel was increased, but there was no changer of staining resistance in Coe-soft, Coe-comfort, and Soft-liner after treatment with monopoly. 2. The staining resistance of the Coe-comfort was the least in all soft denture liners. 3. The staining resistance of Visco-gel and Soft-liner were decreased after thermocycling.

4.The Relationships between Thyroid Hormone Levels and Lower Urinary Tract Symptoms/Benign Prostatic Hyperplasia

PURPOSE: We examined the association between thyroid hormone and lower urinary tract symptoms (LUTS)/benign prostatic hyperplasia (BPH). MATERIALS AND METHODS: A total of 5,708 middle aged men were included. LUTS/BPH were assessed using the international prostate symptom score (IPSS), total prostate volume (TPV), maximal flow rate (Qmax), and a full metabolic workup. Thyroid stimulating hormone (TSH) and free thyroxine (FT4) levels were measured using chemiluminescence immunoassay. We divided participants into quartiles based on their TSH and FT4 levels: first to fourth quartile (Q1–Q4). RESULTS: There was a significant increase in the percentage of men with IPSS>7, Qmax<10 mL/s, and TPV≥30 mL with increase of FT4 quartile. The adjusted odds ratio (OR) for TPV≥30 mL and IPSS>7 were significantly different between FT4 quartile groups (ORs; [5–95 percentile interval], p; TPV≥30 mL, Q1: 0.000 [references]; Q2: 1.140 [0.911–1.361], p=0.291; Q3: 1.260 [1.030–1.541], p=0.025; Q4: 1.367 [1.122–1.665], p=0.002; IPSS>7: Q1: 0.000 [references]; Q2: 0.969 [0.836–1.123], p=0.677; Q3: 1.123 [0.965–1.308], p=0.133; Q4: 1.221 [1.049–1.420], p=0.010). In men with above median levels of testosterone, the FT4 correlated positively with TPV, even after adjusting for confounders. However, the FT4 was not correlated with TPV in men with below median levels of testosterone. TSH was not related to LUTS/BPH measurements. CONCLUSIONS: TPV, IPSS, and Qmax were significantly related to FT4. TPV and IPSS were significantly and independently related to FT4. Additionally, the relationship between FT4 and TPV was distinct when testosterone levels are high.

6.Changes in the Body Mass Index after Total Knee Arthroplasty in Asians

Purpose:
This study examined the changes in the body mass index (BMI) and the effects of BMI on the knee function and risk factors for an increase in BMI after total knee arthroplasty (TKA) in Asians.
Materials and Methods:
A prospective observational study was conducted on 100 cases of TKA performed by one surgeon. Obesity was defined as BMI ≥25 kg/m 2 . The BMI and Knee Society Score, consisting of the Knee Score and Function Score, were measured on the day before surgery and one year after surgery.
Results:
The BMI decreased or was maintained in 34 cases (34%) and increased in 66 cases (66%) one year after surgery. An increase in BMI of 5% or more was considered a clinically significant increase in BMI and occurred in 28 cases (28%). The patients with an increased BMI by more than 5% showed a significantly lower knee function score. The age and preoperative function score were significantly associated with an increased BMI by more than 5%. As the age increased by one year, the odds ratio at which the BMI would increase by more than 5% one year after surgery was 0.905. Moreover, as the preoperative function score increased by one point, the odds ratio that the BMI would increase by 5% one year after surgery was 0.902. In other words, a younger preoperative age and a lower preoperative function score mean a higher risk of a more than 5% postoperative increase in BMI.
Conclusion
The BMI increased by more than 5% one year after surgery in 28% of the TKA patients examined. They had a lower functional score. The risk factors for a more than 5% increase in BMI after TKA were young age and a low preoperative function score. Therefore, surgeons should educate patients, particularly those who are young or have a low preoperative function score, so that the patient’s BMI would not increase after surgery.
8.Cementless Total Hip Arthroplasty for Avascular Necrosis of the Femoral Head: The influence of the extent of involvement in outcome.

The purpose of this study is to evaluate the influence of the extent of involvement in the results of cementless THA in patients with idiopathic AVN of the femoral head. We reviewed 70 hips (52 patients) who had undergone cementless THA for idiopathic AVN of the femoral head. Anatomic femoral components were used in 43 hips and Harris Galante porous prosthesis were employed in 27 cases. The hips were classified according to International Classification (Association Research Circulation Osseous). The length of the follow-up period ranged from 3 to 5 years (average 53.5 months). Preoperative and postoperative clinical documentation and radiographs were evaluated. Statistical analysis was performed on the results of three analysis groups (Group A, comparison of results among Stage IIIA, IIIB, IIIC, and IV; Group B, between stage III and IV; Group C, between IIIA+IIIB and IIIC + IV). Of the 70 hips in the present study, there were 47 hips in stage III (IIIA, 15; IIIB, 19; IIIC, 13) and 23 hips in stage IV. The average postoperative Harris hip score at the time of study was 91.5 (Analysis Group C, p=.009). Femoral subsidence of more than 5 mm occurred in 5 hips (7.1%). Two hips were in Stage IIIC and three hips were in Stage IV (Group C, p=.024). The hips lower than -10 on Engh's index were present in one hip in Stage IIIC and three hips in Stage IV (Group C, p=.011). This study demonstrated that the clinical and radiological results of cementless THA were poor when the extent of involvement was greater or in the case of late stage subjects, particularly those above IIIC (p<05), with avascular necrosis of the femoral head.

9.The Effectiveness of Hepatoprotectants on Elevated Liver Enzyme Induced by Atypical Antipsychotics.

OBJECTIVE: Atypical antipsychotics have been reported to induce the elevation of serum transaminase frequently in Korea, although most of them don't cause severe liver injury. Some hepatoprotectants are commonly prescribed in order to reduce the serum level of transaminase in patients with schizophrenia. We performed the chart review retrospectively for investigating the effect of two hepatoprotectants, biphenyl dimethyl dicarboxylate+garlic oil combination (BDD), and silymarin+silybin combination (SMR14) on the serum transaminase (AST/ALT) elevated by atypical antipsychotics. METHODS: We reviewed the clinical records of 54 schizophrenic patients who experienced the elevation of serum AST/ALT after the treatment with atypical antipsychotics. Patients with preexisting liver disease or elevated AST/ALT above in-house normal limitation at admission were excluded. We obtained the level of serum AST/ALT at the time of hepatoprotectants administration, 1 week, 2 weeks and 4 weeks after the administration. Repeated Analyses of variance were conducted in order to identify sequential change of serum AST/ALT level, and Fisher's exact test were performed to compare the number of patients whose AST/ALT levels were normalized after 4 weeks between two groups. RESULTS: 33 males and 21 females were included in this study and the mean age of those subjects was 36.28+/-10.92. Among all the patients, 35 were treated with BDD and 19 were treated with SMR14. After administration of hapatoprotectants, both serum AST and ALT level were significantly reduced during 4 weeks (F=10.56, p<0.001;F=17.92, p<0.001). BDD was superior to SMR14 in the number of patients whose ALT level reduced below in-house upper limitation after 4 weeks of treatment with hepatoprotectants (p=0.012), but there was no difference between BDD and SMR14 in aspect to AST level. CONCLUSION: Both hepatoprotectants, BDD and SMR14 were effective in reducing serum AST/ALT level which had been elevated by atypical antipsychotics. BDD was superior to SMR14 in normalizing serum ALT level within 4 weeks. Increased liver enzyme is prevalent in patients during the treatment with atypical antipsychotics. More research will be needed in this field.
