1.A Study on the Evaluation of the Psychogenic Etiology by the Symptom Checklist-90-Revigion(SCL-90-R) and Treatment in Female Urethral Syndrome Patients.

From March 1983 to February 1988, 248 patients of female urethral syndrome were studied retrospectively. The results were as follows: 1. Both 113 cases of the patients group who showed female urethral syndrome and the same numbers of 113 as the control group who had been treated at this hospital with other medical diseases, were tested the Korean Manual of Symptom Checklist-90-Revision. In this way, female urethral syndrome patients scored up significantly higher on the anxiety, depression, and phobic anxietic symptoms than did the control group, and the results showed the evidence or psychosomatic etiologies on the female urethral syndrome (p<0.05 ). 2. Among the patients, the 135 patients were treated with transurethral fulguration, urethral dilatation and antibiotics. Tranquilizer and antidepressants were added to above mentioned treatment in another patients group, 113 patients. In this group, excellent result was obtained in cure rate. The cure rate was 83.2% (94 out of 113 cases) while only 57.8% (78 out of 135 cases) in group without tranquilizer and antidepressants in their treatment (p<0.06). 3. The tranquilizer and antidepressants did not influence on recurrence rate( p value ; not significant).

3.A Clinical Study on Patients with Chronic Lymphocytic Leukemia.

PURPOSE: Chronic lymphocytic leukemia (CLL) is the most common form of leukemia in western countries but it has been reported rarely in Korea. The purpose of this study was to evaluate the clinical characteristics, treatment responses and prognosis of the patients with CLL. MATERIALS AND METHODS: We retrospectively reviewed the medical records of 17 patients with CLL diagnosed at the Catholic University of Taegu-Hyosung & Kyungpook National University Hospital from Jan. 1986 to Mar. 1996. RESULTS: There were 9 males and 8 females and the mean age was 55 years. The majority of patients had systemic symptoms (77%) and advanced clinical stage (Binet stage C, 70%) at the time of diagnosis. The bone marrow histology was reviewed in 13 cases and patients were subdivided into those with a diffuse (9cases) and nondiffuse (4cases) pattern of bone marrow histology. In patients with advanced clinical stage, diffuse pattern was predominated and showed poor survival. 14 patients were treated with chlorambucil/steroid or combination chemotherapy. Among the 13 evaluable patients, 1 (8%) achieved a complete response and 7 (54%) achieved partial responses, and median survival duration was 18months. 11 patients died after 2~18 months of follow-up and infection was the main cause of death. CONCLUSION: Poor prognostic factors, such as high blood lymphocyte counts, diffuse bone marrow involvement pattern & advanced clinical stage, are common in our patients at presentation & showed poor survival.

6.Effects of Basic Hospital Infection Control Methods on the Isolation Rate of Methicillin: Resistant Staphylococcus aureus.

BACKGROUND: Methicillin-resistant Staphylococcus aureus (MRSA) is highly prevalent in hospitals in Korea. The overall rate of MRSA in hospitals in Korea was estimated over 60%. Recently hospital infections caused by MRSA are getting increased and problematic in Korea, Our hospital Seoul Paik Hospital, has been trying to solve this problem. We had not applied exact methods for hospital infection control. Therefore, we adapted strict methods under the control of hospital infection control committee (HICC) and assessed the effectiveness of these methods. METHODS: Every month, the number of MRSA isolates was collected and analyzed; the results of the analysis were reported to every ward. All wards were supplied with Microshild(R) (Johnson & Johnson, Australia), Clean N' Fresh(R) (Carroll, USA), and standard paper towels. Health care workers were regularly educated for hospital infection control. The use of antibiotics was controlled by the HICC. We compared MRSA isolation rate between before and after applying the strict infection control methods under the regulation of HICC. Mann-Whitney U test, one-way ANOVA, and Chi-square test were used for statistical analysis of the results. RESULTS: After restricting the uses of antibiotics, glycopeptides and carbapenems were prescribed 15% less (20.3 vs 17.3 DDD /1000 patient-days) and 35% less (9.3 vs 6.0 DDD /1000 patient-days) respectively, compared to before restricting antibiotics. Methicillin resistance rate was decreased from 78% to 69% compared to before the methods(p=0.02). The isolation rate of MRSA was reduced by 31-42% from 2.33 to 1.35-1.60/1000 patient-days compared to before adapting strict infection control methods (p=0.04-0.07). CONCLUSION: This study showed that applying the strict infection control methods in the hospital can be effective to reduce the isolation rate of MRSA.

7.Endovascular Treatment of Direct Carotid Cavernous Fistulas Using Detachable Balloon(s) and Coils.

PURPOSE: To evaluate the merits of this technique and to provide a standard for comparison with futuretreatment alternatives. MATERIALS AND METHODS: We retrospectively reviewed the records of 52 patients withdirect CCF treated at the Yonsei Medical Center between 1983 and 1997 by transarterial or transvenous embolizationusing detachable balloon(s) and/or coils. RESULTS: Among the 52 cases, 51 were traumatic in origin, while onlyone was the result of a ruptured aneurysm. The three most common presentations were chemosis, bruit, andpulsatile proptosis. Forty-five patients were successfully treated with detachable balloon(s) and/or coils andinternal carotid blood flow was preserved in 35. When coils were used (1993-7), the ICA preservation rate washigher than when they were not used (1983-92). To confirm statistical significance, however, more studies areneeded. Four patients who initially presented with incomplete occlusion showed spontaneous occlusion on follow upangiography. In two patients, surgical ligation was performed because embolization failed and there was incompleteocclusion of the fistula. We experienced complications such as transient 3rd and 6th cranial nerve palsy,migration of deflated balloons and coils to the lung, and loss of vision. CONCLUSION: Transarterial andtransvenous embolization with detachable balloon(s) and/or coils provides a high rate of fistula obliteration withlow morbidity. In particular, the use of coils led to an increased rate of ICA preservation, thus and isconsidered as a good treatment modality for CCF obliteration.

8.A Case of Left Brachiocephalic Vein Total Occlusion Due to Acute Thrombosis Soon after Permanent Pacemaker Insertion.

Symptomatic pacing lead-associated thrombosis is very uncommon occurring in 0.5-3.5% of pacemaker implants. Especially thrombisis-induced total occlusion occures almost in late stage over several months to years but acute thrombosis occurring several days after venous pacing has not been reported. In this case, We performed upper limb venography in the patient who presented edema and pain of neck, left upper limb and headache as well as intermittent cough occurring in bending forward. A venogram confirmed acute thrombus completely occluding the left brachiocephalic vein and the patient received intravenous heparin and was maintained on warfarin. Repeated veno- graphy after treatment for 30 days revealed persistent thrombus with total occlusion which not be improved signi- ficantly copmpared to previous venogram and collateral veins diverting the blood to the contralateral side and into the superior vena cava was developed. The patient's symptoms resolved almost and that is likely to be due to the development of collateral venous channels.

9.15 Years of Experience in Laparoscopic Cholecystectomy by a Single Surgeon.

PURPOSE: A laparoscopic cholecystectomy is the treatment of choice for gallbladder disease. The aim of this study is to analyse 2,239 cases of laparoscopic cholecystectomy (LC) and observe the changes of 15th-year gallbladder disease. METHODS: We performed a retrospective analysis of 2,239 patients that underwent a LC by a single surgeon, from March 1993 to December 2007. We divided it into 3 groups; group 1 was from March 1993 to December 1996 and group 2 was from January 1998 to December to 2001, group 3 was from January 2003 to December 2007. RESULTS: The mean age, hospital stay, mean operation time and conversion rate was decreased. Gallbladder cancer of group 1 was 0.6%, group 2 was 0.5%, and group 3 was 1.6% (P<0.05). Gallbladder polyps of group 1 was 3.3%, group 2 was 5.1%, and group 3 was 11.8%. There was statistically significance (P<0.05). Gallstone was significantly decreased (group 1 was 98.1%, group 2 was 97.6%, group 3 was 92.8%) in proportion, pigment stone was significantly decreased (group 1 was 31.5%, group 2 was 27.5%, group 3 was 21.7%) and Cholesterol & Mixed stone was significantly increased (group 1 was 68.2%, group 2 was 72.5%, group 3 was 78.3%)(P<0.05). CONCLUSION: As surgical experience increased with a rising number of cases, the conversion rate, complications, hospital stay and mean operation time was decreased. Pigment gallstone was decreased and cholesterol gallstone was increased owing to western diet, old age and fatness.

Adrenal myelolipomas are rare, benign tumors cornposed of mature fat and bone marrow elements. Most are small, asymptomatic tumors found incidentally at postmortem examination, Fine needle aspiration biopsy can be used to confirm the diagnosis and avoid an unnecessary operation. We report a case of adrenal myelolipoma confirmed by fine needle aspiration biopsy. A 77-year-old woman with complaining of upper abdominal pain for 2 days was found to have an adrenal mass. She took dexamethasone frequently for 3 years due to multiple arthralgia. Ultrasono-graphy showed a 6cm-sized, suprarenal mass and a stone in the gall bladder with thickened wall. Computed tomography and magnetic resonance imaging scan also presented a suprarenal mass. Endocrinologic results were within normal limits. Adrenal myelolipoma was confirmed by computed tomography-guided fine needle aspiration biopsy without surgery.
