1.Changes of Histopathological Findings with Time in the Dermographism Lesion.

Changes of histopathological findings with time were studied after scratching the skin of 37 patients with dermographism. Biopsies were also done in 13 normal healthy controls for comparison with unstroked skin of the patients. 1. Biopsies of unscratched skin of the patients showed no histologic difference from those of the skin from controls. 2. Neutrophils increased in number with time after scratching and maximum neutrophil count (mean 16.08+/-24.17/HPF) was observed at 90 minutes after scratching 3. Eosinophilic infiltration was also similar to that of neutrophils. Maximum eosinophil count (mean 324+/-4.76/HPF) was found at 60 minutes after scratching. 4. L ymphohistiocytic infiltration showed a similar tendency to that of neutrophils, but the degree of change was not so prorninent. 5. Before scratching, mast cell count in patients with dermographism showed no difference in number when compared with norrnal controls. In patients with dermographism, mast; cell count inclined to decrease after scratching. 6. Edema and lymphatic dilatation in the upper dermis were most prominent at 5 minutes after scratching and disappeared slowly thereafter.

2.A Case of laparoscopic cystectomy for large adenomyotic cyst within myometrium with bilateral endometriomas.

Adenomyotic cysts are not uncommon disease, but their sizes are mostly lesser 5 mm. Reports of large adenomyotic cyst ( >5 mm ) are vere rary. These large sized hemorrhagic cysts arise from small sized cyst and may be caused by progressive expansion of cyst due to progressive menstrual bleeding. And the active secreting of cystic wall may play a part in making the large cyst. Authors observed a case of large adenomyotic cyst within myometrium occuring in o 39-year-old woman, and she was accompanied with both ovarian endometriomas. The cyst was 3 *3 cm sized, and had chocolate-colored thick viscous contents. Histologically, cystic wall was lined with endometrial type epithelium. Epithelium were composed of single layer columnar, partly ciliated cells. Stroma under glandular epithelium were thin throughout the cyst and resembled morphologically endometrial stroma in endometriosis and had red cells and hemosiderin-laden macrophages in places. We experienced one case of large adenomyotic cyst, so we report the case with a brief review of the concerned literatures.

3.Treatment and Clinical Application Of a Triangular Splint Method for Fracture of the Humerus

Fracture of the shaft of the humerus had been treated by many different way, such as hanging cast, shoulder spica cast, abduction splint, and Velpeau bandage, etc, The authors have employed the new trianglar splint designed by ourselves for the 54 cases of fracture of the humeral shaft from Jan., 1965 to Oct., 1976 in the department of Orthopedic surgery of Han-il Hospital. In our experience, the triangular splint method is useful for not only fracture between the head of the humerus and supracondylar region, but also shoulder joint injuries and diseases which need firm immobilization. The triangular splint method has merits, such as spontaneous reduction by the fractured arm weight with gravity, the stability during care, relatively comfort during procedure. Satisfactory results had been obtained by the triangular splint method.

5.Ischemic Preconditioning in Isolated Rabbit Heart : Effect on Left Ventricular Function, Infarct Size, and Protein Kinase C*.

BACKGROUND: It has been demonstrated that ischemic preconditioning(IP, repetitive brief period of ischemia and reperfusion) enhances recovery of post-ischemic contractile dysfunction and reduces incidences of reperfusion-arrhythmia and infarct size after a prolonged ischemia. A lot of mechanisms have been proposed, however, controversies still remain. Recent studies suggested that IP could activate protein kinase C(PKC). Therefore, we measured left ventricular function, myocardial creatinin and PKC activities, and infarct size to assess whether IPs cardioprotective effect is related to PKC activation using isolated rabbit hearts. METHODS AND RESULTS: Hearts isolated from New Zealand White rabbits(1.5-2.0kg body weight) were perfused with Tyrode solution by Langendorff technique. After stabilization of baseline hemodynamics, the hearts were subjected to 60-min ischemia followed by 120-min reperfusion with IP(IP group, n=15) or without IP(control group, n=14), IP was induced by 4 cycles of 5-min global ischemia and 5-min reperfusion. Left ventricular function including developed pressure(LVEDP), dp/dt, heart rate(HR), and coronary flow(CF) was measured to determine the recovery of LVEDP, RPP(rate-pressure product, HRXLVEDP) and CF to baseline measurement. Frequency of arrhythmia was counted on reperfusion. Myocardial CK-MB, myocardial cytosolic and membrance PKC were measured and the infarct size was determined by staining with tetrazolium salt and planimetry. Data were analyzed by one-way ANOVA, Tukey's post-hoc test and t-test. There was no significant differences in the recovery of LVEDP, dp/dt, RPP, and CF and frequency of arrhythmia during reperfusion between the control and the IP groups. In comparison with the control groups, however, CK-MB was significantly lowered in the IP group(P < 0.05). Cytosolic PKC was significantly decreased but membrance PKC was increased(p < 0.05). These findings indicate that PKC was translocated and activated by IP. Furthermore infarct size was smaller and limited to the antero-lateral or posterior wall and papillary muscle in the IP group(p < 0.05). CONCLUSION: These results indicate that IP dose not improve post-ischemic contractile dysfunction after a prolonged ischemia of 60 minutes but has an infarct-limiting effect. This cardioprotective effect of IP may be related to PKC activation.
