1.A Case of Intramural Pregnancy.

Intramural pregnancy is a rare variant of ectopic pregnancy, which was first reported in 1924 by Perli, and only 21 cases have been reported in the literature. The pathologic diagnosis of an intramural pregnancy can be made by the myometrium surrounding the products of conception that separate it from the endometrial cavity or fallopian tubes. Recent development of radiologic ultrasonography and radioimmunoassay of the beta-subunit of human chorionic gonadotropin has markedly enhanced the ability to diagnose intramural pregnancy during very early gestation. Many reports have noted that there would be possible relationships between the intramural pregnancy and prior uterine traumatic factor-curettage, cesarean section, myomectomy, salpingectomy, and manual removal of the placenta. We experienced a 28-year-old female patient with amenorrhea for 7 weeks and sudden vaginal spotting. She underwent dilatation and curettage at 4 weeks of gestation. We report this case with a brief review of the literature.

2.Expression of CDK 4, Connexin 32, and PCNA in N-diethylnitrosamine-Induced Lesions of Rat Liver.

Cellular altered foci (CAF), hyperplastic nodules (HN) including regenerating and adenomatous nodules, and hepatocellular carcinomas (HCC) were induced in Sprague-Dawley rat liver by prolonged administration of N-diethylnitrosamine (DEN, 200 ppm). Immunohistochemical expression of connexin 32 (Cx 32), cyclin-dependent kinase 4 (CDK 4), and proliferating cell nuclear antigen (PCNA) was assessed for the evaluation of preneoplastic potential of CAF. Regardless the duration of DEN administration, basophilic cell foci were the most frequently observed lesion in both CAF and cellular expanding hyperplastic nodules. Eosinophilic cell foci, however, were concomitantly increased with adenomatous nodules in later experimental groups. Cx 32 showed perimembranous spot-like expression. Its number was 7.25 2.10 per cell in normal hepatocytes. CAF and adenomatous nodules showed markedly decreased Cx 32 spots. Moreover, its reduction was more prominent in HCC. PCNA-labelled hepatocytes were scattered in the most CAF, showing no significant difference between each CAF. PCNA labelling index (LI) in adenomatous nodule and HCC was markedly increased. CDK 4 was localized in the cytoplasm and/or nucleus of hepatocytes. Eosinophilic cell foci revealed more nuclear expression of CDK 4 than other types of CAF, of which expression incidence was comparable to that of adenomatous nodule. Nuclear CDK 4 expression in HN and HCC was increased, although significant difference between regenerating nodule and adenomatous nodule was not seen. In conclusion, the incidence of CDK 4 was concomitantly increased with PCNA LI, however, reciprocally decreased with Cx 32 in accordance with the advance of DEN-induced HCC in rat. Phenotypically altered foci manifested as CAF are early valuable preneoplastic marker lesion for evaluation. In addition, basophilic cell foci can be considered a discernible marker of cellular expansion within nodules. However, eosinophilic cell foci might be an indeterminate marker for the advance of DEN-induced HCC in rat.

3.CD44 Standard and Variants Expression in Cutaneous T Cell Lymphoma.

BACKGROUND: CD44 is a family of glycoproteins involved in cell to cell and cell to matrix interactions. Overexpression of CD44v6(variant exon 6) form has been reported in several malignant tumors such as stomach cancer, uterine cervical cancer, colorectal cancer, breast cancer and keratinocytic skin tumors, such as, basal cell carcinoma and squamous cell carcinoma. However, CD44 expression in cutaneous T cell lymphoma has not been investigated thoroughly. OBJECTIVE: The purpose of this study is to examine whether there is any difference in the expression of CD44s & CD44v6 between mycosis fungoides(MF), angiocentric T cell lymphoma, subcutaneous panniculitic T cell lymphoma, Ki lymphoma and unspecified peripheral T cell lymphoma. We also evaluated the statistical significance between the expression of CD44v6 and systemic involvement of the diseases. METHODS: Routine paraffin sections of formalin-fixed 33 tissues (11 MF, 8 angiocentric T cell lymphoma, 5 subcutaneous panniculitic T cell lymphoma, 2 Ki lymphoma, 1 unspecified peripheral T cell lymphoma, 2 psoriasis, 2 lichen planus, 2 erythema nodosum) were labeled with anti-CD44 monoclonal antibody using a avidin-biotin-peroxidase complex. Normal skin served as the negative control. RESULTS: 1. Eccrine glands, hair follicles and the epidermis, except the cornified layer, showed positive staining for CD44s. In inflammatory skin diseases, it showed positive staining for CD44s, however CD44v4/5 and CD44v6 stainings were negative. 2. All(4 out of 4) of the tumor stages of MF showed positive CD44s staining, and 3 out of 4 showed positive CD44v6 staining(p=0.024). However, none of them expressed CD44v4/5. 3. All 8 cases of angiocentric T cell lymphoma were positively stained for CD44s, but not for CD44v4/5. In contrast to other peripheral T cell lymphoma, CD44v6 was expressed in 3 out of 8 cases of angiocentric T cell lymphoma. In subcutaneous T cell lymphoma, only the CD44s was expressed, and Ki lymphoma was positively stained for CD44s and negatively stained for CD44v4/5 and CD44v6. 4. In CD44v6 positive angiocentric T cell lymphoma (3 out of 8) and CD44v6 positive tumor stages of MF (3 out of 4), 5 out of 6 patients had systemic involvement suggesting a statistical significance between CD44v6 expression and patient's systemic involvement(p=0.015). CONCLUSION: These results suggest that CD44v6 may serve as a useful prognostic marker in the tumor stage of mycosis fungoides and angiocentric T cell lymphomas.

6.The Efficacy on the Immunotherapy with Paternal Lymphocytes in Unexplained Infertility.

The aim of present study was to determine the efficacy of immunotherapy with paternal lymphocytes for unexplained infertility. It has been apparent that reproductive success may be affected by the presence of abnormal autoantibodies. Unexplained infertility and repeated pregnancy wastage has been reported in the presence of abnormal autoantibodies. These data suffest that abnormal immune function may be an important pathologic entity contributing subfertility in patients with unexplained infertility. Therefore, immunotherapy may be a possible treatment modality for patients with unexplained infertility. Some investigators have reported that a proportion of infertile couples with repeatedly unsuccessful ET showed close histocompatibility similar to those of spontaneous recurrent abortion. Recently, it has been noted that immunotherapy with paternal lymphocytes achieves a high efficacy in preventing subsequent abortion in women with primary recurrent abortion of unknown cause, which was mediated by immune reaction including blocking antibody. To substantiate the hypothesis, we applied immunotherapy preceding Peritoneal Oocyte and Sperme transfer (POST) to 43 patients, 47 cycles of 82 patients, 89 cycles with at least three previous IUI failure from April, 1993 to February, 1995. There were no significant differences between treatment and control group in clinical response and hormonal response to controlled hyperstimulation. there was no significant difference between treatment and control group in pregnant rates per cycles (42.6% versus 28.6%), but a significantly lower abortion rate per pregnancy in treatment group, with 10.0% (2/20) compared with 50.03% (6/12) in control group. This study may suggest that immune therapy for patients with unexplained infertility with paternal lymphocytes might be beneficial.

This study was performed to evaluate histopathologic and physical characteristics of anterior capsule during capsulorhexis with a new high-frequency diathermy.The authors investigated the histopathologic characteristics of anterior capsule according to coagulation time and anterior capsular diameter in diathermy capsulorhesxis(DC),comparing with those in continuous curvilinear capsulorhexis(CCC). Thermally induced histopathologic alter-ation was localized at anterior capsule with coagulation for 0.5 second, while it was extended into cortex with coagulation for 2 seconds.When capsulorhexis was performed with diameter of 6 mmand the margin was pulled with iris retractor,anterior capsular edge was fully extended both in CCC and DC.But with diameter of 3.5 mm,DC mostly led to tear of capsular edge on its extension,while CCC permitted full extension of anterior capsular edge.These results suggest that strength of anterior capsular edge in DC is inferior to that in CCC.However diathermy capsulorhexis is a useful alternative technique in cases,which CCC is difficult to perform,such ashypermature or congenital cataracts.
