2.Microendoscopic discectomy for lumbar intervertebral disc herniation

Objective:To investigate the operation technique and the effect of the posterior microendoscopic discectomy (MED) system on lumbar intervertebral disc hermiation (LIDH). Methods: We retrospectively reviewed and analyzed 25 cases of LVDH treated with the MED system from March 2004 to August 2005. Results: All the patients were ambulant 20 hours to 7 days (mean 2.4 days) after the operation, with a mean hospital stay of 5.2 days. And all were healed within the first stage except 1 case of infection. The patients were followed for 2-15 months (mean 6 months). Excellent results were achieved in 18 cases, good results in 5 and fairly good results in 2, the first two groups making up 92%. Conclusion: The key to successful MED with no complications is familiarity with the equipment, proper choice of indication, exact surgical location and skilful operation.
3.Analysis of misdiagnosis and treatment of acute superior mesenteric arterial embolism

Objective To explore the early diagnosis and treatment of acute superior mesenteric arterial embolism (ASMAE).Methods The clinical data of 25 patients with acute superior mesenteric arterial embolism were retrospectively analyed.Results All 25(100%) patients had severe abdominal pain with abdominal signs,21(84%) patients had history of valvular heart disease or atrial fibrillation,and 17(68%) patients had hematochezia.All the cases were misdiagnosed preoperatively,including 11 patients were misdiagnosed as abdominal pain of unknown causes,3 patients as acute pancreatitis,3 patients as acute gastroenteritis,2 patients as necrotizing enteritis.1 patient as acute appenditis,1 patient as acute myocardial infarction,and 1 patient as acute cholecystitis;but 3 patients were preoperatively suspected to have impairment of mesenteric blood flow.All of the 25 patients underwent necrotic bowel resection,6 patients died,and 7 patients abandoned treatment because of serious complications such as short bowel syndrome etc.Mortality rate was 51%(13/25).Conclusions Early diagnosis of patients with acute superior mesenteric arterial embolism is difficult;the main cause of misdiagnosis is lack of recognition of its clinical signs.Fully grasping the characteristics of patients with early stage disease and effective early intervention are the fundamental measures for reducing mortality.
5.Analysis and prevention of complications in abdominal disease patients with liver cirrhosis

Objective:To study the methods of prevention and occurrence of complications in abdominal operation patients with liver cirrhosis.Methods:Analys is the occurrence of postoperative complications and influencing factors in 41 cases in abdominal diseases patients with liver cirrhosis.Results:The incidence rate of postoperative complications was 63.4%(26/41),19 patients with 1 complication,7 patients with 2 or more than 2 complications.Five patients died,Mortality was 12.2%(5/41).Ascites,Child classification,portal hypertension,esophageal varices and intraopera-tive bleeding were influencing factors of occurrence of postoperative complications in abdominal disease patients with liver cirrhos is analyzed using Logistic regression analysis.Conclusion:Positive perioperative management,reducing the intraoperative and postoperative bleeding,Intra-abdominalinfection prevention and ascitic treatment are importent factors in reducing postoperative complications and mortality.
6.Simple high position ligation of internal spermatic vein under laparoscope in the treatment of children's varicocele

Objective To explore the method and superiority of laparoscopic management of children's varicocele. Methods High position ligation of varicocele under laparoscope was performed and internal spermatic arteries were reserved intraoperatively. Results 32 cases had smooth recovery after operation, and no complication occurred. The hospital stay was (1~3)days postoperatively. 29 cases had been followed up for 6 months~1 year and no relapse and testis atrophy occurred. Conclusions The method has the advantages of simpleness, minimal invasion and quicker recovery. Reservation of internal spermatic artery can prevent testis atrophy.
7.Efficiency of internal fixation on the treatment of pertrochanteric hip fractures

Objective: To observe the effects of different internal fixations on the treatment of pertrochanteric hip fractures. Methods: 158 cases of pertrochanteric hip fractures treated with different internal fixations, including anatomic steel, Gamma nail and dynamic hip screw, from January 1999 to October 2004, combined with functional exercise, were summarized and the advantages and disadvantages of different fixations were evaluated. Results: All of 158 cases were followed up for 6 to 36 months with a mean of 14 months. The rate of excellent or good in all patients was 94.61% by Harris criterion and 17 cases have surgical complications (10.18%). The complication rates of the three types of treatment had no difference. Conclusion: Anatomic steel, Gamma nail and dynamic hip screw are all ideal and reliable internal fixation material for pertrochanteric hip fractures as long as they are selected properly before operation.
8.Study on differentially expressed genes in Jurkat cells treated with phosphonoformate using DNA microarray

Objective To screen the genes differentially expressed in gene expression in human lymphoma cell line Jurkat cells treated with phosphonoformate (PFA). Methods cDNA microarray technique was employed to detect the mRNA expressed in Jurkat cells treated with PFA or 0.9 percent sodium chloride, respectively. Results Among 1 152 genes there were 94 genes with different expression, of which 38 genes were upregulated and 56 genes were downregulated in Jurkat cells treated with PFA compared with those treated with 0.9 percent sodium chloride. Conclusions cDNA microarray technique was successfully used to screen the genes with different expression in Jurkat cells treated with PFA, and the results brought some new clues for the study of the immune regulation mechanism of PFA.
9.Surgical treatment of unstable pelvic fractures combined with acetabular fractures.

OBJECTIVETo discuss the treatment strategy of acetabular fractures and unstable pelvic fracture of the hip and to evaluate its outcome.
METHODSRetrospective analysis of clinical data in 32 patients with unstable pelvic fracture and acetabular fractures from January 2007 to June 2013 were collected. There were 18 males and 14 females aged from 18 to 62 years old (means 38 years old). According to Tile classification of pelvic fracture, 11 cases were type B1, 8 were type B2.1, 7 were type B2.2, 3 were type C1.1, 2 were type C1.2, 1 was type C3. According to Judet-Letournel classification, anterior column fracture was in 1 case, transverse fracture in 8, transverse plus posterior wall fracture in 6, T-type fracture in 1, anterior column plus half transverse fractures in 5, double column fracture in 11. Other combined injuries were treated early, the surgical operation were performed after stable condition. The hip joint function and the fracture reduction were assessed during follow-up.
RESULTSThe operative time was from 1.8 to 6.5 hours (averaged 3 hours). Two fat patients' incision occurred in fat liquefaction and healed after dressing, no incision infection happened. Only 1 case was lost to follow-up, 31 patients were followed up with a mean time of 23 months (6 to 42 months). The healing time of pelvic fracture was from 8 to 18 weeks (averaged in 10.6 weeks). The hip function was evaluated according to the Matta and Tornetta standard postoperatively, the result was excellent in 15 cases, good in 14 cases, fair and poor in 1 case respectively. The Majeed score of the hip function was 83.65? 7.67, the result was excellent in 15 cases, good in 12 cases and fair in 4 cases. The healing time of acetabular fractures was from 8 to 16 weeks (averaged in 10.2 weeks). The fracture reduction was assessed by Matta standard, the result was excellent in 15 cases, good in 12 cases and fair in 4 cases. The heterotopic ossification was evaluated by Brooker standard, 4 cases were grade I, 1 case was grade II . There were no infection, nonunion and necrosis of the femoral head in all patients. The nerve damage symptoms in 5 patients disappeared during 4 to 6 months after operation.
CONCLUSIONPatients with unstable pelvic fractures and acetabulum fractures were in a critical condition early, using the concept of damage control to save lives in a timely manner. Grasp the operation time in the late treatment, acetabulum fractures reach anatomical reduction as far as possible, pelvic fractures are given priority to stable reconstruction. The operation order was fixed pelvic ring first, according to the condition to choose the appropriate surgical approach and fixed mode.
