1.A Traditional Chinese Medicine Research on the Principal Antidepressive Effect in Kaixin Jieyu Prescription Based on Uniform Distribution

Objective To pick up the Chinese materia medica with principal antidepressive effect in Kaixin Jieyu prescription based on uniform distribution. Methods 11 groups of concerted application with different dosages of six simplex Chinese materia medica (X 1-X 6) of Kaixin Jieyu prescription were arranged by the designlist and sustentacular table of U11(1110). Positive and negative control groups were also formed. Mice were randomized into 13 groups according to body weight and fed with medicine 14 d and estimated by tail suspension test, forced swim test and autonomic activities test. Results Relationship of percentage of immovability time (Y 1) in the tail suspension test and X 1-X 6 were analyzed with multivariant non-linear regression analysis and resulted in Y 1=1.133 1-0.842 3×X 1+0.438 2×X 3-0.712 9×X 6. It indicated that both Radix Ginseng and Fructus Aurantii Immaturus had obvious antidepressive function, and Radix Paeoniae Rubra may facilitate depression. Relationship of immovability time (Y 2) in forced swim test and X1-X6 were analyzed with multivariant non-linear regression analysis and resulted in Y 2=167.69-2.99×X 12-2.14×X 1-1.65×X 6. It also indicated that both Radix Ginseng and Fructus Aurantii Immaturus had obvious antidepressive function. Relationship of autonomic activities index (Y 3) in autonomic activities test indicated that X 1-X 6 had no obvious influence on Y 3 and antidepressive function of KaixinJieyu prescription was not concerned with excitability to the central nervous system. Conclusion Radix Ginseng and Fructus Aurantii Immaturus were the Chinese materia medica with principal antidepressive effect in Kaixin Jieyu prescription.
2.Expression and role of TLR4/MyD88 in the hippocampus of rats with post‐traumatic stress disorder induced by single prolonged stress

Objective To explore the expression and role of Toll-like receptor 4 (TLR4)/myeloid differentiation factor 88 (MyD88) in the hippocampus of a rat model of post‐traumatic stress disorder (PTSD) induced by single prolonged stress (SPS). Methods Forty-five rats were randomly assigned into control group (n = 15) or PTSD group (n = 30; 15 each were examined on days 4 and 7 after modeling,respectively). PTSD was induced by SPS,which was recognized internationally. The Morris water maze test was carried out to measure the spatial memory of the rats. The morphological changes of the hippocampus were examined with Nissl staining. The expression of TLR4,MyD88,and downstream nuclear factor-kappa B (NF-κB) was determined by Western blot. Results Compared with the control group,the PTSD group showed a significantly prolonged mean escape latency (P<0.05); sparsely arranged and lightly stained neurons and a reduced number of Nissl bodies within cytoplasm in the hippocampus; and significantly increased protein expression of TLR4,MyD88,and NF-κB on days 4 and 7 after SPS stimulation (all P<0.05). Conclusion SPS can impair the spatial memory of rats,which may be associated with the up-regulation of TLR4/MyD88/NF-κB in the hippocampus. The activation of TLR4/MyD88 signaling may be one of important molecular mechanisms in the development of SPS-induced PTSD.
3.Symptoms and psychological experience of elderly COPD patients with urinary incontinence: a qualitative research

Objective:To explore the symptoms and psychological experience of elderly patients with chronic obstructive pulmonary disease (COPD) and urinary incontinence.Methods:From January to February 2021, a total of 17 patients in General Hospital of Ningxia Medical University and Wuzhong People's Hospital were enrolled by phenomenological method and interviewed using semi-structured interview method. KJ method was used to analyze, summarize and refine the interview data.Results:A total of 4 themes were extracted, including confusion in cognition and management between COPD and urinary incontinence symptoms, affected compliance of lung rehabilitation, intertwined physical and mental burden and psychological adjustment, and eager for medical staff's attention and help to symptom interaction.Conclusions:Elderly patients with COPD and urinary incontinence have many trouble under the interaction of symptoms. Medical staff should pay attention to the complexity of symptom process and the interaction between symptoms, and carry out timely and effective symptom evaluation and intervention, so as to improve patients' compliance of lung rehabilitation and quality of life.
4.An analysis of the problems and countermeasures of health file construction for patients with brucellosis in Qinghai Province

The construction of health file of patients with brucellosis in Qinghai Province was based on the Qinghai Provincial Public Health Network Information Platform. It was jointly participated and implemented by animal husbandry and veterinary departments, animal disease control centers, disease control centers and hospitals at all levels, to achieve standardized management of early detection, early diagnosis, early treatment, as well as follow-up, review, and efficacy evaluation. After analyzing the health file of brucellosis patients, we proposed to strengthen the propaganda of brucellosis, collect information from multiple diseases, share resources with multiple departments, integrate human resources and exert management responsibilities, and explore opinions and countermeasures suitable for local working conditions. The establishment of a health file for patients with brucellosis could fully grasp the diagnosis and treatment of brucellosis patients, facilitate the management of follow-up and review, and it was helpful to understand the distribution and prevalence of brucellosis in Qinghai Province, and to warn the epidemic trend.
5.Photodynamic therapy with intralesional injection of aminolevulinic acid in a rat model of acneiform inflammatory nodules: evaluation of therapeutic efficacy and histopathological changes

Objective:To assess the therapeutic efficacy and histopathological effect of photodynamic therapy (PDT) with intralesional injection of aminolevulinic acid (ALA) in a rat model of acneiform inflammatory nodules.Methods:Forty specific pathogen-free (SPF) SD rats were randomly and equally divided into normal control group, model control group, ALA injection group and topical ALA group. Rats in the normal control group received no treatment, and those in the other 3 groups were inoculated with Propionibacterium acne suspension on the right auricle for the establishment of a rat model of acneiform inflammatory nodules. After successful modeling, rats in the model control group received no other treatment, those in the ALA injection group were intranodularly injected with 5% ALA followed by red light irradiation, and those in the topical ALA group were topically treated with 5% ALA on the acneiform inflammatory nodules followed by red light irradiation. The treatment was performed once a week for 2 weeks. Twenty-four hours after the last treatment, general appearance and histopathological changes of rat ears were observed in each group, the thickness of rat auricles was measured, and liver and kidney functions were evaluated. Statistical analysis was carried out by using one-way analysis of variance and least significant difference- t test for comparisons among the groups. Results:The thickness of rat auricles significantly differed among the normal control group, model control group, topical ALA group and ALA injection group (0.435 ± 0.006, 1.269 ± 0.071, 1.088 ± 0.098, 0.699 ± 0.095 mm, respectively, F = 235.60, P < 0.001) , and was significantly higher in the model control group than in the normal control group, topical ALA group and ALA injection group ( t = 24.18, 5.24, 16.48 respectively, all P < 0.01) , but significantly lower in the ALA injection group than in the topical ALA group ( t = 11.24, P < 0.01) . Compared with the model control group, the topical ALA group and ALA injection group showed decreased degree of local redness and swelling as well as number of nodules on the rat auricle, and decreased quantity of infiltrating inflammatory cells in the dermis and subcutaneous tissues. Compared with the topical ALA group, nodules regressed more markedly in the ALA injection group, and no clumps of inflammatory cells or microabscesses were observed in the ALA injection group. There was no significant difference in levels of alanine aminotransferase, aspartate aminotransferase, creatinine or urea nitrogen among the 4 groups (all P > 0.05) . Conclusion:PDT with intralesional injection of ALA is more effective for the treatment of acneiform inflammatory nodules in rat models than PDT with topical application of ALA.
6.Effects of Kai-Xin Jie-Yu Pill on Neural Plasticity in Rats with Depression

This article was aimed to investigate the function and mechanism of Kai-Xin Jie-Yu (KXJY) pill on depression.The depression model rats were established by chronic unpredictable mild stress and separation.The sucrose preference test and Morris water maze (MWM) were performed.The regulation effects of neural plasticity-related genes including glycogen synthase kinase-3β (GSK-3β),cAMP-response element binding protein (CREB),5-hydroxytryptamine 2C receptors (HTR2C),and vascular endothelial growth factor (VEGF) were determined.The results showed that among depression model rats,KXJY pill significantly increased the sucrose preference and the escape latency of MWM in a certain extent.The neural plasticity-related genes including GSK-3β and HTR2C mRNA were decreased significantly,whereas CREB and VEGF mRNA were increased significantly with the treatment of KXJY pill.It was concluded that KXJY pill had the function of reducing depression-like behavior,which might be mediated by the regulation of GSK-3β,CREB,HTR2C and VEGF.
7.Non-Hodgkin lymphoma with hemophagocytic syndrome as the first manifestation:report of two cases and review of literature

Objective To explore the features of non-Hodgkin lymphoma (NHL) with hemophagocytic syndrome (HPS) as the initial manifestation at diagnosis. Methods The morbidity and treatment of two B-cell NHL patients with HPS as the initial manifestation were retrospectively analyzed, and the literature were reviewed at home and abroad. Results Two B-cell NHL patients with HPS were treated with the HLH-94 protocol to control febrile condition. After then, R-CHOP protocol (dexamethasone substituted for prednisone) was used and their results were good. In reviewing the literature, it was felt that the main sub-type of B-cell lymphoma-associated HPS (B-LAHPS) was diffuse large B-cell lymphoma. However, sub-types of T-cell lymphoma-associated HPS (T-LAHPS) had no significant difference. The incidence of T-LAHPS was significantly higher than that of B-LAHPS in China and South Korea, but there was no significant difference in the Western countries. T-cell lymphomas were more vulnerable to HPS. The median age of B-LAHPS patients at diagnosis was significantly higher than that of T-LAHPS patients. The median survival and overall survival time of B-LAHPS patients were longer than those of T-LAHPS patients by virtue of the application of rituximab. Conclusion B-cell NHL with HPS as the initial manifestation treated with the 2004-HLH plus R-CHOP protocol could achieve good outcomes.
8.A Cohort Study of Invigorating Qi and Dissipating Depression Herbs on Depressive Disorders in the Clinical Efficacy

Objective To evaluate the dynamic efficacy of Invigorating Qi and Dissipating Depression Herbs in the treatment of depressive disorders and its effect on recurrence rate and suicidal thoughts.Methods The prospective cohort study was used to divide the patients into three cohorts according to the different exposure factors(treatment regimens):Western medicine group,integrated traditional Chinese and Western medicine group,and Invigorating Qi and Dissipating Depression Herbs group,and followed up for 2 years,and the treatment effects of each cohort at half a year,one year,one and a half years,and two years were observed respectively,and the evaluation indicators were Montgomery Depression Scale(MADRS),Hamilton Anxiety Scale(HAMA)and Social Deficit Screening Scale(SDSS)reduction rates,and endpoint events(relapse,suicidal ideation)incidence;By comparing the efficacy and endpoint event rate of the three cohorts,the effect of Invigorating Qi and Dissipating Depression Herbs on improving depressive disorders was evaluated.Results A total of 409 patients(67 in the Western medicine group,131 in the integrated traditional Chinese and Western medicine group,and 211 cases in the Invigorating Qi and Dissipating Depression Herbs group)were enrolled,and the rate of score reduction after treatment with MADRS,HAMA and SDSS increased compared with the pre-treatment treatment with the extension of the drug taking time.The difference between the MADRS and SDSS reduction rates in the Invigorating Qi and Dissipating Depression Herbs group on taking the drug for half a year and the SDSS reduction rate on one year of taking the drug were statistically significant compared with the Western medicine group,and there was no significant difference compared with the integrated Chinese and Western medicine group.On one and a half years and two years of medication,the integrated Chinese and Western medicine group had more advantages in MADRS reduction rate than the Invigorating Qi and Dissipating Depression Herbs group.The recurrence rates of depression in the three groups within 2 years were 20.9%,30.5%and 36.0%,and the difference was not statistically significant,and the length of taking medication was used as a protective factor by logistic regression analysis.The incidence rates of suicidal thoughts were 44.8%,19.1%and 17.5%,and the differences were statistically significant,and the exposure factors and income level were the protective factors by logistic regression analysis.Conclusion Invigorating Qi and Dissipating Depression Herbs can significantly improve the depressive state of patients,promote the recovery of social function,reduce the occurrence of long-term suicidal ideation,and appropriately extend the time of taking medicine can increase the long-term efficacy and reduce recurrence.
9.Expression of cyclophilin A/CD147 in carotid atherosclerotic plaque and the intervention of atorvastatin.

OBJECTIVE:
To explore the expression of CyPA and CD147 in rabbit models of vulnerable carotid atherosclerotic plaque and the therapeutic effect of atorvastatin.
METHODS:
Twenty-four male New Zealand rabbits were randomly divided into 3 groups. Eight rabbits were served as a normal diet group (Group A), and the remaining 16 rabbits underwent balloon-induced endothelial injury in the right carotid artery and thereafter were fed on high-cholesterol diet (1% cholesterol) for 12 weeks, then they were divided into 2 groups: a AS group (Group B), an atorvastatin group [Group C, 2.5 mg/(kg.d)]. 4 weeks later, plaque disrupture was triggered by China Russell's viper venom and histamine. Serum levels of TC, TG, LDL-C and HDL-C were measured at different timepoint. The damaged carotid arteries were collected to undergo pathological examination. The macrophage, expression of CyPA and CD147 were detected by immuno-histochemical analysis, and the mRNA levels of CyPA and CD147 were examined by reverse transcription polymerase chain reaction (RT-PCR).
RESULTS:
Compared with the Group A, the serum levels of TC and LDL-c in the Group B and Group C were significantly increased (all P<0.01). Compared with the Group B, the serum levels of TC and LDL-c in the Group C were reduced significantly after atorvastatin intervention for 4 weeks (all P<0.01). The plaques disruption and thrombosis occurred in 4 out of the 6 rabbits in the Group B, while only 1 rabbit demonstrated plaques disruption and thrombosis in the Group C. Compared with the Group B, the levels of CyPA, CD147 and macrophage in carotid atherosclerotic plaque in the Group C were decreased significantly (all P<0.01).
CONCLUSION
The up-regulation of CyPA and CD147 may be involved in pathogenesis of vulnerable carotid atherosclerotic plaque. Atorvastatin could stabilize the plaque through inhibiting the CyPA and CD147 expression.

10.Multi-center retrospective clinical survey of neonatal purulent meningitis in china

Objective To study the clinical characteristics,diagnosis and clinical outcomes of neonatal purulent meningitis in southwest and northwest regions of China.Method There were 5 tertiary hospitals in southwest and northwest regions of China with well capacity of treating neonatal diseases collaborated as the neonatal meningitis network.From January 2010 to December 2014,clinical symptoms,signs and outcomes of cases with the diagnosis of neonatal meningitis were retrospectively studied.Result During the study period,a total of 63 514 neonates were admitted to 5 neonatal departments in Chongqing and Shanxi Province,China,and 407 cases were diagnosed of bacterial meningitis.255 (62.7%) cases were males.The mean gestational age was (37.1 ±3.2) weeks.125 (30.7%) were preterm infants and69 late-preterm infants.The mean birth weight was (2 859 ± 771) g,90 (22.1%) cases were low birth weight infant.The incidence of neonatal purulent meningitis were 0.64% (From 2010 to 2014,0.30%,0.72%,0.64%,0.89% and 1.12% respectively),and 77.6% of the cases were the late-onset infection.The mortality rate were 9.1% (From 2010 to 2014,6.3%,4.0%,9.8%,8.4% and 8.5% respectively).The incidence of neurological sequelae was 5.9% (From 2010 to 2014,12.5%,8.0%,6.1%,6.3% and 11.6% respectively).The mean duration of hospital stay was 21 days.Mean duration of antibiotic treatment was 19 days.The remission rate was 85.0% (346/407).Conclusion From 2010 to 2014,on average,the incidence of neonatal purulent meningitis was 0.64%,mortality rate was 9.1%,neurological sequelae was 5.9%,and remission rate was 85.0%.Among these years the mortality and neurological sequelae of neonatal purulent meningitis were not decreasing.
