1.Oral health status of 4-17-year-old orphan children and adolescents of Chongqing.

OBJECTIVETo examine the oral health status among the orphan children and adolescents of Chongqing and assist in planning of the oral health programs.
METHODSAccording to the third national oral health investigation of epidemiology, the dental caries, gingival bleeding and calculus was examined among 317 orphan living in Chongqing, by cluster sampling. The statistical software SPSS 17.0 was used for the data analysis.
RESULTSIn primary and permanent teeth, the prevalence of dental caries and mean DMFT (dmft) were found to be 50.00%, 1.94 +/- 2.81 and 39.53%, 0.90 +/- 1.38. There were no significant difference between female and male (P>0.05). But there was significant difference of the prevalence of permanent tooth caries between the age group under 12 (include 12) and above 12 (P < 0.05). It was also found that 35.25% of the orphan children and adolescents had caries in the first permanent molar, and there was significant difference between female and male (P < 0.05). Neither of the caries teeth above was treated nor the healthy teeth were sealed. The prevalence rate of gingival bleeding was 78.22%, and the calculus rate was 67.66%.
CONCLUSIONThe oral status of the orphan is poor, and this community has experienced a low utilization of preventive or therapeutic oral health services. In the future work, orphans should be considered as a priority group when make plans for oral health care.

2.The evaluation of on-line hemodiafiltration in acute cerebrovascular disease patients complicated with hyperosmolar nonketotic diabetic coma

Objective To access evaluate the efficacy and safety of on-line hemodiafiltration for treating a-cute cerebrovascular disease (ACVD) patients complicated with hyperosmolar nonketotie diabetic coma (HNDC).Method Totally 11 patients of ACVD complicated with HNDC were observed for this prospective control study.All of them underwent on-line hemodiafiltrafion for 90 minutes using Fressnius 4008S Hemodialysis Machine withONLILNEplus TM and F60 one hour after final diagnosis was made. The bicarbonate ultrafiltrate rate was set at 500ml/min and blood flow at 150 ~ 180 ml/min; and the substitute fluid were infused with post-dilution at 50 ~ 60ml/min. The symptom of brain edema and cardiac insufficiency during the same course of treatment was observed.Blood were taken from the patients to detect serum kalium, serum glucose, serum natrium, BUN and plasma os-motic pressure at one hour before treatment and six hours after treatment, respectively. The change of in conscious-ness and adverse effects were evaluated at 24 hours after treatment. Results All patients were treated successful-ly. The blood glucose, serum natrium, serum kalium, BUN and plasma osmotic pressure were decreased after on-line hemodiafiltration. Among the 11 patients, consciousness was improved obviously in 8 patients, 3 patientsdied, accounting for 73% of successful effectiveness. Conclusions The on-line hemodiafiltration was effectiveand safe for treating ACVD patients complicated with HNDC.
3.The relationship between post-traumatic stress disorder and self psychology elastic in children

ObjectiveTo discuss the relationship between post-traumatic stress disorder(PTSD) and self psychologieal elastic for and to provide the scientific basis early for PTSD and psychological intervention.Methods 592 students were selected with the method of random cluster sampling to be investigated with children self psychological elastic scale and post-traumatic stress disorder and screening of questionnaire.ResultsThere were 45 students were PTSD screening positive group,accounting for 7.60% of the total number,and 15 male students,accounting for 2.53% of the total number,there were 30 female students,accounting for 5.07% of the total number.Male and female in psychological elastic scale score were no significant differences (P> 0.05 ),PTSD positive group psychological screening elastic total cent and factors points were significantly lower than negative group(P< 0.05),PTSD total score was a significantly negative correlation( r =- 0.454,P =0.000).ConclusionPsychology resilience can be used as predictors of incidence of PTSD.
4.Expression and significance of matrix metalloproteinases and their tissue inhibitors in the infiltration and metastasis of lung cancer.

BACKGROUNDTo determine the expression of matrix metalloproteinases (MMPs) and tissue inhibitors of metalloproteinases (TIMPs) in lung cancer specimens, and to find out the possible roles of MMPs and TIMPs in the infiltration and metastasis of lung cancer.
METHODSThe expression of MMP-1, MMP-2, MMP-9, MMP-13, TIMP-1, TIMP-2 were detected in 104 lung cancer tissues by immunohistochemical method.
RESULTSThe expressions of MMPs and TIMPs were up-regulated in lung cancer tissues. The expression of MMP-2 was related to differentiated degree of tumor cells. MMP-9 correlated with lymph node metastasis of lung cancer. The positive rate of TIMP-1 was related to TNM stage and lymph node metastasis. In lung cancer tissues, there was positive correlation between MMP-2 and TIMP-2, and between MMP-9 and TIMP-2.
CONCLUSIONSMMPs and TIMPs may play the important roles in the development of lung cancer. MMP-9 and TIMP-1 might promote the infiltration and metastasis of lung cancer.
5.Relationship between the expression of E-cadherin and MVD and its significance in lung cancer.

BACKGROUNDTo study the relationship between the expression of E-cadherin and MVD in lung cancer and its significance.
METHODSThe expressions of E-cadherin and factor VIII were detected in 104 lung cancer tissues by immunohistochemical method, and MVD was calculated by image analysis system.
RESULTSThe expression of E-cadherin was significantly related to the differentiation of lung cancer (P < 0.05). A negative correlation was observed between E-cadherin expression and MVD in lung cancer tissues (P= 0.047).
CONCLUSIONSDownexpression of E-cadherin and increase of MVD may play an important role in the invasion and metastasis of lung cancer, and may also be used as a useful marker for tumor prognosis.
6.The clinical characteristics of 346 patients with IgG4-related disease

Objective To analyze the clinical characteristics of IgG4-related disease (IgG4-RD)so as to improve the understanding of IgG4-RD in China.Methods IgG4-RD patients were recruited from Peking Union Medical College Hospital between January 2011 and January 2016.All patients were followedup for more than 6 months.The demographic characteristics,symptoms,organ involvements,laboratory examinations and treatment efficacy were evaluated and analyzed.Results A total of 346 patients were finally enrolled,including 230 males (66.5%) and 116 females (33.5%).The mean age of disease onset was (53.8 ± 14.2) years old.The mostly common involved organs were lymph nodes (56.4%) and submandibular glands (52.6%).Other affected organs and manifestations included:swelling of the lacrimal glands (46.5%),autoimmune pancreatitis (38.4%),pulmonary involvement (28.0%),sclerosing cholangitis (25.4%),naso-sinusitis (23.4%),parotid gland swelling (21.7%),retroperitoneal fibrosis (19.9%),large arteries involvement (9.5%),kidney involvement (obstructive nephropathy caused by retroperitoneal fibrosis was excluded) (6.9%),skin lesions (6.4%).Rare features consisted of thyroid glands,pituitary glands,gastrointestinal tract,pachymeningitis,pericardium,sclerosing mediastinitis and orchitis.The majority of patients had multi-organ involvement,such as 74.3% patients with 3 and more,18.2% and 7.5% patients with 2 and single organ involvement respectively.The average IgG4-RD responder index (IgG4-RD RI) was 13.21 ±5.70.History of allergy was found in 172 (49.7%) patients.As to the laboratory tests,elevated serum IgG4 levels were confirmed in 285 (94.1%) patients,which was positively correlated with IgG4-RD RI.There were 33.5% patients receiving monotherapy of glucocorticoid,52.6% treated with glucocorticoids combined with immunosuppressive agents,4.9% patients with immunosuppressant only,and 9.0% patients with mild disease not receiving medication.The majority (336,97.1%) patients improved the above regimens.Conclusion IgG4-RD is a systemic fibro-inflammatory disease with multiple organ involvement.The mostly common involved organs include lymph node,submandibular glands,and pancreas.Glucocorticoids and immunosuppressive agents were effective for IgG4-RD.
7.Investigation on mental health status of front-line anti-epidemic medical staff during the COVID-19 outbreak

ObjectiveTo investigate the mental health status of the front-line anti-epidemic medical staff during the COVID-19 outbreak, so as to provide references for the targeted psychological intervention and improvement of mental health status. MethodsA total of 162 front-line medical staff who worked in Lanzhou Heavy Particles Hospital of Gansu Province from October to December 2021 were were enrolled, and assessed using self-designed general information questionnaire, Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS). ResultsA total of 144 medical staff completed the valid questionnaire survey, and 17 (11.81%) and 19 (13.19%) cases were found to have anxiety and depression, respectively. The detection rate of anxiety yielded statistical difference among medical staff with different anti-epidemic working hours (χ2=10.602, P<0.01), and the detection rate of depression demonstrated statistical differences among medical staff with different occupations, family history of depression and anti-epidemic working hours (χ2=5.486, 4.039, 7.526, P<0.05). ConclusionDuring the COVID-19 outbreak, front-line medical staff develop varying degrees of anxiety and depression. Furthermore, the detection rate of anxiety is relatively high among the staff with long anti-epidemic working hours, and the detection rate of depression is relatively high among the doctors, the staff with family history of depression and long anti-epidemic working hours.
8.Can urine albumin/creatinine ratio replace 24 hours urinary albumin?

Objective To analyze the correlation between urinary albumin/creatinine ratio (ACR) and 24-hour urinary microalbumin (UMA) and evaluate the predictive value of ARC for early diabetic nephropathy.Methods A total of 368 patients with type 2 diabetes mellitus were retrospectively collected.Early diabetic nephropathy was defined as 24h UMA 30~<300 mg/24h.The correlation between ACR and 24hUMA,and the area under the receiver operating characteristic (ROC) curve of ACR in diagnosis of early diabetic nephropathy were calculated.Gender,age,course of disease,fasting venous blood glucose,glycosylated hemoglobin,blood pressure,triglyceride and total cholesterol were used as adjusting variables to establish univariate and multivariate logistic models of ACR for early diabetic nephropathy,respectively.A regression model was used to evaluate the diagnostic value of ACR for early diabetic nephropathy.Results The correlation between ACR and 24h UMA was 0.658.The area under ROC curve of ACR for early diabetic nephropathy was 0.907 before and 0.933 after adjustments of gender,age,course of disease,fasting venous blood glucose,glycosylated hemoglobin,blood pressure,triglyceride and total cholesterol,respectively.The OR value of ACR of diabetic nephropathy was 2.016 before and 2.762 after same adjustments.The calibration of Hosmer-Lemeshow chi-square test evaluation model was 19.362 before (P=0.13) and 14.928 after adjustments (P=0.061).Conclusion ACR is a better predictor for early diabetic nephropathy although its value is influenced by gender,age,course of disease,blood sugar,lipid,and blood pressure.
