1.From Bench to Clinic: the Potential of Therapeutic Targeting of the IL-22 Signaling Pathway in Atopic Dermatitis.

Atopic dermatitis (AD) is the most common pruritic inflammatory skin disease characterized by thickening of epidermis and dermis as well as by the infiltration of multiple pathogenic polarized T lymphocytes, including Th2, Th17, and Th22 cells. Significant progress has been made to develop targeted therapeutics for treating AD, e.g., Food and Drug Administration-approved dupilumab, an antibody for dual targeting of IL-4 and IL-13 signaling pathways. Additionally, a growing body of published evidence and a promising result from the early stage of the clinical trial with ILV-094, an anti-IL-22 antibody, strongly support the notion that IL-22 is a potential therapeutic target for treating AD. Moreover, we also experimentally proved that IL-22 contributes to the pathophysiology of AD by employing a murine model of AD induced by epicutaneous sensitization. Here, we review recent preclinical and clinical findings that have advanced our understanding of the roles of IL-22 and Th22 cells in skin inflammation. We conclude that blockade of IL-22 signaling may be a promising therapeutic approach for the treatment of AD.

As the incidence of early gastric cancer increases, laparoscopic surgery has become one of the treatments of choice for gastric cancer. With the increase of laparoscopic surgery, the chance of discovering aberrant anatomy during the operation also increases. We present a case of laparoscopic total gastrectomy in gastric cancer patients with intestinal malrotation. Intestinal malrotation occurs in one in every 500 births. We found that laparoscopic total gastrectomy in such patients can be performed successfully when it is performed with a proper Roux limb orientation through an alternative minilaparotomy.

5.Evaluation of Epilepsy-Related Information on the Korean Web Sites.

BACKGROUND: The internet has a great amount of medical information for patients, but because no standards exist regarding the publication of medical literature on the internet, the quality of the internet information is quite variable. The purpose of our study was to assess the content and ethics of information on epilepsy, which is available on the Korean internet. METHODS: The search word "epilepsy" or "seizure" was entered into the five most commonly utilized Korean internet search engines. The web sites were divided into the higher or lower rank site on the basis of popularity ranking, and then evaluated by using both ethic and content value scales. The comparison between two groups was analyzed. RESULTS: Twenty eight epilepsy web sites were identified and divided into two equal groups. The average scores of ethic and content value scales were below half of the total score in the higher rank group as well as the lower. The score of higher rank group was not different statistically to that of lower rank group in both value scales. In addition, the score in the epilepsy specialists group was not higher than that in the non-specialists as well. DISCUSSION: The internet is easily accessible source of medical information including epilepsy. Moreover, because lots of epileptic patients are using the web for obtaining the information, the web information should be standardized. The results of our study show that clinicians should not recommend the epilepsy web site as a source of information for epileptic patients unless the epilepsy websites have been evaluated and assessed.

6.Atypical presentation of complex regional pain syndrome: neuropathic itching - A case report -

Background:
In some patients with neuropathic pain (NP), such as complex regional pain syndrome (CRPS), itching rather than pain is the main symptom making diagnosis and treatment difficult.Case: We report a case of a 23-year-old man with a history of hypoxic brain damage who presented with pruritus of the left foot and ankle. His left foot was fractured, and he underwent surgery 6 months previously. After the operation and cast application, he developed uncontrolled pruritus, swelling, sweating, and flushing of the left foot skin with limping. On examination, he showed well-known features of CRPS without pain. He was diagnosed with an atypical CRPS with neuropathic itching (NI). With treatment modalities used for NP and CRPS, his pruritus subsided gradually, and the his ankle mobility improved.
Conclusions
Unexplained itching can be the main symptom in some CRPS patients. Treatment according to NP can improve symptoms of NI in CRPS patients.
7.GEE: An Informatics Tool for Gene Expression Data Explore.

OBJECTIVES: Major public high-throughput functional genomic data repositories, including the Gene Expression Omnibus (GEO) and ArrayExpress have rapidly expanded. As a result, a large number of diverse high-throughput functional genomic data retrieval systems have been developed. However, high-throughput functional genomic data retrieval remains challenging. METHODS: We developed Gene Expression data Explore (GEE), the first powerful, flexible web and mobile search application for searching whole-genome epigenetic data and microarray data in public databases, such as GEO and ArrayExpress. RESULTS: GEE provides an elaborate, convenient interface of query generation competences not available via various high-throughput functional genomic data retrieval systems, including GEO, ArrayExpress, and Atlas. In particular, GEE provides a suitable query generator using eVOC, the Experimental Factor Ontology (EFO), which is well represented with a variety of high-throughput functional genomic data experimental conditions. In addition, GEE provides an experimental design query constructor (EDQC), which provides elaborate retrieval filter conditions when the user designs real experiments. CONCLUSIONS: The web version of GEE is available at http://www.snubi.org/software/gee, and its app version is available from the Apple App Store.

Wernicke's encephalopathy is an acute neurological deterioration due to a reversible brain lesion caused by thiamine deficiency. Most of the affected patients are thiamine-depleted alcoholics, and the condition usually involves the medial thalami, mammillary bodies, and periaqueductal area. However, there are rare reports of lesions in the cerebellum and cerebral cortex, especially in patients with nonalcoholic Wernicke's encephalopathy. We report a case of nonalcoholic Wernicke's encephalopathy involving atypical diffuse cortical regions, and review previously reported cases.

9.Analysis of Risk Factors for Recurrent Ischemic Stroke: Based on Data of Outpatient Clinic in an University Hospital.

BACKGROUND: Risk factors for recurrent stroke have been evaluated in a few community-based epidemiologic studies. This study aims to estimate the risk factors of recurrent ischemic stroke. METHODS: All patients enrolled as acute ischemic stroke patients from Jan. 1998 to Dec. 2000, and who had visited out-patient clinics at regular intervals with a good compliance were included as subjects. Subjects with poor compliance and those expired were excluded. Subjects with recurrent ischemic stroke were selected according to the diagnostic criteria for recurrent ischemic stroke. Their demographic characterisics (age, sex), risk factors of stroke (hypertension, diabetes mellitus, hyperlipidemia, cardiac disease, transient ischemic attack, smoking, alcohol), and types of stroke (TOAST classification) were analyzed to identify the factors responsible for recurrent ischemic stroke. RESULTS: Among 599 patients with ischemic stroke, 43 patients (7.2%) were identified as having recurrent stroke (27 men and 16 women; mean age=66.3 years). Hypertension and hyperlipidemia were the risk factors which were statistically significant in inducing recurrent ischemic stroke. According to the TOAST classification, cardioembolism was more prevalent in recurrent ischemic stroke. CONCLUSIONS: Hypertension, hyperlipidemia, cardioembolism plays a significant role in recurrent ischemic stroke. Therefore, the control of these risk factors appears to be important for reducing further recurrent ischemic stroke.

Purpose:
The incidence and prognostic implications of atrial fibrillation (AF) in patients with severe aortic stenosis (AS) undergoing transcatheter aortic valve implantation (TAVI) are controversial, especially for Korean patients. Furthermore, the pattern of antithrombotic therapy for these patients is unknown. The present study sought to identify the impact of AF on Korean patients undergoing TAVI and demonstrate the status of antithrombotic therapy for these patients.
Materials and Methods:
A total of 660 patients who underwent TAVI for severe AS were recruited from the nationwide K-TAVI registry in Korea. The enrolled patients were stratified into sinus rhythm (SR) and AF groups. The primary endpoint was all-cause death at 1-year.
Results:
AF was recorded in 135 patients [pre-existing AF 108 (16.4%) and new-onset AF 27 (4.1%)]. The rate of all-cause death at 1 year was significantly higher in patients with AF than in those with SR [16.2% vs. 6.4%, adjusted hazard ratio (HR): 2.207, 95% confidence interval (CI): 1.182–4.120, p=0.013], regardless of the onset timing of AF. The rate of new pacemaker insertion at 1 year was also significantly higher in patients with AF than in those with SR (14.0% vs. 5.5%, adjusted HR: 3.137, 95%CI: 1.621–6.071, p=0.001).Among AF patients, substantial number of patients received the combination of multiple antithrombotic agents (77.8%), and the most common combination was that of aspirin and clopidogrel (38.1%).
Conclusion
AF was an independent predictor of 1-year mortality and new pacemaker insertion in Korean patients undergoing TAVI.
