1.EFFECT OF GYNOSTEMMA PENTAPHYLLUM ON THE LIFE SPAN OF FRUIT FLY

The influence of Gynostemma pentaphyllum extract ( G. P ) on the life span of fruit fly was observed. G. P (0.5% and 1.0%) significantly increased mean life span by 11.8% and 12.2%, respectiyely, when fed to male adults.At 0.25% concentraction, the female adults were found to yield an increase of 24.1% in mean life span and the male an increase of 18.5%.Rearing flics on 0. 5% G. P during both the developmental and adult stages proved to be even more effective than just feeding during the adult stage. The mean life span of male and female adults were increased by 53.3% and 46.1% , respectively, but at the higher concen-tration of 1.0%, the increase was 14.4% and 9.7%, respectively,G. P obviously improved the growth of larvae when given aconcentration of 0.5%,No marked change was observed in the copulatory rate of fruitflies after the treatment of 0.5% G.P.
2.RAPD analysis of the genetic diversity in WHBE rabbit, Japanese white rabbit and New Zealand white rabbit

Objective To analyze the genetic variation among white hair black eyes (WHBE) rabbit, Japanese white ( JW) rabbit and New Zealand white ( NZW) rabbit using random amplified polymorphic DNA ( RAPD) technique . Methods Thirty rabbits (male/female 1∶1) of each strain were used in this study.The genomic DNA was extracted from 90 rabbits.Sixty arbitrary primers were used to amplify DNA of rabbits with RAPD-PCR method.Based on the preliminary experiments , polymorphic primers were selected to analyze the genetic variation among the three rabbit strains .The experi-mental data were analyzed using Popgene 3.2 software.Results (1) Twenty-five polymorphic primers were selected among 60 arbitrary primers.493 amplified fragments were detected ranging from 100 bp to 1800 bp.Sixteen primers among 25 arbitrary primers could not only amplify the common DNA bands of 3 rabbit breeds , but also amplify particular alleles in the WHBE rabbit.(2) 234 RAPD sites were detected by agarose gel electrophoresis in WHBE rabbit , among which 166 sites were polymorphic , accounting for 70.94%.228 RAPD sites were detected by agarose gel electrophoresis in the JW rabbit, while 122 sites of them were polymorphic , accounting for 53.51%.231 RAPD sites were detected by agarose gel e-lectrophoresis in the NZW rabbits , with 94 sites being polymorphic, accounting for 40.69%.(3) The Shannon genetic di-versity index of WHBE rabbit, JW rabbit and NZW rabbit was 0.3385, 0.2222 and 0.1905, respectively.(4) The genet-ic similarity between JW rabbit and NZW rabbit was highest among the three rabbit breeds (0.8443), followed by that be-tween WHBE rabbit and JW rabbit (0.8204), and the genetic similarity between WHBE rabbit and NZW rabbit (0.7862) was the lowest .Conclusions Our results demonstrate that there are both genetic similarities and genetic variations among WHBE rabbit, JW rabbit and NZW rabbit .The RAPD technique can be used to delect the genetic relationships among dif-ferent breeds and different individuals of the same breed of rabbits .
3.Functional changes of dendritic cells in the WHBE rabbits with allergic rhinitis induced by ovalbumin

Objective To observe and compare the function of peripheral blood derived dendritic cells (DC) in white hair black eyes (WHBE) rabbits and Japanese white (JW) rabbits with allergic rhinitis (AR) induced by ovalbumin (OVA),and to explore the mechanism of sensitivity to allergen in WHBE rabbits.Methods For the AR induction,rabbits were sensitized intraperitoneally everyday with OVA emulsified in Al(OH)3 followed from day 17 onward by 5 times nasal challenges with OVA in each nostril.General symptoms and histopathological changes of the nasal mucosa were observed.Expressions of CD86 on cell surface and antigen uptake of peripheral blood-derived dendritic cells were detected by flow cytometry at 6 days of culture.The mannose receptor (MR) mRNA expression was tested by real-time PCR.Proliferation of CFSE [5-(and 6-)-carboxyfluorescein diacetate succinimidyl ester]-labelled T cells stimulated by DC were observed by flow cytometry.Results The rabbits sensitized by OVA showed typical AR symptoms and pathological changes.Expressions of CD86 on the cell surface of dendritic cells in WHBE rabbits with AR were significantly upregulated not only compared with the normal control (NC) rabbits,but also with the JW rabbits with AR (P<0.01).The result of real-time PCR assay showed that MR mRNA expression of DC in the NC group of WHBE rabbits were significantly higher than that of the JWrabbits(P<0.01).Moreover,MR mRNA expression of DCs in the WHBE rabbits with AR were not only significantly higher than that in the NC rabbits (P<0.05),but also higher than that in the JW rabbits with AR (P<0.05).Meanwhile,OVA647 internalization percentages of DCs in the WHBE rabbits with AR were not only significantly higher than that in the NC rabbits,but also obviously higher than that in the JW rabbits with AR (P<0.01).Conclusions The sensitivity of WHBE rabbits to allergen may largely depend on the function of dendritic cells with high expression of mannose receptor and their strong ability of maturation and antigen uptake.
4.Prefabricated maxillofacial and excessively expanded cervical skin flap for aesthetic unit reconstruction in the cheek

Objective To explore a method for repairing the appearance and function of the cheek aesthetic unit.Methods A single volume of 100 ml to 150 ml of tissue expander was implanted at the lower edge of the injury area on the cheek.The first injection was performed 5 days after the operation and twice a week after the injection.It took an average of about 4 months to complete the expansion,with 3-5 times over expanded.The lesions were resurfaced with the expanded flaps,and long term follow-up was observed for flap survival.Results 17 cases of cheek lesions had been successfully reconstructed,with the color,texture of the expanded flaps well matched to the surrounding skin after 3-12 months follow-up.The facial expression functions and configurations were satisfied.Conclusions Excessive expansion of the prefabricated skin flap of the jaw and neck can repair the cheek aesthetic unit successfully,which is a practical method and meets the needs of aesthetic unit repair.
5.The establishment of diagnosis related grouping model and grouping tool in hospitals of Shanghai

Based on the Australian AR-DRGs, this research gathered the diagnosis related information on the first page of medical records and accomplished the computerization of data collection and analysis through the data-base of Shanghai Shenkang Hospital Development Center.According to the clinical practices, the DRGs model has been adjusted to complete the localization and a severity-based-DRGs model and grouping tool have been established for the municipal hospitals in Shanghai.
6.Design and application of the hospital performance assessment indicators based on disease se-verity

The performance evaluation is an effective method to guide the public hospitals behavior and pro-mote their sustainable development.The introduction of disease severity could make the performance evaluation sys-tem more scientific and reasonable, and encourage the hospitals to improve their technical skills and services ability. Based on the Shanghai diagnosis related groups system, a case-mix index ( CMI) was introduced and a severity score of disease was developed after the relative weight ( RW) adjustment to evaluate different types of disease severity in hospitals.The results have shown that CMI effectively reflects the patients'disease severity and the intensity of treat-ment in those hospitals taken into consideration.Therefore, the scientific indicators should be selected and the evalu-ation system should be improved thereby establishing an effective monitoring and dynamic adjustment mechanism and further developing the diagnosis related group application.
7.Predictive value of ischemia-modified albumin level in secondary cerebral infarction after transient ischemic attack in anterior circulation

Objective To predict a value of ischemia modified albumin (IMA) levels for assessing secondary cerebral infarction in patients with transient ischemic attack (TIA) in anterior circulation.Methods 105 patients with TIA in anterior circulation admitted to the hospital within 3 hours were retrospectively studied.Combined with ABCD2 score,the correlations of IMA levels at 3 h,6 h and 12 h with secondary cerebral infarction after anterior circulation TIA were analyzed.Results IMA level was 75.28 u/L within 3h after TIA,and the sensitivity and specificity of TIA in anterior circulation were 66.7% and 76.2% respectively.In the total of 105 patients,16 cases (15.2%) suffered from secondary cerebral infarction within 7d,and 21 cases (20.0%) within 8~30d.The serum IMA levels were (87.43±19.89)U/L,(63.88±12.51)U/L and (61.21±12.28)U/L at 3h,6h and 12h after TIA,respectively.A simple analysis showed that there was a linear correlation between the IMA level and ABCD2 scores (P=0.000,r=0.666).Kaplan-Meier survival curve analysis showed that the increased IMA level within 3h,and moderate to high ABCD2 score were the risk factors for secondary cerebral infarction after TIA in anterior circulation (P=0.012,0.041).Conclusions Early detection of IMA has a clinical value similar to ABCD2 score to predict secondary cerebral infarction in patients with TIA in anterior circulation.
8.Percutaneous translumiaal angioplasty for the treatment of thromboangitis obliterans

Objective To investigate the value of endovascular surgery for the treatment of thromboangitis obliterans (TAO). Methods Sixteen patients (18 limbs) with thromboangitis obliterans were treated by percutaneous transluminal plasty (PTA). Results Angiographic success on the limb basis was achieved in 15 of 18 treated limbs and the initial technical success rate was 83.34%. Doppler anklebrachial index (ABI) increased from 0.33±0.16 to 0.79±0.23 one week after PTA. The follow-up time was 2~24 months, the average time was 10.84 months. The 3-month, 12-month accumulated primary patency rate were 81.33% and 60. 23% respectively. Conclusion PTA is effective primary invasive treatment for thromboangitis obliterans yielding acceptable primary clinical success with a low complication rate and resulting in moderate long-term clinical patency and a high limb salvage rate.
9.A comparative study on transilluminated powered mini-phlebectomy (TriVexTM) and pointed phlebectomy for the treatment of varicosis of the great saphenous vein of the lower limbs

ObjectiveTo compare the therapeutic effects of a new surgical endoscopic technique, the transilluminated powered phlebectomy (TriVex System, Smith+Nephew) and pointed phlebectomy in the treatment of varicosis of the great saphenous vein of the lower limbs. Methods Thirty-nine patients (46 limbs) received TriVex operations under spinal, or epidural anesthesia. The powered vein resector and an irrigated illuminator device-a minimally invasive system was used for varicose vein surgery. During the same period, 41 patients (46 limbs) underwent pointed phlebectomy. Results The average postoperative hospital stay was 4.6 days in patients receiving TriVex, and 8.1 days for pointed phlebectomy. Compared with the pointed phlebectomy, the incision of TriVex procedure was shorter and the number less. Conclusion This new surgical device is easy to operate, minimally invasive, efficacious and time saving with satisfactory results.
10.Comparison of operative risks in the infrarenal aortic artery reconstruction between abdominal aortic aneurysm and aortoiliac occlusive disease

Objective To compare operative risks of the infrarenal aortic artery reconstruction between abdominal aortic aneurysm (AAA) and aortoiliac occlusive disease (AIOD), and to analyze the causes of the difference. Method Clinical data of 340 cases undergoing aortic artery reconstructions including 222 AAA cases and 118 AIOD cases , were analyzed retrospectively. Results The perioperative mortality was 7.6% and the mortality in AAA group (5.4%) was lower than that in AIOD group (11.9%)(P
