1.An Application Study on the Cognitive-Behavioral Group Therapy in Medical Students with Social Anxiety

Objective:To explore the effects of cognitive-behavioral group therapy on relieving social anxiety of medical students and thus improve the communication ability of medical students. Methods: The medical students with social anxiety in our school were randomly divided into two groups. The subjects in the control group and the experiment group were given with the routine mental health education and the cognitive-behavioral group therapy respectively. The cognitive, emotional, and behavioral levels were measured before and after intervention using Interaction Anxious-ness Scale ( IAS ) , Fear of Negative Evaluation Scale ( FNE ) , Social Avoidance and Distress Scale ( SAD) , and in order to evaluate the effects of cognitive-behavioral group therapy. Results:After the intervention of cognitive-behavioral group therapy in medical students, all the three levels of anxiety were im-proved significantly in the experimental group than those in the control group ( P<0 . 05 ) . Conclusion:Cognitive-behavioral group therapy could effectively decrease the level of social anxiety for medical students.

3.Arthroscopic rotator cuff repair with double-row allograft bone suture anchor

Objective To discuss the value of double-row allograft bone suture anchor in repair of rotator cuff injury. Methods Under assistance of arthroseope, 16 cases of rotator cuff lesions were repaired by using double-row allograft bone suture anchors. There were 10 males and 6 females at average age of 44 years (32-58 years). According to Bigliani classification, there were 6 cases of small ruptures, 8 medial ruptures and 2 large ruptures. After rotator cuff was trimmed and acromial bone was re-shaped under arthroscopy, 2 bone anchors attached to suture lines were implanted in the attachment point of rotate cuff of humerus greater tuberosity side by side. Then, the 4 ends of the 2 suture lines were vertically sutured through tendon of supraspinatus mnscle and tied separately, crossing the 4 ends of 2 knots and penetrated each 2 ends into one bone anchor. Finally, these 2 bone anchors were embedded into 2 predrilled bone holes which were parallel to the former 2 anchors and lower in humerus greater tuberosity. Thus, a web was formed and overlapped on the ruptured rotator cuff. The number of bone anchors was according to the size of rupture. Results All cases were followed up for average 14 months (6-23 months). The mean UCLA score was (21.30±5.6) points preoperatively and (32.4±5.8) points postoperatively. The function of shoulders recovered well, with no pain, recurrence or pulling out of bone anchor. Function was excellent in 10 cases, good in 5 and fair in 1. Conclusions As for repair of rotator cuff injury, double-row bone-anchors under arthroscope can provide a tendon-bone interface better suited for biologic healing of rotator cuff. Bone suture anchor can not leave any foreign object residual and costs less.
4.Detection of secondary infections of acute necrotizing pancreatitis: a comparison study of 99mTc-ciprofloxacin scintigraphy and CT

Objective To evaluate 99mTc-ciprofloxacin (Infecton) scintigraphy as a method for detecting secondary infections associated with ANP in swine,in comparison with CT.MethodsTwenty-eight healthy swine were randomly assigned to control group (n =6),non-infected ANP (n =6) and infected ANP group( n =16).ANP model was induced by retrograde injection of sodium taurocholate and pancreatic protease mixture into the biliary and pancreatic duct.Two days after ANP induction,swine in infected ANP group were injected with 3 x 108 E.coli into pancreatic tissue,while swine in non-infected ANP group were injected with inactivated E.coli.At 7 d after inoculation,at 0.5,1,2,3,4,and 6 h after intravenous administration of 370 MBq of Infecton,SPECT scan was performed.Then 64-slice spiral CT scan was performed.Then swine were sacrificed,and histopathology examination and bacterial culture of pancreatic tissue were performed.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of the two methods to detect secondary infections were determined.ResultsThere were no abnormality in the normal pancreas and the bacterial culture was negative.There were pancreatic necrosis in the non-infected ANP group,but the bacterial culture was negative.There were pancreatic necrosis and infection in the infected ANP group and the bacterial culture was positive.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of the Infecton method were 93.8% ( 15/16 ),91.7% ( 11/12 ),92.9% ( 26/28 ),93.8 %(15/16) and 91.7% ( 11/12),whereas these values for CT were 12.5% (2/16),100.0% ( 12/12),50.0%(14/28),100.0% (2/2) and 46.2% (12/26),respectively.The sensitivity,accuracy,and negative predictive value of the Infecton method were significantly higher than those in CT group (P <0.01 ).ConclusionsInfecton scintigraphy may be a better procedure for detecting ANP secondary infections than CT.
5.Evaluation of Sterile Effect of Arthroscope Surgical Instruments Using Low-temperature Plasma Sterilizer

OBJECTIVE To evaluate the feasibility and sterile effect of low-temperature plasma sterilizer produced by Chinese Academy of Sciences for the sterilization of arthroscope surgical instruments.METHODS To explore the cleaning,package,arrangement and sterile effect monitoring of instruments in the process of arthroscope sterilization using low-temperature plasma sterilizer.RESULTS In the sterilization process of arthroscope surgical instruments using low-temperature plasma sterilizer,the chemical indicators and indicating patch were discolored,the monitoring of bio-box and the sterilization monitoring of surgical instruments were all negative.CONCLUSIONS Using the low-temperature plasma sterilizer machine for the sterilization of endoscope surgical instruments can ensure the security of sterilization,make less injury of precise instruments,shorten the sterilization time,accelerate the surgical turnover,enhance the frequency of surgical instruments application,and increase the efficiency.
6.Information needs and influencing factors of cardiac rehabilitation in patients undergoing cryobaiioon ablation for atrial fibrillation

Objective:To explore the changes in cardiac rehabilitation information needs of patients undergoing cryobaiioon ablation (CBA) for atrial fibrillation, analyze the influencing factors of different trajectory categories, so as to provide reference for rehabilitation guidance.Methods:From January 2022 to April 2023, convenience sampling was used to select patients who were planning to undergo CBA in the Cardiovascular Department at the First Affiliated Hospital of Xi'an Jiaotong University as participants. Patients were surveyed using the Information Needs in Cardiac Rehabilitation Scale at one day before surgery (T0), one month after surgery (T1), three months after surgery (T2), and six months after surgery (T3) .Results:A total of 257 questionnaires were distributed, and 218 valid questionnaires were collected. The total scores of the Information Needs in Cardiac Rehabilitation Scale for 218 patients at T0, T1, T2, and T3 were (187.86±35.28), (183.27±34.98), (174.03±34.01), and (167.35±33.96), respectively. There was a statistically significant difference in the total scores of the Information Needs in Cardiac Rehabilitation Scale among 218 patients at different time points ( F=18.237, P<0.01). Four potential trajectory categories were fitted, namely the sustained medium high need group (40.83%, 89/218), the medium high need-decline group (36.70%, 80/218), the medium low need-improvement group (7.34%, 16/218), and the medium low need-decline group (15.14%, 33/218). Age, whether to merge underlying chronic diseases, place of residence, social support, and postoperative complications were potential trajectory factors affecting the cardiac rehabilitation information needs of atrial fibrillation patients ( P<0.05) . Conclusions:Medical and nursing staff should assess the development trajectory of patients' cardiac rehabilitation information needs as early as possible, emphasize the importance of cardiac rehabilitation, establish remote and controllable cardiac rehabilitation programs, encourage rural residents to participate in local community related cardiac rehabilitation activities, mobilize patients' social support resources, and improve their needs and compliance with cardiac rehabilitation.
7.Construction and psychometric evaluation of core competency evaluation scale for nurses in Cardiology Department

Objective To construct the core competency evaluation scale for nurses in Department of Cardiology, and evaluate its psychometric characteristics. Methods The core competency evaluation scale for nurses in Department of Cardiology was developed by literatures review and clinical situation. A total of 176 nurses in 5 class Ⅲ grade A hospitals in Shaanxi Province were selected by convenient sampling method and investigated anonymously. Results The choice rates of all alternatives were lower than 80%. One item was deleted because its coefficient of variation was less than 20%. Discrimination correlation coefficients were between 0.340 - 0.732. The content validity index was 0.928. Five factors were extracted by exploratory factor analysis, with the cumulative contribution of 67.6%. Five factors were expected. The factor analysis confirmatory showed that χ2/df=4.013, SRMR=0.051, RMSEA=0.062. The split half reliability was 0.842; Cronbach's alpha was 0.856, and the retest reliability was 0.908. Conclusions The core competency evaluation scale for nurses in department of cardiology has good validity and reliability which can be used to evaluate the core competency.
8.Preparation of Al 18F-PSMA-11 and its preliminary PET/CT imaging

Objective:To optimize the preparation conditions and methods of Al 18F-prostate specific membrane antigen (PSMA)-11 and evaluate the feasibility of clinical transformation. Methods:PSMA- N, N′-bis(2-hydroxy-5-(carboxyethy)benzyl)ethylenediamine- N, N′-diacetic acid (HBED-CC) dissolved in CH 3COONH 4 buffer (pH=4.8) was reacted with AlCl 3·3H 2O dissolved in pure water at a molar ratio of 1∶1 (60 ℃, 10 min), and then purified by tC18 column and freeze-dried to obtain [Al]-PSMA-11. [Al]-PSMA-11 was labeled by 18F - and the effects of reaction temperature and pH value on the labeling rate were investigated. The labeled products were purified by tC18 column and filtered through sterile filter to obtain Al 18F-PSMA-11. The comparison of biodistribution between Al 18F-PSMA-11 and 68Ga-PSMA-11 was analyzed on 5 healthy volunteers (age (56±8) years). The differences of SUV max between two groups were analyzed by independent-sample t test. Besides, the early and delayed imaging of Al 18F-PSMA-11 PET/CT were performed on a patient (70 years old) with recurrent prostate cancer for assessment of its potential for prostate cancer recurrence monitoring. Results:The labeling rate was (42.3±3.2)% reacting in aqueous phase (60 ℃, pH=4.8) for 15 min. After being purified with tC18 cartridge, the radiochemical purity of the product was still more than 95% after placement at room temperature for 3 h. Preliminary application demonstrated that there was no significant difference in the biodistribution of Al 18F-PSMA-11 and 68Ga-PSMA-11 among lacrimal gland, parotid gland, submandibular gland, liver, spleen, kidney, bladder and part of intestine and SUV max of targeted organs were also not different ( t values: 0.19-1.95, all P>0.05) between two groups. Multiple bone metastases were observed by Al 18F-PSMA-11 PET/CT delayed imaging (3 h) in a patient with recurrent prostate cancer. Conclusion:Al 18F-PSMA-11 produced with pre-conjugated [Al]-PSMA-11 meets the requirement of the PET imaging application, and it has good potential of localization and imaging for prostate cancer metastatic lesions.
9.Case report of children with Addison′s disease complicated by torsades de pointes

This article retrospectively analyzes the clinical data of a patient with Addison′s disease complicated by torsades de pointes treated in Children′s Hospital Affiliated to Xi′an Jiaotong University in July 2021.The patient, female, aged 12 years, was hospitalized multiple times due to recurrent seizures, syncope, and coma, and had been successively diagnosed with fulminant myocarditis, supraventricular tachycardia, etc.She was later transferred to Children′s Hospital Affiliated to Xi′an Jiaotong University, where during hospitalization, electrocardiogram (ECG) monitoring revealed torsades de pointes associated with the attacks.The ECG between attacks showed a prolonged Q-T interval, with the longest Q-Tc of 564 ms.Echocardiography suggested a slight enlargement of the left ventricle and reduced left ventricular ejection fraction.After a comprehensive examination, she was diagnosed with Addison′s disease.Treatment with intravenous Hydrocortisone for 3 days, followed by oral Hydrocortisone tablets, was administered sequentially.After treatment, symptoms such as syncope did not recur, and the Q-T interval gradually returned to normal.After continued treatment for 1 year, echocardiography revealed no abnormality.This report aims to enhance pediatricians′ understanding and research on the relationship between pediatric endocrine diseases and arrhythmias.
10.Beta-sitosterol improves cerebral ischemia-reperfusion injury in rats by inhibiting endoplasmic reticulum stress

Objective:To reveal the effect of β-sitosterol on cerebral ischemia-reperfusion injury (CIRI) in rats and whether its mechanism is related to endoplasmic reticulum stress (ERS).Methods:Fifty-three CIRI rats (CIRI models established by modified Longa method) were randomly divided into model group ( n=14), β-sitosterol low-dose group ( n=13), β-sitosterol medium-dose group ( n=13) and β-sitosterol high-dose group ( n=13); 12 rats underwent the same operation without blocking the middle cerebral artery were selected as sham-operated group. Rats in the sham-operated group and model group were given intragastric administration of 1 mL 5 g/L sodium carboxymethyl cellulose daily. Rats in the β-sitosterol low-dose group, β-sitosterol medium-dose group and β-sitosterol high-dose group were given intragastric administration of 1 mL β-sitosterol at 10, 20 and 40 mg/kg/d (dissolved in 5 g/L sodium carboxymethyl cellulose), respectively, for 14 consecutive d. Neurological function was evaluated according to Zea Longa 5 method. Rats were sacrificed and brain tissues were collected. Volume of cerebral infarction was measured by 2,3,5-triphenyl tetrazolium chloride (TTC) staining. Brain injury and neuronal apoptosis were evaluated by HE staining, Nissl staining and TUNEL. Superoxide dismutase (SOD), glutathione peroxidase (GSH-Px) and malondialdehyde (MDA) contents were detected by water-soluble tetrazolium 1 (WST-1) method, colorimetric method or thiobarbituric acid (TBA) method, respectively. The mRNA and protein expression levels of protein kinase R-like endoplasmic reticulum kinase (PERK), inositol-requiring enzyme-1 (IRE-1), activated transcription factor-6 (ATF-6), glucose-regulated protein 78 (GRP78), C/EBP homologous protein (CHOP) and Caspase-12 in the brain tissues were detected by qRT-PCR or Western blotting. Results:Compared with the sham-operated group, the model group had significantly increased neurological function score, cerebral infarction volume and TUNEL positive rate, decreased SOD and GSH-Px content, increased MDA content, and increased mRNA and protein expressions of PERK, IRE-1, ATF-6, GRP78, CHOP and Caspase-12 ( P<0.05). Compared with the model group, the β-sitosterol low-dose group, β-sitosterol medium-dose group and β-sitosterol high-dose group had significantly decreased neurological function score, cerebral infarction volume, and TUNEL positive rate, increased SOD and GSH-Px content, and decreased MDA content ( P<0.05); the β-sitosterol low-dose group, β-sitosterol medium-dose group and β-sitosterol high-dose group had significantly decreased mRNA and protein PERK expressions (mRNA: 2.17±0.17, 1.79±0.07 and 1.33±0.07; protein: 5.11±0.52, 2.91±0.26 and 1.98±0.17), IRE-1 expressions (mRNA: 1.75±0.18, 1.65±0.08 and 1.32±0.08; protein: 5.00±0.31, 4.05±0.27 and 1.98±0.14), ATF-6 expressions (mRNA: 2.24±0.12, 1.77±0.14 and 1.37±0.13; protein: 4.93±0.45, 4.04±0.30 and 3.10±0.20), GRP78 expressions (mRNA: 2.67±0.16, 2.11±0.16 and 1.69±0.11; protein: 5.02±0.38, 2.97±0.26 and 2.05±0.22), CHOP expressions (mRNA: 2.01±0.16, 1.70±0.19 and 1.40±0.10; protein: 4.92±0.39, 4.02±0.27 and 3.08±0.22) and Caspase-12 expressions (mRNA: 1.85±0.09, 1.61±0.09 and 1.30±0.09; protein: 3.03±0.20, 2.19±0.11 and 1.82±0.11) compared with the model group (mRNA: 2.99±0.28, 2.27±0.12, 2.57±0.21, 3.46±0.20, 2.50±0.23 and 2.35±0.16; protein: 6.98±0.48, 6.03±0.58, 5.98±0.63, 7.10±0.45, 6.00±0.53 and 5.02±0.43, P<0.05). Conclusion:β-sitosterol attenuates CIRI in rats, whose mechanism may be related to inhibition of ERS signal pathway.
