1.The effects and mechanisms of Forsythia suspense on the expression of Foxp3 on splenocytes and level of Treg in peripheral blood in severely burnt rats

AIM: To explore the immunity modulation function of aqueous of Forsythia suspense (AFS) and its possible mechanisms. METHODS: Rats of burned model group were burned with vapor under 3mpa pressure and 108℃ temperature for 8 seconds to achieve deep partialthickness bum, to make a thirty percent total body surface area (TBSA)bum. The experiment were divided into five groups: Control group: without any treatment; 8 PBH group: 8 h after burn; the rats of AFS1 guoup, AFS2 group and AFS3 group of them were given AFS 5 g/kg, 2.5 g/kg, 1.25 g/kg once a day by Po. pathway for seven days before burns, respectively. Rats were sacrificed before and 8h after burn, The percentage of Treg cells in CD4~+ T cells was detected by flow cytometry(FCM) ; the expression of Foxp3 mRNA on splenocytes were measured by RT-PCR, and the protein of Foxp3 activity was evaluated by immunohistochemistry staining. RESULTS: Compared with Control group, the expression of Foxp3mRNA and protein on the splenocytes were upregulated markedly(P <0.01), and the percentage of Treg were significantly increased (P < 0.01) in the 8PBH group. AFS1, AFS2 and AFS3 significantly attenuated these increases (P < 0.01), which was dose-dependent. CONCLUSION: AFS has immunity modulation function and mechanism of it is corrected with Foxp3 mRNA on splenocytes.
2.Design and application of modified multifunctional dressing bag with glauber's salt for relieving swelling and pain in early patients with limb fracture

Objective:To explore the effect of modified multifunctional dressing bag with glauber's salt for relieving swelling and pain in early patients with limb fracture.Methods:A total of 111 patients with limb fracture in Tongde Hospital of Zhejiang Province were divided into two groups according to the admission time. The control group consisted of 53 patients with limbs fracture using common mirabilite bag with glauber′s salt and ice bag for external application admitted from January to August 2018, and the observation group consisted of 58 patients with limbs fracture using modified multifunctional dressing bag with glauber′s salt admitted from September 2018 to January 2020. The degree of pain, the time of appearance of dermatoglyphia, the cases of severe swelling, waiting time for operation, compliance and comfort of the patients were observed within 72 hours after injury.Results:The degree of pain, the time of appearance of dermatoglyphia, the waiting time, the cases of severe swelling for surgery in the observation group were (1.8±0.7) points, (4.5±1.2) days, (5.7±1.2) days,1 case, respectively, which were all lower than those in the control group (2.2±0.8) points, (6.0±1.5) days, (6.5±1.4) days, 19 cases, the differences between the two groups were statistically significant ( t values were -2.6,-5.6,-3.3, χ2 value was 21.8, all P<0.05). The total compliance rate in the observation group was 82.76% (48/58), higher than that in the control group 56.60% (30/53), the differences were statistically significant ( χ2 value was 9.34, P<0.05). The comfort of the observation group was 82.76% (48/58), significantly higher than that of the control group 3.77% (2/53), the difference was statistically significant ( χ2 value was 69.8, P<0.05). Conclusions:The modified multifunctional dressing bag with glauber′s salt can reduce the swelling of limbs, reduce the appearance of skin blisters, stop bleeding and pain, and shorten the waiting time for operation, at the same time, it improves the compliance and comfort of the patients.
3.Expressions and clinical significances of Eag1 and VEGF in colorectal cancer tissues

Objective To study the expressions of Eag1 and vascular endothelial growth factor (VEGF) in colorectal cancer,and to explore the relationships between Eagl and VEGF expressions and clinicopathologic features of colorectal cancer.The correlation between Eag1 and VEGF and their clinical significance in colorectal cancer were explored.Methods A total of 73 cases of colorectal cancer tissues,25 cases of colorectal adenoma and 10 cases of normal colorectal mucosa from January 2015 to December 2016 in Hunan Provincial People's Hospital were collected.The expressions of Eag1,VEGF protein and mRNA in 73 cases of colorectal cancer tissues,25 cases of colorectal adenoma and 10 cases of normal colorectal mucosa were detected by immunohistochemistry and real-time fluorescence quantitative polymerase chain reaction (RT-qPCR).The relationships between the two factors and the clinical pathological features of patients with colorectal cancer were analyzed.Results Immunohistochemistry showed that the positive rate of Eag1 in colorectal cancer group was 86.30% (63/73),which was significantly higher than that in colorectal adenoma group (20.00%,5/25) and normal colorectal mucosa group (10.00%,1/10),with significant differences (x2 =38.539,P <0.001;x2 =24.874,P < 0.001).The positive rate of VEGF in colorectal cancer group was 89.04% (65/73),which was significantly higher thah that in colorectal adenoma group (56.00%,14/25) and normal colorectal mucosa group (30.00%,3/10),with significant differences (x2 =10.980,P <0.001;x2 =16.911,P <0.001).There was a significant positive correlation between Eag1 and VEGF in colorectal cancer and colorectal adenoma tissues (c =0.580,P =0.010;c =0.669,P < 0.001).The results of RT-qPCR showed that the expressions of Eag1 and VEGF mRNA in colorectal cancer group,colorectal adenoma group and normal colorectal mucosa group were 4.50 ±0.34,3.14 ±0.44;1.42 ±0.22,1.61 ±0.15 and 1.00 ±0.16,1.00 ±0.53,respectively,and the differences were statistically significant (F =15.546,P =0.014;F =5.363,P =0.025).The relative expression levels of Eag1 and VEGF mRNA in colorectal cancer group was about 4.50 times (t =31.851,P =0.003) and 3.14 times (t =6.870,P =0.014) than those in normal colorectal mucosa group.The mRNA expression levels of Eag1 and VEGF in colorectal cancer were correlated with degree of differentiation (F =840.725,P < 0.001;F =102.950,P < 0.001),depth of invasion (t =4.754,P < 0.001;t =8.557,P <0.001),Duke staging (F =179.902,P < 0.001;F =55.911,P < 0.001) and lymph node metastasis (t =20.337,P < 0.001;t =25.218,P < 0.001).But they were not related to age (t =0.196,P =0.845;t =0.534,P =0.595) and sex (t =0.409,P =0.684;t =1.078,P =0.285).Conclusion Eag1 and VEGF are highly expressed in colorectal cancer tissues with positive correlation,and are closely related to the degree of differentiation,depth of invasion,Duke staging and lymph node metastasis of colorectal cancer.Both of them play important roles in the occurrence and development of colorectal cancer,which is expected to provide favorable research directions to the early diagnosis,treatment and prognosis of colorectal cancer.
4.Effectiveness and safety of low-dose alteplase in elderly patients with acute ischemic stroke

Objective To investigate the effectiveness and safety of intravenous thrombolytic therapy with low-dose alteplase for elderly patients with acute ischemic stroke.Methods The elderly patients with acute ischemic stroke were enrolled prospectively (onset within 4.5 h,aged ≥75 years).They were randomly divided into either a low-dose group (0.6 mg/kg) or a standard-dose group (0.9 mg/kg).The primary outcome was the modified Rankin Scale (mRS) score determined the good outcome (mRS score 0-2) and poor outcome (mRS score >2) at 90 d after treatment;the secondary outcome was intracranial hemorrhage and symptomatic intracranial hemorrhage at 24 h after thrombolysis,and death within 90 d.Results A total of 65 elderly patients with acute ischemic stroke were enrolled,including 32 in the low-dose group and 33 in the standard-dose group.There were no significant differences in all baseline clinical data between the two groups.Compared with before treatment,the proportions of patients with the mRS score 0-2 were significantly higher in both groups at 7 d,14 d,and 90 d after treatment,but there were no significant differences in good outcome rate at the same time point after treatment between the two groups.In addition,there were no significant differences in intracranial hemorrhage (36.4% vs.25.0%;x2 =0.985,P =0.321),symptomatic intracerebral hemorrhage (21.2% vs.9.4%;x2 =1.749,P =0.186) at 24 h after treatment,and incidence of death within 90 d after treatment (12.1% vs.6.3%;x2 =0.151,P=0.697)between the two groups.Conclusions The effectiveness and safety of intravenous thrombolytic therapy with low-dose alteplase (0.6 mg/kg) in elderly patients with acute ischemic stroke is equivalent to the standard dose.
5.Can we early diagnose metabolic syndrome using brachial-ankle pulse wave velocity in community population?

BACKGROUNDThe prevalence of metabolic syndrome (MetS) increased recently and there was still not a screening index to predict MetS. The aim of this study was to estimate whether brachial-ankle pulse wave velocity (baPWV), a novel marker for systemic arterial stiffness, could predict MetS in Chinese community population.
METHODSA total of 2 191 participants were recruited and underwent medical examination including 1 455 men and 756 women from June 2011 to January 2012. MetS was diagnosed according to the criteria of the International Diabetes Federation (IDF). Multiple Logistic regressions were conducted to explore the risk factors of MetS. Receiver operating characteristic (ROC) curve was performed to estimate the ideal diagnostic cutoff point of baPWV to predict MetS.
RESULTSThe mean age was (45.35±8.27) years old. In multiple Logistic regression analysis, the gender, baPWV and smoking status were risk factors to MetS after adjusting age, gender, baPWV, walk time and sleeping time. The prevalence of MetS was 17.48% in 30-year age population in Shanghai. There were significant differences (χ(2) = 96.46, P < 0.05) between male and female participants on MetS prevalence. According to the ROC analyses, the ideal cutoff point of baPWV was 1 358.50 cm/s (AUC = 60.20%) to predict MetS among male group and 1 350.00 cm/s (AUC = 70.90%) among female group.
CONCLUSIONBaPWV may be considered as a screening marker to predict MetS in community Chinese population and the diagnostic value of 1 350.00 cm/s was more significant for the female group.

6. Endovascular thrombectomy after intravenous recombinant tissue plasminogen activator (bridging therapy) for embolic stroke due to cardiac myxoma: a case report

Myxomas are the most frequent, cardiac benign cardiac tumors which often present with stroke caused by tumorous orthrombotic emboli. The treatment of embolic stroke due to cardiac myxoma is still a clinical and technical challenge. A 61-year-old man who had an embolic stroke in the left middle cerebral artery was admitted to the Third Poeple′s Hospital of Huizhou. The initial National Institutes of Health Stroke Scale (NIHSS) score was 16. He received endovascular thrombectomy after intravenous recombinant tissue plasminogen activator (rt-PA) one hour after stroke onset. No intracranial hemorrhage developed. Pathological study of embolus showed a myxoma. A cardiac mass was found in the left atrium and removed surgically three weeks after stroke. Pathological study of the tumor showed a myxoma. At the one-month follow-up after excision of myxoma, the NIHSS score was 1 and the modified Rankin scale score was 0. No recurrence of embolism occurred after surgical resection. Endovascular thrombectomy after intravenous rt-PA (bridging therapy) for embolic stroke due to cardiac myxoma is safe and effective, and early resection of atrial myxoma can effectively avoid recurrence of cerebral infarction.
