1.Indirect Evaluation of Sarcoplasmic Reticulum Ca~(2+) Release Function in Skeletal Muscle Strips

Objective To set up a simple method to measure the SR Ca2+ release function in skeletal muscle strips.Methods The time from peak tension to 75 % relaxation(TR75) during intermittent tetanic contraction(fatigue contraction) of isolated musculus soleus strips was analyzed.It was prolonged rapidly,and then shortened slowly.The ratio of maximal TR75 to initial TR75(R-TR75) indicated a balance between SR Ca2+ release and uptake.If the SR Ca2+-ATPase(SERCA) activity and inhibition extent of SERCA in fatigue contraction were identical,R-TR75 should be an index for SR Ca2+ release function.Results Higher stimulation voltage or 5 mmol/L caffeine perfusion which increased the SR Ca2+ release channel open probability induced an increase of R-TR75 from 2.5 in control to 3.0.On the contrary,inhibition of SR Ca2+ release with 5 mmol/L magnesium sulfate perfusion significantly decreased R-TR75.Inhibition of SR Ca2+ release by metabolites during fatigue contraction was recovered slowly.So R-TR75 at the second fatigued contraction was reduced after 5 or 10 min of recovery,but no significant difference after 60 min of recovery.During intermittent tetanic contraction R-TR75 was increased to be 2.9 times of normal in unloaded soleus.Conclusion The above results suggest that R-TR75 during muscle strip fatigue contraction without any change of SERCA activity can indirectly reflected SR Ca2+ release function.In 2-week unloaded soleus,the enhanced R-TR75 indicates an increase in SR Ca2+ release function.
2.The Effect of Dezocine Analgesia on Cognitive Function of Patients after Gynecological Laparoscopic Surgery

Objective To discuss the application of Mini Mental State Scale (the mini-mental state examination MMSE) test in assess the effect of dezocine analgesia therapy on the cognitive function of patients with agitation in recovery period of general anesthesia after gynecological laparoscopic surgery.Methods We observed 210 cases of elective gynecologic laparoscopic surgery in our hospital from January 2015 to December 2015,ASA grade Ⅰ or Ⅱ.All patients were transrered to PACU (ICU) after extubation,and were divided into three groups:A,B,C group,each with 70 cases in each group.Patients with severe agitation in A group were timely given 10mg of dezocine,patients with mild agitation in B group were given 10mg of dezocine after a delay of 5 ~ 10 minutes,and patients without agitation in C group were not given dezocine.Then the MMSE test was performed to evaluate the cognition scores of patients in the three groups.The MMSE scores immediately,15 minutes,30 minutes,60 minutes after given dezocine and 24 hours after operation were recorded.Results The statistical results showed that there were significant differences in the MMSE scores of patients immediately and at 30 minutes after given dezocine between three groups (P<0.05);and there was no significant difference in the MMSE scores of patients at 60 minutes after given dezocine and 24 hours after operation between three groups (P >0.05).Conclusion Dezocine analgesia therapy has no effect on the cognitive function of patients with agitation in recovery period of general anesthesia after gynecological laparoscopic surgery.Dezocine can be safely used for analgesia and sedation treatment for patients with agitation in recovery period of general anesthesia after gynecological laparoscopic surgery analgesia,sedation agitation.
3.Surveillance of bacterial resistance in Tongling People′s Hospital during 2013

Objective To investigate the antimicrobial resistance of clinical isolates in Tongling People′s Hospital during 2013. Methods A total of 2 281 nonduplicate clinical isolates were collected.Kirby-Bauer disc diffusion method was employed to study the antimicrobial susceptibility.The data were analyzed with WHONET 5.6 software according to CLSI 2012 breakpoints. Results The top 5 most frequently isolated microorganisms were E.coli (479,21.0%),K.pneumoniae (360,15.8%),A. baumannii (271,11.9%),P .aeruginosa (240,10.5%),S.aureus (171,7.5%).Gram negative and gram positive microorganisms accounted for 76.5% and 23.5%,respectively.The prevalence of methicillin-resistant strains in S.aureus (MRSA)and coagulase negative Staphylococcus (MRCNS)was 38.6% and 73.1%,respectively.The resistance rates of MR strains to beta-lactams and other antimicrobial agents were much higher than those of MS strains.No staphylococcal strain was found resistant to vancomycin or teicoplanin.E.faecalis showed relatively lower resistance to penicillin,ampicillin and nitrofurantoin.E.faecium strains were more resistant than E.faecalis to most of the antibiotics tested.Approximately 50.5% of E.coli and 44.5% of Klebsiella isolates produced extended-spectrum beta-lactamases (ESBLs).The ESBLs-respectively.And 29.8% and 23.4% of the P .aeruginosa strains were resistant to imipenem and meropenem.Nearly all (94.0%)P .aeruginosa isolates were susceptible to amikacin.Conclusions There appears a trend of increasing resistance in the clinical bacterial isolates in this hospital,especially the carbapenem-resistant Enterobacteriaceae,which is of great concern.It is mandatory to take effective antibiotic policy and infection control measures.
4.The Development of Mandarin Monosyllable Lexical Neighborhood Test

Objective Base on the neighborhood activation model(NAM), to develop the Mandarin monosyllable lexical neighborhood test(M-LNT-monosyllable), which is expected to be useful for evaluating speech perception performance in children with hearing loss. Methods Test items were based on the lexical characteristics of word frequency and neighborhood density which addressed in the neighborhood activation model (NAM). M-LNT -monosyllable consisted of two parts: Lexically "easy" words with high word frequency, which were low phonemically similar to the target word and lexically "hard" words with low word frequency, which were high phonemically similar to the target word. 34 children of 3~5 year old with normal hearing were choosed as subjects to verify easy word and hard word lists. Results 1 979 words for children contained 487 easy words and 419 hard words. Three easy word lists and three hard word lists were developed to estimate the performance of word recognition among normal- hearing children. There were no differences among scores of three easy words lists(P>0.05), and no difference among scores of three hard words lists(P>0.05). But there were significant differences between scores of easy and hard words lists(P<0.01). Conclusion The development of the lexicon was affected by the lexical characteristics. Normal-hearing children with some lexical techniques were affected by the lexical characteristics when they recognized the spoken words, but children with less lexical technique didn't show the same result because the recognition was processed on the phonetic level.
5.Application effect of homemade visible stomach tube guiding device in patients with gastric canal implantation under general anesthesia

ObjectiveTo evaluate the application effect of homemade visible stomach tube guiding device in patients with gastric canal implantation under general anesthesia.MethodsFrom January to December 2015,90 patients who would undergo gastrointestinal or hepatobiliary surgery under general anesthesia with indwelling gastric tube were selected as the research object by purpose sampling method. 90 cases were divided into group A (conventional catheter),group B (guidewire guided) and group C (homemade visible stomach tube guiding device guided),with 30 cases in each group. All patients received gastric canal implantation immediately after endotracheal intubation under general anesthesia. We recorded the time of one-time successful gastric tube implantation,the times of implantation,the number of successful cases that implanted at a time,the total number of cases with successful implantation and the number of cases with throat mucosal injury.ResultsThe intubation time of group A and group B were longer than that of group C (P<0.05). The time in group A was the longest. Only 10 cases in group A were successful implanted in one time,19 cases in group B were successfully implanted in one time,and all the cases in group C were successfully implanted in one time. The one-time success rate of gastric canal implantation of group C (100.0%) was higher than that of group A (33.3%) and group B (63.3%) (P<0.05). The total success rate of gastric canal implantation of group A (53.3%) was lower than that of group B (90.0%) and group C (100%) (P<0.05). The incidence of throat mucosal injury of group C (3.3%) was lower than that of group A (33.3%) and group B (26.7%) (P<0.05).ConclusionsThe homemade visible stomach tube guiding device can quickly improve the one-time success rate of gastric canal implantation in patients under general anesthesia,reduce the time of implantation and decrease the incidence of throat mucosal injury.
6.Effect of psychological nursing on anxiety of patients undergoing coronary artery bypass surgery

Objective? To explore the effect of psychological counseling on postoperative anxiety in patients with coronary artery bypass grafting (CABG). Methods? A total of 58 patients with CABG admitted to Beijing Anzhen Hospital Affiliated to Capital Medical University from April 2016 to July 2017 were selected by convenience sampling method. All the research subjects were divided into observation group and control group with 29 cases in each by random number table method. The patients in the control group were given routine perioperative nursing, while the patients in the observation group were given psychological nursing intervention. The anxiety level of the patients in the two groups before and after operation was evaluated by Hamilton Anxiety Scale (HAMA). Results? There was no statistical significance difference of HAMA scores of the two groups before operation (P> 0.05). After operation, the difference was statistically significant in HAMA score (P<0.05). Conclusions? Psychological intervention in CABG patients can effectively reduce the mortality of the patients and relieve the postoperative anxiety of the patients.
7.Application of intensive blood glucose monitoring in blood glucose management of patients after coronary artery bypass grafting

Objective:To investigate the effect of intensive blood glucose monitoring and nursing program on blood glucose variability of patients transferred to general wards after coronary artery bypass grafting (CABG) .Methods:A convenient sampling method was used to select 106 patients with coronary heart disease and hyperglycemia who were transferred from the ICU within 72 hours after CABG from Beijing Anzhen Hospital from June to December 2018. They were divided into control group (54 cases) and research group (52 cases) according to the random number table method. The control group was given routine nursing care, and the research group adopted a comprehensive nursing strategy including intensive blood glucose monitoring based on the control group. The average daily blood glucose level (MGL) , standard blood glucose standard deviation ( SD) , daily average blood glucose fluctuation range (MAGE) , and blood glucose variation coefficient ( CV) of the two groups were compared within 3 days after CABG patients were transferred to the general ward. Results:After the patients were transferred from the ICU to the general ward, there were time effects, inter-group effects, and time-group interaction effects in SD; inter-group effects in SD; time- and inter-group effects in MAGE; and inter-group effects and time and group in CV interaction effect. Conclusions:Within 1 to 3 days after the patient is transferred from the ICU to the general ward, compared with the conventional nursing group, continued intensive blood glucose monitoring and nursing can reduce the degree of blood glucose variation.
8.Risk assessment and influencing factors of pressure injury in patients transferred from ICU after coronary artery bypass grafting

Objective:To investigate the occurrence and influencing factors of high risk of pressure injury (PI) in patients transferred from ICU after coronary artery bypass grafting (CABG) .Methods:From January 2018 to June 2019, the convenient sampling method was used to select 277 patients with coronary heart disease who were transferred from ICU to general ward in Cardiovascular Surgery Department of Beijing Anzhen Hospital and underwent CABG or CABG combined with valve replacement as the research objects. The general information questionnaire, Morse Fall Scale (MFS) , Barthel Index of activities of daily living and Waterlow Score Scale were used to investigate the patients to evaluate the occurrence of high risk of PI. Logistic regression analysis was used to explore the influencing factors of high risk of PI after CABG.Results:A total of 14.4% (40/277) of CABG patients were at high risk of postoperative PI. Multivariate Logistic regression analysis showed that activities of daily living score ( OR=0.951, 95% CI: 0.909-0.995, P=0.031) , age ( OR=1.151, 95% CI: 1.075-1.234, P<0.001) , B-type brain natriuretic peptide ( OR=11.038, 95% CI: 2.263-53.826, P=0.003) , BMI ( OR=1.366, 95% CI: 1.178-1.585, P<0.001) , ejection fraction ( OR=0.938, 95% CI: 0.883-0.998, P=0.042) and operation time ( OR=1.912, 95% CI: 1.137-3.216, P=0.015) were the influencing factors of high risk of PI after CABG. Conclusions:A variety of factors are independently related to the high risk of postoperative PI in CABG patients, and clinical nurses should pay attention to the risk factors and take preventive measures as soon as possible.

10.Effects of hemodialysis information system on work adaptability and engagement in nursing staffs

Objective By measuring the nursing staffs′psychology of work adaptability and engagement before and after the utilization of hemodialysis information system, multidimensional comparative analysis was used to compare the differences of nursing staffs′ psychology between before and after the utilization of hemodialysis information system. Methods To select nursing staffs working in the Sixth People′s Hospital Affiliated Shanghai Jiaotong University, the nursing staffs were investigated by work attitude scale ( WA) and Utrecht work engagement scale ( UWES) before the implementation of hemodialysis information system. The collected data was statistically analyzed by SPSS 19. 0. Results Before the implementation of hemodialysis information system, score of WA was (20. 79 ± 3. 39), and the differences of nursing staffs in different age group had statistical significance (P <0. 01); the differences were statistically significant in WA scores of nursing staffs with different educational background after 4, 6 and 10 weeks implementation of hemodialysis information system (P<0. 05); the differences were no statistically significant in different work experiences between before and after implementation of the system (P>0. 05). The score of UWES was (48. 53 ± 9. 02) before usage of the system;when staffs had different age, education background and length of work experience, the differences had statistical significance (P >0. 05). Conclusions Hemodialysis information system can reduce obstacles of nursing staffs′ work adaptability working in Hemodialysis Room, enhance work engagement and achieve digital, scientific and modernized nursing service, so as to provide a safe and comprehensive quality nursing service for hemodialysis patients.
