1.Epidemiological investigation of a maternal Listeria monocytogenes ST2 infection case

Abstract
On September 26, 2024, a municipal hospital in Jiaxing City reported a maternal case of Listeria monocytogenes infection. In order to clarify the source of infection, the Jiaxing Center for Disease Control and Prevention immediately conducted the epidemiological investigation, laboratory testing and related disposal work. The case presented with fever (37.9 ℃), gradually intensifying paroxysmal abdominal pain without obvious cause, and went to hospital on the day of onset. Due to fetal intrauterine distress, a male infant was delivered by cesarean section on the same day. The epidemiological investigation identified that the case usually consumed fruits, often store fruits such as watermelon and grapes in the refrigerator alongside raw meat, and the refrigerator had never been cleaned or disinfected, posing a risk of cross contamination. Laboratory tests on amniotic fluid sample from the pregnant woman, infant blood sample showed positive results for Listeria monocytogenes infection. One strain of Listeria monocytogenes was detected in a smear sample from the inner wall of the refrigerator, and all the strains were ST2 type. Consuming fruits contaminated with Listeria monocytogenes may be the main source of infection. Food safety education for pregnant women and their family members should be strengthened to reduce the risk of infection.
2.A comprehensive report on the clinical,pathological,and genetic characteristics of Stormorken syndrome attributed to a STIM1 gene mutation,diagnosed in adulthood

To report a case of Stormorken syndrome(STRMK)diagnosed in adulthood and to study its clinical,pathological and genetic characteristics.The individual was a 31-year-old male whose symptoms started from childhood with epilepsy as initial symptom.He gradually developed progressive muscle weakness and atrophy.The patient's clinical data,laboratory results,and imaging studies were collected.A biopsy on the patient's left gastrocnemius muscle was conducted.Simultaneously,whole-exome sequencing on both the patient and his parents was performed.The physical examination of the patient showed short stature,proximal muscle weakness with lower limb predominance,accompanied by joint contracture and scoliosis.Abdominal CT examination revealed asplenia and magnetic resonance image(MRI)showed obvious fat infiltration in the bilateral lower limb muscle.Muscle biopsy was consistent with tubular-aggregate myopathy.Genetic test indicated that the patient had a c.326A>G(p.His109Arg)heterozygous mutation in the stromal interaction molecule 1(STIM1)gene,a known pathogenic mutation.This study further enables neurologists to gain a better understanding of this rare disease.
3.Analysis of the risk of class clustering of acute hemorrhagic conjunctivitis from Xiamen City in 2023

Objective:
To explore the intra class transmission pattern of acute hemorrhagic conjunctivitis (AHC) among students, so as to provide a basis for formulating precise prevention and control strategies.
Methods:
The data of AHC students in 2023 was obtained from the teacher-student health information module of the "Xiamen i-Education" platform. Taking the onset date from January 1st to December 31st, 2023 as the search criteria, the epidemiological data of confirmed AHC cases among students in the whole city in 2023 was collected. And the Knox test method was employed to quantify cluster risk at the class level. Subgroup comparisons were conducted to examine differences in cluster risk between academic stages and gender combinations. Cross analysis was performed to investigate the interactive effects of two characteristics on cluster risk.
Results:
In 2023, 708 cases of AHC students were reported in Xiamen, among which 54 class clustered outbreaks were identified. These outbreaks accounted for 40.54% of total cases. Among them, the incidence of class clustered outbreaks with male first onset cases was higher than that with female first onset cases ( χ 2=4.55, P <0.05). The class cluster risk was at a relatively high level within the incubation period (0-2 d) after AHC onset ( RR =4.61-6.43, P <0.05). Subgroup analysis revealed that female-female case pairs had a higher class cluster risk ( RR = 4.81- 10.29) compared to other gender combinations ( P <0.05). Primary school case pairs showed a higher risk ( RR =4.60-6.77) than middle school case pairs ( RR =3.85-4.57) (both P <0.05). Cross analysis indicated that primary school and female-female combinations had a higher risk ( RR =4.70-11.24) than other combinations, while middle school and male-female combinations showed a positive interaction ( RR =3.68-6.68)(both P <0.05).
Conclusions
AHC infection among students is primarily transmitted within classes. Gender and educational stage are key demographic factors influencing class cluster transmission risks. Transmission risks should be assessed by analyzing population characteristics and contact patterns, and targeted interventions should be implemented.
4.Impact of heat wave on the incidence of foodborne diseases

Objective:
To explore the impact of heat wave on the incidence of foodborne diseases, so as to provide the basis for preventing foodborne diseases in summer.
Methods:
Data on foodborne disease cases from June to September each year in Jiaxing City from 2018 to 2023 were collected through the Zhejiang Provincial Foodborne Disease Surveillance Platform. Meteorological data were obtained from the Jiaxing Meteorological Bureau. Air quality data were collected from the National Urban Air Quality Real-time Release Platform. Heat wave were defined based on temperature thresholds and durations. The impact of heat wave on the incidence of foodborne diseases were analyzed by a time stratified case-crossover design with a distributed lag nonlinear model (DLNM). The cumulative relative risk (CRR) and attributable fraction (AF) were calculated to compare the cumulative lagged risks and disease burdens across different genders, age groups, and disease types.
Results:
From June to September each year between 2018 and 2023, a total of 13 026 foodborne disease cases were reported in Jiaxing City, including 6 646 male cases and 6 380 female cases. Among them, 746 cases were aged <15 years, 10 910 cases were aged 15-65 years, and 1 370 cases were aged >65 years. There were 1 425 cases of infectious diarrhea, 9 956 cases of acute gastroenteritis, and 1 645 cases of other foodborne diseases. The DLNM analysis revealed that heat waves defined in different ways increased the risk of foodborne diseases (all CRR>1, P<0.05), and the risk of disease incidence rose with increasing temperature thresholds and durations. When the daily mean temperature was not lower than the 95th percentile of the daily mean temperature from 2018 to 2023 and lasted for at least 4 days, the Akaike Information Criterion was minimized, and the cumulative lagged risk and disease burden of foodborne diseases were relatively high, with a CRR value of 1.32 (95%CI: 1.09-1.60) and an AF value of 2.81% (95%CI: 1.05%-4.31%). Among them, females, individuals aged 15-65 years, and cases of acute gastroenteritis were more significantly affected by heat wave days, with CRR values of 1.37 (95%CI: 1.12-1.67), 1.35 (95%CI: 1.06-1.73), and 1.35 (95%CI: 1.09-1.66), respectively, and AF values of 3.00% (95%CI: 0.82%-4.73%), 3.08% (95%CI: 1.27%-4.61%), and 3.00% (95%CI: 1.04%-4.64%), respectively.
Conclusions
Heat wave have lagged and cumulative effects on the risk of foodborne disease incidence, increasing both the risk and disease burden. Females, individuals aged 15-65 years, and cases of acute gastroenteritis are more significantly affected by heat wave days.
5.Research progress on postoperative chronic pain in adolescents with idiopathic scoliosis

Spinal correction surgery is a complex procedure with a long operative duration and extensive tissue damage, frequently resulting in chronic postoperative pain. This type of pain imposes significant physical and psychological burdens on patients, as well as considerable economic strain on families and society. This paper provides an overview of the concept, prevalence, assessment tools, influencing factors, and preventive and therapeutic strategies for chronic postoperative pain in adolescents with idiopathic scoliosis. The aim is to offer a reference for managing postoperative chronic pain in these patients and to inform further research in this area.

8.Correlation between preoperative plasma fibrin degradation products level and clinicopathological features in patients with non-small cell lung cancer

Objective:To investigate the relationship between preoperative plasma fibrin degradation products (FDP) level and clinicopathological features of patients with completely resected non-small cell lung cancer (NSCLC).Methods:A retrospective case series study was performed. The clinical data of 521 patients who were pathologically diagnosed with NSCLC in Beijing Friendship Hospital Affiliated to Capital Medical University from January 2016 to December 2017 were retrospectively analyzed. Among 521 cases, 406 cases were postoperatively pathologically confirmed as non-lymph node and non-distant metastasis (non-metastasis group) and 115 cases were postoperatively pathologically confirmed as lymph node or distant metastasis (metastasis group). The preoperative FDP level and clinicopathological characteristics as well as the clinicopathological characteristics of NSCLC patients with different FDP levels were compared between the two groups. The correlation between preoperative FDP level and TNM staging was analyzed by using Spearman correlation analysis.Results:Among 521 NSCLC patients, 266 cases were female, 255 cases were male; the age [ M( Q1, Q3)] was 59 years (54 years, 65 years); 441 cases were adenocarcinoma and 70 cases were squamous cell carcinoma. The preoperative median FDP level was 2.78 mg/L (2.35 mg/L, 3.13 mg/L) and 2.99 mg/L (2.56 mg/L, 4.16 mg/L), respectively of NSCLC patients in non-metastasis group and metastasis group, and the difference was statistically significant ( Z = 6.13, P < 0.001). The preoperative FDP level was 2.56 mg/L (2.35 mg/L, 3.20 mg/L) and 2.99 mg/L (2.56 mg/L, 3.20 mg/L), respectively in the early-stage NSCLC (stage Ⅰ-Ⅱ) and advanced NSCLC (stage Ⅲ-Ⅳ) patients, and the difference was statistically significant ( Z = 8.42, P < 0.001). Spearman correlation analysis showed that preoperative FDP level was positively correlated with tumor diameter ( r = 0.287, P < 0.001). There was a positive correlation between preoperative FDP level and the number of metastatic lymph nodes in 115 patients with lymph node metastasis ( r = 0.679, P < 0.001). According to the preoperative median FDP (2.78 mg/L), all patients were divided into FDP ≤2.78 mg/L group and FDP >2.78 mg/L, and there were statistically significant differences in age, metastasis, tumor staging, tumor diameter, the metastatic number of lymph node and histological types of NSCLC patients in both groups (all P < 0.05). Conclusions:The increase of preoperative plasma FDP level may be related to the tumor metastasis and clinical stage of NSCLC patients
9.Comparison on Quality of Honey Processed Lilii Bulbus Prepared by Traditional Empirical and Pharmacopoeial Methods Based on Chemical Analysis and Sensory Techniques

ObjectiveTo compare the quality of two kinds of honey-processed Lilii Bulbus(HPLB) prepared by empirical and pharmacopoeial methods based on chemical analysis and intelligent sensory technology, and to improve and upgrade the quality standard of HPLB on the basis of excavating and inheriting the traditional experience of famous experts in traditional Chinese medicine(TCM). MethodSamples of HPLB were prepared according to the method in the 2020 edition of Chinese Pharmacopoeia(honey was added before frying) and the empirical method of the old pharmacists(honey was added after frying), and the appearance characteristics, thin layer chromatography(TLC) identification, inspection, leachables and other items of the two kinds of samples were detected according to the methods under the Lilii Bulbus in the 2020 edition of Chinese Pharmacopoeia. The fingerprints of HPLB with different honey addition methods were established by high performance liquid chromatography(HPLC), and the contents of regaloside A, regaloside B and 5-hydroxymethylfurfural(5-HMF) were determined, the quality difference between the two HPLB samples was evaluated by multivariate statistical analysis. Meanwhile, electronic nose and electronic tongue were used to measure the sense of smell and taste, and combined with multivariate statistical analysis, the difference sensors were screened with the variable importance in the projection(VIP) value>1 in order to compare the differences between the two kinds of HPLB. ResultCompared with HPLB prepared by the pharmacopoeial method, the sample prepared by the empirical method had lower water content, slightly higher soluble extract content, better appearance and easy storage. HPLC fingerprints of two kinds of HPLB samples were established, and the similarity between them was≥0.999, indicating that the composition of two species was similar. And 29 common peaks were calibrated, among which peaks 8 and 18 were regaloside A and regaloside B. The quantitative analysis of the index components in the two kinds of HPLB samples showed that the difference in the contents of regaloside A and regaloside B was not statistically significant, whereas the content of 5-HMF in empirical HPLB was significantly higher than that of the pharmacopoeial HPLB(P<0.01). The electronic tongue and electronic nose test results showed that the umami, umami richness and saltiness of the empirical HPLB were higher than those of the pharmacopoeial HPLB. The results of electronic nose detection showed that the four sensors W1W, W2S, W5S and W1S were the differential odor sensors of the two samples, among which the response values of W1S and W2S sensors were higher than those of the pharmacopoeial HPLB, which might be related to the content of 5-HMF. ConclusionCompared with the pharmacopoeial HPLB, the empirical sample is of better quality, but the two can be clearly distinguished by electronic nose and electronic tongue, which can provide a reference for excavating and inheriting the traditional experience of famous experts in TCM and further improving the processing methods and quality standards of HPLB.
10.Value of three-dimensional energy Doppler ultrasound parameters combined with placental growth factor in predicting early-onset fetal growth restriction

Objective To explore the value of three-dimensional energy Doppler ultrasound parameters combined with placental growth factor (PLGF) in predicting early-onset fetal growth restriction (FGR). Methods Eighty pregnant women with early-onset FGR were selected as FGR group, and another 50 healthy pregnant women with prenatal examination were selected as control group. Three-dimensional energy Doppler ultrasonography was performed for all subjects at 11 to 13 weeks plus 6 days of gestation, and placental volume (PV), vascularization index (VI), flow index (FI) and vascularization-flow index (VFI) were collected. Serum PLGF level was measured in all subjects at 14 to 16 weeks plus 6 days of gestation. Results The PV, VI, FI, VFI and serum PLGF level in the FGR group were significantly lower than those in the control group (
