2.Experimental study of a closed-chest pulmonary embolism-reperfusion injury canine model by means of Swan-Ganz catheter

Objective To establish a closed-chest pulmonary embolism-reperfusion animal model by Swan-Ganz catheter and to explore the mechanisms of pulmonary embolism(PE)-reperfusion injury(RI). Methods Experiments were made on 14 mongrel dogs,ranging in weight from 15 to 18 kg,anesthetized with 3% pentobarbital sodium.The dogs were intubated with I.D.7 endotracheal tubes.Under sterile conditions,a 7 F Swan-Ganz catheter via the external jugular vein was positioned in the unilateral pulmonary diaphragmatic lobe(DL)artery.Occlusion/reperfusion of the DL artery was controlled with 1.2 ml diluted contrast agent filled into/drawn from the balloon.After the 24 h PE,the balloon was deflated to result in 4 h reperfusion of the DL.Measurements of blood gases and tumor necrosis factor-?(TNF-?)were made at normal condition,at 24 h PE and at 4 h reperfusion.Thin-section CT scans were performed at normal condition,24 h PE,30 min,1,2,3 and 4 h reperfusion,respectively.At the end of each experiment, tissue specimens of bilateral diaphragmatic lobes were obtained for both wet/dry(W/D)weight ratio and for pathological study.Results Reperfusion pulmonary edema(RPE)was an acute,mixed,noncardiogenic edema that was observed in all 14 dogs who had been successfully established as PE/RI animal models.RPE demonstrated heterogeneous ground-glass opacifications that predominated in the areas distal to the recanalized vessels.It manifested pathologically as an edematous lung infiltrated by inflammatory cells.The mean ofPaO_2 and TNF-? of 4 h reperfusion was(81?4)mm Hg(l mm Hg=0.133 kPa)and(16.0? 2.5)pg/ml,which were significantly different(P

Objective To investigate the technological feasibility,efficacy and morbidity of cr guided ~(125)Ⅰseed implantation for recurrent rectum cancer.Methods Twenty-three patients with recurrent rectum cancer were treated with cr guided interstitial ~(125)Ⅰseed brachytherapy.In 20 patients the procedure was performed under epidural anesthesia and 3 patients under local anesthesia.Treatment planning system was used to calculate the number of seeds,the space distribution and the introduction of the seeding needles.Matched peripheral dose (MPD) of ~(125)Ⅰseed implantation ranged from 90-120 Gy for patients who had had external radiotherapy,and 140- 160Gy for those who had not.The planning target volume(PTV)was clinical target volume(CTV)plus 1 cm margin.The range of radioactivity of the ~(125)Ⅰseeds was 18.5-25.9 MBq.All these 23 patients had CT scan at 5 mm intervals after implantation for quality evaluation,together with routine chest,pelvic X-ray films within 24-48 hours after seed implantation.Three patients received three-dimensioual conformal radiation therapy(3DCRT) to a total dose of45-50 Gy,with 2-3 Gy/f.Follow-up time was from 3 to 28 months.Results All patients was able to tolerate seed implantation well.Complete pain relief was observed in 12/15,and partial relief in 2/15 and no response in 1/15,with a response rate of 93%.The local control rate was 87%.The 1-and 2-year survival rate was 93% and 50% respectively.Two of four patients have died of dissemination to the lung after 8 and 12 months. One seed has migrated into the pelvis without causing any untoward morbidity.Conclusion CT guided ~(125)Ⅰseed implantation for recurrent rectum cancer is safe,minimally invasive,causing only mild morbidity.It possesses a high efficacy,yet it should be given in combination with external beam radiation and chemotherapy,should distant metastasis be observed.

T cell immune receptor with Ig and ITIM domains (TIGIT), a promising new target in cancer immunotherapy, plays a critical role in limiting adaptive and innate immunity against tumors. The extracellular domain of human TIGIT was used to immune BALB/c mice, and a new anti-human TIGIT chimeric antibody (c7D3) was developed. The mice in this study were used in accordance with the international guidelines for the care and use of laboratory animals, and the animal study was approved by the Institutional Animal Ethics Committee of AbMax Biotechnology. The biological activity of c7D3 was studied. The results showed that c7D3 exhibited high affinity for TIGIT and effectively inhibited the interaction between TIGIT and its ligands. Cell-based assays indicated that c7D3 induced strong luciferase signaling in TIGIT/CD155 signaling reporter assay and enhanced cytokine secretion in a T cell stimulation assay. The data showed that c7D3 has high binding affinity and excellent blocking bioactivity, supporting the further advancement for therapeutic application.
6.Short term clinical outcomes of laparoscopic fertility preserving radical hysterectomy in the management of early stage cervical cancer.

OBJECTIVETo investigate the feasibility and effectiveness of laparoscopic radical trachelectomy and lymphadenectomy in the treatment of early-stage cervical cancer.
METHODSThe clinical data of 6 patients (stage 1a2 to 1b1), who underwent laparoscopic fertility-preserving radical operation for cervical cancer in our department from February 2009 to October 2010, were retrospectively analyzed in terms of operation duration, intra-operative blood loss, postoperative pathology, complications, and pregnancy.
RESULTSBoth radical resection of cervical and pelvic lymph node dissection were completed under laparoscopy, and only the cervical and vaginal cuffs were closed from vagina. The operation duration ranged 155-210 min (mean: 185 min) and the intra-operative blood loss was approximately 60-120 ml(mean: 105 ml). The average length of hospital stay was 18 days without complications, postoperative infection, and bleeding. Postoperative pathology showed no lymph node metastasis, and no ligament, blood vessels, vaginal cutting margin, or upper part of cervix was invaded by tumor cells. During the 8-20-month follow-up, 1 patient had become pregnant for 4 months and no case experienced tumor recurrence.
CONCLUSIONLaparoscopic fertility-preserving lymphadenectomy and radical trachelectomy is feasible for patients with early-stage cervical cancer who have strong wish to have a child.

The purpose of this study was to establish a model of trigeminal neuralgia (TN) through an approach from lower edge of cheekbone and to observe the functional changes in the voltage-gated potassium currents in the cultured trigeminal ganglion (TG) neurons. Thirty Sprague-Dawley male rats were divided into two groups, the sham-operated (sham) group and the operated group. The TN model was carried out by using a chronic constriction injury of the infraorbital nerve (ION-CCI) from lower edge of cheekbone. Peripheral pain threshold test and whole-cell patch clamp recording were used to determine the difference between sham and ION-CCI rats. The withdrawal threshold of whisker pad in operated side of ION-CCI rat was decreased significantly from 6 d after operation and then maintained until 21 d, with the lowest on the 15th day. The threshold of whisker pad in non-operated side of operated rats was also decreased significantly compared with that in the sham group. Delayed rectifier potassium current (I(K)) in cultured ION-CCI TG neurons was decreased significantly compared with that in the sham group. Transient outward potassium currents (I(A)) in both operated and non-operated sides of TG neurons from ION-CCI rats were also reduced significantly compared with that in the sham group. The present study provided a new method of ION-CCI. In this model, the decrease of I(A) and I(K) might contribute, at least in part, to the decrease in mechanical pain threshold of whisker pad and the subsequent hyperalgia.

8.Effect of DAPC on Adhesion of Streptococcus Mutans

Objective To analyze the property of the new antibacterial agent (N, N-di-n-decyl-N, N-dimethyl-ammonium 5-oxopyrrolidine-2-carboxylate, DAPC) to prevent Streptococcus mutans' adhesion on tooth surface.Method determine the minimal inhibitory concentration and minimal bactericidal concentration of DAPC and Chlorhexidine gluconate by liquid dilution method.Set the Chlorhexidine gluconate as the positive control, while PBS as the negative control.Use the crystal violet staining to measure the quantity of biofilm.Re s ult The MIC of DAPC on Streptococcus mutans was 0.0031250% and MBC was 0.0062500%. The MIC of Chlorhexidine gluconate was 0.0015625%, while the MBC was 0.0031250%.When concentration of the two antibacterial agents was MIC, the quantity of biofilm have no significance among three groups.However, when concentration of Chlorhexidine gluconate and DAPC were 0.12% and 0.24% respectively, biofilm of experimental group was lower than PBS, and there is no significance between Chlorhexidine gluconate group and DAPC group after 24 hours incubation.Conclus ion New antibacterial agent DAPC have significant effect in inhibiting of Streptococcus mutans.And the residual of DAPC on teeth can maintain a long time of antibacterial effect to inhibit Streptococcus mutans' adhesion.
9.Application of carbon nanoparticles in the laparoscopic sentinel lymph node detection in patients with cervical cancer.

OBJECTIVETo explore the role of laparoscopic sentinel lymph node(SLN) detection with carbon nanoparticles tracer in cervical carcinoma.
METHODSTotally 21 patients with confirmed early cervical cancer were enrolled in this study.Before laparoscopic extended hysterectomy and pelvic lymphadenoetomy(and para-aortic lymphadenoectomy) , they were injected with carbon nanoparticles suspension injection tracer from cervical neck before surgery. The black-staining lymph nodes were cut as SLN under the laparoscope for routine pathological examination.
RESULTSOf these 21 patients, at least one SLN was successfully detected in 20 patients(95.24%) , and a total of 158 SLNs were detected.The conventional pathology results suggested that 5 patients(23.81%) had positive lymph nodes(n=16, including 14 in 4 patients) . The new approach showed a sensitivity of 80.0%(4/5) , accuracy of 100.0%(20/20) , and negative predictive value of 100.0%(16/16) for SLN detection.
CONCLUSIONLaparoscopic SLN detection with carbon nanoparticles tracer is a relative safe and sensitive method for in cervical carcinoma.

10.Effectiveness and safety of laparoscopic presacral neurectomy in treating endometriosis-associated pain.

OBJECTIVETo explore the effectiveness and safety of laparoscopic presacral neurectomy (LPN) in treating endometriosis-associated pain.
METHODSTotally 64 patients with endometriosis were divided into two groups using prospective non-random method. Patients in the control group received only the conventional laparoscopic resection of endometriosis lesions, while patients in the LPN group underwent LPN in addition to the resection of endometriosis lesions. The pre-operative pain scores, intra-operative staging results, surgical duration, intra-surgical blood loss, post-operative pain relief were compared between these two groups.
RESULTSThese two groups showed no significant differences in terms of age, body weight, pre-operative pain score, surgery staging, surgical duration, and intra-operative blood loss (all P > 0.05). All patients were followed up for 6 to 18 months (median: 12.8 months). The post-operative pain relief rate was 89.28% (25/ 28) in LPN group and 61.29% (19/31) in the control group (P = 0.030).
CONCLUSIONLPN can effectively and safely in treating endometriosis and its associated pain.
