Chinese Journal of Information on Traditional Chinese Medicine 2017;24(6):119-120
Restless legs syndrome is one of the common complications of maintenance hemodialysis. At present, there is no exact efficacy of Western medicine treatment. Deficiency of spleen and kidney, and deficiency of qi and blood are the TCM pathogenesis for this syndrome. LI Dong-yuan's theory of Yin Fire based on deficiency of spleen and stomach, down in the kidney, and clear Yang not rising. The creation of Shengyang Sanhuo Decoction can enrich Yang Dao and scatter Yin Fire. Yang three needles can warm yang, tonify qi and invigorate the kidney, and has a yang from yin therapeutic effect. Syndrome differentiation of medicine and acupuncture is consistent with the pathogenesis of restless legs syndrome in hemodialysis patients. It can provide new ideas and new targets for optimization of TCM treatment, and provide low cost, simple and effective treatment mode for primary hospitals.

OBJECTIVE:To explore the effects of alendronate sodium on bone mineral density (BMD) and bone marker in prostate cancer elderly patients after medical castration therapy. METHODS:In perspective study,84 elderly patients undergoing medical castration therapy were selected and divided into treatment group(45 cases)and control group(39 cases)according to ran-dom number table. Control group received medical castration therapy+Calcium carbonate D3 tablets,1 tablet,po,qn;treatment group was additionally given Alendronate sodium tablets 70 mg,po,once a week,1 week after routine treatment,on the basis of control group. Treatment course of 2 groups lasted for 12 months. The levels of 25-OH-D,testosterone,BMD and bone marker were observed in 2 groups, and the occurrence of ADR was recorded. RESULTS:3 cases of treatment group and 1 case of control group dropped out of the study. Before treatment, there was no statistical significance in above indexes between 2 groups (P>0.05). After treatment,25-OH-D levels of 2 groups were increased slightly, but there was no statistical significance(P>0.05);tes-tosterone level of 2 groups were decreased significantly compared to before treatment, with statistical significance (P<0.05). 12 months after treatment,BMD of lumbar vertebra, collum femoris and hip were increased significantly in treatment group, while those were decreased significantly in control group;there was statistical significance in above indexes between 2 groups(P<0.05). The levels of N-terminal midfragment of osteocalcin, procollagen Ⅰ N-terminal propeptide and β-crosslaps were decreased signifi-cantly in treatment group, while those were increased significantly in control group;there was statistical significance in above in-dexes between 2 groups (P<0.05). There was no statistical significance in the incidence of ADR between 2 groups(P>0.05). CONCLUSIONS:Alendronate sodium can prevent bone loss and reduce the rate of bone turnover in elderly patients with prostate cancer receiving medical castration therapy.

Objective To explore diagnosis-related groups(DRGs) case mixes and development approaches for medicare expense standard fitting patients with respiratory system diseases in Sichuan province. Methods 280 717 cases of respiratory system diseases were sampled from the homepages of medical records of general hospitals in Sichuan. These cases were grouped by means of the exhaustive chi-square automatic interaction detector in the decision tree model and the medicare costs standard was derived using the relative-ratio weighting coefficient. Results The main classification nodes of respiratory diseases were age and patient clinical complexity level (PCCL).Patients were classified into 158 disease diagnosis related groups, including 122 DRGs of internal medicine and 36 DRGs in surgical medicine. The max relative-ratio weighting coefficient was 14.04 and the min one was 0.29.And the extreme inpatients' expenses can affect the identification of classification nodes, calculation of relative weighting coefficient and medicare cost standard. Conclusions Large sample size is advantageous in establishing DRGs and calculating the medicare costs standard based on relative-ratio weighting coefficient. It is however imperative to strengthen monitoring on extreme inpatients' costs and control the homepage quality of medical records.

Human adenovirus (HAdV) is one of the most important pathogens in infants and young children with acute respiratory infections and other diseases. This article reviews the literature on HAdV, including its molecular biological characteristics, detection and typing, and pathogenic mechanism, the clinical features and epidemiological characteristics of HAdV-related diseases, and the prevention and control of HAdV infections. So far, 67 types of HAdV have been identified, including recombinant variants discovered in recent years. The major epidemic strains that cause acute respiratory infections are HAdV-3 and HAdV-7, both of which belong to the subgroup B. HAdV often leads to acute respiratory infections, but it also causes diseases of other systems. HAdV-related diseases have similar clinical manifestations as those caused by other respiratory viruses, but often accompanied by gastrointestinal symptoms. The pathogenic mechanism of HAdV remains unclear, especially for the new recombinant variants, due to few studies on their association with diseases. Because there are no prospective, large randomized controlled trials of HAdV infections, the treatment of HAdV infections is controversial. Vaccine is the most effective measure to reduce respiratory HAdV infections, but it is still not commercially available.

Objective To investigate the effect of chronic intermittent hypoxia(CIH)on serum high-density lipoprotein cholesterol(HDL-C)and the changes of HDL-C metabolism-related indicators such as the expressions of peroxisome proliferators-activated receptor a (PPARα)and adenosine triphosphate binding cassette transporterA1(ABCA1)in liver in male SD rats. Methods Obstructive sleep apnea syndrome(OSAHS)-induced CIH rats were randomly allocated into 6 groups:10%CIH-3 weeks,5%CIH-3 weeks,5%CIH-3 weeks +RH(Removal of hypoxia-3 weeks),10%CIH-3 weeks + RH(Removal of hypoxia-3 weeks),control group-3 weeks, and control group-6 weeks.Serum lipids were measured and compared. To observe and compare the liver pathology, the expression levels of PPARα and ABCA1 in liver tissue of CIH rats were detected by immunohistochemical method. Results The levels of serum TC,TG and LDL-C was significantly higher in CIH rats than in control group. The levels of TC,TG and LDL-C were significantly lower in reoxygenation groups than in CIH groups. There was no significant difference between experiment groups and correspondent control groups(all P>0.05).Compared with control group,CIH rats had significantly lower levels of serum HDL-C;Compared with CIH groups, the levels of HDL-C were significantly higher in reoxygenation groups. There was no significant difference between experiment groups and correspondent control groups(all P>0.05).Compared with control group, the expression of PPARα and ABCA1 of CIH group was significantly lower;Compared with CIH groups, the expression of PPARα and ABCA1 was significantly higher in reoxygenation groups; There was no significant difference in the expression of PPARα and ABCA1 between reoxygenation groups and correspondent control groups,in which the oxygen level was recovered to normal. Conclusions The serum HDL-C level was obviously decreased in OSAHS-induced CIH rats, and the decreased serum HDL-C can be effectively improved by reoxygenation intervention.OSAHS-induced CIH may lead to the dyslipidemia through PPARα-ABCA1 pathway, and reoxygenation intervention for three weeks can effectively recover the expression levels of PPARα and ABCA1 to normal levels, which suggests that if CIH is effectively intervented, the change of PPARα and ABCA1 of liver can be reversed, thereby reversing dvslipidemia.
