OBJECTIVE:To evaluate the economical efficiency of 3 therapeutic plans for acute upper respiratory tract infec-tion caused by group A β-hemolytic streptococcal (GAS). METHODS:In retrospective study,71 GAS positive patients with acute upper respiratory tract infection in our hospital during Mar. 2014-Mar. 2016 were selected and divided into group A(16 cases),B(30 cases),C(25 cases)according to medication method. Group A,B,C were given intramuscular injection of ben-zathine benzylpenicillin(1200000 U),oral administration of penicillin V potassium(0.5 g,q8 h,10 d)or oral administration of amoxicillin sulbactam(0.5 g,bid,10 d),respectively. Clinical efficacy and bacterial clearance were observed, and econom-ics of 3 therapeutic plans were evaluated. RESULTS:The cure rates of 3 groups were 87.50%,90.00%,and 92.00%;bacterial clearance rates were 88.24%,87.87%,and 92.59%,respectively;there was no statistical significance(P>0.05). Cost-minimi-zation analysis was adopted for pharmacoeconomic evaluation. The costs of 3 groups were 237.79,279.49,400.40 yuan,respec-tively. The cure rate and bacterial clearance rate of therapeutic plan C were the highest, but its cost also was the highest. The re-sults of sensitivity analysis were consistent with the results of cost-minimization analysis. CONCLUSIONS:The intramuscular in-jection of benzathine benzylpenicillin can be served as first choice therapeutic plan for recurrent upper respiratory tract infection caused by GAS and patients with rhenmatic fever risk. The patients who are reluctant to be treated with penicillin but guarantee medication time can take penicillin V potassium orally. The patients with poor efficacy of common penicillin can take amoxicillin sulbactam pivoxil.

Journal of Huazhong University of Science and Technology (Medical Sciences) 2004;24(5):452-5
In order to determine whether the variations in the calpain-10 gene constitutes risk of type 2 diabetes (T2DM) in Chinese, the frequency of UCSNP-43, 44 in 268 adults newly diagnosed with T2DM (according to the 1999 ADA criteria) and 153 non-diabetic control subjects was investigated. For all subjects, the height, weight, waist-to-hip ratio (W/H) and blood pressure, as well as following parameters were measured: (1) 75-g oral glucose tolerance test with insulin, C-peptide, HbA1c and blood lipid profiles; (2) Genomic DNA extracted from peripheral blood lymphocytes was genotyped for UCSNP-43 (calpain-10-g. 4852 G/A) and UCSNP-44 (calpain-10-g. 4841 T/C) by sequencing a polymerase chain reaction (PCR)-amplified fragment. PCR product was selected by single strand conformation polymorphism (SSCP) and then sequenced. The results showed that there was significant difference between T2DM group and normal control group in allele frequencies, haplotype frequencies, or haplotype combinations of UCSNP-43 and -44 either. But in newly diagnosed T2DM group, it was found that the individuals with the genotype UCSNP-44 T/C + C/C had significantly increased fasting and post-challenge insulin levels (Fins and P2hIns), consistent with reduced insulin sensitivity. In the BMI> 25 subgroup, the differences were even more significant. It was demonstrated that the Calpain-10 gene polymorphism UCSNP-44 was associated with insulin sensitivity and Fins and P2hIns in newly diagnosed T2DM, although Calpain-10 doesn't appear as a major diabetes susceptible gene in this population.

Chinese Journal of Experimental Ophthalmology 2015;33(10):924-929
Background The pathogenesis of allergic conjunctivitis has not been clearly established.Current researchers indicate that interleukin-4 (IL-4), IL-5 and IL-13 may play an important role in allergic conjunctivitis. But whether the roles of these inflammatory factors are same in different types of allergic conjunctivitis remains unclear. Objective This study was to investigate the expressions of IL-4, IL-5 and IL-13 in ocular surface with different types of allergic conjunctivitis. Methods A prospective cohort study was designed.Eighty individuals were recruited in Shanxi Eye Hospital from April 2013 to September 2014, including 20 patients with vernal keratoconjunctivitis (VKC), 20 patients with seasonal allergic conjunctivitis (SAC), 20 patients with perennial allergic conjunctivitis (PAC) and 20 normal healthy subjects.Surficial tissues were binocularly scraped using disinfected scraper from upper eyelid conjunctiva, and 4 μl of tear fluid was obtained with capillary tube. The expressions of IL-4, IL-5 and IL-13 protein and mRNA in the conjunctival epithelial cells were detected by immunohistochemistry and real-time fluorescence quantitative PCR.The IL-4, IL-5 and IL-13 concentrations in tear fluid were assayed by Luminex method. This study complied with Declaration of Helsinki and the research protocol was approved by the Shanxi Eye Hospital Ethics Committee.Written informed consent was obtained from each subject prior to entering the cohort. Results IL-4, IL-5 and IL-13 were positively expressed in cytoplasm of conjunctival epithelial cells in the VKC group,SAC group and PAC group, but the expressions of IL-4,IL-5 and IL-13 were absent in the normal control group. The relative expression levels of IL-4 mRNA were 4.11±1.24,2.71±0.71 and 2.00±0.80;the relative expression levels of IL-5 mRNA were 4.02±0.43,2.07±0.45 and 1.47±0.50;and the relative expression levels of IL-13 mRNA expression levels were 6.44±0.66,4.35±1.26 and 2.39±0.86 in the VKC group,SAC group and PAC group, showing significant differences among the 4 groups (F =51.32,220.18,162.49, all at P<0.01).The relative expression levels of IL-4,IL-5 and IL-13 mRNA were significantly higher in the VKC group than those in the SAC group and PAC group;and those in the SAC group were significantly elevated in comparison with the PAC group (all at P<0.05).No IL-4, IL-5 and IL-13 were detected in the tear fluid in the normal control group;while the concentrations of IL-4,IL-5 and IL-13 in the tear fluid were (14.06±3.50), (10.88±1.82) and (34.28±8.42) pg/ml in the VKC group, and (7.71 ±0.65), (5.10± 1.33), (23.77±6.29) pg/ml in the SAC group as well as (3.30± 1.50) pg/ml, (2.43± 1.28) pg/ml and (17.67 ± 4.28) pg/ml in the PAC group, showing significant differences among the 3 groups (F =200.29,260.49,128.23, all at P<0.01).IL-4, IL-5 and IL-13 concentrations in the tear fluid were significantly higher in the VKC group than those in the SAC group and PAC group, and those in the SAC group were significantly raised in comparison with the PAC group (all at P<0.01).Conclusions IL-4,IL-5 and IL-13 participate in the pathogenesis of multiple allergic conjunctivitis, but their expressions in the ocular surficial tissue are discriminatory in different types of allergic conjunctivitis.

AIM: To investigate the refractive distribution and analysis risk factors for aircrew ametropia.
METHODS: The number of 49 cases with ametropia from 1031 aircrew during May 2013 to May 2014 were reviewed. Various types of refraction composition, age, type, position, time of flight with the subjective assessment of aircrew were analyzed and compared.
RESULTS:Of 49 cases, 43 cases (88%) were myopia, 6 cases ( 12%) were hypermetropia. , Detection rates were higher in age over 50 years aircrew and flight time more than 3000h. Detection rates were lower in self-conscious symptom heavy aircrew, fighter aircrew and good habit of using eyes.
CONCLUSION: The myopia incidence in aircrew with age >50 years and long flight time is higher, than that of fighter pilots and good habit of using eyes. We should pay attention to the increasing late-onset myopia of aviators and habit of using eyes, work intensity and time of using eyes about aircrew.

Journal of Shanghai Jiaotong University(Medical Science) 2009;29(6):656-659
Objective To evaluate the mechanical properties of SEI silicone for maxillofacial prosthesis before and after heat aging and explore its clinical value. Methods Two kinds of silicones, SEI and A-2186, were performed heat aging for 24, 48 and 72 h. The parameters of tear strength, tensile strength, elongation at break and hardness of them were detected under ordinary state and at different time points after heat aging. Statistic comparison and analysis were conducted. Results Tear strength, tensile strength and elongation at break of A-2186 silicone were more favorable than those of SEI silicone under ordinary state, while the parameter of hardness was an exception (P < 0.05). After heat aging and with time progress, tear strength, tensile strength and elongation at break significantly decreased and hardness significantly increased for both A-2186 silicone and SEI silicone (P < 0.05). After heat aging, tear strength and tensile strength of A-2186 silicone were more favorable than those of SEI silicone, while the latter had a minor change in hardness. Conclusion Compared with the worldwide commonly used A-2186 silicone, there is room for improvement in tear strength and tensile strength for SEI silicone. Given the favorable property of minor change in hardness after heat aging, SEI can be used as a prosthetic material for facial soft tissue defect.

[Abstract ] Objective To understand the identification and in vitro antifungal susceptibility of Penicillium marneffei(PM)in yeast phase, and guide clinic antifungal application. Methods Strains isolated from blood and bone marrow of 23 patients infected with PM in a hospital between 2009 and 2016 were collected, colony morpholo-gy of PM in yeast phase was observed, susceptibility to itraconazole, voriconazole, amphotericin B,and fluconazole were detected with E-test method. Results Colony morphology of PM were as follows:direct microscopic examina-tion of Wright’s staining of tissue specimens found visible oval or round spore with apparent septum, and mainly lo-cated in macrophage;Gram staining of blood culture specimens found that strains were with bulbous and slightly curved ends, occasionally branched and with septum. PM was dimorphic fungi, presented mycelium at 28° C,pro-duced red pigment and diffused into medium;PM presented yeast form at 35° C,there were typical colony morpholo-gy. Minimum inhibitory concentrations (MICs)of itraconazole, voriconazole, amphotericin B,and fluconazole to PM in yeast phase were 0.002-0.016,0.012-0.125,0.002-0.500,and 0.500-16.000μg/mL respectively. Conclusion Typical colony morphology and fungal spore of PM in bone marrow and peripheral blood are important features for identification. PM is most susceptible to itraconazole, followed by voriconazole and amphotericin B, while fluconazole is less susceptible.
