1.Study of clinical application of HPV L1 capsid protein combined with HPV typing and TCT detec-tion in diagnosis and treatment of cervical lesion

Objective To investigate the expression of human papilloma virus L1 (HPV L1)capsid protein in cervical lesions and different human papillomavirus ( HPV) subtypes, and to guide clinical triage management and best individual treatment.Methods Retrospective analysis of 2012 January to 2014 Janu-ary in Jiaxing Hospital of Traditional Chinese Medicine gynecology clinic for HPV L1 protein combined with HPV type, and liquid-based cytology test ( TCT) of 176 patients data.Results The positive expression rate of HPV L1 protein with TCT examination in the negative for intraepithelial lesion or malignancy ( NILM) , atypical squamous cells of undetermined significance( ASCUS) , low-grade squamous intraepithe lial lesion ( LSIL) , atypical squamous cells not except high lesion ( ASC-H) , high-grade squamous intraep-ithelial lesion ( HSIL) , and squamous-cell carcinoma ( SCC) was 28.99%, 44.19%, 64.44%, 22.22%, 12.50%, and 0, respectively.No significant differences were found between the NILM and ASCUS groups ( P >0.05) .The positive expression rate of HPV L1 protein in LSIL group was the highest, and it was sta-tistically significantly different from ASC-H and HSIL groups (χ2 =3.88,5.50, P <0.05) , and 0 expres-sion in SCC group.It suggests that the positive expression rate of HPV L1 protein decreased with the severi-ty of cervical cytology lesions.The top five diseases with positive expression of HPV L1 protein were inflam-mation (44.76%), cervical intraepithelial neoplasia (CIN)Ⅰ(52.94%), CIN Ⅱ(23.81%), CIN Ⅲ(8.33%), and squamous cell carcinoma (0); no significant difference was found between inflammation group and CIN Ⅰ group ( P >0.05 ); and statistically significant difference was found between CIN Ⅰgroup and CINⅡ, CINⅢgroup (χ2 =4.53,5.56, P <0.05), and 0 expression in SCC group.It sug-gests that the positive expression rate of HPV L1 protein increased with the increased level of cervical le-sions.The positive expression rate of HPV L1 protein in top five frequency of HPV infection was 60.78%in multiple infection, 17.65%in type 16, 50%in type 52, 66.67%in type 58, and 12.5%in type 18, re-spectively.The expression rate of HPV L1 protein of Type 16 infection was significantly lower than that of type 52, type 58 infection and multi infection(χ2 =4.15, P <0.05, χ2 =7.88,15.44, P <0.01).The positive expression rate of HPV L1 protein of type 18 infection was lower than the other three, but significant differences occurred only compared to multiple infection(χ2 =4.70, P <0.05).The rest had no significant difference( P >0.05) .Conclusions Detection of HPV L1 protein is of clinical value to evaluate the risk of cervical lesions.HPV L1 protein combined with HPV type and TCT detection is helpful for traffic man-agement and personalized treatment, and benefit patients with cervical lesions.
2.Effect of Catalpol on the Activity and Apoptosis of Osteoclast in the Osteoblasts-osteoclasts Co-culture Sys-tem and Its Mechanism Study

OBJECTIVE:To study the effect of catalpol on the activity and apoptosis of osteoclasts (OC) in the osteoblasts (OB)-OC co-culture system and its mechanism. METHODS:OB and OC were isolated respectively from the SD rats of 1-3 days and 5-7 days old to establish OB-OC co-culture system. After treated with 0(blank control),0.05,0.5,5,50 and 100 mg/L catal-pol for 48,72 and 96 h,the number of bone absorption lacuna for OC was observed by inverted microscope to reflect OC activity. After treated with 0(blank control)and 0.05 mg/L catalpol for 48,72 and 96 h,the activity of tartrate resistant acid phosphatase (TRACP)in OC was detected,and the apoptosis rate of OC was calculated. After treated with 0(blank control)and 0.05 mg/L ca-talpol,mRNA expression of osteoprotegerin(OPG)in OB was detected. RESULTS:In OB-OC co-culture system,the number of bone absorption lacuna in 0.05-50 mg/L catalpol groups was significantly lower than blank control group(P<0.01),indicating ca-talpol could inhibit OC activity,especially 0.05 mg/L catapol. Compared with blank control,0.05 mg/L catapol lowered the activity of TRACP but increased the apoptosis rate of OC(P<0.05);mRNA expression of OPG was up-regulated in OB(P<0.01). CON-CLUSIONS:In OB-OC co-culture system,catalpol can inhibit the activity of OC and induce the apoptosis of OC,and its mecha-nism may be associated with the mRNA expression up-regulation of OPG in OB.
3.Fungal distribution and drug sensitivity analysis in 113 cases of lesion secretions

Objective To analyze the fungal distribution and drug sensitivity analysis in 113 cases of lesion secretions .Methods Identification of fungi and drug sensitive test were done in 113 cases of lesion secretions .Results Among the 113 cases of lesion se-cretions ,there were Candida albicans 75 cases(66 .4% ) ,Candida dublin 29 cases(26 .7% ) ,Candida parapsilosis 6 cases(5 .3% ) , Candida krusei 3 cases(2 .7% ) .For Candida albicans ,the drug sensitive rates of 5-fluorocytosine and amphotericin B were 94 .7%and 97 .3% respectively .For Candida dublin ,the drug sensitive rate of amphotericin B was 93 .1% .For Candida parapsilosis ,the drug sensitive rates of voriconazole and amphotericin B were both 83 .3% .For Candida krusei ,the drug sensitive rates of 5-fluoro-cytosine and amphotericin B were both 100 .0% .Conclusion Strengthening the fungal distribution and drug sensitivity analysis be-fore treatment in fungal lesion secretions may provide direction for the clinical treatments .
4.Observation on the Efficacy and Safety of Alprazolam by Progressive Dose Increasing in the Treatment of Chronic Tinnitus

OBJECTIVE:To observe the efficacy and safety of alprazolam by progressive dose increasing in the treatment of chronic tinnitus. METHODS:Totally 50 patients with chronic tinnitus were included. They were orally treated with Alprazolam tab-let 0.4 mg in the first 1 and 2 week(s),once every night at bedtime;0.4 mg in 3 and 4 weeks,once in the morning and once in the evening;0.4 mg in 5 and 6 weeks,three times a day. If the treatment of tinnitus was invalid,then the gradual withdrawl was conducted by twice a day for continuous 3 d,0.4 mg each time;then once a day,0.4 mg each time,for continuous 3 d. The re-sponders were maintained 12 weeks,and gradual withdrawal was conducted,and followed by follow-up for 3 months. The clinic data was observed,including the clinical efficacy,tinnitus disability scale(THI)score,visual analogue scale(VAS)score,tinnitus loudness(TI)and incidence of adverse reactions before and after treatment. RESULTS:The effective rate was 66.67%;after treat-ment,the scores of THI and VAS,and TI were significantly lower than before,with significant differences(P<0.05). There were no obvious adverse reactions during treatment. CONCLUSIONS:The alprazolam by progressive dose increasing has obvious effica-cy in the treatment of chronic tinnitus,with good safety.
6.The effect of the PXh haplotype combining estrogen receptor gene Xba Ⅰ, Pvu Ⅱ polymorphisms and osteocaicin gene Hind Ⅲ polymorphism on bone mineral density in postmenopausal women

Objective To investigate the correlation between bone mineral density(BMD)and the PXh haplotype combining estrogen receptor (ER) gene Xba Ⅰ , Pvu Ⅱ polymorphisms and osteocalcin gene Hind Ⅲ polymorphism in postmenopausal women.Methods In 307 subjects,the BMD of lumbar vertebrae and proximal femur were measured by dual energy X-ray absorptiometry and the Xba Ⅰ and Pvu Ⅱ polymorphisms of ER gene and the Hind Ⅲ potymorphism of osteocalcin gene were detected by polymerase chain reaction and restriction fragment length polymorphism (PCR-RFLP).Results (1)The BMD of greater trochanter was significantly lower in XX genotype group than in xx genotype group ( P<0.05).The BMD of femoral neck, greater trochanter and Ward's triangle were lower in Xx genotype group[(0.695±0.087)g/cm2 , (0.592±0.106)g/cm2, (0.500±0.115) g/cm2] and X allele group[(0.697±0.088)g/cm2 , (0.594±0.105)g/cm2, (0.505±0.123)g/cm2] than in xx genotype group[(0.737±0.108) g/cm2,(0.653±0.119)g/cm2 ,(0.554±0.130)g/cM2] and non-X allele group[(0.737 ± 0.108) g/CM2, (0.653 ± 0.119) g/cm2 , (0.554 ± 0.130) g/cm2] ,respectively (all P<0.05 ).(2)The BMD of Ward's triangle was lower in PP genotype group and P allele group than in pp genotype group and non-P allele group (P<0.05).(3)The BMD of femoral neck, greater trochanter and Ward's triangle were lower in hh genotype group and h allele group than in I-IH genotype group, and were lower in non-h allele group than in HH genotype group(all P<0.05).(4)Women carrying PX, PXh haplotypes combining ER gene and osteocalcin gene had lower BMD at femoral neck than those not carrying PX,not carrying PXh haplotypes, respectively (all P<0.05).ConclusionsER gene(Xba Ⅰ) polymorphism and osteocalein gene(Hind Ⅲ) polymorphism are associated with BMD in postmenopausal women.The presence of X allele or h allele shows negative influence on the BMD of postmenopausal women.The PXh haplotype is a suitable genetic marker of postmenopausal women osteoporosis in Fuzhou area.
7.Application of clinical pathway in the management of single disease cost

Objective To explore the application of clinical pathway in the management of single disease cost. Methods Inpatients with breast cancer from June 2006 to June 2007 were divided into the test group and the control group according to different payment models.Patients in the test group had medical insurance while patients in the control group did not.The general data in the two groups had no statistical difference.The test group were given nursing intervention by clinical pathway while the control group received routine nursing measures. Results The mean inhospital days,mean days waiting for operations and mean inhospital expenses in the test group were shorter or lower than those in the control group (P < 0.05).The mastering degree of health knowledge and the satisfaction degree of patients in the test group were higher than those in the control group (P < 0.05). Conclusions The application of chnical pathway in the single disease cost patients with medical insurance could effectively control the medical expenses and guarantee the quality.So we consider that the application of clinical pathway in single disease cost is applicable.

10.Importance of data management with statistical analysis set division.

Testing of hypothesis was affected by statistical analysis set division which was an important data management work before data base lock-in. Objective division of statistical analysis set under blinding was the guarantee of scientific trial conclusion. All the subjects having accepted at least once trial treatment after randomization should be concluded in safety set. Full analysis set should be close to the intention-to-treat as far as possible. Per protocol set division was the most difficult to control in blinded examination because of more subjectivity than the other two. The objectivity of statistical analysis set division must be guaranteed by the accurate raw data, the comprehensive data check and the scientific discussion, all of which were the strict requirement of data management. Proper division of statistical analysis set objectively and scientifically is an important approach to improve the data management quality.
