Objective To investigate the effect of standardized usage of antibiotics and hand hygiene on carbapenem-re-sistant Enterobacteriaceae (CRE) infection in neonates. Methods According to the situation of standardized management of an-tibiotics in our hospital, the study period, from January 2011 to December 2013, was divided into pre-intervention, transition and post-intervention. Retrospective study was employed to analyze the antibiotics use density (AUD), the hand hygiene compliance rate among the medical staff, the positive rate of CRE and the nosocomial infection percentage of the neonatology department. Results From pre-intervention to post-intervention period, the AUD was decreased from 12.93 DDDs to 8.10 DDDs;hand hy-giene compliance rate of medical staff was increased from 53.49%to 83.33%;the detection rate of CRE was decreased from 3.71%to 2.27%. The differences were all signiifcant among different periods (P<0.01). The nosocomial infection percentage decreased from 0.22%to 0.17%and there was no signiifcant difference (P=0.441). Conclusions Standardized usage of antibiotics and hand hygiene can be served as economical yet effective approaches to controlling CRE infection in neonates.

Objective To investigate a simple and dependable approach to establish the hypoxic-ischemic encephalopathy model in neonatal rat. Methods Twenty-one neonatal rats of 7 days old were randomly divided into control group, hypoxic group, and hypxic-ischemic group.Every group was randomly divided into 3 hours,6 hours,1 day,3 days,7 days, 14 days, and 21 days group, according to the time of killing.Left common carotid artery of neonatal rats at age of 7 days in hypoxic-ischemic group were ligated. Then, the rats in hypoxic and hypoxic-ischemic group were put in a state of 8% oxygen for 2.5 hours. Brain tissues of rats in 3 groups were observed with HE staining under light microscope. Results In hypoxic-ischemic group, there was found mild brain damage after hypoxic-ischomic 3 hours, the brain lesion was most severe at 1 day, glial cell proliferation was found at 3 days, much neur were losed at 14,21 days, and colloid scar was formed in cortex, striatum and hippocampi. Conclusion The method that left common carotid ontery of neonatal rats were ligated and then put in 8% oxygen for 2.5 hours is simple, rapid and dependable, which can be applied widely.

ObjectiveTo explore the video-urodynamic characteristics of neurogenic bladder caused by suprapontine neuropathy.Methods65 patients with neurogenic bladder caused by suprapontine neuropathy were involved from February 2004 to May 2009. The data were analyzed retrospectively, including clinical manifestation, diagnosis, voiding management, ultrasound, uroflow, post void residual, filling cystometry, pressure-flow study and the radiology. The results were compared with those of the suprasacral spinal cord injury.ResultsNo significant difference was found in the urodynamic parameters between various kinds of suprapontine neuropathy. Compared with suprasacral spinal cord injury, suprapontine neuropathy had less detrusor-sphincter dyssynergia, less reflux and upper urinary tract dilation but more normal micturiton reflex.ConclusionThe main video-urodynamic characteristic of neurogenic bladder caused by suprapontine neuropathy is detrusor overactivity, and the detrusor-sphincter dyssynergia, reflux and upper urinary tract dilation are rare.

ObjectiveTo recognize the features of genitourinary dysfunction in multiple system atrophy (MSA) and the importance of videourodynamics.Methods5 cases of MSA were reviewed.ResultsGenitourinary dysfunction in MSA included voiding problem, nocturnal urinary frequency, urgency, impotence and so on. The characteristic appearance of videourodynamics was that postvoiding volume more than 100 ml, detrusor-external sphincter dyssynergia (DESD) and open bladder neck at the start of bladder filling. The abnormal sphincter electromyography comprised of abnormal spontaneous activity, motor unit potentials (MUPs) more than 13 ms, polyphasic potentials more than 60%. MRI showed brain atrophy and the "hot cross bun" signal in the pontocerebellar degeneration.ConclusionMSA is a disorder characterized by progressive neuronal atrophy at certain sites of the central nervous system that control the urogenital function. MSA-related urological symptoms are analogous with symptoms of bladder outlet obstruction. It is necessary to take some reasonable investigations for avoiding misdiagnosis and unnecessary surgery.
