1.Cytogenetic Analysis in 543 cases of amniocentesis.

OBJECTIVE: To analyze 543 cases of amniocentesis with indications, cytogenetic results. METHOD: This study includes 543 cases cytogenetic study results which amniocentesis to detect fetal chromosomal abnormality in the Cytogenetic Laboratory at Eun Hospital in Kwang-Ju from August 1996 to December 2000, as gestational ages, indications of amniocentesis, maternal age distributions, chromosome aberrations with cytogenetic results. The cytogenetic results of chromosome aberration was identified by parents inheritance and de novo karyotypes as parents periperal blood cytogenetic study. RESULTS: Amniocentesis performed mostly from 15 weeks to 20 weeks of gestaional ages. Requested indication of amniocentesis presents abnormal maternal serum screening (37%), infertility (23%) and maternal old age (> or =35) (17%). Chromosome aberration according to amniocentesis indication was suspected fetal anomaly by ultrasonogram (8.6%), previous family history (5.7%). Chormosome aberration following maternal ages, more 40 years old women were found highly 7.9%. The frequency of chromosome aberration was 5.5% but de novo chromosome aberration was 2.2%. Numerical aberration was overall new karyotypes (1.3%). Structural aberration was inheritance karyotypes (3.3%) and de novo karyotypes (0.9%). CONCLUSION: Amniocentesis is a effective diagnostic tools in fetal chromosome aberration. Indication of fetal chromosomal anormality by ultrasonography and advanced maternal ages is an important diagnostic method with chromosome aberrations. Prenatal fetal chromosome aberrations included inheritance and de novo karyotypes. Especially, identification of de novo chromosome aberrations may predict fetal anomaly and counsel the fetus for pregnant parents.

3.Normal Values of rNeonatal Periumbilical Skin Length.

Malformations of umbilicus are a feature of many dysmorphic syndromes and the measurement of periumbilical skin length should be considered as the basic step in the description of the umbilical malformations. So, we measured the periumbilical skin length in the 103 normal neonates and obtained the following results. 1) The means (+SD) of the periumbilical skin length were the 11.7mm (+3.0) in the cranial site and the 8.2mm (+2.5) in the caudal site, and the cranial length was 3.5mm longer than the caudal length. 2) No difference was observed between the male and female groups in the periumbilical skin length. 3) Age was the only statistically significant variable associated with the periumbilical skin length and it was observed that the periumbilical skin length decreased according to the advance of age (hour).

4.Testicular Injuries-Efficacy of the Organ Injury Scale Developed by the American Association for the Surgery of Trauma.

PURPOSE: According to the AAST (American Association for the Surgery of Trauma) injuries to the testes are rare, but there are many reports in the literature on other organs using the AAST Organ Injury Scale (OIS). In the present study, the authors evaluated the usefulness of the AAST OIS when applied to the testis. MATERIALS AND METHODS: A retrospective review was performed on 65 patients (mean age 30.0 years) with a testis injury treated between March 1986 and July 2006. Of the 65 patients, preoperative ultrasonography was performed on 51. Injuries were classified according to the OIS of the AAST, based on history taking, a physical examination and scrotal ultrasonography. RESULTS: Of the 51 patients, 46 and 5 underwent surgical intervention and conservative management, respectively. Of the included patients, 38 were found to have abnormal testis contours on ultrasonography, 29 had a rupture of the tunica albuginea, with a sensitivity of 76.3%. Of the 13 patients with normal contours, the tunica albuginea was normal in 7, and 8 underwent operation, with a specificity of 87.5%. Of the 12 Grade I patients, 7 underwent an operation, but the testes contours were normal. Of the 8 Grade II patients, only 1 underwent a primary repair with partial orchiectomy, but of the 45 Grades III, IV or V patients, 35 underwent a primary repair with a partial orchiectomy or orchiectomy with a hematoma evacuation performed in 10. CONCLUSIONS: Ultrasonography was found to be the most effective modality for detecting a rupture of the tunica albuginea, and the authors conclude that the AAST OIS can be used to determine the treatment modality. Our recommended treatment policy is as follows; conservative for Grade I, exploration and repair for Grade II, and exploration, and a partial orchiectomy or orchiectomy for Grades III, IV or V. In particular, an orchiectomy is the most commonly used modality for Grade V.

5.Stress, Self-esteem, and Powerlessness in Korean Husbands Married to Foreign Wives.

PURPOSE: The purpose of this study is to investigate levels of stress, self-esteem, and powerlessness in Korean husbands married to foreigner wives. METHODS: The subjects were 183 Korean men who were living with foreigner wives in the Gwangju, Jeonnam and Jeonnbuk areas. Data were collected from January to May 2012 and analyzed by descriptive statistics, t-test, one-way ANOVA, Scheffe test, Pearson's correlation coefficient, and backward multiple regression with the SPSS PC WIN version 18.0. RESULTS: The average level of stress was 4.48, with difficulties in international marriage experienced by the subjects due to differences in language and culture. The average reported self-esteem score was 29.69, and the average powerlessness score 117.76. There were significant differences in perceived powerlessness depending on the employment statuses, income levels, and nationalities of the participants' wives, and their perceived levels of satisfaction with living with a foreigner wife. Powerlessness was positively correlated with stress (r=.41, p<.001), and negatively correlated with self-esteem(r=-.63, p<.001). The factors affecting the perceived powerlessness included self-esteem (beta=-.54, p<.001), stress (beta=.28, p<.001), lower income (beta=.13, p=.016), and working wife (beta=.12, p=.036). CONCLUSION: Healthcare providers need to explore strategies to improve self-esteem and powerlessness in Korean men married to foreigner wives.

6.Expression of Spinal c-fos in a Rat Model of Postoperative Pain.

BACKGROUND: The oncogene, c-fos, is a useful marker of nociceptive neurons activated by various types of pain.However, the relationship between c-fos and acute pain induced by surgical incision has not been studied.The purpose of this study was to describe the expression of spinal c-fos in a rat model of postoperative pain. METHODS: Sprague-Dawley rats were used to enumerate spinal fos-like immunoreactive (fos-LI)-positive neurons after making sham, skin, or skin-muscle incisions on the plantar surface of the rat hindpaw. The spinal cords were divided into the following 4 segments for immunohistochemical detection of fos-LI at 3 time points after the incision (4 hours, POD1, and POD3):thoracic, upper lumbar, lower lumbar, and sacral.For analysis of the laminar distribution of fos-LI, the dorsal horn was divided into three regions (I-II, III-IV, and V-VI). RESULTS: The number of fos-LI was increased in the ipsilateral spinal dorsal horn in the lower lumbar segment 4 hours post-incision and was significantly greater in rats with skin-muscle incisions than in rats with skin-alone incisions.In the skin-muscle group, laminae I-II and V-VI had a greater number of fos-LI neurons than laminae III-IV.The increase in number of spinal fos-LI neurons normalized by POD1 and POD3. CONCLUSIONS: c-fos expression is confined to neurons of the spinal dorsal horn known to be activated by nociceptive stimulation. The larger the injury elicited, the greater the expression of c-fos, suggesting that the expression of c-fos can serve as a useful marker of activated nociceptive neurons caused by acute pain following surgical incision.Further study is required to elucidate why the degree of tissue injury affects the laminar distribution of c-fos expression.

Congenital adrenal hyperplasia is inherited disorder of adrenal steroidogenesis. 21-hydroxylase deficiency is the most commone enzymatic defect and is divided into classic and late-onset or nonclassic forms. Both classic non-classic 21-hydrozylase deficiencies are inherited in a recessive manner as allelic variants. But it is rare that happened in twin infants. Chief complaints of affected twins in our case were ambiguous genitalia, hyperpigmentation and dehydrations. They were revealed into hyponatremia, hyperkalemia and increased amount of serum progesterone, 17-hydroxyprogesterone and urinary 17-ketosteroid excretion and were administered with DOCA, 9alpha-fluorohydrocortisone, hydrocortisone to control the electrolyte imbalance. And now, both of them are going to normal ratio of weight gain and body growth.

10.Aggressive Osteoblastoma: A case report

Osteoblastoma is a rare tumor which forms osteoid and bone with highly vascular stroma, account for less than 1 percent of all bone tumors. The lesion had been though to be controlled with curettage and local excision. In recent years, osteoblastomas have been described as aggressive osteoblastoma or malignant osteoblastoma, separated from the innocuous behavior of usual osteoblastoma. It represents the histologic characteristics of the presence of epithelioid osteoblasts, trabecular rather than lace-like osteoid, low mitotic rate with no atypical form with prominent giant cells of osteoclastic type. The differential diagnosis between aggressibe osteoblastoma and osteogenic sarcoma is extremely difficult, but its peculiar histologic pattern and its different clinical and radiologic features and better prognosis are helpful. We report here a case of aggressive osteoblastoma of 17 years old female patient with 3 years and 4 months follow-up period and review the literature.
