In view of the current conditions and reasons about the reform of medical and health system in China,the paper expounds the feasibility of authorized operation with state-owned assets of public hospital and the concrete operational pattern from the angle of hospital operational mechanism, which proposes a new idea about how to effectively solve the contradiction between the limited investment on state-owned health resources and the growing demands on medical health.

Toxoplasma gondii is an intracellular protozoan parasite that infects all warm?blooded animals. The surface anti?gens of T. gondii with the potential for application as antigens of diagnosis and vaccines have been studied extensively in recent years especially for P43 P35 P30 P23 and P22. The studies on the surface antigen in tachyzoites of T. gondii are reviewed in this paper.

AIM: To explore the changes in serum TGF ? 1 in type 2 diabetes mellitus. METHODS: Forty-five cases type 2 diabetes mellitus patients were divided into three groups according to urine albumim excretion rate(UAER): normoalbuminuria(NA)group and microalbuminuria(MA) group and macroalbuminuria group (Overt DN). Serum TGF ? 1, fasting blood glucose(FBG), HbA 1c ,BUN,Cr,Ccr,lipidemia were detected in all cases. Objective To comparatively analyze Toxoplasma gondii separated from HIV-positive people and RH strain GRA6 gene. Methods By using the nested PCR the amplification of Dali HIV-positive blood samples and RH strains of Toxo-plasma GRA6 genome was performed. The GRA6 gene amplification positive product was selected and the electrophoresis imag-ing was performed by being digested with the Mse I endonuclease and the gene sequences were measured and analyzed. Re-sults The GRA6 gene fragment 800 bp was successfully amplified and about 600 bp and 200 bp bands were got by Mse I. The sequencing results showed that T. gondii GRA6 gene positive samples had 2 nucleotide variation compared with T. gondii strain RH namely 447 base pair at C becoming G and 623 base pair at G becoming T. At 146 bp and 690 bp the Mse I restric-tion sites TTAA were found. Conclusion The preliminary judgment shows that the Dali HIV-positive T. gondii genotype is consistent with RH strain belonging to genotype I.

Objective To explore the effects of different vitamin A(VA) levels on thyroid cells apoptosis and its gene expression of mice taking excessive iodine. Methods Kunming mice were randomly divided into 6 groups according to body weight 3 weeks after born: normal control(NI) group, high iodine(HI) group, low vitamin (LVA) group, high iodine plus low vitamin A(HI+LVA) group, high iodine plus vitamin A1 (HI+VA1) group, high iodine plus vitamin A2(HI+VA2) group. The VA was given in food(4000,4000,0,0,8000,16 000 U/kg), and the iodine was given as potassium iodate in water (I-:50,3000,50,3000,3000,3000 μg/L). The apoptosis was tested using in situ end labehng(TUNEL) method. Reverse transcription polymerase chain reaction (RT-PCR) were used to measure the level of mRNA of apoptosis gene(Fas, FasL, Bcl-2) in tissues. Results Apoptotic index measured by TUNEL method was rising along with the mice age. Compared to NI group[(14.09±5.68)%], apoptotic index was significantly increased in HI[(20.91±9.57)%], HI+LVA[(20.29±9.90)%]and HI+VA2 [(19.51±8.25)%]groups in the three months(P < 0.05). Compared to NI group[(16.80±9.90)%], apoptotic index was significantly increased(P < 0.05) in HI[(23.22±8.58)%],LVA[(22.56±6.17)%],HI+LVA [(25.99±9.62)%],HI+VA1 [(21.65±7.74)%]groups in the six months. Compared with the NI group(Fas: 1.29±0.25,1.27±0.26; FasL: 1.60±0.13,1.65±0.13), the mRNA levels of Fas and FasL in HI group(Fas: 1.57±0.36,1.49±0.35; FasL: 1.85±0.46,1.84±0.32) were increased, but the differences were not remarkable(P > 0.05) in the three and six months. Compared with the HI group, the mRNA levels of Fas in HI+ VA1, HI+VA2(1.33±0.35, 1.30±0.26) groups were decreased to the level in NI group in the six months. The mRNA levels of Fas and FasL were not different (P > 0.05) between HI+LVA(I.60±0.27,1.88±0.46) and HI groups in the three months. The mRNA levels of Bcl-2 were not remarkably differences in the three months (1.05±0.19,0.96±0.33,0.95±0.26,1.18±0.27,1.10±0.19,0.98±0.36, all P > 0.05), and in the six months (1.35±0.28,1.60±0.25,1.48±0.18,1.71±0.26,1.66±0.29,1.56±0.35, all P > 0.05). Conclusions Excessive iodine can cause thyroid cells apoptosis in mice. Supplementation of suitable amount of VA can regulate the levels of the apoptosis-related genes expression, and partly antagonize the apoptosis caused by high iodine.

Objective To explore the relationship between serum 25 (OH)D level and cognitive function in patients with Alzheimer's disease (AD).Methods Totally 113 AD impatients were enrolled in this study.Their serum 25 (OH) D2,25 (OH) D3,and total 25 (OH) D levels were determined by liquid chromatography-tandem mass spectrometry (LC-MS/MS).Patients were divided into Vitamin D severe deficiency group [25 (OH)D level≤25 nmol/L],deficiency group [25 (OH)D levels:25-50 nmol/L],insufficiency group [25 (OH) D level:50-75 nmol/L],and sufficiency group [25 (OH) D level ≥ 75 nmol/L] according to the criteria proposed by US Institute of Medicine.The cognitive function was assessed by MMSE score.The association between serum 25 (OH) D level and cognitive function was systematically analyzed.Results The serum levels of 25 (OH) D,25 (OH) D2,and 25 (OH) D3 were (27.08 ± 15.33) nmol/L,(1.23 ± 0.93) nmol/L and (24.50 ± 13.04) nmol/L in AD patients.The proportions of severe deficiency,deficiency,insufficiency,and sufficiency were 60.18％ (68/113),30.97％ (35/113),7.97％ (9/113),and 0.88％ (1/113),respectively,among these AD patients.A positive correlation was found between serum 25 (OH) D concentration and MMSE score in AD patients.Conclusions Impatients with AD often have severe vitamin D deficiency and need vitamin D supplementation.Serum 25 (OH) D concentration is associated with cognitive function,and therefore vitamin D supplementation may improve cognitive function.
