1.Assessment of infantile hypertrophic pyloric stenosis with ultrasonic measurements

Objective To determine whether pyloric measurements with ultrasound, that muscle thickness and channel of pyloric, correlated with weight and age in patients with hypertrophic pyloric stenosis (HPS). Methods A retrospective analysis was conducted on 111 cases diagnosed with HPS by operation from 2008 to 2012. Pearson correlation and linear regression analyses were used to determine if there were sta?tistically signiifcant associations between these combinations of factors:age and pyloric muscle thickness, weight and pyloric muscle thickness, age and pyloric length, and weight and pyloric length. Results Patients’mean age was 39.1 d (8-92 days). Their mean weight was 4.3 kg (2.2-7.9 kg). Mean pyloric muscle thickness was 4.8 mm (2-4.6 mm), and mean pyloric length was 17.5 mm (12-23.5 mm). Pearson correlation coefifcient analysis showed a signiifcant correlation between age and muscle thickness (r=0.6, P<0.001) as well as weight and muscle thickness (r=0.486, P<0.001). No signiifcant correlation was found be?tween pyloric length and age or weight. Linear regression analysis demonstrated similar results. Conclusions In patients with HPS, pyloric muscle thickness was directly related to age and weight. Smaller and younger infants with suspected diagnosis of HPS should be followed up even though the minimum diagnostic criterion for muscle thickness or length was not found on ultrasound.
2.Endovascular management versus open surgery for subclavian steal syndrome

Objective To evaluate the therapeutic effects and safety of open surgery vs.endovascular procedures for subclavian steal syndrome (SSS).Methods A retrospective analysis was made on 112 patients from July 2005 to July 2014 including 72 undergoing endovascular therapy and 40 cases treated by open bypass surgery.Results There were not difference between endovascular therapy group and surgical group in technical success rate (95.8% vs.100%,P =0.239),postoperative subclavian artery diameter [(8.3 ± 1.6) mm vs.(8.1 ± 0.7) mm,P =0.374],postoperative suffering/contralateral systolic pressure ratio [(0.95 ±0.12) vs.(0.96 ±0.15),P =0.518].Nor there were the differences in mortality [0(0%) vs.3 (7.5%),P =0.018],postoperative renal dysfunction [2 (2.8%) vs.5 (12.5%),P =0.042],wound inffections [1 (1.4%) vs.4 (10%),P=0.034],duration of operation [(1.5 ±0.32) h vs.(2.7±0.51) h,P=0.027],days in hospital [(5.7±3) dvs.(9.3±3) d,P=0.013].After 1,3,5 years postoperative follow-up the two groups were not significantly different in patency rates,subclavian artery diameter,suffering/contralateral systolic pressure ratio (all P > 0.05).Conclusion For subclavian artery steal syndrome,endovascular treatment is safe,effective and minimally invasive.
3.Establish Evaluation System to Track Implementation Effect of Clinical Research Projects

Objective To establish a suitable evaluation index system to track implementation effect of clinical research program.Methods Delphi method was used to creat the evaluation index system.The weighted average method was adopted to determine the weight of each index.Results After two rounds of expert consultation,twenty seven evaluation indices were selected,including three first-class indices,eight second-class indices and sixteen third-class indices,and the weight of each index was determined.Conclusions The evaluation index system reflects the purpose of tracking clinical research to a certain extent.This index system is simple and easy to be used.
4.Management of arterial reocclusion after endovascular treatment for diabetic feet

Objective To investigate the causes of arterial reocclusion in diabetic feet patients after endovascular treatment and its remedial measures.Methods From January 2009 to October 2013,clinical data of 371 arterial reocclusion of diabetic feet patients after endovascular treatment in Tianjin First Central Hospital were reviewed retrospectively.We summarized the causes of reocclusion,treatment methods and the short term results.Results According to the Trans-Alantic Inter-Society Consensus (TASC) Ⅱ grading standards,the first time when the endovascular treatment started there were 37 cases of grade A,85 cases of grade B,143 cases of grade C,106 cases of grade D.Arterial re-occlusion developed from one day to 36 months,averaging at (21 ± 8) months.Causes of re-occlusion included intimal hyperplasia in 263 cases (70.9%),thrombosis in 65 cases (17.5%),dissection in 19 cases (5.1%),stent fracture in 17 cases (4.6%),vascular rupture in 7 cases (1.9%).Remedial therapy adopted for arterial reocclusion was repeated endovascular treatment in 327 cases (88.1%),arterial bypass surgery in 23 cases (6.2%),conservative treatment in 13 cases (3.5%),amputation (cut toe) in 4 cases (1.1%),4 cases (1.1%) died perioperatively.275 cases were followed up for 1 to 36 months,the average was (13 ± 8) months.patency rate was 82.9%,71.3% and 63.0% at 6 months,1 year and 2 years.Amputation rate was 1.1%,1.8% and 2.5% at 6 months,1 year and 2 years.Conclusions Intimal hyperplasia is to blame for arterial reocclusion after endovascular treatment of diabetic foot.In this case most patients still can benefit from second time endovascular treatment,with a satisfactory short term patency rate.
5.Blood flow field characteristics of cerebral aneurysm before stent implantation

BACKGROUND:Cerebral aneurysm is a kind of mortal hemangioma, and its treatments such as endovascular embolization and clipping both cause high postoperative recurrence rate and mortality. So the stent implantation for cerebral aneurysm is coming into being. OBJECTTVE:To evaluate the hemodynamic parameters after stent implantation into cerebral aneurysm and to provide a novel feasible strategy for clinical treatment. METHODS:A retrospective analysis was preformed based on the CT image data of 11 patients with cerebral aneurysm from the Affiliated Hospital of Xinjiang Medical University. Firstly, the flexible and solid model of cerebral aneurysm was established by the MIMICS and reverse engineering. Secondly, the matching stent model was implanted into the cerebral aneurysm, and then the blood flow structure of cerebral aneurysm was analyzed by the fluid dynamics theory and the Fluent with the method of two-way flow solid coupling. Final y, comparative analysis of the kinetic parameters of cerebral aneurysm before and after implantation, including wal pressure, blood velocity, path line of the blood flow, wal shear stress, wal deformation was conducted, and blood flow characteristics after stent implantation were analyzed under different entrance velocity. RESULTS AND CONCLUSION:After implantation, the wal surface pressure was reduced about 61.1%;the blood flow velocity around the stent and the inside of the cerebral aneurysm was decelerated obviously;under setting 2 000 lines of blood flow, the number of path line of blood flow into the cerebral aneurysm reduced about 75.0%, the maximum wal shear stress decreased about 79.3%, and the maximum wal deformation reduced to a lower level. The entrance velocity was respectively v1=0.1 m/s, v2=0.2 m/s, v3=0.3 m/s and the wal pressure was in a gradient ascent;the wal shear stress increased with the velocity, meanwhile,τzou (left neck of aneurysm)<τzhong (aneurysm )<τyou (right neck of aneurysm). The path lines of blood flow mainly concentrated in the top of the aneurysm, and the blood velocity markedly affected the surface deformation. These results indicate that main hemodynamic parameters are obviously improved after stent implantation into cerebral aneurysm, and the blood velocity should never be neglectful in the treatment process.
6.Effect of lipoxin receptor agonist on human cytomegalovirus replication and proliferation

BACKGROUND:The anti-inflammation and protective effects of lipoxin have been verified in several immunity-related disease models. Preliminary studies of our research group have shown that, lipoxin receptor agonist BML-111 has negative regulation effects on the human cytomegalovirus (HCMV)-induced immunological injury. However, the effect of BML-111 on the HCMV replication remains unclear.
OBJECTIVE:To observe the influence of lipoxin receptor agonist BML-111 on HCMV replication and proliferation in THP-1 macrophages and human embryonic lung fibroblasts.
METHODS:THP-1 macrophages were infected by HCMV AD169 strain, and were divided into three groups:mock infection, HCMV infection, HCMV+BML-111. The final concentration of BML-111 was 100 nmol/L. cells in each group were col ected at 0, 1, 2, 4, 12, 36, 48 hours, the mRNA levels of IE86 and pp65 in the THP-1 macrophages were tested by RT-PCR method. Human embryonic lung fibroblasts were infected with HCMV (MOI=0.1), and were divided into two groups:HCMV infection and HCMV+BML-111. The patho-morphous changes of human embryonic lung fibroblasts were observed under light microscope, and the cellnumber was measured. The infective virus titer changes in human embryonic lung fibroblasts were examined by plaque assay.
RESULTS AND CONCLUSION:After the macrophages were infected by HCMV, compared with the mock infection group, the mRNA levels of IE86 and pp65 in the HCMV group and HCMV+BML-111 group were increased significantly;compared with the HCMV infection group, the mRNA levels of IE86 and pp65 in the HCMV+BML-111 group were increased significantly in the early stage (within 4 hours) after infection, but the pp65 mRNA levels were decreased significantly in the medium and late stages (24-72 hours) after infection. After human embryonic lung fibroblasts were infected by HCMV, the degree of the patho-morphous in the HCMV+BML-111 group reached 100%2 days earlier than the of HCMV infection group. The infective virus titer reached the peak 2 days earlier than the HCMV infection group, but no significant difference was found between the two groups. BML-111 accelerates the replication of HCMV in the early stage of infection, but inhibits the expression of pp65 gene in the late stage. BML-111 has no impact on the proliferation of the infective HCMV titer in vitro.
7.Analysis about different type of health education on diabetes related costs

Objective To know the influence of short-term hard health education combined with net-working fellow-up on diabetes related costs. Methods Divided 83 diabetes patients into the experimental group (41 cases) and the control group (42 cases) randomly. Short-term hard health education combined with networking fellow-up and traditional health education was used in the two groups respectively, and then evaluat-ed the diabetes related costs between the two groups on the time points of the third, the sixth and the dozenth month. Results The costs of health products in the control group was significant higher than that of in the experimental group from the first month to the third month after the intervention, while the inspection expenses in the experimental group was significant higher than that of in the control group from the fourth month to the sixth month after the intervention. Conclusions Short-term bard health education can not change the average level of diabetes related costs, but it can change the constituent ratio of costs.
8.The clinical characteristics of postinfectious irritable bowel syndrome in Qingdao

Objective To investigate the clinical characteristics of postinfectious irritable bowel syndrome (PI-IBS) in Qingdao. Methods Two hundred and four PI-IBS and 2068 non-PI-IBS patients were investigated with questionnaire including general information, symptoms and quality of life scores with microecological study before and after therapy. Results (1) The morbidity rate of PI-IBS in female was 2. times of that in male, which was similar to that in non-PI-IBS. (2) Brainwork labors dominated in both PI-IBS and non-Pl-lBS patients. (3) As to the simultaneous presence of extra-gastrointestinal symptoms,there was no statistical difference between the rate of physical symptoms in PI-IBS and non-PI-IBS patients (X<'2>= 10. 5, P>0.05) ,but the rate of mental symptoms was higher in PI-IBS than in non-PI-IBS patients, and the difference was significant(X<'2>= 28.7, P<0.05). (4)The alteration of intestinal microflora rate in PI-IBS was obviously higher than that in non-PI-IBS patients. (5) The quality of life scores in PI-IBS was improved after treatment with Birid Triple Viable , and there was significant difference(t =3. 8, P<0.01),but there was no statistical difference in non-Pl-IBS (t = 1.5, P>0.05). Conclusion There was some difference in certain clinical characteristics between PI-IBS and non-PI-IBS patients in Qingdao.
9.Qianlongtong capsule elevates the Smad4 gene expression in prostate stromal cells.

OBJECTIVETo investigate the effects of the plasma containing Qianlongtong Capsule (QLT)-containing plasma on the expression of the Smad4 gene in prostate stromal cells in vitro and provide some experimental evidence for the treatment of benign prostatic hyperplasia (BPH) with Chinese medicinal compound.
METHODSFifteen cases of BPH were equally randomized to three groups to be treated with QLT at a high dose (6 capsules once), a medium dose (3 capsules once), and a low dose (1.5 capsules once), tid, for 7 days consecutively. QLT-containing plasma was collected from the patients. Prostate stromal cells were identified by immunofluorescence when they became monolayered and cultured in the QLT-containing plasma for 24 hours, followed by detection of the expression of the Smad4 gene by real-time quantitative PCR and that of the Smad4 protein by Western blot.
RESULTSAfter treatment with the QLT-containing plasma, the expression of the Smad4 gene in the stromal cells was significantly increased in a dose-dependent manner as compared with the blank control and no-QLT groups (P < 0.01). The expression of the Smad4 protein was also markedly elevated after treatment. The differences were statistically significant between the blank control and medium-dose groups (P < 0.01), low-dose and medium-dose groups (P < 0.05), and high-dose and the other groups (P < 0.01), but not between the blank control and low-dose groups (P > 0.05).
CONCLUSIONQLT-containing plasma could inhibit the proliferation and improve the apoptotic index of prostate stromal cells in vitro, which was related to the elevation of the mRNA and protein expressions of Smad4.

10.Effects of lipoxin receptor agonist BML-111 on IFN-βand IE86 mRNA expression of macrophages infected by human cytomegalovirus

Objectives To investigate the effects of lipoxin receptor agonist BML-111 on IFN-βand IE86 mRNA expression of macrophages infected by human cytomegalovirus (HCMV). Methods Macrophages were infected with HCMV (MOI=0.5), and the cultured cells were randomly divided into control group, HCMV group, HCMV+BML-111 group, and HCMV+MP group. The cells were collected at 0,1,2,4,8 and 12 h after infection, and the levels of IFN-βand IE86 mRNA were tested by real-time PCR. Results Compared with HCMV group, the levels of IFN-βmRNA in HCMV+BML-111 group increased significantly (P < 0.05), while the levels of IFN-βmRNA in HCMV+MP group decreased significantly (P < 0.05); Compared with HCMV group, there were no significant differences of the levels of IE86 mRNA in HCMV+BML-111 group (P>0.05), while the levels of IE86 mRNA in HCMV+MP group increased significantly (P < 0.05). Conclusion BML-111 exerts antiviral activity by promoting the expression of IFN-βmRNA at the early stage of HCMV infection.
