Objective To investigate the inhibitory effect of sorafenib on the proliferation of cervical cancer HeLa cells and explore the mechanism of its antitumor mechanism.Methods Cervical cancer HeLa cells were treated with sorafenib at a final concentration of 0.01,0.1,1.0,2.5,5.0 mg · L-1 (experimental group).Control group was not given any drug treatment (HeLa cells).The proliferation of cervical cancer HeLa cells cultured for 1,2,3 d were detected by MTT assay.The expression of vascular endothelial growth factor-C (VEGF-C) and phosphorylated extracellular signal-regulated kinase (p-ERK) protein were detected by immunofluorescence assay and enzyme-linked immunosorbent assay (ELISA).Results After 3 d,the proliferation inhibition rates in 0.01,0.1,1.0,2.5,5.0 mg · L-1 experimental groups were (9.16 ± 1.58)％,(23.07 ± 1.86)％,(30.86 ± 2.67)％,(55.94 ± 3.92)％,(68.75 ±4.48)％,had significant difference with that in control group,which was (5.15 ± 1.67)％ (P ＜0.05).Immunofluorescence assay showed that,the green fluorescence in control group enhanced significantly,and the green fluorescence decreased significantly after treated with different concentrations of sorafenib.The expression of VEGF-Cin 0.01,0.1,1.0 mg L-1 experimental groups were (1305.64 ± 119.82),(1235.57 ± 102.34),(1083.33 ±89.61)pg · mL-1,had significant difference with that in control group,which was(1326.54 ± 139.57)pg· mL-1(P ＜0.05).The expression of p-ERK in 0.01,0.1,1.0 mg · L-1 experimental groups were (2.21 ±0.22),(1.56 ± 0.21),(0.96 ± 0.13) μg · mL-1,had significant difference with that in control group,which was(2.34 ± 0.28)μg · mL-1 (P ＜ 0.05).The expression of VEGF-C and p-ERK protein in HeLa cell culture supernatant decreased gradually with the increasing the sorafenib concentration (P ＜ 0.05).Conclusion Sorafenib can inhibit the proliferation of cervical cancer HeLa cells,and the proliferation inhibition rate is positively related to time and concentration.The mechanism of tumor suppressor may be related to its ability to inhibit the expression of VEGF-C and p-ERK protein in cervical cancer HeLa cells.

Objective To introduce a new technique for urethral coverage in Snodgrass hypospadias repair,and to evaluate its effectiveness and complications.Methods From April 2003 to February 2006, this new procedure was performed in 289 children with hypospadias aged 3 months to 12 years (mean age,2. 4 years).The native meatus of urethra was identified subcoronal in 78 cases,penile/shaft in 136,penoscrotal in 36 and scrotal in 16;and 23 cases had undergoneⅡstage operation and re-operation.The overlapping coverage with bilateral shaft based vascularized dartos pedicle was done in the new urethra by Snodgrass hy- pospadias repair in these children.Results All the cases were followed for 3 months to 2 years.Postoper- atively,urinary fistulas developed in 32 cases (11%).Of them,11 were cured spontaneously within 4 weeks. The incidence of actual urinary fistula was 7% (21/289).Of the 21 fistulas which were not cured,11 (5%) occurred in 214 cases of distal hypospadias;and 10 (13%) in 75 cases of proximal hypospadias,Ⅱstage and re-operation.No dehiscence and diverticulum was found.Combined with mucosal collar technique,the ventral skin of the penis was sewn on the midline.During the follow-up,excellent cosmetic results with normal-ap- pearing circumcised penis were achieved in most patients.Conclusions Bilateral shaft based vascularized dartos pedicle urethral coverage procedure is a reliable and effective method for preventing urethral cutaneous fistulas and dehiscence.This method can reconstruct a satisfactory cosmetic appearance of the penis.

Objective To report the therapeutic effect of free dursum flap lnbule grafting to repair soft tissue defects in two or three fingers result from electric bum in just one operation.Methods Seven patients with electric injuries in two or three fingers were treated by the dorsum flap (or composite flap) lohule graft- ing.The procedure is followed:after dehridement,a trifoliated flap composed of medial pedal,first dorsal met- atarsal and lateral pedal flaps or a bifoliated flap composed of first dorsal metatarsal and lateral pedal flaps was designed.The combined flap contained anterior tihial artery and dorsal pedal artery as the stem vessel for anas- tomosing with the supplying vessel.The three tributaries of dorsal pedal artery:medial tarsal artery,first dor- sal metatarsal artery and lateral tarsal artery were enntained in the three leaves respectively.They could contain extensor digitorum longus tendons and cutaneous nerves and be used to cover the skin defect of two or three fin- gers,reconstruct the flexor tendons and finger arteries and nerves at the same time.Results All the flaps were survived completely,the wounds in all fingers healed primarily,however,skin graft in two feet necrosised partly,one got to healing required wound dressing for three weeks,another required further operation of trans- ferring medial supramalleolar flap due to tendon exposure.The patients were followed up from 3 to 24 months, all the flaps were of appropriate thickness,good texture and satisfactory sensation,all the fingers recovered good function of flection and extension except two fingers existing little limitation.Conclusion Free dorsum flap lohule grafting is an ideal way to treat electric injuries of multiple fingers because it can recnnstruct the skin,tendons,blood vessels and nerves in two or three fingers in just one operation.

OBJECTIVETo identify patients with SARS coronavirus infection who have only mild symptoms.

METHODEnzyme-linked immunosorbent assay was employed to detect serum antibody against SARS coronavirus in the lysate of whole SARS coronavirus from 19 SARS patients and 200 medical staff members without obvious SARS symptoms after possible exposure to the virus during routine medical practice.

RESULTSSerum IgG antibody against SARS coronavirus was detected in all the 19 SARS patients, and among the 200 staff members, 20 (10%) were found positive for the antibody but with no obvious or only mild symptoms.

CONCLUSIONSerum IgG antibody against SARS coronavirus is positive in a small proportion (around 10%) of the medical staff members exposed to the virus in our hospital, but may not cause obvious symptoms, suggesting SARS coronavirus infection might in some cases have mild or even no clinical manifestations.

BACKGROUNDThe lung is one of the most important organs that are sensitive to ischemia. We hypothesized that remote postconditioning (RPostC) induced by brief occlusion and reperfusion of the pulmonary artery could attenuate myocardial reperfusion injury.

METHODSThirty rabbits were randomized into three groups. Group ischemia-reperfusion (IR) (n = 10) were anesthetized rabbits subjected to 30-minute occlusion of the left anterior descending coronary artery followed by 180-minute reperfusion. Group RPostC (n = 10) had the left pulmonary artery blocked for five minutes followed by a 5-minute reperfusion, and the left anterior descending coronary artery (LAD) occluded for 30 minutes with a 180-minute reperfusion. Group L-N(w)-nitro-L-arginine methylester (L-NAME) + RPostC (n = 10) had the left pulmonary artery blocked for five minutes followed by a 5-minute reperfusion and intravenous infusion of L-NAME (10 mg/kg), and the LAD occluded for 30 minutes with a 180-minute reperfusion. Blood samples were taken for levels of creatine kinase (CK), superoxide dismutase (SOD) and malondialdehyde (MDA) at three different time points. At the end of the experiment, tissue samples of the infarcted region were harvested to calculate the cardiomyocyte apoptosis index (AI) by TUNEL. A piece of left and right lung tissue was harvested to evaluate the damage to the lung.

RESULTSAfter reperfusion for 180 minutes, the concentration of CK was lower in group RPostC, (4.79 ± 0.27) U/ml, than that in group IR, (6.23 ± 0.55) U/ml (P < 0.01), and group L-NAME + RPsotC, (5.86 ± 0.42) U/ml (P < 0.01). The concentration of MDA was lower in group RPostC, (6.06 ± 0.36) nmol/ml, than that in group IR, (11.41 ± 0.91) nmol/ml (P < 0.01), and group L-NAME + RPostC, (11.06 ± 0.62) nmol/ml (P < 0.01). The activity of SOD was higher in group RPostC, (242.34 ± 25.02) U/ml, than that in group IR, (148.05 ± 18.24) U/ml (P < 0.01), and group L-NAME + RPostC, (160.66 ± 9.55) U/ml (P < 0.01). The apoptosis index was lower in group RPostC, (14.25 ± 5.20)%, than that in group IR, (35.77 ± 10.09)% (P < 0.01), and group L-NAME + RPostC, (30.37 ± 7.76)% (P < 0.01). No significant difference caused by pulmonary ischemia was found in the lung tissue among the three groups.

CONCLUSIONSRPostC may attenuate myocardial ischemia-reperfusion injury connected to the activity of endothelial nitric oxide synthase. Brief pulmonary ischemia may not be harmful to lungs.

OBJECTIVETo explore the results of repairing widespread defects of traumatic soft tissue in heel by microvascular anastomotic anterolateral thigh flaps.

METHODSTwenty-six consecutive free anterolateral thigh flaps in 26 patients were transplanted for repairing widespread defects of traumatic soft tissue in heel from October 1997 to March 2005, suturing the descending branch of the lateral circumflex femoral vascular and posterior tibial vascular to reconstruct the blood supply of transplanted flaps, repairing lateral femoral cutaneous nerve to recover their sensation, fixing tensor fasciae in the calcaneum to add their stabilization.

RESULTSAll flaps survived completely,the wounds healed in the initial treatment, follow up 3 to 48 months, twenty-six cases achieved partial sensation, good contour and stabilization.

CONCLUSIONSThe anterolateral thigh flap is an ideal flap for repairing widespread defects of traumatic soft tissue in heel, because it has such advantages as adequate blood supply, big dermatosis area and covert donor site, furthermore, nervi cutaneous femoris lateralis and tensor fasciae offer the good sensation and adequate stabilization.

Objective To learn the changes of blood lead levels and serum biochemical parameters of the school agechildren from different areas. Methods All research objects, the school age children, were from three different areasrespectively, including a mountainous area (L area), an island area (H area) where there is not history of Pb pollution,and an industry area (N area) in relation to Pb pollution. The morning urine and peripheral venous blood samples werecollected from the school age children. Pb in blood (PbB), δ-aminoaevulinic acid in urine (ALA), Ca2+, BUN, Cr inserum, and Thyroid Stimulating Hormone (TSH), thyroxin (T4), free thyroxin (FT4) levels were detected. ResultsPbB levels [M was 36.0 ppb] of the school age children from N area were significantly higher than that of L area [22.0 ppb] andH area [23.8 ppb]. On the contrary, serum Ca2+ levels of the school age children from N area were significantly lower than thatof L area and H area. Serum T4 of N area was significantly lower than that of L area and H area. Serum FT4 of H area wassignificantly higher than that of L area and N area. And TSH of N area and H area were both obviously lower than that of L area.But all of these thyroxin indexes were in the range of normal values. Conclusion It should be widely concerned that thesignificant elevation of PbB levels may have a negative impact on school age children in the future.

Objective To study the curative effect of behavior therapy(military training and growth eduction)on conduct disorder of adolescent inpatients. Methods Using a random grouping and control-study method，87 inpatients with conduct disorder were divided into the study group and the control group. On the basis of drug therapy，the study group adopted the behavior therapy(military training and growth education),and was assessed 1 week and 12 weeks after admission using Achenbach's Child Behavior Checklist(CBCL). The assessment results were statistically treated. Results After 12 weeks of behavior tllerapy. the CBCL scores of the study group and the control group were(13.46±3.28)and(18.41±4.26)in anxiety and obsession,(8.65±3.34)and(12.57 4-3.28)in schizophrenia like，(11.87±3.21)and(14.85±3.55)in depression and withdrawn，(8.62±3.67)and(15.74±2.21)in immaturity，(9.23±2.41)and(15.62±2.51)in violation of discipline，(15.24±3.62)and(26.54±3.11)in aggressiveness，(7.16±3.32)and(11.89±2.65)in hyperactive syndrome. There Was a statistical meaning between the two groups(P<0.01).In addition，there Was a statistical meaning between before and after the therapy in the study group(P<0.01). Conclusions Behavior therapy(military training and growth education)is an effective method in the treatment of conduct disorder of adolescent inpatients.
