1.Dynamic determination of the medicine concentration of poisoned blood of acephate.

OBJECTIVE:
To observe rule of medicine concentration of blood and the last concentration that through hemoperfusion after poisoned by acephate.
METHODS:
Utilizeng the patient annual bonus venous blood in hospital emergency room, the content of acephate in plasma was analyzed by gas chromatography.
RESULTS:
After hemoperfusion, the concentration of acephate showed a rapid drop and the characteristic that the concentration drops quicker if medicine concentration of blood before hemoperfusion is higher.
CONCLUSION
Hemoperfusion is able to rapidly reduce the concentration of acephate in blood, its speed is determined by initial concentration and the beginning time of hemoperfusion etc.

2.The value of high-resolution contrast-enhanced three-dimensional magnetic resonance angiography in the evaluation of hepatic arterial anatomy

Objective Compared with digital subtraction angiography(DSA),to evaluate the value of high-resolution contrast-enhanced three-dimensional magnetic resonance angiography(3D MRA) in defining hepatic arterial anatomy.Methods The data about abdominal high-resolution contrast-enhanced 3D MRA and DSA of 26 patients (24 patients with primary liver cancer,2 patients with metastatic liver tumor) was retrospectively analyzed.The display quality of different segmental hepatic artery was scored with 4 grades and the agreement between high-resolution 3D MRA and DSA was determined with the weighted Kappa statistic.The depiction of hepatic arterial anatomy/anomalies and vascular pathology on high- resolution 3D MRA was assessed and compared with DSA.Results With respect to display quality,there was good or fair correlation between high-resolution 3D MRA and DSA for the common hepatic artery (the mean score respectively was 3.96,3.96 and Kappa value 0.99),gastroduodenal artery (the mean score respectively 3.85,3.88 and Kappa value 0.85 ),right hepatic artery(the mean score respectively 3.92, 3.96 and Kappa value 0.65 ),left hepatic artery (the mean score respectively 3.77,3.92 and Kappa value 0.43 ),left gastric artery(the mean score respectively 3.73,3.85 and Kappa value 0.43 ),right anterior artery (the mean score respectively 3.35,3.70 and Kappa value 0.53),right posterior artery (the mean score respectively 3.31,3.73 and Kappa value 0.46)and 1V segment artery (the mean score respectively 2.92,3.46 and Kappa value 0.51 );Poor correlation was found for the Ⅱsegment artery (the mean score respectively 2.15,3.35 and Kappa value 0.18)and Ⅲsegment artery (the mean score respectively 2.19, 3.35 and Kappa value 0.21 ).Compared with DSA,18 normal hepatic arterial anatomy and 7 arterial anomalies were accurately demonstrated(accuracy ratio 96.1% (25/26)),the obliteration of gastroduodenal artery correctly depicted in 1 patient on high-resolution 3D MRA image.Conclusions High-resolution 3D MRA can provide accurate evaluation of hepatic artery and has the capacity of depicting hepatic segment artery.
3.Initial experience of demonstrating hepatic artery in orthotopic liver transplanation candidates using high resolution contrast-enhanced MR angiography accelearated by generalized autocalibrating partial parallel acquisition

0.75).The distal branch of hepatic right/left artery were well delineated in 39 patients(scored 4),hepatic right/left artery in 23 patients(scored 3),proper hepatic artery in 4 patients(scored 2)and hepatic common artery in 1 patients(scored 1).The average score was 3.49.Fifty three cases with normal hepatic arterial anatomy and 14 cases with anomalies were accurately detected on high-resolution MRA images.Conclusion High- resolution contrast-enhanced MRA accelerated by GRAPPA could delineate the hepatic artery accurately.
4.Inoculation of bacille calmette guerin combined with intraperitoneal adenosine injection enhances antibacterial effect of macrophage against Mycobacterium tuberculosis in mice.

OBJECTIVEThe protection rate of inoculation with BCG vaccine is only 50 percent, and most of patients with tuberculosis had a history of BCG vaccine inoculation. Adenosine (ADO) has an immunomodulating effect; it promotes immune reaction by increasing number of macrophage and enhancing phagocytosis. The present study was designed to investigate if combined use of adenosine with BCG enhances the anti-Mycobacterium tuberculosis effect of macrophage in mice.
METHODSFifty BALB/C mice were divided randomly into 3 groups: BCG group (n = 21), BCG plus ADO group (n = 21) and control group (n = 8). The mice in BCG and BCG plus ADO groups were inoculated with 0.1 ml BCG intradermally and the mice in BCG plus ADO group were injected intraperitoneally with ADO 30 mg/(kg.d) for 5 days. The mice in BCG group and control group were injected with NS 0.1 ml/d for 5 days. Six weeks after the last injection, all mice were challenged with intravenous 1 x 10(6) CFU human Mycobacterium tuberculosis virulent strain. After challenging, lung and spleen specimens were taken at the 10th, 20th and 30th days from the mice of BCG and BCG plus ADO groups and at the 30th day from mice in control group. The pathological examinations of lung and spleen sections were performed after HE staining and acid-fast staining, and detection of cell apoptosis was also performed.
RESULTSConsolidation with neutrophil infiltration was found in most of the lung tissue taken at the day 30; there were a lot of tuberculous granulomas and Mycobacterium tuberculosis in the lungs of control group. The alveolar septum in BCG gradually became wide and in interstitium lymphocyte infiltration dominated, and there were less tuberculous granulomas but there were large number of Mycobacterium tuberculosis in the lungs from 10th to 30th days after challenging. The widening of alveolar septum and consolidation of lung tissue in BCG plus ADO group became milder with monocytes infiltration, and there were few tuberculosis granulomas and Mycobacterium tuberculosis in the lungs from 10th to 30th days after challenging.
CONCLUSIONADO could increase the number of monocyte-macrophages and promoted anti-bacterial effects of these cells.

5.Life-threatening meningitis resulting from transrectal prostate biopsy.

After antibiotic prophylaxis with metronidazole and levofloxacin, a transrectal sextant biopsy was performed under the guide of transrectal ultrasonography (TRUS) for a 75-year-old suspicious patient with prostate adenocarcinoma. Although antibiotics were also given after this procedure, the patient still developed fever, anxious, agrypnia and headache. Blood cultures remained negative. Lumbar puncture was performed and was consistent with Escherichia coli bacterial meningitis.

6.Effects of electroacupuncture on the expression of GDNF and Ret in Parkinson's disease model rats.

OBJECTIVETo explore the mechanism of electroacupuncture therapy on Parkinson's disease (PD).
METHODSFifty Wistar rats were randomly divided into a normal group, a sham-operation group, a model group, a Fengfu-Taichong group and a Shuanggu Yitong group. PD model was duplicated by microinjection of 6-Hydroxyl-Dopamine into right corpora striata, and by microinjection of normal saline in sham-operation group. Rats in normal group, sham-operation group and model group were not treated. In Fengfu-Taichong group, the rats were treated by electroacupuncture at "Fengfu" (GV 16) and "Taichong" (LR 3) on the basis of the PD model, and by electroacupuncture at "Fengfu" (GV 16), "Taichong" (LR 3), "Guanyuan" (CV 4) and "Zusanli" (ST 36) in Shuanggu Yitong group, once daily for 2 weeks. GDNF and Ret expression were detected by immunohistochemistry and western blotting, respectively.
RESULTSThe number of GDNF positive cells and the content of Ret receptor increased significantly in the two electroacupuncture groups compared with those in the other groups (all P < 0.01), and the expression of GDNF increased significantly in Shuanggu Yitong group compared with that in Fengfu-Taichong group (P < 0.01).
CONCLUSIONElectroacupuncture can not only increase the expression of GDNF, but also enhance its effect. "Shuanggu Yitong" method is better than simple acupuncture at "Fengfu" (GV 16) and "Taichong" (LR 3) in increasing expression of GDNF.

7.Therapeutic effect of ankle bath associated with chiropractic technique on the patients with ankle fractures in later period

Objective To observe the therapeutic effect of ankle bath by traditional Chinese medicine associated with chiropractic technique on the patients with ankle fractures in later period so as to explore the nursing and its influencing factors.Methods From June 2009 to December 2011,a total of 76 cases with ankle fractures were selected and radomly divided into control group and observation group.Control group took medicine that could activate blood circulation to dissipated blood stasis,bathed ankle in warm water and took functional exercise,while the observation group received Chinese medicine bath and massage followed by functional exercise.Results After 10 days,the difference between observation group and control group in the degree of pain and activity of ankle and joint function appeared ( 14.86 ±7.21vs 8.68 ± 8.98,7.18 ± 1.91vs 4.34 ± 1.66,2.84 ± 1.46 vs 1.97 ± 1.17 ) and the difference was statistically significant ( t =3.31,6.90,2.85 ;P<0.05).And 20 days later,the improvement rate of observation group was 92.1% and 71.06% of control group,and the difference was statistically significant ( u =2.07,P < 0.05).Conclusions Bath foot by traditional Chinese medicine associated with chiropractic technique can make qi and blood be unobstructed,activate blood circulation to dissipate blood stasis,promote ankle joint swelling and analgetic,and finally enhance joint function to recover.
8.Effects of apolipoprotein E genetic polymorphism on susceptibility of depression and efficacy of antidepressants.

OBJECTIVETo assess the effects of apolipoprotein E (APOE) polymorphism on the susceptibility of depression and the efficacy of antidepressants.
METHODSA total of 275 patients with depression, who met the diagnostic criteria of both CCMD-3 and DSM-4, were randomly assigned into venlafaxine group (n=136)and paroxetine group(n=139). Another 202 healthy subjects were enrolled as the control group. Hamilton Rating Scale for Depression (HAMD)-17 was adopted as the primary rating instrument to evaluate the severity of depression on the baseline and the end of the 1st, 2nd, 4th, 6th week after treatment, respectively. HAMD scores ≤7 was defined as remission, and the reduction of HAMD scores ≥50% was defined as response while <50% was defined as invalid. PCR-restriction fragment length polymorphisms (PCR-RFLP) was applied to detect the genetic polymorphism of the APOE in the case groups and control group.
RESULTSIn the venlafaxine group, the remission rate was 52.9%(n=72), the response rate was 26.5%(n=36), and the invalid rate was 20.6%(n=28), whereas the corresponding data in the paroxetine group wee 42.4%(n=59), 31.7%(n=44), and 25.9% (n=36), respectively. There were no significant differences in the efficacy between the two groups(p>0.05). In the venlafaxine group, there were no significant differences in the genotypes and the allele distribution frequency of APOEΕ2/Ε3/Ε4 between the remitters, nonremitters, and healthy controls at the end of the 6th week(p>0.05), but there was significant differences in the allele distribution frequency between the nonremitters and healthy controls(p=0.02). In paroxetine group, there were no significant differences in the genotypes and the allele distribution frequency of APOEΕ2/Ε3/Ε4 among the remitters, nonremitters and healthy controls at the end of the 6th week(p>0.05), but there were significant differences in the allele distribution frequency between the nonremitters and healthy controls (p=0.04); in addition, there were also significant differences in Ε2/Ε3 and Ε4 allele between the two groups (p=0.014).
CONCLUSIONSThe APOE gene may not play a major role in the pathogenesis of major depression. The efficacy of venlafaxine is same as paroxetine after treatment for six weeks. The APOE (Ε2+Ε3) allele may be an indicator of the bad efficacy of paroxetine treatment.

9.The comparative study of diffusion-weighted MR imaging with modified sensitivity encoding technique for small hepatocellular carcinoma lesions

Objective To prospectively evaluate whether DWI with mSENSE(mSENSE-DW1)can help to improve image quality and affect the calculation of lesion's ADC,compared with DWI with conventional phase encoding(conventional DWI)in patients with small HCC lesions.Methods Thirty-two patients with 47 small HCC lesions underwent single-shot spin-echo echo-planar.(SE-EPI)DWI with conventional phase encoding and mSENSE.Three b values of 300,500 and 800 s/mm~2 were used.Image quality and CNR between conventional DWI and mSENSE-DWI group were compared with different b value. Lesion's ADC were also calculated and compared.Matched-pairs Wilcoxon signed rank test and Friedman test were used to test statistical significance.Results The image quality was assessed with a five-point scale.For conventional DWI group,image quality score of 2 was assigned in 5 cases,score of 3 in 24 cases, score of 4 in remaining 3 eases with b=300 s/mm~2,score of 2 in 9 eases,score of 3 in 22 cases,score of 4 in remainingl cases with b=500 s/mm~2,score of 1 in 2 cases,score of 2 in 24 cases,score of 3 in remaining 6 cases with b=800 s/mm~2.For mSENSE-DWI group,image quality score of 3 was assigned in 4 cases,score of 4 in 25 cases,score of 5 in 3 eases with b=300 s/mm~2,score of 3 in 10 cases,score of 4 in 20 cases,score of 5 in remaining 2 cases with b=500 s/mm~2,score of 2 in 7 cases,score of 3 in 23 eases, score of 4 in remaining 2 cases with b=800 s/mm~2.Higher image quality scores were achieved at mSENSE- DWI group than conventional DWI group(Z=-5.578,-5.488,-4.796 respectively,P

Objective To reasonably make use of the social support system,help the patients adapt to the disease process,increase the quality of life,improve the curative effect and promote the recovery of patients.Methods The status of social support.The quality of life and the correlation of the two were surveyed among 235 primly hver CanCer patients using a questionnaire survey method.Results The total score of social support acquired by the liver cancer patients was(44.40±7.54).And the social support maily came from the family;The total score of life quality Was(48.75±7.31).There Was a positive correlation between the social support and the whole quahty of life(P<0.01).The social support had positive correlations with the five dimentions of quuty of life(P<0.01). Condusions Good social support is the important foundation for the fiver cancer patients to cope the disease,overcome the pain ofdisease and increase their therapy confidence.It can playan active role in improving the quality of life,maintaining the physical and mental state,and promoting the rehabilitation.
