Approximately 3 to 5% of newly diagnosed metastatic cancers are of unknown primary tissue origin due to difficulties identifying a primary tumor using standard diagnostic approaches.MicroRNAs (miRNAs) have recently been demonstrated to be able to assist pathologist with improved accuracy in diagnosing cancers of unknown primary origin (CUP).In this short commentary,we will highlight some of the recent advancements in miRNA based cancer diagnosis as well as some future directions for the field.
2.Randomized-controlled study on anti-inflammation and safety of three drugs after Nd : YAG laser posterior capsulotomy

Background Nd: YAG laser posterior capsulotomy is an important way for after cataract.Usually the patient will use glucocorticoid eye drops to treat the anterior chamber inflammation after operation,but there is potential risk of elevating intraocular pressure (IOP).Objective This study was to compare the clinical effectiveness and safety of loteprednol etabonate ophthalmic suspension,tobramycin+ dexamethasone eye drops and fluorometholone eye drops following Nd: YAG laser posterior capsulotomy.Methods A randomized-controlled clinical trail was performed.One hundrcd and seventy-onc cycs of 127 paticnts who received Nd: YAG laser posterior capsulotomy for after cataract were randomly divided into four groups.Loteprednol etabonate ophthalmic suspension,fluorometholone eye drops,tobramycin+dexamethasone eye drops and systane eye drops was topically administered respectively in the four groups after laser posterior capsulotomy and 6 times per day for 5 days.IOP was measured with Goldmann tomometer 1 hour before operation and 1 hour,1 day,3 days and 7 days after operation.The ocular anterior segment inflammatory response was examined under the slit lamp and scored based on the Peizeng criteria.Written informed consent was obtained from each patient before any relevant medical procedure.Results The IOP was (18.2 ±4.7),(20.1 ±5.7),(18.7±5.5),(19.0 ±4.1),(19.5 ±3.5) mmHg in various time points in the loteprednol etabonate group; (18.7 ±5.3),(20.9±5.7),(21.3±4.5),(21.0±4.9),(22.5±6.5) mmHg in the fluorometholone eye drops group ; (17.9± 6.3),(20.3 ± 6.1),(23.0 ± 3.7),(24.7 ± 4.9),(24.5 ± 6.5) mmHg in the tobramycin +dexamethasone group and(18.4±6.3),(20.7±3.7),(22.7±6.5),(19.6±4.8),(18.5±3.5) mmHg in the systane group,showing a significant difference among the 4 groups (Fgroup =3.876,P =0.023).With the time lapse,the IOP was gradually reduced in the loteprednol etabonate group and systane group,but that in the fluorometholone group and tobramycin+dexamethasone group was elevated,showing a significant difference among them (Ftime =3.801,P =0.031).No any ocular and systemic adverse effect was found in various groups.The percentage of grade 1 and 2 of aqueous inflammatory cells was lower in the loteprednol etabonate group and tobramycin+dexamethasone group than the fluorometholone group and fluorometholone group and systane group(H =8.276,P =0.012).The percentage of Ⅰgrade of aqueous flare was 8% in the loteprednol etabonate group,22% in the fluorometholone group,18% in the tobramycin+dexamethasone group and 30% in the systane group,with a significant difference among them (H=9.305,P=0.000).Conclusions The use of corticosteroid eye drops can relieve the inflammatory response of ocular anterior chamber after Nd: YAG laser posterior capsulotomy.Loteprednol etabonate ophthalmic suspension has a better anti-inflammatory effect and less influence on IOP.
3.The Apoptosis Induction of CT 26 Cells by Recombinant Clostridium difficile Toxin B

To investigate the induction of apoptosis of mouse colonic adenocarcinoma CT26 cells by recombinant Clostridium difficile toxin B (rTcdB), CT26 cells were exposed to different concentrations of rTcd B. Inhibition of cell proliferation was measured by MTT assay. The activation of Caspase 3 was measured by colorimetric method. Cell morphological analysis and flow cytometry were performed to confirm cell apoptosis. rTcd B inhibited the proliferation of CT26 cells in a timeand dose-dependent manner. Caspase 3 activity in CT26 cells was elevated remarkably after rTcd B exposure for 6 h, 12 h, 18 h or 24 h, as compared with the control group. Morphological changes were observed by fluorescence microscopy. The exposure of rTcd B to CT26 cells induced a timeand dose-dependent apoptotic cell death as determined by flow cytometry analysis. The results showed that recombinant Clostridium difficile toxin B induced apoptosis of CT26 cells.

5.Effect of nursing intervention on psychological stress factors of patients in the ophthalmology department

Objective To explore psychological stress factors of patients in the ophthalmology department and psychological nursing interventions as well as effect, to promote the rehabilitation of patients. Methods A total of 289 patients from our ophthalmology department was investigated with the self-designed questionnaire. Results Major psychological stress factors of Ophthalmology inpatient were knowing nothing a variety of points for attention(92%), fearing and worrying the disease and surgery(85%), worrying about the level of health care technology of medical staff (84%). Psychological stress of patients released through psychological nursing intervention measures such as cognitive intervention and environmental intervention. 243 patients of fearing and worrying the disease and surgery reduced to 35 before and after intervene, The difference was statistically significant (X2 = 299.84, P < 0. 01). Conclusions We should properly guide and inspire positive mental patients based on patients' mental state, give mental nursing intervention, promote the rehabilitation of eye disease.
6.Differential diagnosis of hyperdensities on computed tomography immediately after intra-arteriai thrombolysis in patients with acute ischemic stroke

Objective The present study was to differentiate the hyperdensities on CT immediately after intra-arterial thrombolysis in patients with acute isehemic stroke.Methods Twenty two patients with acute ischemic stroke were treated with intra-arterial combining with intravenous thrombolysis within 6 hours after onset.All patients underwent nonenhaneed CT scans before,immediately and 24 hours after thrombolytic therapy.The hyperdensities on CT after intra-arterial thrombolysis were analyzed retrospectively.Results Five hyperdense areas were seen in 22 patients immediately after thrombolytie therapy.According to their locations, CT values and follow-up CT scans,the hyperdensities on CT imaging were classified into two groups:contrast enhancement and hemorrhagic transformation.The former was characterized by rapid clearance of the hyperdensity lesion with maximum Hounsifild Unit<90,on the contrary,the latter was noted by persistence of hyperdensity lesion after 24 hours CT scan with maximum Hounsifild Unit>90.Two of the five hyperdense lesion patients were confirmed to be contrast enhancement with location in cerebral cortex,the other were hemorrhagic transformation,mostly located in basal ganglia.Hyperdensity in patients with contrast enhancement showed neurological improvement although no further medical cares were offered.Conclusions Different kind of hyperdensity on CT immediately after intra-arterial thrombolysis in patients with acute ischemic stroke can be differentiated according to its location,CT value and follow-up CT scan.When contrast enhancement occurred,no further medical care is needed.
7.Effects of tetrapod-like zinc oxide whiskers incorporation on antibacterial activity of composite resin

Objective To evaluate effects of incorporating tetrapod-like zinc oxide whisker (T-ZnOw) antibacterial agent on the antibacterial activity of composite resin, compared with that of the silver-based inorganic antibacterial agent. Methods The minimal inhibitory concentration (MIC) and minimal bactericidal concentration (MBC) of the two different antibacterial agents against Streptococcus mutans were investigated using the broth dilution test Then the antibacterial activities of the self-cured composite resin specimens with different incorporating concentrations of the two antibacterial agents were evaluated using direct contact test And the antibacterial activities of the resin specimens were examined again after 3 months of accelerating aging. Results The MIC (MBC) of the T-ZnOw antibacterial agent and silver-based inorganic antibacterial agent were 0.15625(0.3125) g/L and 0. 15625(0.15625) g/L When the concentrations of T-ZnOw added to composite resin were 3%, 5% , and 10% (w/w), the antibacterialratios of the resin specimens were (84.85±5.16)%, (94.22 ± 3.73)%, and (99.43±0.48)%respectively. When the concentrations of the silver-based antibacterial agent added to composite resin were 1%, 3%, and 5%, the antibacterial ratios were (71.01±7.67)%, (90.76±5.91)%, and (97. 19 ±4.10)% respectively. The antibacterial ratios of the resin specimens containing 5% of both antibacterial agents were (89.89 ± 5.55) % and (78.79 ± 7.81) % respectively after 3 months of accelerating aging.Conclusions Incorporation of the T-ZnOw antibacterial agent in composite resin can improve the antibacterial performance of the resin, and the resin exhibits better antibacterial performance than that incorporating silver-based inorganic antibacterial agent after 3 months of aging.
8.Effects of three different zinc oxide incorporation on the antibacterial activity against Streptococcus mutans of composite resin.

OBJECTIVETo investigate the effects of incorporating three different zinc oxide (ZnO) on the antibacterial activity of composite resin.
METHODSThe minimal inhibitory concentration (MIC) and minimal bactericidal concentration (MBC) of nano-ZnO, tetrapod-like zinc oxide whiskers (T-ZnOw), micro-ZnO against Streptococcus mutans were examined by the broth dilution test. Then the three different ZnO were added to the powder of one kind of bicomponent self-cured composite resin at 5% respectively, and the antibacterial activities of the resin specimens were evaluated using the membrane covering method before and after 3-month accelerating aging.
RESULTSThe MIC values of the three different ZnO against Streptococcus mutans were 78.13, 312.50 and 1 250.00 microg/mL respectively and the MBC values were 156.25, 625.00, 1,250.00 microg/mL respectively. The antibacterial ratios of the resin specimens incorporating with 5% of the three different ZnO were (93.58+/-5.95)%, (89.42+/-4.11)% and (78.97+/-3.90)% respectively, while after 3-month accelerating aging those were (89.01+/-7.91)%, (84.63+/-4.72)% and (72.27+/-3.89)%.
CONCLUSIONThe three different ZnO could improve the antibacterial activity of the composite resin. The nano-ZnO exhibit the strongest antibacterial activity, while the micro-ZnO weakest. The T-ZnOw presents comparatively strong antibacterial activity although with smaller specific surface area.

9.Ultrasound surveillance of cervical lymph node metastasis in thoracic esophageal carcinoma.

OBJECTIVETo improve the accuracy of preoperative evaluation of cervical lymph node metastasis in thoracic esophageal squamous carcinoma.
METHODSForty-two patients with thoracic esophageal squamous carcinoma underwent neck ultrasonography. Enlarged lymph nodes with their long axis greater than 10 mm and a short-to-long axis ratio greater than 0.5 were considered as metastatic.
RESULTSPreoperative neck ultrasonography revealed the enlarged lymph nodes in 16 patients, but only in 5 (31%) cases the nodes were palpable. Among them 9 were classified as metastatic (cM(1-LN)), including 4 patients with palpable nodes. In 5 cM(1-LN) patients surgical intervention was canceled and the remaining 37 patients underwent trans-thoracic esophagectomy. Cervical node metastasis (pM(1-LN)) was confirmed pathologically in 6 surgical patients, 4 with tumors invading the adventitia (pT3) and the other 2 into the surrounding structure (pT(4)) (pT(1), pT(2) vs. pT(3), pT(4), P = 0.020). All 6 pM(1-LN) patients had concomitant mediastinal node metastasis and 4 of them had upper abdominal node metastasis. Statistically significant relationship was detected between cervical and abdominal nodal status (r = 0.536, P = 0.007). In comparison with the results of pathological examination and treatment response, the accuracy and sensitivity were 81% and 95% (P = 0.043), 36% and 82% (P = 0.081), respectively, for palpation and ultrasonography. Five out of 39 (13%) patients had their therapy changed due to ultrasonographic findings.
CONCLUSIONSNeck ultrasonography for cervical lymphadenopathy is of high sensitivity and accuracy, which plays an important role in the preoperative evaluation and therapeutic decision-making.

Nine compounds were isolated from an ethanol extract of the roots of K. roxburghii by using a combination of various chromatographic techniques including column chromatography over silica gel, MCI gel, Sephadex LH-20, and reversed-phase HPLC. On the basis of physical-chemical properties and spectroscopic data analysis, their structures were identified as munjistin (1), 1-methoxy-3,6-dihydroxy-2-hydroxymethyl-9,10-anthraquinone (2), 1,2,3-trihydroxy-9,10-anthraquinone (3), arjunolic acid (4), hyptatic acid-A (5), hyptatic acid-B (6), 2α,3β,24-trihydroxyurs-12-en-28-oic acid (7), 2α,3β,23-trihydroxyurs-12-en-28-oic acid (8), and daucosterol (9). Compounds 1-9 were obtained from this genus for the first time.
