Vitamin D deficiency is common among the elderly,which may be an important pathogenic factor in Alzheimer's disease(AD).Vitamin D has multiple functions including immune regulation,anti-oxidation,regulation of neurotrophic factors/acetylcholine neurotransmitters,and clearance of amyloid beta peptide; meanwhile,it may be helpful to avoid hyperparathyroidism.Vitamin D supplementation among the elderly may prevent the loss of neurons and slow down the AD-related cognitive decline,and therefore may play a role in the multi-targeted therapies for AD.
3.Incidence of cerebral venous sinus thrombosis in patients with idiopathic intracranial hypertension

Objective To explore the rate of incidence of cerebral venous sinus thrombosis(CVST)in patients with idiopathic in- tracranial hypertension(IIH).Design Restrospective case series.Participants 92 cases with idiopathic intracranial hypertension.Meth- otis All patients diagnosed with papilledema from January 1,2000 through May 1,2007 at our ophthalmology center.Consecutive pa- tients with a diagnosis of papilledema were identified.Patients with space-occupying lesions,hydrocepbalus,or meningitis were excluded. The remaining patients were evaluated with lumbar puncture,magnetic resonance imaging(MRI)and magnetic resonance venography (MRV).Main Outcome Measures The rate of incidence of cerebral venous sinus thrombosis(CVST)in patients with idiopathic in- tracranial hypertension(IIH).Results Excluding patients with mass lesions,meningitis,or hydrocephalus,the occurrence of CVST was 7 (7.6%)of 92 patients with presumed IIH.One additional patients had a diagnosis of suspected CVST.Cerebral venous sinus thrombosis was diagnosed in 1 of the 7 patients with MRI alone,whereas it was evident in all 7 patients with MRV.Conclusions Cerebral venous si- nus thrombosis accounts for 7.6% of patients with presumed IIH in our ophthalmology services.Magnetic resonance venography in com- bination with MRI is recommended to identify this subgroup of patients.(Ophthalmol CHN,2007,16:410-413)
4.Delayed treatment of 51 pediatric displaced femoral neck fractures

Objective To compare open versus closed reduction in treatment of children with displaced femoral neck fracture whose reduction is delayed by more than 24 hours.Methods We retrospectively reviewed 51 children who had been treated at our department for femoral neck fracture between May 2005 and May 2012 after their reduction had been delayed for more than 24 hours.They were 37 boys and 14 girls,1.7 to 15.5 years of age (average,9.1 years).According to Delbet's classification,27 cases belonged to type Ⅱ,18 to type Ⅲ,and 6 to type Ⅳ.Of them,18 underwent closed reduction and 33 open reduction.Their complications were recorded in detail.Their therapeutic results were evaluated according to Ratliff's criteria and compared between the 2 methods of reduction.Results The 51 children were followed up for an average of 36.7 months (range,from 17 to 61 months).Anatomic reduction was achieved in 6 cases in the closed reduction group (33.3%) and 29 cases in the open reduction group (87.9%),respectively.All fractures healed after an average of 10.9 weeks (range,from 9 to 13 weeks),with one exception of nonunion in the closed reduction group.The incidence of femoral head necrosis in the closed reduction group (27.8%,5/18) was significantly higher than that in the open reduction group (15.2%,5/33) (P < 0.05).According to the Ratliff's evaluation at the final follow-ups,the good to excellent rate in the closed reduction group(66.7%,12/18) was significantly lower than that in the open reduction group (84.8%,28/33) (P <0.05).Coxa vara occurred in 3 children in the closed reduction group but in none in the open reduction group.Conclusion Open reduction with internal fixation may lead to better outcomes than closed reduction in the treatment of children with displaced femoral neck fracture whose reduction is delayed by more than 24 hours,probably because open reduction can result in better reduction quality.
5.Prophylactic use of antibiotics for cancer inpatients with leukocyte count ≤ 1 × 109/L

Objective To investigate the infection prevention effect of prophylactic use of antibiotics for cancer inpatients with leukocyte count ≤ 1 × 109/L.Then to provide the basis for rational use of antibiotics.Methods It was retrospectively surveyed that the infection situation and prophylactic usage of antibiotics in the 426 cancer inpatients with leukocyte count ≤ 1 × 109/L caused by radiotherapy and chemotherapy.Results In the 426 inpatients,the infection rates of patients with and without prophylactic use of antibiotics were 11.0% and 20.9% respectively.The difference was statistically significant (x2 =6.815,P =0.009).The leukocyte count median values of patients with and without infection were 0.73 × 109/L and 0.77 × 109/L respectively.The difference had no statistical significance(Z =-1.257,P =0.209).The main infection sites were lower respiratory tract,upper respiratory tract and gastrointestinal tract with infection rates of 48%,12% and 11%,respectively.The top five pathogens were Klebsiella pneumonia,Enterobacter cloacae,Escherichia coli,Enterobacter aerogenes and Staphylococcus aureus.The most use of antibiotics were cephalosporin.Conclusion When the leukocyte counts of cancer inpatients are less than or equal to 1 × 109/L induced by chemotherapy and radiotherapy,reasonable prophylactic use of antibiotics can reduce hospital infection rates.
6. Adding hormone in cocktail promotes rapid recovery after unicompartmental knee arthroplasty

BACKGROUND: Postoperative pain is a common clinical problem in unicompartmental knee arthroplasty, and severe pain can affect postoperative efficacy. Hormone is a common and effective anti-inflammatory drug, and there is some controversy over whether to add hormone to the local injection of cocktail in unicompartmental knee arthroplasty. OBJECTIVE: To evaluate the effectiveness of adding hormones to the cocktail to promote rapid recovery after unicompartmental knee arthroplasty. METHODS: Osteoarthritis patients receiving unilateral unicompartmental knee arthroplasty from October 2017 to March 2019 were randomly divided into a hormone group (local injection of cocktail + 40 mg triamcinolone acetonide) and a control group (local injection of cocktail + equivalent normal saline). Visual analogue scale score, fentanyl consumption, knee function, inflammatory indicators, adverse reactions and complications were observed in both groups after surgery. RESULTS AND CONCLUSION: (1) Finally, 80 patients were included in the study (n=40 in each group). There was comparability between the two groups (P > 0.05). (2) The static and dynamic visual analogue scale scores of patients in the hormone group at 1, 2 and 3 days after surgery were significantly lower than those in the control group (P < 0.05). (3) The dosage of fentanyl in the hormone group was significantly lower than that in the control group at 48 hours after surgery (P < 0.05). (4) At 1, 3, 5 days and 3 months after surgery, the knee flexion range of motion in the hormone group was higher than that of the control group (P < 0.05). (5) Hospital for Special Surgery score of knee in the hormone group was higher than that in the control group at 3 months after surgery (P < 0.05). There was no statistically significant difference in Hospital for Special Surgery scores of the knee between two groups at 6 months after surgery (P > 0.05). (6) C-reactive protein levels at 3 days and interleukin-6 levels at 24 hours after surgery in the hormone group were significantly lower than those in the control group (P < 0.05). (7) The incidence of complications in the hormone group was significantly lower than that in the control group (P < 0.05). (8) Addition of hormones in the cocktail can effectively promote the rapid recovery after unicompartmental knee arthroplasty..
7.Long-term follow-up for neurogenic bladder treated by sacral neuromodulation: 1 case report

ObjectiveTo explore the efficacy and safety of sacral neuromodulation(SNM) for the treatment to neurogenic bladder.MethodsOne patient with neurogenic bladder after spinal bifida underwent the therapy of SNM 42 months ago.The therapeutic efficacy was evaluated and followed up by means of the symptom improvement and voiding diaries.ResultsDuring the test stimulation period,there were significant improvements(>50%) in the objective findings and subjective symptoms.This patient received permanent electrode and neurostimulator implantation and lower urinary tract symptoms were improved continuously until 42 months.ConclusionSNM may be effective for some neurogenic dysfunctions of the bladder.
8.Alteration of serum lipids in patients with acute leukemia and its clinical significance.

To explore the clinical significance on alteration of serum lipids in acute leukemia (AL) patients, the level of serum lipids was monitored in 86 AL cases by using of automatic biochemical analyzer. The results showed that TG was significantly higher (P < 0.05) while TC, LDL-C and HDL-C were obviously lower (P < 0.05) in AL patients than those in normal controls. After chemotherapy, TG decreased but TC, LDL-C and HDL-C were still higher (P < 0.05) as comparing to pre-treatment in complete remission cases. There were no changes of those parameters in non-remission patients. It is concluded that determination of serum lipids level in AL patients is a simple and important accessory index to evaluate curative effect and monitor patient's condition.

10.Constructive Exploration on the Digital Service System of Traditional Chinese Medicine Decoction

Presently, there are many issues in traditional Chinese medicine (TCM) decoction, such as the uncertain sources of TCM, the lack of reminder for medication taboo, the nonstardard herb operation, and difficult supervision, etc. A digital service system of TCM decoction was established to solve the problems mentioned above. The digital service system mainly includes automatic coding for checking in & out, drug medication taboo database, digital operation in decoction, distribution through 2D code, the corresponding application for mobile phone, and the information supervision platform for TCM decoction. The digital service system of TCM decoction can track the quality & duty of the pieces, remind decoction medicine contraindications, improve the standard operation process of decoction, develop decoction distribution & tracking through cell phone, save the waiting time, and hence provides a new supervising method for TCM decoction. The digital service system of TCM decoction solves the key issues for the formula, operation, delivery and supervision of TCM. In the same vein, this system will expand the market share of TCM decoction and promote the development of TCM.
