Carcinoma, Renal Cell*

Hepatoid adenocarcinoma of the stomach is a gastric carcinoma with both adenocarcinomatous and hepatocellular carcinomatous differentiation. It usually produces large amount of serum alpha-fetoprotein. It often occurs in aged person, commonly located in the gastric antrum. Because of early lymph node and liver metastasis, prognosis is poor. A case of hepatoid adenocarcinoma of the stomach in a 49-year-old man is reported. The patient's serum alpha-fetoprotein level was high(3380ng/ml). Liver function test was normal, otherwise. A mass was observed in right lobe of the liver on US and CT. It was hypervascular with increased uptake of lipiodol on hepatic arteriography. A large ulcerative tumor involving the gastric antrum and body was also found at barium study of the stomach. Subtotal gastrectomy and right Iobectomy of the liver resulted in rapid decrease in serum alpha-fetoprotein. Histopathologically the diagnosis was hepatoid adenocarcinoma of the stomach with liver metastasis.

BACKGROUND: Korea is, known to be an endemic area of viral hepatitis B, and 5-6% of population are carriers. Immunization can reduce the spread of hepatitis B infection. Hower, not all people respond to hepatitis B immunsation. 5-15% of primary vaccinees fail to be immunized. After revaccination, only 37.5 44% of the primary failures elicit antibody response. Recent studies have reported that it is effective to inject a double dose vaccination in immunocompromsed patients. In some country, they recommend that neonate whode mother is HBV carrier shoud receive double dose vaccination. This study documented the results of double dose vaccintion only once to nonrsponders toward the HBV immunzation. METHODS: Healthy nonresponders to Hepatitis B immunization were recuited from Sep. 1995 to Aug. 1996. Subjects with their AST/ALT level over 40, or over 65 years old were excluded. They were received 2ml of plasma-derived vaccine(Hepavax-B) intramuscularly in the deltoid muscle. Anti-HBs antibody testing were performed at 1 to 3 months after vaccintion. RESULTS: 17 healthy nanreponders to Hepatitis B immunization(male 10, female 7) were included. Their mean ages were 47(32 to64), AST/ALT level 20/19(16 to 28/11 to 35), smoking 4.3pys(1 to 7). Seroconversion occured in 13 of them(76.5%). CONCLUSIONS: Nonresponders to Hepatitis B vaccintion respond well to double dose vaccination once.

OBJECTIVES: The purpose of this study is to compare the perceptions of the rural healthcare service improvement project' performance and reorganization of public health centers between project staffs and local residents. METHODS: Data collection from this study was performed in 141 project areas using structured questionnaires. Data analysis was used in SPSS 22.0 version. RESULTS: The public health center staffs were more positive about the items for improving health facilities than the local residents. Residents in the Si area generally perceived performance as more positive than residents in the Gun area, while public health center staffs in the Gun area perceived performance as more positive than public health center staffs in the Si area. Local residents expressed negative opinions about the reduction in the number of branches of public health clinics and health medical clinics. CONCLUSIONS: In conclusion, careful improvement projects for rural health care and the establishment of health care systems will be necessary, reflecting the opinions of local residents, along with a variety of regional characteristics.

OBJECTIVES: The purpose of this paper is to determine an efficient operation plan for a smoking cessation clinic in public health centers. To do so, the primary success factors in smoking cessation have been analyzed after classifying the smoking cessation programs of public health centers into urban and rural areas. METHODS: A study was conducted with 262,837 smokers age 19 or older who were provided with smoking cessation services for more than 6 months through smoking cessation clinics at public health centers(250 clinics nationwide) from July 16, 2006 to July 15, 2007. RESULTS: Urban areas indicated that gender, age, social security, the frequency of counseling, re-enrollment in the program, the number of cigarettes per day and alcohol problems were all factors that influenced the success of smoking cessation. On the other hand, in the rural areas the total number of counseling, re-enrollment in the program, the number of cigarettes per day and alcohol problems were the influential factors. Furthermore, the urban area confirmed that the frequency of counseling, re-enrollment in the program, the number of cigarettes per day and alcohol problems affected traveling while the rural area was affected by social security, the frequency of counseling and re-enrollment. CONCLUSIONS: It has been confirmed that the success rate and factors of smoking cessation clinics can vary depending on the region and enrollment method.

Incidence of adenocarcinoma of the kidney in long-term hemodialysis is higher than in the general group. Although renal cancers occuring in dialysis patients are often small and asymptomatic, some patients present with cutaneous, visceral, or skeletal metastasis. We report a case of adenocarcinoma of the kidney in chronic hemodialysis patient. A 36-year-old male was diagnosed as chronic renal failure 6 years ago and then had been recieving hemodialysis. Renal tumor and multiple renal cysts were incidentally found in right kidney by renal ultrasonography and abdominal CT scanning. Radical nephrectomy was done and pathologic diagnosis was renal cell carcinoma and multiple simple renal cysts.
