1.A Case of Carotico-Cavernous Fistula Treated by Intraluminal Occlusion.

A case of carotico-cavernous fistula was managed by intravascular occlusion using a Fogarty catheter and ligation of the carotid artery. The fistula was obliterated by the procedure and neurologic deficits improved progressively without further recurrence. The radiological feature, symptomatology and surgical management were discussed.

2.Development and Evaluation of the Quick Anaero-system-A New Disposable Anaerobic Culture System.

BACKGROUND: We developed a new disposable anaerobic culture system, namely, the Quick anaero-system, for easy culturing of obligate anaerobes. METHODS: Our system consists of 3 components: 1) new disposable anaerobic gas pack, 2) disposable culture-envelope and sealer, and 3) reusable stainless plate rack with mesh containing 10 g of palladium catalyst pellets. To evaluate the efficiency of our system, we used 12 anaerobic bacteria. We prepared 2 sets of ten-fold serial dilutions of the 12 anaerobes, and inoculated these samples on Luria-Bertani (LB) broth and LB blood agar plate (LB-BAP) (BD Diagnostic Systems, USA). Each set was incubated in the Quick anaero-system (DAS Tech, Korea) and BBL GasPak jar with BD GasPak EZ Anaerobe Container System (BD Diagnostic Systems) at 35-37degrees C for 48 hr. The minimal inoculum size showing visible growth of 12 anaerobes when incubated in both the systems was compared. RESULTS: The minimal inoculum size showing visible growth for 2 out of the 12 anaerobes in the LB broth and 9 out of the 12 anaerobes on LB-BAP was lower for the Quick anaero-system than in the BD GasPak EZ Anaerobe Container System. The mean time (+/-SD) required to achieve absolute anaerobic conditions of the Quick anaero-system was 17 min and 56 sec (+/-3 min and 25 sec). CONCLUSIONS: The Quick anaero-system is a simple and effective method of culturing obligate anaerobes, and its performance is superior to that of the BD GasPak EZ Anaerobe Container System.

4.Diagnostic Usefulness of the Multimodality Study with Mammography, Ultrasonography,99mTc-MIBI Scan in Breast Cancer.

PURPOSE: To assess the diagnostic usefulness of multimodality study in the diagnosis of breast cancer. MATERIALS AND METHODS: Eighty-one patients between January 1998 to December 1999 were involved in this study, and who underwent mammography, breast ultrasonography, and 99mTc-MIBI scintimammography in all cases the findings were retrospectively reviewed. Each modality was graded by two physicians specializing in nuclear medicine and two radiologists, all unaware of the pathologic results. A four-grade system(1=definitely benign, 2=probably benign, 3=probably malignant, 4=definitely malignant) was applied to those ofbreast ultrasonography and 99m Tc-MIBI scintimammography and BIRADS was applied to those ofmammography. All breast masses were confirmed by surgery (n=67) or FNA (n=14). Findings of grade 3 or 4 the four-grade system, BIRADS category 4 or 5, or positive coincidence in double and triple combination studies were defined as positive results, and on the basis of the data thus obtained, the sensitivity, specificity, positive and negative predictive value and diagnostic accuracy were calculated for each modality and for multimodality studies. RESULTS: The sensitivity, specificity, positive predictive value, negative predictive value and diagnostic accuracy of 99mTc-MIBI scintimammography were 83.6%, 88.5%, 93.9%, 71.9% and 85.2%, respectively. For ultrasonography, the corresponding figure were 94.5%, 65.3%, 85.2%, 85.0%, and 85.2%, and these for mammography, they were 87.3%, 69.2%, 87.3%, 81.8% and 80.2%. For the ultrasonography and mammography combination, the figures were 83.6%, 50.0%, 93.9%, 100.0% and 95.2%, respectively, and for 99mTc-MIBI scinti-mammography and mammography, the corresponding findings were 72.7%, 69.2%, 95.2%, 100.0% and 96.7%. For the 99mTc-MIBI scintimammography and ultrasonography combination, respective findings of were 80.0%, 61.5%, 95.7%, 94.1% and 95.2%, respectively, and in the triple modality study, respective findings of 70.9%, 50.0%, 97.5%, 100.0% and 98.1% were recorded. CONCLUSION: Among multimodality studies, sensitivity was greatest in the ultrasonography and mammography combination, which is thus extremely suitable for the diagnosis of breast cancer. The findings of two series suggest that in equivocal cases, 99mTc-MIBI scintimammography with its higher specificity and positive predictive value, is a useful adjunctive tool.

5.Hypercholesterolemia and In-Vivo Coronary Plaque Composition in Patients with Coronary Artery Disease: A Virtual Histology - Intravascular Ultrasound Study.

BACKGROUND AND OBJECTIVES: Hypercholesterolemia is a key factor in the development of atherosclerosis. We sought to evaluate the relation between hypercholesterolemia and plaque composition in patients with coronary artery disease. SUBJECTS AND METHODS: Study subjects consisted of 323 patients (mean 61.5 years, 226 males) who underwent coronary angiography and virtual histology-intravascular ultrasound examination. Patients were divided into two groups according to total cholesterol level: hypercholesterolemic group (> or =200 mg/dL, n=114) and normocholesterolemic group (<200 mg/dL, n=209). RESULTS: Hypercholesterolemic patients were younger (59.7+/-13.3 years vs. 62.6+/-11.5 years, p=0.036), than normocholesterolemic patients, whereas there were no significant differences in other demographics. Hypercholesterolemic patients had higher corrected necrotic core volume (1.23+/-0.85 mm3/mm vs. 1.02+/-0.80 mm3/mm, p=0.029) as well as percent necrotic core volume (20.5+/-8.5% vs. 18.0+/-9.2%, p=0.016) than normocholesterolemic patients. At the minimal lumen area site, percent necrotic core area (21.4+/-10.5% vs. 18.4+/-11.3%, p=0.019) and necrotic core area (1.63+/-1.09 mm2 vs. 1.40+/-1.20 mm2, p=0.088) were also higher than normocholesterolemic patients. Multivariate linear regression analysis showed that total cholesterol level was an independent factor of percent necrotic core volume in the culprit lesion after being adjusted with age, high density lipoprotein-cholesterol , hypertension, diabetes mellitus, smoking and acute coronary syndrome (beta 0.027, 95% confidence interval 0.02-0.053, p=0.037). CONCLUSION: Hypercholesterolemia was associated with increased necrotic core volume in coronary artery plaque. This study suggests that hypercholesterolemia plays a role in making plaque more complex, which is characterized by a large necrotic core, in coronary artery disease.

Methods:
Past trends of inflow and outflow of cardiologists were used to make crude projections, which were then adjusted based on demands of services to obtain final projections. Inflow of cardiologists was estimated using second-order polynomial regression and demand for cardiology care was estimated using linear regression.
Results:
There were 1,139 active cardiologists who were under the age of 65 in clinical practice in Korea. The estimated number of cardiologists from 2022 to 2040 showed that the number of cardiologists would peak at 1,344 in 2032 and gradually decrease thereafter. We also estimated an increase of 947,811 cases of heart-related procedures annually from 2023 to 2032. The number of heart-related procedures per cardiologist would increase 1.4 times from 12,964 in 2023 to 17,862 in 2032. The estimated number of emergency patients per cardiologist under 50 years old would almost double from 544 in 2022 to 987 in 2032.
Conclusions
We expect significant shortage of cardiologists in Korea within the next 10 years. The number of emergency patients per cardiologist will increase by nearly 50%, leading to high individual workload for cardiologists. To prevent this imbalance between supply and demand, an organized and collective approach by the specialty of cardiology is imperative to produce a balanced workforce.

9.Suicide Related Indicators and Trends in Korea in 2018

Suicide has been a long-standing problem for global public health, along with almost 800,000 deaths from suicide worldwide in 2016, accounting for 1.4% of all deaths. South Korea was ranked first in suicide mortality in 2018 among countries in the Organization for Economic Cooperation and Development. This study aimed to suggest up-to-date information about suicide-related indicators such as the rate of suicidal ideation, suicide attempt, and suicide death, and its trends by applying sampling weight to make it nationally representative. In this study, we used the data sources: Korea National Health and Nutrition Examination (KNHANES, ‘07–13, ‘15–18), Korean Community Health Survey (KCHS, ‘08–09, ‘13, ‘17), Korean Wealth Panel Study (KOWEPS, ‘12–18), Korea Health Panel Survey (KHP, ‘10–13), and Statistics Korea (1983–2017). The rate of suicidal ideation as recent year was 4.73% (KNHANES, ‘17), 6.96% (KCHS, ‘17), 2.29% (KOWEPS, ‘18), and 5.39% (KHP, ‘13). That of suicide attempt as recent year was 0.51% (KNHANES, ‘18), 0.32% (KCHS, ‘17), and 0.15% (KOWEPS, ‘18). Annual percentage change (APC) of suicidal ideation was -15.4% (KNHANES, ‘07–13, ‘15, ‘17), -2.5% (KCHS, ‘08–09, ‘13, ‘17), -10.8% (KOWEPS, ‘12–18), and -10.9% (KHP, ‘10–13). APC of suicide attempt was -4.4% (KNHANES, ‘07–13, ‘15-18), -4.4% (KCHS, ‘08–09, ‘13, ‘17), and -13.6% (KOWEPS, ‘12–18). APC of death by intentional self-harm was -1.25% (Statistics Korea, ‘07–18). All suicide-related indicators were found to be decreasing in the overall from 2009. Individuals with lower income level were more likely to experience suicidal ideation and suicide attempts. Even though suicide rate had been continuously decreasing from its highest point in 2011 (suicide rate: 31.7 per 100,000 population) to 2017 (suicide rate: 24.3 per 100,000 population), it increased again in 2018 (suicide rate: 26.6 per 100,000 population). This information would be grounded on policy implementation for suicide prevention, thus continuous data observation is necessary.
10.The Effect of Cyanidin-3-O-beta-d-glucopyranoside on the Penile Erection and Corpus Cavernosum in a Rat Model of Diabetic Erectile Dysfunction.

PURPOSE: The aim of this study was to evaluate Cyanidin-3-O-beta-d-glucopyranoside on improvement and protection for erectile function. MATERIALS AND METHODS: Sprague-Dawley rats (12wks old) were divided into three groups (n=12 in each): normal control, diabetes (DM), and diabetes with Cyanidin-3-O-beta-d-glucopyranoside (C3G) concentration materials treatment (DM+C3G). DM and DM+C3G group received a single injection of streptozotocin (50 mg/kg), and 4 wk after induction of diabetes, DM+C3G group were treated with daily C3G (10 mg/kg) dissolved in water for 8 wk. After 12 wk of streptozotocin injections, rats in each group underwent intracavernosal pressure measurement (ICP) and then the corporal tissues were sampled. RESULTS: DM group showed markedly lower erectile parameters than those in the control group, whereas rats in the DM+C3G group showed improved erectile function by minimizing corporal apoptosis. CONCLUSIONS: The current study is the first to suggest that Cyanidin-3-O-beta-d-glucopyranoside may have a potency to improve and protect erectile function in a rat model of diabetic erectile dysfunction.
