3.Nursing Student Academic Achievement depending on College Entrance System-related Factors.

PURPOSE: This study aims to understand the influence of college entrance system-related factors and scores of college scholastic ability test on a first year nursing student's academic achievement. METHODS: Data collected include both a descriptive survey designed to assess college entrance system-related factors with respect to 182 nursing students attending a 4-year nursing college program and their test scores in the first year. The data were analyzed via descriptive statistics, Mann-Whitney U test, Kruskal-Wallis test with Bonferroni's correction, and Spearman's rank correlation coefficient through an SPSS 18.0 program. RESULTS: Student academic achievement with regard to college entrance system-related factors exhibited significant divergence from first year grade point averages (hereafter GPA) and natural science grades. CONCLUSION: This study confirmed the existence of significant differences, depending on the factors regarding the college entrance system. As a result, conducting assessment to evaluate the level of basic knowledge before college entrance is highly recommended. Also recommended is the development of a support program for students projected to have difficulties with respect to their academic achievement.

4.The Effect of Zinc Sulfate in Acnd Vulgaris.

In order to evaluate the effect of zinc sulfate on acne vulgaris, this study was performed during 3 months' period from October, 1979 to December 31, 1979. Total number of acne patients visited at the Department of Dermatology, Han Gang Sung Shim Hospital were 130, but among them 58 patients had completed this atudy. The patients were classified in 3 groups, A, B, C, as follows. Group A (23 patients) who were treated with zinc sulfate (600mg/day) and 2. 5% benzoyl peroxide Group B (20 patients) who were treated with tetracycline (initial: 750mg/day, after 2weeks: 500rng/day) and 2. 5% benzoyl peroxide. Group C (15 patients) who were treated with only 2. 5% benzoyl peroxide. To evaluate the efficacy of treatment in these 3 groups, the number of open comedone, closed comedone, papule and pustule were counted every other week for 8 weeks. (countiuned..)

5.Efficacy of treatment with interferon alpha in hepatitis C.

BACKGROUND/AIMS: Hepatitis C virus(HCV) was known to most common etiologic agent of chronic liver disease in United states and Japan. Although hepatitis B virus(HBV) was well known to be a its major etiologic agent in Korea, it has been showed that HCV and HBV are associated with liver cirrhosis and hepatocellular carcinoma as major causative agent of chronic liver disease. Interferon alpha therapy is generally accepted as effective single agent for chronic hepatitis or to decrease the chronicity of acute hepatitis C. So, we evaluated the efficacy of interferon alpha in hepatitis C. METHODS: 46 patients who were positive for anti-HCV antibody and HCV RNA were included in this study. Liver biopsy was per formed on all patients and all of them were tested as negative for serum HBsAg, anti Hbe. Patients were divided into 2 groups . 30 patients received interferon therapy(treated group) and 16 patients received no therapy(untreated group). We compared the change of liver function test and HCV RNA before and after therapy between two groups. Treated group was subdivided into 5 groups according to response to interferon therapy '. Non-response, partial response, breakthrough, relapse and sustained response. RESULTS: 1) The mean age and sex distribution were 49.9 year old, male 19, female 11 in treated group and 48.7 years, male 12, female 4 in untreated group. 2) The number of patients with acute hepatitis, chronic persistent hepatitis, chronic active hepatitis and liver cirrhosis were 1, 2, 23, 4 in treated group and 0, 1, 12, 3 in untreated group, respectively. 3) The mean follow up period was 1.7 year and 2.3 years in treated and untreated group, respectively. 4) The activity of serum ALT before and after therapy were 195+ 134.6 IU/L, 87.4+ 40.5 IU/L and 186.7+ 106.4 IU/L, 157+ 87.1 IU/L in treated and untreated group, respectively. Serum ALT after therapy in treated group was significantly lower than untreated group(P<0.01). 5) The number of patients for patterns of reponse in treated group was non-response 5, partial response 8, breakthrough 1, relapse 4, sustained response 12 and there was no difference in age among them(P>0.05). 6) The case of negative conversion for HCV RNA in treated group was 12, but there was no case in untreated group. 7) Sex distribution of sustained response was 6(31% ) of 19 male, 6(54.5%) of 11 female and 12 patients(40.0%)(1 of 1 patients with acute hepatitis, 1 of 2 chronic persistent hepatitis, 10 of 23 chronic active heaptitis) included in sustained reponse, but any patients with liver cirrhosis had response. 8) Mean total dose and duration of interferon therapy was non-response 10353.6 million unit(MU)/5.8month(M), partial response 20025.06MU/6.4 M, breakthrough 36000.0MU/5.0M, relapse 11700.0MU/3.3M, sustained response 28100.0MU/6.6M, respectively. 9) 3 of 7 patients who were followed up over 1 year in sustained response and mean time to the relapse was 2.2 years. CONCLUSION: Our results showed that interferon alpha therapy is effective in patients with hepatitis C and further study and attempts should be performed to augument the efficacy of interferon alpha for the treatment of hepatitis C.

6.Clinical Significance of Gray-scale Ultrasound in the Diagnosis of Ureteral Stone: Need of the New Diagnostic Modalities for the Ureteral Stone in Emergency Room.

BACKGROUND: Ureteral stone(UTS) and acute pyelonephritis are the most common causes of nontraumatic acute flank pain in adults. Urography(IVU) is known as the most ideal diagnostic modality of obstructive uropathy to date. However, it has many practical limitations in overcrowding Korean emergency department to perform IVU as the diagnostic test of UTS. So we have performed prospective analytic study to compare IVU with its alternative diagnostic modalities. METHOD AND RESULTS: Our study was performed at Asan Medical Center from March, 1998 to July, 1999. We performed urinalysis, KUB, grayscale ultrasonography, and IVU in 243 patients, suspected to have UTS, based on histories, physical examination. After excluding patients with undetermined results(n=7) or spontaneous passage of stone before IVU(n=24), 212 patients were enrolled in our study. All of the other diagnostic tests were performed within one hour after emergency room presentation except IVU (53+/-24 hours). Diagnostic agreement among tests are as follows: Urinalysis shows good agreements with KUB(0.53) and IVU(0.62). KUB shows good agreement with IVU(0.48). Ultrasonography shows good agreement with IVU(0.58). Sensitivity(%), specificity(%), positive predicitive value(%), negative predicitive value(%), accuracy(%), and odds ratio of each diagnostic methods are as follows: Urinalysis is 91, 19, 78, 42, 74, 3.88; KUB is 55, 33, 72, 19, 50, 2.49; ultrasonography is 58, 92, 96, 41, 66, 45.61, and IVU is 81, 96, 98, 62, 84, 107.72. CONCLUSION: Although IVU seems to be the most ideal diagnostic modality for the UTS to date, it takes too long time for emergency physicians to confirm the diagnostic using IVU. We can performed other diagnostic modalities within one hour. However, emergency physicians suffer from poor diagnostic accuracy. Therefore, it is concluded that we are in need of more accurate alternative diagnostic modalities for UTS.

7.Vitamin D Resistant Rickets

The discovery and synthesis of vitamin D and the elucidation of the role of sunlight in the activation of the vitamin D precursors changed the syndromes of rickets from a therapeutic enigma to a socioeconomic and public health problem. Since Albright in 1937 first described vitamin D resistsnt rickets, which did not respond to treatment with the usual dose of vitamin D, it has progressively become a common form of rickets in practice. In addition, as the result of increasing understanding of renal physiology and careful investigation, a spectrum of renal tubular abnormalities have been identified which cause clinical rickets and which in many cases are insensitive to even large doses of vitamin D. We have reported a case of an unusual form of vitamin D resistant rickets which did not easily respond to treatment with high doses of vitamin D and was associated with hypocalcemia in multiple pathologic fractures in the lower extremities of the patient.

The occurence of stress fracturea at sites such as the metatarsal, tibia, fibula, and femur in military trainee and certain other very physically active groups has been reported frequently, but literature regarding stress fractures of the inferior pubic ramus has been very sparse. 9.A case report of orthodontic treatment of cleft palate accompany teeth congenital missing.

19 years old female had untreated Veau classification class II cleft palate with ectopic eruption of upper right lateral incisor and congenital missing of lower lateral incisors. Upper left lateral incisor, left first molar aid lower left first molar were root restswithperiapicalpathologiclesions. So all root rests were extracted and prosthodontic rehabilitation after orthodontic treatment was planned. She was treated by means of multibanded system with face bow. After 23 months all orthodontic correction were achieved and, as soos as debanding procedure was done she was referred to oral surgeon and prosthodontist for surgical operation and bridge construction.

10.A Case of Dermoid Cyst Causing Deep Erosion of the Skull.

Dermoid cysts develop from sequestration of epithelium along lines of embryonic fusion.The most common locations are the lateral third of the eyebrows, nose, and scalp. These cysts are located in the subcutis; they are often adherent to periosteum, and may invade or erode underlying bane. A 34-year-old female presented with a solitary, skin colored, dome-shaped, child fistsized, subcutaneous mass on her right occiput. At operation, keratinous material was discharged and tufts of hair projected from opening of the cyst wall.The base of the cyst was firmly adherent to periosteum and diffuse depression with focal deep erosions of the outer table of skull was found.
