2.Lower incisor extraction for dental camouflage.

There is a variation in the range of normal occlusion, and we must fit our treatment to the needs of each patient. If the upper or lower incisors are congenitally missing, malformed, or crowded, the extraction of the incisor has some advantages over the extraction of premolars and nonextractions. The advantages are l) simple mechanics, 2) reduced treatment time, 3) less relapse tendency, and 4) fewer facial profile changes. In order to decide which incisor should be extracted, we must consider certain factors: l) discrepancies in anterior arch length, 2) anterior tooth ratio, 3) periodontal and tooth health condition, and 4) the relationship between the upper and lower midline. Diagnostic set-up can be helpful to plan the treatment and show us the post treatment result.

3.Effect of Energy Sources (Glucose, Pyruvate and Lactate) Added to Dulbecco's Modified Eagle Medium (DMEM) on the Mouse 2-cell Embryo Development.

OBJECTIVE : Mammalian embryos undergo changes of energy environment for transfer from oviduct to uterus. Also, the human reproductive organ (oviduct, uterus) contains energy sources of different concentration (oviduct - glucose: 0.5 mM, pyruvate: 0.32mM, lactate: 10.5 mM; uterus - goucose: 3.15 mM, pyruvate: 0.1mM, lactate: 5.87 mM, respectively). This study was conducted to examine the effect of these energy sources added in DMEM with glutamine on the mouse embryo development. METHODS: There was used ICR female mouse. Two cell embryos of mouse are collected by method of 'flushing'. Flushing fluid was used Ham's F-10 added to 20% FBS. The collected 2 cell embryos were cultured in media such as Control (only DMEM), group A and B (DMEM supplemented with 0.5 mM and 3.15 mM glucose), and group C and D (DMEM supplemented with 0.1 mM and 0.32 mM pyruvate), and group E and F (DMEM supplemented with 5.87 mM and 10.5 mM lactate). All experimental media supplemented with 20% hFF, respectively. Pattern of embryo development was observed to interval at 24hr during 96hr. RESULTS : The media with glutamine added glucose (group A: 51.0%; group B: 48.4%) was significantly (p<0.05) higher than other experimental group in development into the morula stage after 24 hr in culture, but not significantly different compared with control and the rate of development into the blastocyst was significantly (p<0.05) low in the both of pyruvate (group C: 7.9% group D: 6.8%) and lactate (group E: 7.1%, group F: 7.1%) treatment group after 48 hr in culture. Development into the hatched and attached balstocyst after 96hr in culture revealed similarly in control (81.9%) and glucose treatment group (group A: 83.3%, group B: 82.8%). However, development into the hatched and attached blastocyst after 96hr in culture revealed significantly (p<0.05) development in the glucose treatment group (group A: 82.3%, group B: 78.5%) than control (63.2%), and its of pyruvate (group C: 34.1%, group D: 34.1%) and lactate (group E: 25.9%, group F: 33.3%) treatment group were significantly (p<0.05) lower than control similar to previous observations. CONCLUSION : The glucose added to the DMEM with only glutamine, as energy source, was highly to the rate of development compared with control, but the other energy sources were not, synthetically. Above refer to, the human reproductive organ (oviduct, uterus) contains energy sources of different concentration. Thus, further studies are will examine continuously to effects by interaction of different energy sources in the mouse embryo culture.

4.Timed Analysis of Mouse 2 - Cell Embryo Development in Dulbecco's Modified Eagle Medium ( DMEM ) in The Presence of Glutamine With of Without Glucose.

OBJECTIVE: To evaluat the effects of a culture medium with glucose in the presence of glutamine on the development of mouse embryos. METHODS: Two-cell embryos recovered from ICR mice at 48 hrs after hCG injection (mated just after hCG injection) were cultured in DMEM (with 20% hFF) supplemented with or without glucose on the presence of glutamine. Embryos were cultured under three different glucose regimens: (1) 0 mM (control); (2) 0.5 mM (group I); or (3) 3.15 mM (group II), and were analyzed at 24, 48, 72 and 96 hours intervals. Chi-square test (x2-test) was used to compare values of groups. RESULTS: No differences were found in the number of embryos showing morula (control: 37.5%; group I: 51.0%; group II: 48.4%), blastocyst (control: 21.5%; group I: 33.3%; group II: 34.4%) and blastocyst and hatching or hatched blastocyst (control: 81.9%; group I: 83.3%; group II: 82.8%) between groups at 24 hrs, 48 hrs or 72 hrs respectively. However at 96 hrs, the number of hatched and attached blastocyst was significantly higher in group I (82.3%) and II (78.5%) than control (63.2%; P<0.05). CONCLUSION: The addition of glucose (0.5 mM) to the DMEM, as energy source, improved the rate of development of late stage embryos in mice.

6.Radiotherapy Results in Stage IIB Uterine Cervix Cancer.

PURPOSE: To evaluate the treatment results and prognostic factors after radiotherapy in stage IIB uterine cervix cancer. MATERIALS AND METHODS: We retrospectively analyzed the records of 90 patients with stage IIB uterine cervix cancer who received radiotherapy between 9/94 and 12/99. Age was ranged from 28 to 79 years (median 57). Tumor size was > or = 4 cm in 64 patients. Preteatment SCC level was measured in 76 patients. Twenty nine patients received conventional radiotherapy (QD) and the others received modified hyperfractionated radiotherapy (BID). Only 7 patients in BID had tumor size <4 cm. All patients received high dose rate brachytherapy (4 Gy x 7 or 5 Gy x 6). No patient received concurrent chemotherapy during radiotherapy. Follow up period was ranging from 9 to 76 months (median 38). RESULTS: The 5-year overall and disease free survival rates were 73.4% and 71.6%, respectively. Local recurrences occurred in 10% of patients, and distant metastasis in 18.9%. There was a significant correlation between OS/DFS and tumor size (<4 cm; OS 95.2%, DFS 91.4%, > or = 4 cm; OS 63.4%, DFS 63.4%). Pretreatment SCC level was one of prognostic factors only in univariate analysis. CONCLUSION: With modified hyperfractionated radiotherapy, there was very low local recurrence rate (6.6%) and high 5-year overall and disease free survival rate (75.4% and 70.5%), which is comparable to results after concurrent chemoradiotherapy in bulky, locally advanced stage IIB uterine cervix cancer.

7.Effect of Preinduction Amino Acid Infusion on Body Core Temperature.

BACKGROUND: It is well known that administration of nutrients, especially protein and amino acids mixtures, stimulates energy expenditure in the unanesthetized patients. Also, there were reports that the mechanisms for nutrient-induced thermogenesis are not impaired during general anaesthesia. The enhanced amino acid-induced thermogenesis would counteract the anaesthesia-induced reduction in metabolism and prevent the development of hypothermia. The present study was designed to see if completing the amino acid infusion before induction of anaesthesia would induce heat production, which would promote energy expenditure and thereby counteract the development of anaesthesia-induced hypothermia. METHODS: Body temperatures and arterial blood gases were measured in 48 patients during perioperative period. 24 patients had an amino acid mixture of 227 cal infused over 2 hours before anesthesia and 24 control patients received Hartman's solution. RESULTS: Amino acid infusion attenuated the development of hypothermia during the period from 60 min to 105 min after induction of anaesthesia compared to control group, however, did not prevent overall hypothermia throughout the study period. CONCLUSION: This result may indicate amino acid infusion before anesthesia can be an adjuvant to prevent intraoperative hypothermia.

8.Histological Evaluation of Bone Ingrowth around Titanium and Hydroxyapatite-coated Titanium Implant in Dogs.

Since the concept of osseointegration has been introduced, a lot of researches were performed to achieve stronger fixation of the implant. In this study, we tried to find the different pattern of bone ingrowth after implanting pure titanium implant and Hydroxyapatite coated titanium implant in dogs. Four typed of cylindrical titanium implants (diameter 6 mm, length 1 mm) were inserted in the distal femur of adult dogs. The histological examination was done at postoperative 2, 4, 8, and 12 weeks. There was no foreign body reaction at contact site and all implants kept direct contact with bone tissue. In the light microscopic finding, after 2 weeks, the cases with Hydroxyapatite-coated Implant (HAP) showed greater formation of immature woven bone in comparison to Titanium implant. After 4 weeks, Hydroxyapatite-coated Implant (HAP) showed more mature lamellar bone. After 8 and 12 weeks, most of the woven bone was transformed into mature lamellar bone. The fluorescence microscopic finding, after 2 weeks, revealed calcein green fluorescence around implants in all cases and it was more prominent in the cases with Hydroxyapatite-coated Implant (HAP). After 4 weeks, the center of trabecula gave out green fluorescent light, the corner of them showed red fluorescent light, and they were more prominent in the cases with Hydroxyapatite-coated Implant (HAP). After 8 and 12 weeks, green fluorescence disappeared gradually, red fluorescence appeared for the most part, and there was no difference in each cases, especially in 12 weeks. As a result, we support the concept that Hydroxyapatite-coated Implant (HAP) induces faster bone ingrowth in 4 weeks than Titanium implant, and that it may be more profitable for early bone ingrowth and implant stability.

9.Clozapine-Induced Acute Hepatitis.

The authors report two cases of clozapine-induced acute hepatitis. Two patients developed asymptomatic hepatitis and got better with conservative care. We decreased the dosage of clozapine and added hepatic protectors, resulting in normalized laboratory findings. The authors also reviewed side effects of clozapine in this report. We reviewed the suggested mechanism of either clozapine or chlorpromazine-induced hepatitis. Clozapine influences the liver cell via cytochrome P 450 and chlorpromazine does so via mild cholestasis. There may be a possibility that a patient who has experienced drug-induced hepatitis is vulnerable to clozapine-induced acute hepatitis. In this respect, those who have experienced drug-induced hepatitis must be observed more closely.

10.A Clinical Observation of 129 Cases of Intussusception in Infants and Children.

A retrospective study of 129 cases of intussusception from January 1972 to July 1977 at Busan Natinal University Hospital was done. Nearly 1,000 cases of intussusception were reported in Korean literatures, but still the causation of this disease is unclear. As the intussusception needs not only urgent diagnosis and treatment, but also it is relatively common during infancy and childhood, we think it will be necessary and worthwhile to analyse our own cases. The results were summarized as follows; 1) In age distribution, 79% of the patients were under 1 year of age and peak incidence was during 6~12months of age. 2) Male was predominant than female and male to female ratio was 2.3 : 1 3) In seasonal incidence, slight prevalence was noted during spring and summer. 4) The cardinal symptoms and signs on admission were revealed as vomitting (66%), abdominal pain, irritability or loud crying(64%), bloody or mucous bloody stool(61%) and abdominal mass(57%). 5) The most common type of intussusception was ileo-colic. 6) In the view of eiology, 96% of cases were idiopathic, but only 4% had organic causes. 7) The 90.8% of intussusception was reduced successfully by barium enema and surgical management with manual reduction or partial segmental resection was performed in only 10.2%. 8) The recurrence and mortality rate were 4% and 1% respectively.
