1.Acute Suppurative Arthritis in Neonatal Period

Acute suppurative arthritis in neonates is very difficult to diagnose early since it does not show typical clinical picture and as a results, delayed diagnosis and improper treatment often leads to permanent disability in patient. To prevent the sequelae follwing acute suppurative arthritis, early surgical treatment and administration of antibiotics are essential. We analysed 15 affected joints in 11 neonates less than 1 month old who were treated by surgical treatment and administration antibiotics. The results were as follows. 1) As for the specific joint involved, those affecting hip joint were most frequent(8 joint: 53%), followed by knee joint(5 joints: 33%) and shoulder(2 joints: 14%). Acute osteomyelitis was simultaneously occurred in 10 joints(67%). 2) As for clinical symptoms and signs, 10 cases (91%) showed limited range of motion with the clinical symptoms and signs similar to neonatal sepsis. 3) As for the causative organism, Gram-negative bacilli was the most frequent one(40%), followed by Staphylococcus aureus(27%) and candida albicans. No organisms were identifiable in four joints(27%). 4) As for the sensitivity test to antibiotics, group of staphylococcus aureus was found sensitive to newer cephalosporins while those of gram-negative bacilli to amikacin in 83%. 5) Average time between the first clinical symptom and operation was 5.14days. 6) The hip joint had yielded most complications(5 cases) followed by knee and shoulder joint, one for each. The accompanying osteomyelitis and delayed treatment were contributing factors to complications.

4.Stress, Self-esteem, and Powerlessness in Korean Husbands Married to Foreign Wives.

PURPOSE: The purpose of this study is to investigate levels of stress, self-esteem, and powerlessness in Korean husbands married to foreigner wives. METHODS: The subjects were 183 Korean men who were living with foreigner wives in the Gwangju, Jeonnam and Jeonnbuk areas. Data were collected from January to May 2012 and analyzed by descriptive statistics, t-test, one-way ANOVA, Scheffe test, Pearson's correlation coefficient, and backward multiple regression with the SPSS PC WIN version 18.0. RESULTS: The average level of stress was 4.48, with difficulties in international marriage experienced by the subjects due to differences in language and culture. The average reported self-esteem score was 29.69, and the average powerlessness score 117.76. There were significant differences in perceived powerlessness depending on the employment statuses, income levels, and nationalities of the participants' wives, and their perceived levels of satisfaction with living with a foreigner wife. Powerlessness was positively correlated with stress (r=.41, p<.001), and negatively correlated with self-esteem(r=-.63, p<.001). The factors affecting the perceived powerlessness included self-esteem (beta=-.54, p<.001), stress (beta=.28, p<.001), lower income (beta=.13, p=.016), and working wife (beta=.12, p=.036). CONCLUSION: Healthcare providers need to explore strategies to improve self-esteem and powerlessness in Korean men married to foreigner wives.

5.Clinical Analysis of Penetrating Keratoplasty in Herpes Simplex Keratitis.

We conducted a retrospective study of 20 penetrating keratoplasty (19patients) for corneal opacity due to herpes simplex keratitis from January 1992 to February 1997 at National Medical Center. We exaniined f requeuey of graft rejection and recurrence of herpetic keratitis after penetrating keratoplasty. The patients were grouped into control & treatinent, group according to the use of antivirial drug after penetrating keratoplasty and we examined the difference in rate of recurrence between two groups. The follow-up period was from 8 months to 68 months, mean follow-up period being 29.6 months. Graft rejection was seen in 3 eyes out of 20 eyes (15.0%) and recurrence was developed 3 eyes out of 20 eyes (15.0%). Recurrent herpetic keratitis developed in 15.4%(2 of 13) of the eyes in control group and 14.3% (1 of 7) of the eyes in treatment group. There was no statistically significant difference in recurrence rate between two groups (p)0.05). Postoperative prophylactic antiviral treatment is not associated with decreased rate of herpes simplex keratitis recurrence.

6.The Effect of Limbal Transplantation & Cyclosporine A for Chemically Damaged Rabbit Cornea.

The authors have evaluated the effect of topically applied 1% cyclosporine A for four weeks after limbal allograft transplantation for ocular surface stability in severely ocular surface damaged rabbits. A total eleven severely damaged rabbits were subdivided into three groups of four limbal autograft (AUTO), three limbal allograft (ALLO), four limbal allograft with topical application of 1% cyclosporine A five times daily for four weeks (ALLO-C). We examined ocular surface smoothness, clarity, corneal vascularization before surgery and three months after surgery, and classified three grade (success, partial success, failure) according to result. In AUTO, three eyes were success, one eye was failure. In ALLO, one eye showed inferior graft detachment and one eye was success, two eyes were failire. In ALLO-C, no graft detachment detected and one eye was success, two eyes were partial success, one eye was failure. We knew that. ALLO-C show more effective ocular surface stability than ALLO. These result suggest ALLO-C can be used as alternative treatment of AUTO in ocular surface reconstruction. But, rnore study for duration and concentration of cyclosporin A will be need.

7.Management of Healthcare Workers and Patients on Exposure to Pandemic Influenza A (H1N1 2009) Virus in a Hospital.

BACKGROUND: This study is aimed at describing the outcomes of the management of the patients, caregivers, and healthcare workers (HCWs) who are exposed to the pandemic influenza (H1N1 2009) virus and at evaluating the adequacy in exposure management and infection control. METHODS: From July 2009 to January 2010, for 7 a month period, we managed patients and healthcare workers without any respiratory protective devices, who came within 1 m distance of H1N1-positive individuals for more than 1 h and performed a 1-week follow-up. RESULTS: The total of 157 cases with exposure to pandemic influenza (H1N1 2009) virus and exposed individuals of 907 were reported. Of the exposed individuals who were under management, 15 were confirmed to be infected with the infection rate being 1.7%. The confirmed individuals did not have a secondary infection after the exposure. Rates of infection of the exposed patients and healthcare workers were 1.8% and 1.6%, respectively, and these figures were not statistically significant. CONCLUSION: The exposure management results at the hospital revealed that the infection had spread by contact with individuals who were positive for the infection. The high incidence of early exposure to the virus warrants the need to ensure the use of protective equipment and the adoption of assertive teaching methods that have long lasting effects.

8.Prostatic Hypertrophic Symptoms, Activities of Daily Living, Satisfaction of Sexual Activities and Well-Being between Subjects with Prostatic Hypertrophic Symptoms and General Persons.

PURPOSE: This study was designed to explore and compare the prostatic hypertrophoc symptoms, activities of daily living, satisfaction of sexual activities and well-being among subjects with prostatic hypertrophic symptoms and the general population. METHOD: One hundred subjects with prostatic hypertrophic symptoms and one hundred general persons were recruited in K medical center. The data were collected from October 20, 2001 to March 30, 2002 by structured questionnaire. RESULT: The results were the following; 1. The percentage of mild(0~7), moderate(8~19) and severe(20~35)symptoms between subjects with prostatic hypertrophic symptoms and general persons were 6% : 60%, 54% : 39%, 40% : 1%. 2. There was a significant difference in prostatic hypertrophic symptoms(t=12.82, p<.001), in activities of daily living(t=-7.77, p<.0001), in satisfaction of sexual activities (t=-4.80, p<.0001), in well-being(t=-4.80, p<.0001) between subjects with prostatic hypertrophic symptoms and general persons 3. There was a significant difference in activities of daily living(F=16.28, p<.0001), satisfaction of sexual activities(F=3.98, p<.05) according to prostatic hypertrophic symptoms in subjects with prostatic hypertrophic symptoms. CONCLUSION: According to the above findings, prostatic hypertrophic symptoms influence activities of daily living, satisfaction of sexual activities, well-being of subjects with prostatic hypertrophic symptoms negatively.

9.Improvement of a Planning Technique Based on Heuristic Target Shaping for Stereotactic Radiosurgery.

Stereotactic radiosurgery (SRS) is a technique to deliver a high dose to a target region and a low dose to a critical organ through only one or a few irradiation. The SRS must be planned exactly. Currently the surgery plan is performed by trial and error method. There are many questions about the reliability and reproducibility of the plan result. This study improve each step of the Oh's method based on heuristic target shaping to obtain the better result. The target was reconstructed using cylinders with same height and the neighbored cylinders were combined according to the difference of each center and diameter. Then, spheres were packed within each cylinders by the packing rules. Two virtual targets were used to compare this method with Oh's method. As a result, the numbers of isocenter were successfully reduced - more than 36% and 26% - without serious differences of prescription isodose to tumour volume ratio (PITV) and maximum dose to prescription dose ratio (MDPD). This technique using cylinder piling and sphere packing will be a helpful tool to planner in stereotactic radiosurgery.

10.Effects on Surgical Repair of VSD by TATV.

Perimembranous ventricular septal defects(PMVSDs) are the most common type of ventricular septal defects(VSDs) and consist morphologically of deficiency of the membranous septum and variable portions of the adjacent muscular septum. Repair of VSD has begun via a right ventriculotomy. Even with this exposure, however, it mght lead to ventricular dysfunction. Transatrial exposure of VSDs is known to a versatile approach to PMVSDs and even malalignment defects can be repaired by this METHOD: Although transatrial exposure can be improved by taking down the atrioventricular valve at the annulus, surgeons have been hesitant to do so because of concern for valvular competence. Therefore, this study was undertaken to clarity the effects of transannular approach of tricuspid valve (TATV) at operation of PMVSD. During last 5 years, twenty eight cases from 96 patients of PMVSD were closed by TATV and follow up study was done from 3 months to 33 months and results were obtained as follows. 1. Age at operation was from 4 months to 38 years and most patients(17, 62%) were above 5 years. 2. Preoperative pulmonary-systemic flow ratio(Qp/Qs) was ranged from 1 to 2.8 and 22 patients(79%) were less than 2. 3. Peak systolic pulmonary artery pressure was below 30mmHg in 8, 30-50mmHg in 17, above 50mmHg in 3 patients and 25 patients(89%) were less than 50mmHg. 4. Preoperative tricuspid regurgitation(TR) is none in 12, trivial in 6, mild in 3, moderate in 5, severe in 2 patients but postoperative TR was none in 18, trivial in 6, mild in 4 patients, so TR in most patients had decreased or not. 5. Indications for operation were based on the presence of a significant shunt. However, in patients with small shunts, indications for operation were included additional factors, tricuspid valve pouch, RVOT obstruction(right ventricular outflow tract obstruction), subacute bacterial endocarditis and associated anomalies. 6. There were no hospital deaths and residual shunts in postoperative echocardiography. Therefore, TATV is especially a good method in PMVSD where patients have trcuspid valve pouch. And it is a safe and effective technique that improves exposure for PMVSD repair and does not adversely affect tricuspid valvular competence.
