1.Reversibility of DNA Ploidy Changes Observed in Pulmonary metastasis in the Murine Osteosarcoma after Orthotopic Implantation

The difference in DNA ploidy pattern between primary and metastatic tumor remains an interesting question. We examined whether the observed change of tumor ploidy is permanent or whether it is reversible after reinoculation of the metastatic foci to an orthotopic site of another mouse. We isolated 14 metastatic pulmonary nodules from 8 mice with MGH-OGS murine osteosarcoma of thigh, and inoculated these nodules into the thighs of twenty nine C3H/Sed mice. In addition, we performed the cytoflowmetric examination of the primary tumor, the inoculated metastatic pulmonary nodules, and the regrown tumor after orthotopic inoculation. In the primary thigh tumor of FS MGH-OGS mice, the ploidy pattern was primarily aneuploid(Average values were % Diploid Cells: 23% and the mean DNA Index: 1.39). Examination of the pulmonary nodule revealed a significant increase in the diploid cell population(Mean: 54%) in 79% of nodules and a significant decrease in the mean DNA Index(Mean: 1.24) in 50% of the nodules. The tumors regrown after orthotopic implantation of metastatic pulmonary nodules however showed the same ploidy pattern and regrown parameters as was seen in the primary tumor, regardless of the ploidy pattern of the implanted metastatic nodules. This data support the concept that the lesion that goes to the lung may have hyperpercentage of surviving diploid cells(rather than aneuploid), but once reimplanted into the "native" site became primarily aneuploid again.

2.Partial Capital Resection in Legg-Calve-Perthes' Diseases

Partial capital resection (cheilectomy, partial capitectomy, excision of a lip) has gained an accepted place in treating the Legg-Calve-Perthes disease with severely malformed femoral head in which containment surgery has rarely been successful. The clinical experiences in six children aged over 10 years who complained marked limitation of hip motion with severe collapse and subluxation of femoral head are presented. Postoperative results including clinical and radiological findings are quite promising.

3.Microsurgical Reconstruction of Extremities by Cutaneous or Myocutaneous Free Flaps

The authors performed microsurgical reconstruction by free cutaneous or myocutaneous flaps transfer in 104 cases, who were sustained of extensive soft tissue injuries on their extremities by accident. These 104 cases were operated in 6 years from 1980 to 1986 and were consisted of 47 cases of latissimus dorsimyocutaneous free flaps, 36 cases of scapular free flaps, 18 cases of dorsalis pedis sensible free flaps, and 3 cases of groin flaps. Most of them were males(80 cases) and were in third decade(28 cases). The youngest case was 2-year-old baby and the oldest one was 76-year-old woman. The traffic accident was the most frequent cause of injuries(55 cases), and the others were burn, industrial injuries, and cultivator accidents. Most of the microsurgical reconstruction were performed in lower extremity injuries(81 cases), especially below the ankle joints(65 cases). The average dimension(width and length) of free flaps were 7.3×14.3cm in latissimus dorsi free flaps, 5.6×8.2cm in scapular free flaps, and 4.5×5.9cm in dorsalis pedis free flaps. The dorsalis pedis free flaps were successful in 100%, the scapular free flaps were in 83.3%, the latissimus dorsi myocutaneous free flaps were in 89.4%, and the groin free flaps were in 33.3%, And the overall viability of 104 free flaps was 87.5%.

5.A Clinical Study of the Ankle Fractures

Ninety six patients treated at Seoul Red Cross Hospital from Jan. 1979 to Dec. 1982 were analized in clinical and radiological aspect. The results obtained from this study were as followings; 1. Among the 96 patients, male was 67 patients and female was 29 patients (M:F=2.5:1). The average age was 32. 2. The causes of the injury were traffic accident, slipping down, falling down and sports injuries in orders. 3. According to the classification of Lauge-Hansen, supination external rotation type (38.5%) was the most common type. 4. 65 cases (67.7%) were treated by open reduction and 31 cases (32.3%) by closed reduction. The better result was obtained by open reduction than closed reduction. 5. Accurate reduction and rigid internal fixation of the lateral malleolus was most important in treatment of the ankle fractures. 6. Classification of Lauge-Hansen was useful in diagnosis and treatment of the ankle fractures.

7.The Changes of Postural Balance in Patients with Total Hip Arthroplasty.

PURPOSE: To investigate the changes in postural balance before and after total hip arthroplasty (THA) using computerized dynamic posturography MATERIALS AND METHODS: This was a prospective study of 18 patients undergoing total hip arthroplasty. We evaluated each patient's postural balance prior to, and at least 12 months after, total hip arthroplasty by using computerized dynamic posturography. We compared the preoperative equilibrium scores with the postoperative equilibrium scores. Clinical results were assessed for all patients preoperatively and postoperatively using the Harris hip scoring system. We investigated whether postural balance improves, and what factors were related to this parameter. RESULTS: Patients showed significant improvement in postural balance by means of proprioception after total hip arthroplasty (p<0.05). We found, during intercepting visual compensation, that an increase in the Harris hip score was correlated with an improvement in balance (p<0.01, r = 0.649). CONCLUSION: Patients with Total hip arthroplasty showed improvement in dynamic postural balance through proprioceptive recovery, and balance improved as the Harris hip score increased. This resulted in decreased pain, recovery of articular function, enhancement of physical activity, and ultimately improvement in postural balance by means of total hip arthroplasty.

8.A Clinical Study of Popliteal Artery Injury

With the increase of motor accident, the rate of popliteal artery injury is increased, As the injuries are critical, early diagnosis and prompt treatment can save the limb. 27 cases of popliteal artery injury patients were reviewed and the result was as follows ; l. Amputation rate was 11%, as 3 out of 27 cases failed. There was no correlation between ischemic time and the rate of amputation. 2. Complete resection of all injured portion of vessel and reconstruction of patency through interposed saphenous vein graft was often indicated to avoid tension at the anastomosis site. 3. The rate of fasciotomy was positively correlated with the ischemic time. 4. Medial approach is preferred to posterior approach, since it makes proximal extension of exploration possible, and decrease the incidence of joint contracture. 5. Complications noted are wound infection, systemic sepsis and venous stasis, but they could be treated with proper management. However, nerve deficit due to associated nerve injuries was resist to treatment.

9.Cervical Myelography in Brachial Plexus Injuries

With the increase of frequency and the recent development of surgical treatment in brachial plexus injury, the requirement for accurate assessment between supra and infraganglionic injury is increased. Cervical myelography has been the popular tool in evaluating the root lesion since the first introduction of Murphey et al. in 1947. However, the invasiveness of procedure has been the impediment to the common-use. But with the utilization of water-soluble contrast media, Metrizamide, such impediment can be alleviated. Author has performed myelography in 27 patients along with surgical exploration in 18 patients since 1984. And the results were as follows. 1. 85% of 27 patient revealed anomaly of root in myelography. 2. Traumatic menimgoceles were typical findings and most of them were located at C7 and C5. 3. Myelography reaveled 70%(28 root) of anomalies among 40 root lesions, confirmed by surgical explorations. 4. Myelography is considered to be very effective in evaluating supra and infraganglionic root lesions when its results is correlated with physical and other neurological examination.

10.A case of Cryptococcal Meningitis.

The clinical picture and CSF findings in cryptococcal meningitis may be identical with those of tuberculous meningitis. The differential diagnosis can be made by finding the budding yeast organism in the counting chamber of in stained smear, the detection of cryptococcal antigen in CSF by the latex agglutination test, and by culture of the fungus on Sabouraud agar. We experienced a case of cryptococcal meningitis in the 48 years old woman, which was confirmed by Indian ink preparation and culture.
