1.Schirmer Test and BUT(Tear Film Breakup time) with Nonspecific Conjunctival Irritations.

Schirmer test and tear film break up time(BUT) were performed in 126 patients who complained of nonspecific conjunctival symptomps. The mean value of Schirmer test was 7.85mm/5min and the mean value of the BUT was 9.68sec. Marked decrease was shown in comparison with previously studied normal values of Korean and according to these results, dry eye was suspicious. Same as for normal eyes, there was no relationship between the Schirmer test and BUT in this study. So, Schirmer test and BUT with detailed slit lamp exam should be necessary for those who complain of tearing, foreign body sensation and visual disturbances and these may helpful for diagnosis and treatment of dry eye patients.

2.A Case of Optic chiasmal and focal hypothalamic infarction.

Ischemia is a rare cause of the optic chiasmal syndrome and is sometimes hard to define. The present report describes a 73 year ~ old male with abruptly developed visual field defect involving bitemporal hemianopsia and inferior binasal quadrantanopsia. The MRI study revealed focal anterior hypothalarnic atrophy which might be due to focal infarction. These findings are suggesting common blood supply of superior chiasmal circulation and anterior inferior hypothalamus.

5.New Index of Combined Systolic and Diastolic Cardiac Performance Using Echo-Dopple Methods: The Utility of Cardiac Performance Index in the Screening Test of Cardiac Dysfunction due to Acute Myocardial Infarction.

BACKGROUND: Because systolic and diastolic dysfunction frequently coexist in acute myocardial infarction(AMI), we hypothesize that a combined measure of ventricular performance using Doppler echocardiography may be more sensitive and time-saving diagnostic tool for the evaluation of patients presenting with cardiogenic chest pain. METHOD AND RESULTS: Seventy-one patients with AMI (47 male, 59+/-11 years) and 45 patients with normal coronary artery (29 male, 52+/-11 years) were included in the study for measurement of cardiac performance index and established parameters of ventricular function using conventional echo-Doppler methods. a new derived index of cardiac performance: (ICT+IRT)/ET, was obtained by subtracting ejection time(ET) from the interval between cessation and onset of the mitral inflow velocity to give the sum of isovolumic contraction time(ICT) and isovolumic relaxation time(IRT). The mean value of the index was significantly different between normal and AMI(p<0.01). The degree of inter-group overlap was smaller for the index compared to other parameters. within functional groups, the value of the index did not appear to be related to heart rate, mean arterial pressure and the degree of mitral regurgitation. CONCLUSION: cardiac performance index is a conceptually new, simple and reproducible Doppler index of combined systolic and diastolic myocardial performance, and it is useful as screening test for patients with cardiac dysfunction due to AMI.

6.A Case of Occult Adenocarcinoma of Cystic Duct Associated with GB Empyema.

A case of adenocarcinoma of cystic duct associated with GB empyema is presented. A 72 year old male was admitted to Kwangju Christian hospital because of abdominal pain in right upper quadrant for 4 days. He underwent explolaparotomy under the impression of GB empyema or GB cancer. For seeking causative factor of GB empyema, serial sections of cystic duct and mapping were performed. We can find adenocarcinoma of cystic duct in the specimen removed at cholecystectomy. The criteria for disgnosis of cystie duct carcinoma which was outlined by Farrar. These criteria are; 1) The growth must be restricted to the cystic duct, 2) There is no neoplastic process in the gall bladder, hepatic and common bile duct. 3) Histopathological examination must be comfirm carcinoma. Here we report a case of adenocarcinoma of cystic duct associated with GB empyema with brief review of Literature.

8.Operative Treatment of the Diaphyseal Fractures of Clavicle (Fresh Fracture and Symptomatic Delayed Union or Nonunion)

All clinical studies reported in the literature have indicated that non-operative treatment is the treatment of choice for the diaphyseal fractures of the clavicle. It has also been suggested by some that open reduction may contribute the development of nonunion. From 1985 to 1989, 23 of 133 diaphyseal fractures of the clavicle were treated by open reduction and internal fixation with the intramedullary pinning with or without cerclage wiring, or prebent plate and screw fixation with supplementary iliac bone graft in each instance. All fractures healed without any complication such as the infection, pin migration, other fixation failure, neurovascular accident, and pseudarthrosis. Based on our experience and review of the literature, we concluded that the indications for open reduction and internal fixation should be: 1. The patient's inability to tolerate prolonged immobilization or recumbency in multiple injury. 2. Widely distracted or displaced fractures in adults. 3. Comminution with one or more large butterfly fragments. 4. Neuro-vascular compromise due to displacement and impingement of the bone fragment. 5. Symptomatic delayed union or nonunion. 6. Refracture. And early operative treatment is safe, reliable and provide excellent results comparable to the conservative treatments.

9.Prebanked Autologous Transfusion using Recombinant Human Erythropoietin: Report of one case

In the orthopaedic field, some elective surgery such such as joint replacement, spinal surgery and limb sparing procedures for musculoskeletal tumors frequently need various amounts of blood trans- fusion. But homologous transfusion occasionally result in various side effect including allergic reaction, febrile reaction, coagulopathies due to reduced platelets and coagulating factors, graft versus host disease(GVHD) and transmission of infectious diseases such as malaria, syphilis, hepatitis and AIDS. Recently these complications especially in elective surgery might result in medicolegal and social problem. Risks of transfusion could be minimized with autologous blood. So many authors are interested in prebanked autologous transfusion in elective orthopaedic surgery to prevent these problems. But there might be need of additional homologous blood transfusion because of the time limited and low yield of autologous blood. Recombinant human erythropoietin(4-HuEPO) has been shown to increase erythropoiesis. The authors could yield 5 units during 14 days using r-HuEPO without any adverse reactions. Therefore we could performed decompression, posterolateral fusion with pedicular screw fixation in spondylolisthesis without additional homologous transfusion.

10.The Association between the Dopamine D1 Receptor Genotype and Treatment Response in Korean Schizophrenic Patients.

BACKGROUND: Dopamine receptors have been regarded as a strong candidate involved in etiology of schizophrenia and a target for various antipsychotic drugs. The purpose of our study was to investigate whether dopamine D1 receptor(DRD1) gene polymorphisms would predict the treatment response to antipsychotics in schizophrenia. METHOD: One hundred thirty-four schizophrenic patients, who met DSM-IV criteria for schizophrenia were entered into a 48-week study. The psychopathology of the patients was assessed at baseline, 12th, 24th, 48th weeks of treatment by PANSS. Responders were defined by a 20% of the reduction in total PAnSS score at end point. The genomic DNA fragment corresponding to nucleotides of dopamine D1 receptor gene was amplified by polymerase chain reaction(PCR). RESULT: Neither allelic frequencies nor genotypes for dopamine D1 receptor differed significantly between responders and non-responders. Also, there was no difference of changes of PANSS scores among three genotype groups of the dopamine D1 receptor. CONCLUSION: Allelic variation in the dopamine D1 gene is not associated with individual differences in antipsychotic response.
