1.CAN WE ESTABLISH CARDIOPULMONARY BYPASS(CPB) THROUGH UMBILICAL ARTERY AND VEIN

Ninety fresh umbilical cords of newborns were harvested and dissected. There were 86 cords with double arteries and single vein and 4 cords with only 1 artery and 1 vein. The outer and inner perimeter of single artery, double arteries and the vein was 3 8?0 3mm vs 6 2?0 3mm, 2 4?0 2mm vs 4 6?0 2mm and 10 0?0 4mm vs 10 8?0 4mm at empty and flatened state, respectively. The diameter of vein at 20mmHg filling pressure and single and double arteries at 80mmHg was 7 2?0 3mm, 4 8?0 1mm and 3 9?0 8mm, respectively. According to these values, various drainage and perfusion metal tubes were designed and evaluated both in vitro and in vivo. By using these tubes,extracorporeal circulation was established successfully via umbilical artery and vein in a newborn who suffered from cardiac arrest after birth. The results suggest that CPB through umbilical artery and vein is simple, practical and effective.
2.THE DISTRIBUTION OF OXYTOCIN IMMUNOREACTIVE NEURONS IN THE SUBEPENDYMAL PLEXUS OF THE THIRD VENTRICLE IN THE RAT

Immunohistochemical method was used to investigate the morphology and distribution of the oxytocin immunoreactive (OXT-ir) neurons and their dendrites in the subependymal plexus (SEP) around the third ventricle (OXT-SEP). The OXT-ir neurons were of magnocellular neurosecretory type which often intercalated between the ependymal cells, their dendrites were generally long and straight and mostly paralleled to or traveled within the ependyma,some of them could protrude into the ventricle, and most of them appeared heavily vacuous and have a strong tendency of grouping into fascicles. The plexus was composed of dorsal and ventral parts, the former corresponding to the extent of the magnocellular paraventricular nuclei and was extensive and dense, the dendrites were well organized; the latter was more sparse. The cell bodies and dendrites within and between different parts might mix or contact with each other. The functional significance of the plexus is discussed.
3.CHANGES OF CALCITONIN GENE-RELATED PEPTIDE-LIKE IMMUNOREACTIVE (CGRP-LI) NERVE FIBERS OF THE ANTERIOR PITUITARY IN THE RAT AFTER ADRENALECTOMY

Using immunohistochemical method in the present study we examined the changes of the CGRP-LI nerve fibers in the anterior pituitary (AP) of the rat after adrenalectomy. 60 male adult rats were divided into 5 groups at random, i.e., normal, sham-operated, adrenalectomized (AdX) plus hydrocortisone, normal plus hydrocortisone and AdX groups. There were some of CGRP-LI nerve fibers sparsely and unevenly distributed in the normal rat AP. The fibers which had some varicosities and varied in their thickness could occurred either individually or in plexus. The quantity of CGRP-LI nerve fibers increased in number, length, branches and the density of the varicosities. The extent of their distribution was also enlarged. With a Leitz computer-assisted image analyzer the quantity of the CGRP-LI nerve fibers in the 5 groups was measured and the results showed that the amount of CGRP-LI nerve fibers in AdX group was significantly higher than those in all the other groups (P
4.Ovarian tissue autografe after cryopreservation in rats

BACKGROUND: Vitrification is a comparatively new technology which applies high concentration cryoprotectant and rapid refrigeration. By the method, the cells were quickly frozen and to avoid damage by ice crystals inside and outside. OBJECTIVE: To compare the effect of four cryoprotectants on morphology and function of ovarian tissue in rats after vitrification. METHODS: The rats were randomly assigned into six groups with 6 rats for each: DMSO + EG, DMSO + EG + sucrose, DMSC +EG + sucrose + acetamide, EG + sucrose + acetamide, ovariectomized, and normal control groups. The ovarian tissues of four freezing groups were treated with the corresponding cryoprotectants, the vitrified ovarian tissues were then resected but not frozen and transplanted; otherwise, tissues were not treated with any treatment in the normal control group. Two weeks after freezing, the tissues were thawed and heterotopic-transplanted into femoribus intemus of hind limb. At 30 days after implantation, vaginal epithelial cells and estrus cycle were observed, while after three months, blood were collected to detect the level of estradiol (E2) and the ovarian tissues were reclaimed to analyze their morphological changes. RESULTS AND CONCLUSION: All ovarian tissues were damaged after cryoprersarvation in four freezing groups. The rates ot healthy primordial follicles were 67.9%, 71.6%, 80.5%, and 59.4%, respectively, while healthy primary follicles were 41.6%, 52.3%, 55.9%, and 36.7%, respectively. In all freezing groups, the rate of the healthy follicles in DMSO + EG + sucrose + acetamide group was higher than DMSO + EG group and EG + sucrose + acetamide group (P < 0.05). No significant difference was found in the proportion of follicles at different development stages among four groups. The typical secondary follicle was not found in four groups. Damaged ovotid showed oocyte pyknosis and vacuolation in cytoplasmic area. There was not typical cell type of all freezing groups. Ovarian autografting gained visible vascularity from surrounding tissue that connected ovarian tissue to form net. There was a lot of blood capillary in transplanted ovarian tissues and clumped primordial follicles in cortical substance. The rates of primary follicles and secondary follicles were lower than primordial follicles. The level of serum estradiol was obviously decreased compared with normal control group (P < 0.01). There was significant difference between DMSO + EG + sucrose + acetamide group and other three freezing groups (P < 0.05). Four kinds of freezing methods have poor effects on different stages of follicles and the structure of ovariarn tissue. DMSO + EG + sucrose + acetamide group is an optimal protocol for cryoprerserving ovarian tissue. Freezing methods still need to explore further because the rats had not appeared disciplinary estrus cycle after ovarian autoqrafting.
6.Development of international general practice system:Influencing factors and policy implications

International experiences can provide evidences for general practitioners ( GPs) system building in China. The paper reviews the development of GP systems from several typical countries of different insurance sys-tems, including national health care systems, commercial medical insurance systems, and social medical insurance systems, etc. The factors that influence GPs systems are analyzed. It is found that the control force of health security system on health system, the control force of specialists on health system, the incentive mechanism of general prac-tice, and the development of general medicine influence the establishment of GPs systems. Finally, this paper pro-vide several policy implications:First, in order to meet the needs of public health, a GPs system with Chinese char-acteristics should be established;Second, the core values of general medicine should be promoted to provide public support for the establishment of a GPs system;Third, a sound incentive mechanism should be established for the de-velopment of a GPs system to improve the status of general practitioners;Fourth, the division of general practitioners and specialists should be promoted to improve the development of general medicine.
7.Optimization on Extracting Technology of Tongluo Granule by Orthogonal Design

Objective To optimize an extracting technology for Tongluo granule by orthogonal design. Methods Water-extracting fraction:with the weight of water-extracting fraction and paeoniflorin content as the indexes,extracting times,water volume and extracting time were screened by L9(34) orthogonal test,Alcohol-extracting fraction:with the weight of alcohol-extracting fraction and hesperidin content as the indexes,alcohol concentration,extracting times,alcohol volume and extracting time were screened by L9(34) orthogonal test. Results The optimal extraction conditions were as follows:water-extracting fraction: extracting 3 times with 12-fold water,1.5 hours for each time;alcohol-extracting fraction:refluxing and extracting 2 times with 10-fold 60% alcohol,1.5 hours for each time. Conclusion The results can provide theoretical basis for production of Tongluo granule.

Methods The primary culture of prostatic stromal cells (including fibroblasts and smooth muscle cells ) have been established and cultured to 4~6 passages. Then the cells were cultured in the medium with various concentration of TGF ? 1(0.01, 0.10, 1.00 and 10.0 ng/ml)for 48h. 10.Effects of myogenic induction, differentiation and transplantation of canine umbilical cord blood stem cells on cell-cell junction

BACKGROUND: Umbilical cord blood-mesenchymal stem cells (UCB-MSCs) following differentiation into cardiomyocytes were transplanted into ischemic myocardium. The transplanted cells can build connection with host cells and repair the infarct myocardium. OBJECTIVE: To detect the cell-cell junction after transplantation of the cardiac-like cell derived from the canine umbilical cord blood stem cells. DESIGN, TIME AND SETTING: A randomized controlled animal study was performed from July 2006 to October 2007 at the Animal Experimental Center of Xinhua Hospital Affiliated to School of Medicine, Shanghai Jiao Tong University. MATERIALS: A total of 2 full-term pregnant canines were used for isolation of UCB-MSCs. A total of 36 adult mongrel canines were divided into cell transplantation group and model control group (n=18) according to the rule of random digits table. METHODS: The MSCs at passage 4 were transfected by Laz-Z. After 3-day culture, MSCs were induced by 10 μmol/L 5-azacytidine (5-aza). The canine models of myocardium infarction were established following 3 weeks of culture. 2 mL (1 ×107)MSCs were transplanted into dogs with acute myocardium infarction by coronary artery infusion and local injection in cell transplantation group. An equal volume of saline was used in the model control group. The specimens were harvested and detected at 2, 4 and 8 weeks, respectively. Cell junction was determined using immunohistochemistry. MAIN OUTCOME MEASURES: The following parameters were measured: gene trensfection, myogenic induction and differentiation results of UCB-MSCs; junction of transplanted cells and host cardiomyocytes. RESULTS: Following 72 hours of transfaction, mass of cells expressed LacZ gene, synthetized galactosidase, and stained blue using X-gal staining. Following 3 weeks of 5-aza induction, the antigen a-Actin, Desmin and Connexin43 were all been positively expressed, but before induction they were all negative. From the myocardial section of 8 weeks after transplantation, the junction was formed between the transplanted cells and the host myocardium as formed between the transplanted cells. In the junction, green-fluorescence positive expression of cadherin and connexin43 could be seen. However, in the model control group, only cadherin and connexin43 expressed positively, but the transplanted UCB-MSCs with red fluorescence could not been observed. CONCLUSION: The UCB-MSCs is able to differentiate into cardiac-like cell in vitro and form cell-cell junction in vivo to communicate with surrounding cells.
