1.A Study on the Prevalence and Associated Factors of Falls in Some Rural Elderly.

BACKGROUNDS: It is to find out ins and outs of falls, and then to study risky factors that are preventable. METHODS: Total 260 elderly people living in Nonsan who are 60 years old and over were analyzed for data. Stereotyped of those are populational, social specific, physical functions, and basis diseases. And concerning falls, it was analyzed if they have had experience of up to 5 times within 3 years recently. RESULTS: The analyzed are total 260 people, men 112(43%), and women 148(57%). And the ones who answered they have had experience of falls are 108(41.6%), men 31(28.7%), and women 77(71.3%), showing a distinguishable difference between two groups. Women have had more experience than men have(p<0.001), and according to their age, the average age of those experienced falls showed to be higher than the one of those who have not(p=0.036). And according to their education, the ones who did not go to any schools have had more experience than those who did, showing the education has something to do with falls(p<0.001). The mean mass index(p=0.043) and average weight(p=0.023) also showed a mere difference between the group of the people who have had experience and the one of those who have not. Taking into account the fact that there were more women in the group who have had experienced falls, it is shown that there is nothing practical to pay attention to. In Activity of Daily Living(ADL) the experienced is 10.8(+/-4.2), and the unexperienced 9.1(+/-3.1), showing a mere difference between two groups(p<0.000). Concerning chronic diseases, there was no difference between those two groups, but individual patient histories showed that the experienced group had less rheumatic trouble than the unexpedenced(p=0.033). Looking at the difference of the causes for the falls according to their gender, the range of their movements(p=0.043), illumination(p=0.012), influence of alcoho1(p=0.001), and the shoes when they were falling down(p<0.001), the first aid treatments after falls(p=0.014), and influence of medications(p<0.001) showed a mere difference between men and women, while did not show any difference in seasons, time of falls, places, and the descriptions of the surfaces of the places. The mean number of falls within recent 3 years of the experienced was 1.79 times, and the comparison of the relation between the frequency and the causes of falls was done between the ones with experience of I fall, and the ones with experience of more twice falls. The average height(p=0.046) was much higher thai those with experience of more than 2 falls. There was also a mere difference between two groups in movements(p=0.009), illumination(p=0.005), influence of alcohol(p=0.003), the shoes (p=0.048), and first aid treatments after falls(p<0.001). CONCLUSION: The group with higher risk rate includes women, age, poor education, skinny physical figures, and lack of ability to act in daily life. Therefore, those results should be considered when a preventive program of falls for elderly people is designed.

2.A case of retroperitoneal hematoma by spontaneous rupture of renal capsule in hemorrhagic fever with renal syndrome presented with anuria and unilateral flank pain.

The hemorrhage in hemorrhagic fever with renal syndrome (HFRS) varies from transient petechial lesions to fulminant and massive bleeding. Also in vital organ such as lung, kidney, spleen, brain and pituitary, hemorrhage occasionally occurs spontaneously or by minor trauma. We report a case of retroperitoneal hematoma by spontaneous rupture of renal capsule in HFRS presented with anuria and right flank pain. A 34-year-old male was admitted to our hospital presenting anuria and right flank pain for 3 days. He also had suffered from fever and myalgia since 5 days ago. Sonography and computed tomography were performed at the day of hospitalization and showed massive perirenal hematoma with ruptured renal capsule and spurtting subcapsular renal artery on the right kidney. He was diagnosed as HFRS and treated with hemodialysis, fluid infusion and transfusion. After conservative treatment, he recovered from HFRS without further blood loss.

3.Relationship between Family Adaptability, Cohesion and Adolescent Problem Behaviors: Curvilinearity of Circumplex Model.

BACKGROUND: The Family Adaptability and Cohesion Evaluation Scale (FACES) III using the circumplex model has been widely used in investigating family function. However, the criticism of the curvilinear hypothesis of the circumplex model has always been from an empirical point of view. This study examined the relationship between adolescent adaptability, cohesion, and adolescent problem behaviors, and especially testing the consistency of the curvilinear hypotheses with FACES III. METHODS: We used the data from 398 adolescent participants who were in middle school. A self-reported questionnaire was used to evaluate the FACES III and Youth Self Report. RESULTS: According to the level of family adaptability, significant differences were evident in internalizing problems (P = 0.014). But, in externalizing problems, the results were not significant (P = 0.305). Also, according to the level of family cohesion, significant differences were in internalizing problems (P = 0.002) and externalizing problems (P = 0.004). CONCLUSION: The relationship between the dimensions of adaptability, cohesion and adolescent problem behaviors was not curvilinear. In other words, adolescents with high adaptability and high cohesion showed low problem behaviors.

4.A case of minimal change glomerulopathy without interstitial nephritis induced by non-opiate analgesics.

The use of nonsteroidal antiinflammatory drugs (NSAIDs) can be complicated by severe forms of renal dysfunction. These include fluid and electrolyte abnormalities, acute renal insufficiency due to alteration in renal hemodynamics, or interstitial nephritis and proteinuria secondary to glomerular pathology, which has the histologic characteristics of minimal change glomerulopathy (MCG). While NSAID-induced nephrotic syndrome characteristically consists of MCG with interstitial nephritis, which is the most common clinical manifestation, it rarely consists of MCG without interstitial nephritis, which has been reported in a handful of patients who took fenoprofen, ibuprofen, sulindac, diclofenac, or zomepirac. We experienced a 66-year-old female patient who presented with low serum albumin, proteinuria and generalized edema and received Geworin for about 2 year before developing symptoms. She histologically had MCG without interstitial nephritis and achieved a complete remission thirty-fifth days after discontinuing the drug. A cause-and-effect relationship of this disease to Geworin administration is strongly suggested by the resolution of the proteinuria after the drug was stopped and by no evidence of any impairment in renal function after twenty eight months of follow-up.

5.The Effect of Frenotomy on the Tongue Length, Motion, and Speech According to Age

Background and Objectives:
Ankyloglossia often results in articulation disorders, which depends on age, articulation ability, and mobility of the tongue. A primary treatment strategy includes surgery and/or speech therapy. This study aimed to evaluate the changes of the tongue length, tongue mobility, and percentage of consonants correct (PCC) after frenotomy.We investigated the optimal surgery timing for ankyloglossia.
Subjects and Methods:
A total of 136 patients underwent frenotomy for the tongue tie between January 2016 and December 2019. There were 90 males and 46 females, with the mean age of 5.23, which ranged from 2 to 10 years. The length of the tongue was measured for all patients during the operation preoperatively and postoperatively. The mobility of the tongue and PCC were performed preoperatively and 1 month after surgery by two speech therapists. Patients were divided according to age into three groups: 2-3 years, 4-5 years, and over 6 years old.
Results:
The mean tongue length of all age groups was significantly elongated after surgery(p<0.05). After frenotomy, the mobility of tongue, including horizontal protrusion, protrusion with upward pointing, circumlocution, and lateral movement, was improved. Also, PCCs of all age groups were significantly improved after surgery (p<0.05).
Conclusion
Frenotomy can be an effective procedure for children with ankyloglossia in all age groups. After frenotomy, length of the tongue, mobility of the tongue and PCC were significantly improved.
6.Angiotensin II Receptor Blocker Induced Inhibition of Cellular Hypertrophy and Differential Expression of Cyclin-dependent Kinase Inhibitors in Cultured Podocytes Stimulated by Long-term High Glucose.

BACKGROUND: Hypertrophy of podocytes is observed in type 2 diabetic patients. Cellular hypertrophy requires combined effects of various mitogen- induced entry into the cell cycle and subsequent cell cycle arrest at the G1/S interphase. This cell cycle arrest is mediated by various cyclin-dependent kinase inhibitors (CKIs). We investigated the effect of angiotensin II receptor blocker (ARB) treatment on podocyte hypertrophy and CKIs expression in cultured podocytes stimulated by long-term high glucose. METHODS: Immortalized mouse podocytes were cultured in media containing 5.6 mM normal glucose (NG), 30 mM high glucose (HG), or NG+angiotensin II (AII, 10(-7)M) for 7 days with or without ARB (L-158,809, 10(-6)M). Cellular hypertrophy was assessed by measurement of cellular protein/cell counts, and CKIs mRNA and protein expression were assessed by reverse-transcription polymerase chain reaction (RT-PCR) and Western blot, respectively. RESULTS: Cellular hypertrophy was induced in podocytes exposed to HG or AII compared to NG cells and this HG-induced cellular hypertrophy was inhibited with ARB treatment by 70% (p<0.05). In addition, there were 1.5-fold and 2.0 fold increases in p27Kip1 mRNA and protein expression, respectively, in HG-stimulated podocytes compared to NG- treated cells (p<0.05). p27Kip1 mRNA and protein expression were also increased in cultured podocytes stimulated by AII by 156% and 199%, respectively (p<0.05). ARB treatment ameliorated HG-induced increase in p27Kip1 mRNA by 75% and protein expression by 70% (p<0.05). In contrast, there were no significant changes in p21Cip1 and p57Kip2 protein expression in cultured podocytes exposed to HG or AII. CONCLUSION: High glucose induced significant cellular hypertrophy and increased p27Kip1 mRNA and protein expression in cultured mouse podocytes, and these changes were effectively inhibited by ARB treatment.

Mucormycosis is an uncommon, frequently fatal, opportunistic fungal infection which usually develops in immunosuppressant patients, especially in patients with AIDS, organ transplantation, diabetic mellitus, administration of steroid, chronic illness, severe malnutrition. It is classified into four groups as clinical forms and one of its, gastrointestinal type is an unusual form, believed that infection of the gastrointestinal tract is acquired through ingestion of the ubiquitous fungal spores, the stomach being the most frequently involved site. Especially, gastric mucormycosis, vascular invasive type is frequently fatal. Recently we experienced a case of gastric mucormycosis, vascular invasive type found in a 74-year old male undergoing hemodialysis for chronic renal failure and he was in severe malnutrition. In the course of the admission, unexpectedly, hematemesis was developed. Upper gastrointestinal fiberoscopy was done and we found a ulcerative lesion, geographic in body of stomach and on histologic examination, characteristic findings of nonseptate hyphae with right angle branchings was observed in the ulcer debris with vessel invasion. The patient was treated with systemic antifungal agent, amphotericin B but expired due to massive hematemesis.

10.A Case of Spontaneous Hemothorax in a Maintenance Hemodialysis Patient.

The 55 years old male patient, undergoing maintenance hemodialysis, was admitted on our department because of painful swelling on left knee joint. At 19th hospital day, he complained of severe dyspnea. He had a large amount of pleural effusion of the left chest that was shown to be hemothorax by thoracentesis. After closed thoracostomy, about amount of 3,000 mL of blood was drained and bleeding was sustained from pleural cavity, and then emergency thoracotomy was done. We cannot find any bleeding focus in thoracic cavity, but oozing blood was seen on entire inner thoracic cavity. He had no previous invasive procedure or history of trauma before 6 months. We think that hemothorax may be developed because of defected coagulopathy and increased bleeding tendency due to platelet dysfunction in renal failure patient. Immediately thoracotomy and cryoprecipitate were helpful for this patient. We report a rare case of spontaneous hemothorax in a maintenance hemodialysis patient with literature review.
