PURPOSE: The purpose of this study was to identify the relationships among nursing students' knowledge, attitudes to advance medical directives and ethics values. METHODS: The study was a cross-sectional and descriptive survey, carried out with 232 students from two schools of nursing, one located in B and C city. Student nurses' knowledge, attitudes for advance medical directives and ethics values were measured using structured self-report questionnaires. Data were analyzed using t-test or one-way ANOVA, and Pearson correlation coefficients. RESULTS: The scores for knowledge, attitudes for medical advance directives were 7.18 and 3.07, and for ethics values, 3.47. Nursing students' knowledge and attitude showed a significant difference depending on grade. Nursing students' ethics values showed significant differences depending on whether a study on ethics had been completed. A positive correlation was observed between nursing students' knowledge, attitudes for advance medical directives and ethics values. CONCLUSION: The results of this study indicate that basic data for the development of an effective education program on ethics is important in establishing a positive attitude toward advance medical directives and the appropriate ethical values in student nurses.

PURPOSE: This study was aimed to explore the work experience of nurses who transitioned from the patient nursing department to administrative departments.METHODS: The participants were 11 nurses who had worked in departments of human resources, education and training, medical insurance review, information technology, and quality improvement. Data were collected through in-depth individual interviews done between December 2017 and July 2018 and were analyzed in terms of Giorgi's phenomenological methodology. The research question was ‘What kind of experience did you have as you transition to a new role from a nurse to a hospital administrative nurse? What are the challenges in carrying out a new role? How do you overcome obstacles and adjust to being a hospital administrative nurse?’RESULTS: The main essence of the administrative nurses' work adjustment experience is represented by six components; 1) Beginning of a new challenge in conflict, 2) Running into barriers, 3) Struggling to take root, 4) Recognizing changes in the hospital, 5) Gradually adjusting to change, 6) Becoming a proficient hospital administrative nurse.CONCLUSION: Based on these findings, it is essential to create systems that enable administrative nurses to adjust to their work without confusion, and continuously develop programs to enhance their competence.

The intradermal (ID) test has been widely used in Korea and several reports about the results of the ID test are known. We examined the egg of Clonorchis sinensis (C.s.) by ID test in 443, stool's egg-counting technique in 79 and direct smear (cellophane thick smear technique) in 1204 subjects. The results are as follows: 1. The positive rate of C.s. was 3.8% out of 1304 persons. 2. The sensitivity of ID test was 82.1% out of 39 persons and the specificity was 64.6% out of 404 persons. 3. The false positive of ID test was 35.4% out of 404 persons and, the false negative was 17.9% out of 39 persons. Intradermal test is a rapid, sensitive and useful supplementary diagnostic tool for the detection of Clonorchiasis infection and must be used as screening test with direct smear of stool but cross reaction with other helminth infections and moderate false reaction are the main disadvantages in its practical application.

BACKGROUND: Postoperative admission to the surgical intensive care unit (S-ICU) is commonly planned to prevent and treat complications, unnecessary admission to the S-ICU increases medical costs and length of hospital stay. This study aimed evaluated outcome and the predictive factors for mortality in patients admitted to the S-ICU after abdominal surgery. METHODS: The 168 patients admitted to the S-ICU immediately after abdominal surgery were reviewed retrospectively from January to December 2011. RESULTS: The mortality rate of patients admitted to the S-ICU after abdominal surgery was 8.9% (15 of 168). Two preoperative factors (body mass index [BMI] < 18.5 kg/m2 [p < 0.001] and serum albumin < 3.0 g/dL [p = 0.018]), two operative factors (the need for transfusion [p = 0.008] or vasopressors [p = 0.013] during surgery), and three postoperative variables (mechanical ventilation immediately following surgery [p < 0.001], sequential organ failure assessment [p = 0.001] and SAPS II [p = 0.001] score) were associated with mortality in univariate analysis. After adjusting for age, gender, and SAPS II by a Cox regression, which revealed that BMI < 18.5 kg/m2 (p < 0.001, hazard ratio [HR] 9.690, 95% confidence interval [CI] 2.990-25.258) and the use of mechanical ventilation on admission to S-ICU (p < 0.001, HR 34.671, 95% CI 6.440-186.649) were independent prognostic factors. CONCLUSIONS: In patients in S-ICU after abdominal surgery, low BMI and postsurgical mechanical ventilation should be considered important predictors of mortality.

We performed a retrospective analysis of the results of surgical treatment of sixteen patient with comminuted fracture of the distal femur from September 1993 to August 1995 and evaluated the efficacy of the angled blade plate as one of the fixation devices used. The fractures were limited to C2 and C3 fracture classifed according to the AO classification. After an average follow-up of 13 months (range, 9 to 24 months), all of the fractures had healed. Postoperatively, the arc of flexion was limited between 90degrees and 125degrees in eight, normal arc in five, and severely restricted arc of joint motion was present unexceptionally in three patients with deep infection. In angular deformity, average 5.8degrees of valgus and 2.4degrees posterior angulation were showed. The functional results by Sanders-Swiontkowski-Rosen-Helfet rating system were showed excellent in 3, good in 7, fair in 3, and poor in 3 cases. The overall results were seen to be excellent to good in ten cases (62.5%) but in twelve patients using the angled blade plate, excellent to good results were showed 10 patients (83.3%). We attribute the favorable results in our series to restoration of the articular surface through the modified extensile approach, sufficient stable fixation using the angled blade plate with conforming to the concept of bridge plate, routinely performing the bone graft for medial buttress and bone healing, and early mobilization of the extremity.

PURPOSE: We report a case of unilateral, focal, pigmented paravenous retinochoroidal atrophy (PPRCA). CASE SUMMARY: A 46-year-old female visited our clinic in complaint of a vague problem with her right eye identified during a general medical examination. The visual acuity (without correction) of both eyes was 1.0. Slit-lamp examination of both eyes revealed no specific signs. Fundus examination of the right eye revealed focal, bony-spicule-shaped retinochoroidal atrophy with pigmentation along the course of the superior retinal vein. A fundus autofluorescence examination revealed principally hypofluorescence with some hyperfluorescence at the margin of the atrophic retinochoroidal lesion. Optical coherence tomography revealed mixed clumping and atrophy of the retinal pigment epithelium (RPE) layer and thinning of the choriocapillaris layer. Fluorescence angiography revealed a window defect and blockage at the site of the lesion (the fluorescent material did not enter the lesion). The site of the window defect was in correlation with the atrophic RPE region. The site of the blockage at lesion also matched with the site of the regional pigment clumping. No definite leakage was observed. CONCLUSIONS: To the best of our knowledge, this is the first case of unilateral focal PPRCA reported from Korea.

Purpose: The purpose of this study was to examine the influence of empowerment, infection control organizational culture, and infection control awareness on infection control performance among nurses in long-term care hospitals. Methods: A descriptive survey study was conducted with 125 nurses as subjects who had been working for at least six months in four long-term care hospitals located in Busan metropolitan city and Gyeongsangnam-do Province. Data were collected from September 30 to October 28, 2021 and analyzed using t-test, one-way ANOVA, Pearson’s correlation coefficients, and stepwise multiple regression with SPSS/WIN 26.0. Results: The results showed that infection control performance had significant correlations with empowerment (r=.36, p<.001), infection control organizational culture (r=.51, p<.001), and infection control awareness (r=.75, p<.001). Multiple regression analysis for infection control performance revealed that the most powerful predictor was infection control awareness (β=.70, p<.001). Empowerment, infection control awareness and awareness of infection control guidelines explained approximately 60.0% of the variance in infection control performance. Conclusion Findings indicated that various factors are related to the infection control performance among nurses in long-term care hospital. Based on the results of this study, further development and application of the programs to enhance empowerment and infection control awareness are needed in order to improve the infection control performance of nurses in long-term care hospitals.

Coronavirus disease-2019 (COVID-19) has led to a decline in restaurant visits, which in turn has changed the decision-making process of consumers. This study aimed to investigate the factors influencing visit intention during the COVID-19 endemic by employing the expanded theory of planned behavior. A total of 291 samples were collected through an online survey for the analysis. Confirmatory factor analysis, correlation analysis, and structural equation path analysis were performed on the collected data using the Analysis of Moment Structures (AMOS) statistical program. The findings of the study revealed the positive effects of attitude and perceived behavioral control and the negative effects of subjective norms on desire. Second, desire played a mediating role between antecedent variables (attitude, subjective norm, perceived behavioral control) and the visit intention. These findings are meaningful in that they expand the theory of planned behavior during the COVID-19 endemic based on factors that affect visit intention.

The purpose of this study is to identify personal factors, social factors, and environmental factors related to physical activity in older adults in urban and non-urban areas. Methods: We used source data from the 2017 Community Health Survey. The subjects of this study included some older adults aged 65 and over, and analyzed the data of 23,043 older adults living in the urban and 34,063 older adults living in the non-urban area. Results: The common factors influencing physical activity in older adults by region include current smoking and drinking, BMI, sleep duration, and subjective health status, help with neighbors, frequency of meeting with neighbors and friends, participation in social and leisure activities, and falls experience (p<.001). However, the living environment, public transport satisfaction, and medical service use significantly associated with physical activity for only older adults living in the urban area (p<.001). Conclusion: In order to improve physical activity in older adults in the community, it is necessary to consider not only the improvement of individual factors that practice health behaviors but also health promotion strategies that take into account social and environmental factors because there are environmental differences among regions.
