The purpose of this study was to identify the relationship between the burden of family with the mentally ill and professional needs. Data collection period was from June 29, 1998 to July 30, 1998. The subjects for the study was 134 family members of psychiatric in-patients and out-patients in Taejon and Chungnam. The questionnaire developed by OK-KYUNG YANG to measure the family burden and professional needs was used for this study. The tools used for this study were composed of General characteristics(30 items), family burden(43 items), professional needs(26 items). The data was analyzed by using on SPSSWIN program and included percentage, mean, S.D., t-test, ANOVA and Pearson correlation coefficient. The results of the analysis were as follows; The mean score of family burden was 134.26, which is higer than the mean score of the instrument. The mean score of professional need, the family caregiver who supporting of mentally ill patients, was .48 which is lower than the mean score of the instrument. Family burden was statistically correlated with professional needs(gamma=.6139, P<.001). There were statistically differences in family burden for respondent's relation, location of living, most care money giver, substant care money giver. There were statistically significant difference in professional need for family age, marriage status, substant care money giver, information status. The conclusion that can be drawn from this study is that addressing professional needs would contribute to reduce burden of the family caregivers with mentally ill. Therefore, Nursing interventions are needed to reduce family burden.

Acute suppurative arthritis in neonates is very difficult to diagnose early since it does not show typical clinical picture and as a results, delayed diagnosis and improper treatment often leads to permanent disability in patient. To prevent the sequelae follwing acute suppurative arthritis, early surgical treatment and administration of antibiotics are essential. We analysed 15 affected joints in 11 neonates less than 1 month old who were treated by surgical treatment and administration antibiotics. The results were as follows. 1) As for the specific joint involved, those affecting hip joint were most frequent(8 joint: 53%), followed by knee joint(5 joints: 33%) and shoulder(2 joints: 14%). Acute osteomyelitis was simultaneously occurred in 10 joints(67%). 2) As for clinical symptoms and signs, 10 cases (91%) showed limited range of motion with the clinical symptoms and signs similar to neonatal sepsis. 3) As for the causative organism, Gram-negative bacilli was the most frequent one(40%), followed by Staphylococcus aureus(27%) and candida albicans. No organisms were identifiable in four joints(27%). 4) As for the sensitivity test to antibiotics, group of staphylococcus aureus was found sensitive to newer cephalosporins while those of gram-negative bacilli to amikacin in 83%. 5) Average time between the first clinical symptom and operation was 5.14days. 6) The hip joint had yielded most complications(5 cases) followed by knee and shoulder joint, one for each. The accompanying osteomyelitis and delayed treatment were contributing factors to complications.

OBJECTIVE: This study was conducted to perform histomorphometric evaluations of the bone surrounding orthodontic miniscrews according to their proximity to the adjacent tooth roots in the posterior mandible of beagle dogs. METHODS: Four male beagle dogs were used for this study. Six orthodontic miniscrews were placed in the interradicular spaces in the posterior mandible of each dog (n = 24). The implanted miniscrews were classified into no loading, immediate loading, and delayed loading groups according to the loading time. At 6 weeks after screw placement, the animals were sacrificed, and tissue blocks including the miniscrews were harvested for histological examinations. After analysis of the histological sections, the miniscrews were categorized into three additional groups according to the root proximity: high root proximity, low root proximity, and safe distance groups. Differences in the bone–implant contact (BIC, %) among the root proximity groups and loading time groups were determined using statistical analyses. RESULTS: No BIC was observed within the bundle bone invaded by the miniscrew threads. Narrowing of the periodontal ligament space was observed in cases where the miniscrew threads touched the bundle bone. BIC (%) was significantly lower in the high root proximity group than in the low root proximity and safe distance groups. However, BIC (%) showed no significant differences among the loading time groups. CONCLUSIONS: Regardless of the loading time, the stability of an orthodontic miniscrew is decreased if it is in contact with the bundle bone as well as the adjacent tooth root.

Purpose: This study examined the effect of vibration exercise on the thickness of the oblique extrinsic, oblique abdominal, and biceps muscles, which are trunk muscles, targeting children with spastic cerebral palsy. Methods: The participants in this study were 20 children (8 male and 12 female) with cerebral palsy aged 5–10 years. They were classified into two groups using a randomized allocation method, and the trunk muscle thickness was measured using an ultrasound-imaging device before and six weeks after the experiment. A paired t-test was used for the within-group changes, and an independent t-test was used for the inter-group changes. The significance level was set to α = 0.05. Results: There was a significant increase in the inter-group change in the experimental group and control group in the intra-group change in the external oblique muscle and internal oblique muscle. After six weeks, there was a significant increase in the experimental group compared to the control group. Conclusion Vibration exercise had a positive effect on the trunk muscle thickness of children with cerebral palsy. Vibration exercise produced a significant difference in the changes in the trunk muscle thickness in children with cerebral palsy compared to no vibration exercise. These results may provide basic data for future research and as a training method for strengthening the trunk muscles in clinical trials.

Purpose: The purpose of this study was to examine the effect of sling exercise on pain, trunk strength, and balance in patients with chronic low back pain in their 40s and 50s. Methods: Twenty patients with chronic low back pain were divided into two groups, 10 patients in the exercise group using a sling and 10 patients in the waist stabilization exercise group, applying a random assignment, draw-out method, and sling exercise was applied for 50 minutes a day, 3 times a week, for a total of 5 weeks. For data analysis, a corresponding t-test was performed for within-group changes and an independent t-test was performed for inter-group changes, and the significance level was α = 0.05. Results: After 5 weeks, there were significant differences in all items of within-group changes, and the inter-group changes after 5 weeks demonstrated significant differences in pain, trunk flexion strength, and balance. Conclusion The results of this study showed that exercise using a sling had positive effects on pain, trunk flexion strength, and balance changes in chronic low back pain patients. It is suggested that sling exercise can be used as an intervention method for pain reduction and functional improvement of patients with chronic low back pain in clinical practice.

BACKGROUND: The purpose of this study was to evaluate the efficacy of suction drain use following arthroscopic rotator cuff repair by comparing early pain score and range of motion (ROM) between groups with and without suction drains. METHODS: The study included 153 patients with rotator cuff tears who underwent arthroscopic repairs at our clinic from April 2014 to March 2015. Following surgery, a suction drain was used in 85 patients (group D) and not used in 68 patients (group ND). There was no statistical difference between the groups in terms of age, gender, or total operation time. The clinical outcome with regard to pain (assessed by pain scores and analgesic requests) and passive ROM was assessed preoperatively and postoperatively. RESULTS: Immediate postoperative analgesic requirement was significantly higher in group D (p=0.001), although there was no difference in pain outcomes between the groups during the 3-month follow-up period. A statistically significant difference in passive ROM was observed at the postoperative 2- and 6-week follow-ups (p=0.036, 0.035, and 0.034 in forward elevation (FE), external rotation at the side (ER) and 90 ER at weeks 2, respectively; 0.045 and 0.009 in FE and ER at weeks 6, respectively); however no significant difference was observed at the end of 3 months. During the study period, no complication was reported in either group. CONCLUSIONS: Use of suction drains after arthroscopic rotator cuff repair provided little benefit in terms of ROM or pain in the early postoperative period (up to 3 months).

We evaluated the effect of M-VAC (methotrexate, vinblastine, adriamycin and cisplatin) chemotherapy in 63 patients with invasive transitional cell carcinoma of the bladder between January 1987 and December 1993. The patients consisted of 59 male and 4 female. Patient age ranged from 35 to 80 years with a mean of 61.5 years. All patients were given 1-7 cycles(mean 2.7 cycles) of M-VAC chemotherapy and followed for 1 to 8 years. Ten patients(16%) achieved a clinical complete remission(CR), 22(35%) partial remission(PR), 11(17.5%) minor response(MR), 11(17.5%) stabilization(STAB), and 9(14%) progression(PROG). The overall clinical response rate was 51%. Of 22 patients who underwent surgery(radical cystectomy in 15, partial cystectomy in 7), 3 patients(14%) achieved objective pathologic response. The estimated 5-year survival rate according to response of primary tumor to chemotherapy was 86%, 55% in patients with response and non-response, respectively. This difference between the groups was statistically significant(P<0.05). Overall 3-years and 5-years survival rates were 73% and 71%, respectively, and mean survival was 3.1 years. The toxicity of the regimen was generally acceptable, but 79% of the patients experienced myelosuppression, 8% hepatic toxicity and 6% stomatitis. In conclusion, the patients who achieved a clinical response seem to have a better prognosis and neoadjuvant M-VAC chemotherapy may result in bladder preservation for selected patients with muscle invasive bladder cancer.

Familial lichen planus is an uncommon disease, and there are 104 cases reported in the English literature. Several theories regarding the etiopathogenesis of lichen planus have been proposed. However, immunological mechanisms and genetic susceptibility have been emphasized the most. Several reports showed HLA analysis in lichen planus patients, and recently 2 cases have revealed an increased frequency of DR1 antigen in patients with lichen planus compared to a control group. We observed two brothers with typical skin lesions of lichen planus of almost simultaneous onset. We confirmed lichen planus by skin biopsies in both brothers and analyzed HLA typing. We also observed the occurrence of DR1 antigen in both of them.
