2.Association between periodontal disease and coronary heart disease.

Coronary heart disease is the leading cause of mortality in adult population. Whereas the association between periodontal disease and coronary heart disease (CHD) are controversial, recent studies reported the association between periodontal disease and acute myocardial infarction or prognosis of CHD. This study was aimed to investigate the relationship between periodontal disease and angiographically defined CHD, and acute myocardial infarction, and the prognosis of treated CHD. Patients under the age of 60 who had undergone the diagnostic coronary angiography were enrolled in this study. Subjects were classified as positive CHD (+CHD, n=37) with coronary artery stenosis more than 50% in at least one of major epicardial arteries, and negative CHD (-CHD, n=20) without stenosis. After recording the number of missing teeth, periodontal disease status was measured by means of plaque index (PI), gingival index (GI), bleeding on probing (BOP), probing depth (PD), and clinical attachment level (CAL). Positive CHD subjects were classified into acute myocardial infarction group (AMI), and non-AMI with angina pectoris and old myocardial infarction. Six months postoperatively, positive CHD subjects were followed and had undergone the coronary angiography again. Even though there was no significant difference in the periodontal parameters and status between positive CHD and negative CHD, some periodontal parameters, such as mean probing depth and proportion of sites with probing depth greater than 4mm or 6mm were significantly different between AMI and Non-AMI(p<0.05). There was no significant difference in the periodontal parameters according to in angiographically follow-up status. These results indicate that periodontal disease may be associated with the occurrence of acute myocardial infarction.

3.Clinical Usefulness of Electro-Oculography in Differentiating the Vertigo of Central Origin from that of the Peripheral.

The study wasdesigned for the evaluation of diagnostic usefulness of standard electro-oculography (EOG) battery in differentiating the vertigo of central origin from that of peripheral one. Twelve patients of definite posterior fossa lesion proved by MRI or CT scan were selected as a central group and fourteen patients of peripheral vestibulopathy as a peripheral group. Using our laboratory standard methods of EOG battery, age-matched normal valuse of bithermal caloric responses(fixation supperssion, vestibular paresis, and directional preponderance) and the gains of pursuit and optokinetic nystagmus were obtained. Abnormal vestibular paresis was found in 21 patients of the peripheral group, but in only one patient of the central. Directional preponderance did not show significant difference between the peripheral and central group (p<0.01). In peripheral group, the fixation suppression index (percent change in slow-phase velocity with visual fixation during the period of maximum intensity of caloric nystagmus) was 56.8+8.3%, p>0.01). However, failure of fixation suppression was noted in eleven patients of central group and its mean value of 92.6+7.3%, which was significant statistical difference compared with peripheral and control group (p<0.01 respectively). Defective suppression of caloric nystagmus was more remarkable when the direction of caloric nystagmus was induced toward the lesion site. Such findings were demonstrated in six patients of eight patients with definits unilateral cerebellar hemispheric lesions. In addition, the gains of pursuit and optokinetic nystagmus were significantly reduced in the patients showing defctive fixation suppression compared with whom showed normal pattern of fixation suppression (p<0.01). Therefore, fixation suppression, vestibular paresis, and the gains of pursuit and optokinetic nystagmus would be useful diagnostic paremeters in differntiaging the vertigo of central origin from that of the peripheral.

4.E-Cadherin Expression in Renal Cell Carcinoma according to the Mainz Classification.

According to the Mainz classification, renal cell carcinoma (RCC) consists of three subtypes: each has characteristic genetic alterations within the chromosomal or mitochondrial DNA. The three subtypes are: clear cell type, chromophil type, and chromophobe type. E-cadherin is a Ca++-dependent adhesion molecule which plays a major role in the maintenance of intercellular adhesion in epithelial tissues. In a normal kidney, E-cadherin is expressed in the distal tubule and the collecting duct, but not in the proximal tubule. We reclassified 110 cases of RCC according to mainz classification. Immunohistochemical staining for E-cadherin was done on twenty eight cases of RCC, including 18 cases of clear cell type, four cases of chromophil type, and six cases of chromophobe type. The results were as follows: 1) of the 110 cases of RCC, 96 cases (87.3%) were of clear cell type, four cases (3.6%) of chromophil type, and ten cases (9.1%) of chromophobe type, 2) there was no significant correlation between the nuclear grade and clinical stage according to each subtype, 3) E-cadherin expression showed a strong positive reaction along the cell membranes in all six cases of chromophobe type. The differential expression of E-cadherin in RCC may suggest that the chromophobe type may have different biologic characteristics from other types of RCC.

5.A Case of Carotidynia Mimicking Trigeminal Neuralgia.

Carotidynia is defined as an atypical craniofacial pain syndrome caused by the dilatation or distension of the extracranial arteries. We report an unusual case of carotidynia mimicking trigeminal neuralgia caused by an arterioscle-rotic tortous carotid artery. A 68-year-old woman suffered from frequent episodes of severe electrical shock-like radiating pain around her left ophthalmic and maxillary division of the trigeminal nerve for 5 years. Initially, under the impression of trigeminal neuralgia, carbamazepine was tried. She was reevaluated due to an unsatisfactory pharmacological response. On examination, no abnormalities were found on the craniofacial region except for a tortous pulsating tender mass on the region of the left carotid artery. Routine laboratory findings and a connective tissue study were normal. Neck sonography and a 4-vessel angiography showed a tortous left internal carotid artery with stenosis extending near to the skin. The patient was treated for 2 weeks with a nonsteroidal anti-inflammatory agent, steroids, a prophylctic beta blocker, and a TCA antidepressant. The pain gradually subsided.

7.Job Stress and Health Status of Circulation Enterprise Workers.

PURPOSE: The purpose of this study was to measure the level of job stress and health status and find out factors affecting the health status among circulation enterprise workers. METHOD: This study was a cross-sectional descriptive study. The subjects were 128 workers whose mean age was 33.2 years. Korean occupational stress questionnaire Short Form was used to measure job stress. Todai Heath Index that was made by Aoki in 1977 and translated by Kim(1997) was used to measure the health status. RESULTS: The total job stress scores of men and woman fell in the lower 50% of Korean workers. Job autonomy and interpersonal conflicts as a subcategories fell in upper level 50% in both men and women and job insecurity fell in the upper level 50% in women. The mean score of health status was 51.47, indicating slightly higher frequency of complains of health symptoms. There was no significant correlation between total job stress and total health status. Finally, factors influencing the health status were organizational system and age. CONCLUSION: Based on the results of this study, it is necessary to develop comprehensive stress management programs including interventions of personal and organizational levels for promoting workers' health.

8.Vestibuloneuritis Developed Concurrently in Ipsilateral Site with Herpes-Zoster Oticus Syndrome .

BACKGROUND:Acute vestibular neuronitis is the disease of which the etiology and pathophysiology are largely unknown . But the viral infection and ischemia of the labyrinth and the vestibular nerve are considered as general etiology. This study was performed to support the viral infection rather than the ischemic theory. MATERIALS & METHODS:We studied seventy years old female patient who showed painful vesicles on left auricle and vertigo with spontaneous nystagmus to the right side. We performed physical examination, serologic test, ENG test, pure tone audiogram, brain magnetic resonance imaging and polymerase chain reaction. RESULTS:We found small vesicles and vascular injection in left EAC, herpes zoster IgG positive, spontaneous right beating in electronystagmograpy, 54% left canal paresis in Caloric test , decreasing left side Tc in velocity step rotatory test, decresed gain, deviation to left in symmetry and phase lead in sinusoidal harmonic acceleration test, normal range hearing in pure tone audiogram, microangiopathy on cortex in brain MRI and negative PCR. CONCLUSION:This case supports viral infection etiology rather than ischemia in vestibular neuritis. But more studies to find the etiology of vestibular neuronitis are required.
